BackgroundAlthough patient advocates have developed templates for standard consent forms, evaluating patient preferences for first in human (FIH) and window of opportunity (Window) trial consent forms is critical due to their unique risks. FIH trials are the initial use of a novel compound in study participants. In contrast, Window trials give an investigational agent over a fixed duration to treatment naïve patients in the time between diagnosis and standard of care (SOC) surgery. Our goal was to determine the patient-preferred presentation (...) of important information in consent forms for these trials.MethodsThe study consisted of two phases: (1) analyses of oncology FIH and Window consents; (2) interviews of trial participants. FIH consent forms were analyzed for the location(s) of information stating that the study drug has not been tested in humans (FIH information); Window consents were analyzed for the location(s) of information stating the trial may delay SOC surgery (delay information). Participants were asked about their preferred placement of the information in their own trial’s consent form. The location of information in the consent forms was compared to the participants’ suggestions for placement. Results34 [17 FIH; 17 Window] of 42(81%) cancer patients approached participated. 25 consents [20 FIH; 5 Window] were analyzed. 19/20 FIH consent forms included FIH information, and 4/5 Window consent forms included delay information. 19/20(95%) FIH consent forms contained FIH information in the risks section 12/17(71%) patients preferred the same. Fourteen (82%) patients wanted FIH information in the purpose, but only 5(25%) consents mentioned it there. 9/17(53%) Window patients preferred delay information to be located early in the consent, before the “Risks” section. 3/5(60%) consents did this.ConclusionsDesigning consents that reflect patient preferences more accurately is essential for ethical informed consent; however, a one-size fits all approach will not accurately capture patient preferences. We found that preferences differed for FIH and Window trial consents, though for both, patients preferred key risk information early in the consent. Next steps include determining if FIH and Window consent templates improve understanding. (shrink)

Physical Relativity explores the nature of the distinction at the heart of Einstein's 1905 formulation of his special theory of relativity: that between kinematics and dynamics. Einstein himself became increasingly uncomfortable with this distinction, and with the limitations of what he called the 'principle theory' approach inspired by the logic of thermodynamics. A handful of physicists and philosophers have over the last century likewise expressed doubts about Einstein's treatment of the relativistic behaviour of rigid bodies and clocks in motion in (...) the kinematical part of his great paper, and suggested that the dynamical understanding of length contraction and time dilation intimated by the immediate precursors of Einstein is more fundamental. Harvey Brown both examines and extends these arguments, after giving a careful analysis of key features of the pre-history of relativity theory. He argues furthermore that the geometrization of the theory by Minkowski in 1908 brought illumination, but not a causal explanation of relativistic effects. Finally, Brown tries to show that the dynamical interpretation of special relativity defended in the book is consistent with the role this theory must play as a limiting case of Einstein's 1915 theory of gravity: the general theory of relativity.Appearing in the centennial year of Einstein's celebrated paper on special relativity, Physical Relativity is an unusual, critical examination of the way Einstein formulated his theory. It also examines in detail certain specific historical and conceptual issues that have long given rise to debate in both special and general relativity theory, such as the conventionality of simultaneity, the principle of general covariance, and the consistency or otherwise of the special theory with quantum mechanics. In many research studies, tumor biopsies are an unavoidable requirement for achieving key scientific aims. Yet some commentators view mandatory research biopsies as coercive and suggest they should be optional, or at least optional until further data are obtained regarding their scientific usefulness. Further complicating the ethical picture is the fact that some research biopsies offer a potential for clinical benefit to trial participants. We interviewed and surveyed a convenience sample of participants in phase I clinical trials at a single (...) institution. Our primary aim was to describe phase I participants’ understanding of whether a research biopsy offered them the prospect of medical benefit. We also endeavored to describe participants’ views about biopsies—specifically, the benefits of biopsies, if any, and whether biopsies were acceptable, risky, or discouraged trial participation. Finally, we collected data on demographics and attitudes to see if any strong correlations with misunderstanding, acceptability, or riskiness existed. Overall, the respondents tended to view research biopsies as acceptable, though they did not succeed in identifying the lack of benefit of a research biopsy. These findings call for renewed efforts in consent conversations and documents to carefully describe the benefits, or lack thereof, of research biopsies. (shrink)

It is argued that Minkowski space-time cannot serve as the deep structure within a ``constructive'' version of the special theory of relativity, contrary to widespread opinion in the philosophical community.

One of the widespread confusions concerning the history of the 1887 Michelson-Morley experiment has to do with the initial explanation of this celebrated null result due independently to FitzGerald and Lorentz. In neither case was a strict, longitudinal length contraction hypothesis invoked, as is commonly supposed. Lorentz postulated, particularly in 1895, any one of a certain family of possible deformation effects for rigid bodies in motion, including purely transverse alteration, and expansion as well as contraction; FitzGerald may well have had (...) the same family in mind. A careful analysis of the Michelson-Morley experiment (which reveals a number of serious inadequacies in many text-book treatments) indeed shows that strict contraction is not required. (shrink)

It is argued that awareness of the distinction between dynamical and variational symmetries is crucial to understanding the significance of Noether's 1918 work. Specific attention is paid, by way of a number of striking examples, to Noether's first theorem, which establishes a correlation between dynamical symmetries and conservation principles.

In a comparison of the principles of special relativity and of quantum mechanics, the former theory is marked by its relative economy and apparent explanatory simplicity. A number of theorists have thus been led to search for a small number of postulates - essentially information theoretic in nature - that would play the role in quantum mechanics that the relativity principle and the light postulate jointly play in Einstein's 1905 special relativity theory. The purpose of the present paper is to (...) resist this idea, at least in so far as it is supposed to reveal the fundamental form of the theory. It is argued that the methodology of Einstein's 1905 theory represents a victory of pragmatism over explanatory depth; and that its adoption only made sense in the context of the chaotic state state of physics at the start of the 20th century - as Einstein well knew. It is argued that an unheralded moment marking the beginnings of relativity theory occurred in 1889, when G. F. FitzGerald, no doubt with the puzzling 1887 Michelson-Morley experiment fresh in mind, wrote to Heaviside about the possible effects of motion on inter-molecular forces in bodies. Emphasis is placed on the difference between FitzGerald's and Lorentz's independent justifications of the shape distortion effect involved. Finally, the importance of the their `constructive' approach to kinematics---stripped of any commitment to the physicality of the (...) ether--- will be defended, in the spirit of Pauli, Swann and Bell. Context: Distributed language and interactivity are central members of a set of concepts that are rapidly developing into rigorous, exciting additions to 4E cognitive science. Because they share certain assumptions and methodological commitments with enactivism, the two have sometimes been confused; additionally, while enactivism is a well-developed paradigm, interactivity has relied more on methodological development and on a set of focal examples. Problem: The goal of this article is to clarify the core conceptual commitments of both interactivity-based and enactive approaches (...) to cognitive science by contrasting the two and highlighting their differences in assumptions, focus, and explanatory strategies. Method: We begin with the shared commitments of interactivity and enactivism - e.g., antirepresentationalism, naturalism, interdisciplinarity, the importance of biology, etc. We then give an overview of several important varieties of enactivism, including sensorimotor and anti-representationalist enactivism, and then walk through the history of the “core” varieties, taking care to contrast Maturana’s approach with that of Varela and the current researchers following in Varela’s footsteps. We then describe the differences between this latter group and interactivity-based approaches to cognitive science. Results: We argue that enactivism’s core concepts are explanatorily inadequate in two ways. First, they mis-portray the organization of many living systems, which are not operationally closed. Second, they fail to realize that most epistemic activity depends on engagement with non-local resources. Both problems can be dealt with by adopting an interactivity-based perspective, in which agency and cognition are fundamentally distributed and involve integration of non-local resources into the local coupling of organism and environment. Implications: The article’s primary goal is theoretical clarification and exposition; its primary implication is that enactive concepts need to be modified or extended in some way in order to explain fully many aspects of cognition and directed biological activity. Or, read another way, the article’s primary implication is that interactivity already provides a rich set of concepts for doing just that, which, while closely allied with enactivism in several ways, are not enactivist concepts. Modern insolubility proofs of the measurement problem in quantum mechanics not only differ in their complexity and degree of generality, but also reveal a lack of agreement concerning the fundamental question of what constitutes such a proof. A systematic reworking of the (incomplete) 1970 Fine theorem is presented, which is intended to go some way toward clarifying the issue.

The influence of historiography on aspects of political thought in France, Italy and Germany. In recent years the overlap between political thought and historiography has changed the boundaries of intellectual history. Donald Kelley, the longtime editor of The Journal of the History of Ideas has played a leading part in this process. These essays by his friends and former students follow in his footsteps. We discuss what we take to be three possible misconceptions in the foundations of general relativity, relating to: the interpretation of the weak equivalence principle and the relationship between gravity and inertia; the connection between gravitational redshift results and spacetime curvature; and the Einstein equivalence principle and the ability to ``transform away" gravity in local inertial coordinate systems.

The conceptual model presented in this article argues that corporations exhibit specific behaviors that signal their true level of moral development. Accordingly, the authors identify five levels of moral development and discuss the dynamics that move corporations from one level to another. Examples of corporate behavior which are indicative of specific stages of moral development are offered.

We review the dynamical approach to spacetime theories---in particular, its origins in the development of special relativity, its opposition to the contemporary `geometrical' approach, and the manner in which it plays out in general relativity. In addition, we demonstrate that the approach is compatible with the `angle bracket school'.

Abstract The historical evolution of the principle of relativity from Galileo to Einstein is briefly traced, and purported difficulties with Einstein's formulation of the principle are examined and dismissed. This formulation is then compared to a precise version formulated recently in the geometrical language of spacetime theories. We claim that the recent version is both logically puzzling and fails to capture a crucial physical insight contained in the earlier formulations. This paper investigates what the source of time-asymmetry is in thermodynamics, and comments on the question whether a time-symmetric formulation of the Second Law is possible.

Between 1964 and 1990, the notion of nonlocality in Bell's papers underwent a profound change as his nonlocality theorem gradually became detached from quantum mechanics, and referred to wider probabilistic theories involving correlations between separated beables. The proposition that standard quantum mechanics is itself nonlocal became divorced from the Bell theorem per se from 1976 on, although this important point is widely overlooked in the literature. In 1990, the year of his death, Bell would express serious misgivings about the mathematical (...) form of the local causality condition, and leave ill-defined the issue of the consistency between special relativity and violation of the Bell-type inequality. In our view, the significance of the Bell theorem, both in its deterministic and stochastic forms, can only be fully understood by taking into account the fact that a fully Lorentz-covariant version of quantum theory, free of action-at-a-distance, can be articulated in the Everett interpretation. The Past Hypothesis defended by David Wallace in his 2011 account of macroscopic irreversibility is technically distinct from, but in the same spirit as, that of David Albert in his 2000 book Time and Chance. I am concerned in this essay with the role of objective probability in both accounts, which I find obscure. Most of the analysis will be devoted to the classical treatments by both authors, but a final section will question whether Wallace's quantum version involving unitary dynamics (...) removes this obscurity. Heisenberg'sgendanken experiments in quantum mechanics have given rise to a widespread belief that the indeterminacy relations holding for the variables of a quantal system can be explained quasiclassically in terms of a disturbance suffered by the system in interaction with a quantal measurement, or state preparation, agent. There are a number of criticisms of this doctrine in the literature, which are critically examined in this article and found to be ininconclusive, the chief error being the conflation of this disturbance with (...) the projection postulate. We present a critique of the disturbance theory based on the fact that the required disturbance will in general depend on the interaction time of the system and state-preparer. This point is exploited in the construction of a spin-interaction model which acts as a counterexample to the disturbance doctrine, while remaining faithful to the spirit of Heisenberg'sgedanken experiments. Several consequences of this result are discussed. (shrink)
