Behandlungsfehler in der Psychotherapie sind bisher kaum erforscht. Eine empirisch gestützte Kategorisierung von Behandlungsfehlern stellt einen ersten Schritt dar, sich evidenzbasierten ethischen Empfehlungen zum Umgang mit solchen Fehlern zu nähern. Zielsetzung dieser Arbeit ist es, dafür erste Grundlagen zu erarbeiten, die auf Erfahrungen von Praktikern Bezug nehmen. Nach einer systematischen Literaturrecherche wurden 30 semistrukturierte Interviews mit approbierten Psychotherapeuten unterschiedlicher Ausrichtungen (Schulen) geführt und anhand der qualitativen Inhaltsanalyse nach Mayring ausgewertet. Die beschriebenen, alltäglich auftretenden Behandlungsfehler konnten in technische, normative, (...) Einschätzungs- und Systemfehler klassifiziert werden. Viele der technischen und Einschätzungsfehler wurden als reversibel angesehen; sie könnten sogar konstruktiv für die Behandlung nutzbar gemacht werden. Das Versäumnis, einen Fehler zu korrigieren, wurde als Hauptfehler betrachtet. Bei normativen Fehlern sei mit rechtlichen oder berufspolitischen Konsequenzen, aber auch mit Vertrauensverlust und Therapieabbruch zu rechnen. Für Systemfehler fühlten sich die befragten Therapeuten nicht verantwortlich; hier seien berufspolitische Änderungen nötig. Die Ergebnisse zeigen, dass die Befragten zu der Empfehlung tendieren, Psychotherapiepatienten in passender Form über Behandlungsfehler aufzuklären und in die entstehenden Konsequenzen einzubeziehen. Fazit: Psychotherapeuten äußern sich aufgeschlossen gegenüber einer transparenten, konstruktiven Fehlerkultur – eine wesentliche Voraussetzung für Fehlerprävention. Häufig resultiert erst durch die fehlende Korrektur eines (alltäglichen) Fehlers ein Behandlungsfehler, der Konsequenzen hat (z. B. Scheitern der Therapie). Um diesem entgegenzuwirken, zeichnet sich eine Befürwortung für eine passende Form der Patientenaufklärung über Fehler ab. (shrink)

The people there, particularly Judith Thomson, who is still at MIT, called Continental philosophy “stone-age philosophy,” and wouldn’t let me teach in the graduate program at all, because they thought that it would just corrupt the students and waste their time. Qualitative research has tended to evoke rather stereotyped objections from the mainstream of social science. Ten standardized responses to the stimulus "qualitative research interview" are discussed: it is not scientific, not objective, not trustworthy, nor reliable, not intersubjective, not a formalized method, not hypothesis testing, not quantitative, not generalizable, and not valid. With the objections to qualitative interviews highly predictable, they may be taken into account when designing, reporting, and defending an interview study. As a help for new qualitative (...) researchers, some of the issues, concepts, and arguments involved are outlined, and the relevancy of the standard objections is discussed. Alternative conceptions of qualitative research, coming from phenomenological and hermeneutical traditions, are suggested. The qualitative interview based on conversation and interaction here appears as a privileged access to a linguistically constituted social world. (shrink)

The reanalysis of genomic sequencing results has the potential to provide results that are of considerable medical and personal importance to recipients. Employing interviews with forty-seven predominantly medically underserved families and ethnographic observations we argue that there is pressing need to standardize the approach taken to reanalysis. Our findings highlight that study participants were unclear as to the likelihood of reanalysis happening, the process of initiating reanalysis, and whether they would receive revised results. Their reflections mirror the lack a (...) specific focus upon reanalysis within consent and results sessions as observed in clinical settings. Mechanisms need to be put into place that standardize the approach to reanalysis in research and in clinical contexts. This would enable clinicians and genetic counsellors to communicate clearly with research participants with respect to potential for reanalysis of results and the process of reanalysis. We argue that that the role of reanalysis is too important to be referred to in an ad-hoc manner. Furthermore, the ad-hoc nature of the current process may increase health inequities given the likelihood that only those families who have the means to press for reanalysis are likely to receive it. Background Gerontechnologies are increasingly used in the care for older people. Many studies on their acceptability and ethical implications are conducted, but mainly from the perspective of principlism. This narrows our ethical gaze on the implications the use of these technologies have. Research question How do participants speak about the impact that gerontechnologies have on the different phases of care, and care as a process? What are the moral implications from an ethic of care perspective? Research design Secondary analysis of (...) semi-structure interviews, whose segments on specific technologies were analysed through reflexive thematic analysis. Participants and research context Sixty-seven Swiss stakeholders involved in the use of gerontechnologies, including professional caregivers, informal caregivers, and older persons themselves. Ethical considerations The research study was evaluated by the Ethics Commission of Northwest and Central Switzerland (EKNZ). All participants received an information document before the interview date detailing the purpose, procedure, and anonymization measures. After explaining the study during the agreed upon interview time and upon receiving their written informed consent, the interview process began. Findings/results Four themes are identified: Identifying care needs, Taking responsibility, Hands-on work, Responding to care. As part of these themes, many codes highlighting the ambivalent impact of gerontechnologies are created, ranging from ‘Expanded capacity for…identifying care needs’ to ‘Create new & (un)necessary…hands-on work’. The moral implications of these results from the care ethics perspective are discussed, through the ethical elements of: attentiveness, responsibility, competence, and responsiveness. Conclusions The moral implications of gerontechnologies on care phases from the care ethics perspective open up several questions on whether they actually help give care a central role in social life and provide more competent care. Because organic certification standards institutionalized a product-based rather than process-based definition, certified organic produce can be grown on large-scale industrial monocrop farms. Besides toxicity of inputs, these farms operate in much the same way as conventional production. Scholars emphasize the fact that labor rights have been left out of certification criteria, and because of that, organic farms reproduce the same labor relations as conventional. Empirical studies of organic farm labor, however, rely primarily on the perspective of farmers. In this study, (...) I ask the farmworkers themselves how harvesting on organic farms compares to conventional, and found that working in organic negatively impacts farmworker livelihoods. Qualitative interviews with 36 strawberry harvesters in Oxnard, California reveal that farmworkers make more money in conventional strawberry production because of the interaction between wage structure and size of the berry. Conventional strawberries are larger and therefore fewer of them fill up a box. Farmworkers routinely pick more boxes in conventional than in organic, thus earning more, since under the piece rate system, farmworkers are paid per box. With short-term economic survival rather than long-term occupational health concerns in mind, strawberry harvesters would rather work on conventional farms because “la fresa orgánica es más chiquita”. Values‐based practice is deemed essential for healthcare provision worldwide. In England, values‐based recruitment methods, such as multiple mini interviews (MMIs), are employed to ensure that healthcare students’ personal values align with the values of the National Health Service (NHS), which focus on compassion and patient‐centeredness. However, values cannot be seen as static constructs. They can be positively and negatively influenced by learning and socialisation. We have conceptualised students’ perceptions of their values over the duration of their education programme as (...) a ‘values journey’. The aim of this hermeneutic longitudinal focus group study was to explore the ‘values journey’ of student nurses and midwives, recruited through MMIs, across the 3 years of their education programme. The study commenced in 2016, with 42 nursing and midwifery students, originally recruited onto their programmes through multiple mini interviews. At the third and final point of data collection, 25 participants remained. Findings indicate that students' confidence, courage and sense of accountability increased over the 3 years. However, their values were also shaped by time constraints, emotional experiences and racial discrimination. We argue that adequate psychological support is necessary as healthcare students embark on and progress through their values journey, and propose a framework for this. (shrink)

These autobiographical and philosophical essays, in the form of expertly probing interviews, provide a superb introduction to the work of one of the most significant contemporary political philosophers and a marvelously readable perspective on the French intellectual and political arenas from the 1970s to the present. Those already familiar with Manent's work will find an indispensable reflection on his transition from the critique of modernity brilliantly represented in his earlier books (most notably Tocqueville and the Nature of Democracy and (...) The City of Man), a critique at once original and significantly indebted to Leo Strauss, toward a perspective that emerges in his recent The Metamorphoses of the City, a monumental and profoundly original study that endeavors to situate modernity within the original Greek founding of the act of politics. The autobiographical passages in this vivid and engaging work invite the reader into, first, the world of postwar France in which Manent grew up, in which he was presented with the choice between the Communist hopes of his father and the opposing power and prestige of all things American. There is also an impressive portrait of the rigors and spirit of a provincial lycee where the first sparks of philosophical eros ignited in Manent's soul, and of his studies in the legendary Ecole Normale Superieur in the midst of the ideological confusion associated with the ferment of 1968. Personal data protection is an ethical issue. In this study we analyzed how research ethics committees (RECs) and data protection officers (DPOs) handle personal data protection issues in research protocols. We conducted a mixed-methods study. We included heads (or delegated representatives) of RECs and DPOs from universities and public research institutes in Croatia. The participants provided information about data protection issues in research and their mutual collaboration on those issues through structured interviews that contained closed and open-ended questions. Qualitative (...) description was used to analyze open-ended questions. The results showed that 55% of the REC representatives were not aware who was DPO in their institution. Among RECs, 65% never contacted the DPO. There were 61% of RECs who reported that they received no training from the organization on personal data protection. When asked about barriers to personal data protection in their institutions, 26% of REC members highlighted the lack of a clear protocol for assessing personal data protection issues, while 30% of DPOs mentioned lack of knowledge among researchers about personal data. In conclusion, we found that when it came to protecting personal data in research protocols, RECs and DPOs hardly ever worked together. When developing future personal data protection policies for academic and scientific research institutions, it is essential that RECs and DPOs should collaborate and both continue to expand/update their knowledge on personal data protection procedures. BackgroundEvaluation of decision-making capacity to consent to medical treatment has proved to be difficult in patients with dementia. Studies showed that physicians are often insufficiently trained in the evaluation of decision-making capacity. In this study, we present findings from a secondary analysis of a qualitative interviews with physicians. These interviews were initially used to assess usability of an instrument for the evaluation of decision-making capacity. By looking at difficult cases of decision-making capacity evaluation in patients with dementia, we (...) provide recommendations for such evaluations in clinical practice.MethodsWe used thematic coding to analyse physicians’ narratives of problematic decision-making capacity evaluations in patients with dementia to uncover challenging issues of decision-making capacity evaluation.ResultsIn this study, decision-making capacity evaluations in patients with dementia were mainly perceived as challenging when they pertained to treatment refusals and treatment unrelated circumstances, such as psychiatric consultation, advance directives, and new living arrangements. Furthermore, the physicians reported training needs regarding situation-independent challenges with decision-making capacity evaluation.ConclusionsUpon further examining self-reported training needs and challenging cases, we have developed recommendations to improve decision-making capacity evaluations in clinical practice. In these recommendations, we argue that being able to evaluate decision-making capacity is an integral part of the informed consent process. (shrink)

This paper focuses on the relationship between journalism and right wing populist discourses in the context of broadcast news interviews. We analyse a specific feature of question design in which the public is invoked as a source of opinionated positions in adversarial interviewing. Analysing data from a range of socio-political contexts, we identify a shift in adversarial questioning along a scale of ‘soft’ populism, that is the attribution of views and concerns to a generic public ‘in crisis’, to ‘hard’ (...) populism, where interviewers construct hypothetical scenarios in which populist positions are attributed to ‘some people’. We argue that the democratic role of journalists as public watchdogs, holding politicians and public figures accountable on behalf of the public, is challenged by this normalisation of populist moral order discourses in a routine journalistic practice, both drawing on and contributing to the propagation of populist agendas and anti-democratic populist rhetoric. Success in job interviews depends largely on the interviewers' favourable opinion of the candidates' presentation, and how well candidates have managed to build solidarity with their interviewers. This article explores the ways in which candidates shape their talk to interact interpersonally with their interviewers, so as to construct affiliation and solidarity. Drawing on Systemic Functional Linguistics Appraisal theory, in particular its system of Attitude, this article examines a set of authentic job interviews in French, or French and English. (...) The interview analyses are complemented by the candidates' comments on the impressions they had tried to convey and the interviewers' comments about their impressions of the candidates. The analysis identifies two strategies whereby candidates may construct affiliation and solidarity with their interviewers: expressing their enthusiasm for and interest in their work and profession, and demonstrating their professional ability. It also highlights the damaging effect of expressing negative feelings and opinions. (shrink)

This article investigates how talk show interviews are brought to an end. The closing process of televised talk shows is analysed and compared with Clayman's characterization of news interview closings. The study concludes that closings in both genres share features relating to the structural organization and the participants' behaviour which can be accounted for in terms of the institutional context in which the speech events take place. Nevertheless, the particular closing structure of each type of institutionally situated talk, as (...) well as its content and style, is genre-specific and shaped by the specific goal pursued in each speech event and by the relationship between the parties to the encounter. Thus, unlike news interviews, the entertainment purpose of talk show interviews ultimately justifies the complexity of the closing structure, its interpersonal content and informal style. ZusammenfassungDie finanziellen Ressourcen im deutschen Gesundheitssystem sind begrenzt. Diese Mittelknappheit führt im Rahmen der ärztlichen Tätigkeit zu medizinischen, ökonomischen, juristischen und ethischen Problemen, welche sich in den kommenden Jahren weiter verschärfen dürften. Aus ethischer Perspektive sind die Probleme einer gerechten Verteilung knapper Ressourcen sowie mögliche Rollen- oder Gewissenskonflikte der ärztlichen Profession besonders relevant. Mit Hilfe von qualitativen Interviewstudien lässt sich der aktuelle ärztliche Umgang mit der Mittelknappheit in der klinischen Versorgung in seiner Komplexität und seinen ethisch relevanten Aspekten untersuchen. An (...) fünf Kliniken mit unterschiedlichem Versorgungsauftrag wurden Tiefeninterviews mit fünfzehn leitenden Ärzten in den Bereichen Intensivmedizin und Kardiologie durchgeführt. Die auf Tonband aufgezeichneten Interviews wurden nach dem Verfahren der Grounded Theory ausgewertet. Die Ärzte berichten ein heterogenes Spektrum von Einflussfaktoren, Konsequenzen und Reaktionen im Zusammenhang mit ihrem Handeln bei Mittelknappheit. Zuteilungskriterien wie der medizinische Nutzen oder die Kosteneffektivität werden von den befragten Ärzten sehr unterschiedlich interpretiert und führen damit möglicherweise zu inkonsistenten Allokationsentscheidungen. Gleichzeitig bestätigen die Untersuchungen aber auch, dass die Ärzte nicht leichtfertig mit den Allokationsentscheidungen umgehen, im Gegenteil: Die Mittelknappheit und die dadurch hervorgerufenen Verteilungsprobleme können zu Gewissenskonflikten, emotionalem Stress und Gefühlen der Überforderung führen. Um Lösungskonzepte für ethische Probleme im Umgang mit der Mittelknappheit in der klinischen Praxis entwickeln und erfolgreich implementieren zu können, müssen die in dieser Arbeit systematisch zusammengestellten Verständnisweisen, Einflussfaktoren und Konsequenzen im Zusammenhang mit dem Kostendruck berücksichtigt werden. Jean-François Lyotard (1924-1998) was one of the most important French philosophers of the Twentieth Century. His impact has been felt across many disciplines: sociology; cultural studies; art theory and politics. This volume presents a diverse selection of interviews, conversations and debates which relate to the five decades of his working life, both as a political militant, experimental philosopher and teacher. Including hard-to-find interviews and previously untranslated material, this is the first time that interviews with Lyotard have been (...) presented as a collection. The emergence of the incorporation of culture into EFL education is a growing trend in Taiwan. The purpose of the study was to examine: the effects of the ethnographic interview project on Taiwanese students' cognitive development in understanding native English speakers and their cultures; changes in students' self‐awareness and understanding of both the target culture and their own; and students' perceptions of the ethnographic interview project employed in EFL college classes. Data were collected through pre–post questionnaires, oral and written reports, (...) classroom observation and interviews. Results indicated that participation in the ethnographic interview project helped facilitate the development of cross‐cultural awareness and communication skills by providing opportunities for students to gain insight into the values of target language countries, learn to view their own culture in new ways, increase their confidence in using English to communicate and view authentic communication as the goal of EFL learning. The majority of the participants saw the ethnographic interview project as an effective means of facilitating intercultural/interpersonal communication and understanding. (shrink)

This paper argues that the personal interview method is particularly appropriate for the kind of exploratory and complicated theory-building research that ethical decision-making, as a topic, represents at present. In doing so, it examines the key tasks of the ethics researcher, the suitability of interviews for obtaining the kind of data needed to accomplish these tasks, and the ensuing problems faced by the interview methodologist. It concludes with suggestions for enhancing the validity and reliability of interview-based ethics research.

Performance appraisal interviews are carried out on the basis of known-in-advance written materials such as preparation forms and interview guides. This article demonstrates how participants manage interviews by following a question–answer–response format fit to address interview guide entries one at a time. Two recurring supervisor responses to employees’ talk about problems in work are investigated: positive prediction and advice. It is suggested that these responses serve to establish supervisor authority and deter participants from discussing issues raised in employee (...) answers and thus go against norms emanating from literature on management communication. Results obtained in interviews are put down in writing along the way and subsequently summarized in documents to be signed by both participants, that is, employee and supervisor. The article demonstrates how participants use positive prediction and advice to coordinate talk-in-interaction with handling written materials and note taking, as well as formulating conclusions suitable for writing. The analyses shed light on how talk in an institutional context becomes a middle-medium, that is, a bridge between writings, and how this process is accomplished in turn-taking procedures. (shrink)

The assessment of nanotechnology applications such as nanocarrier-based targeted drug delivery has historically been based mostly on toxicological and safety aspects. The use of nanocarriers for TDD, a leading-edge nanomedical application, has received little study from the angle of experts’ perceptions and acceptability, which may be reflected in how TDD applications are developed. In recent years, numerous authors have maintained that TDD assessment should also take into account impacts on ethical, environmental, economic, legal, and social issues in order to lead (...) to socially responsible innovation. Semi-structured interviews were conducted with French and Canadian researchers and research trainees with diverse disciplinary backgrounds and involved in research related to emerging technologies. The interviews focussed on scenarios presenting two types of TDD nanocarriers in two contexts of use. Content and inductive analyses of interviews showed how facets of perceived impacts such as health, environment, social cohabitation, economy, life and death, representations of the human being and nature, and technoscience were weighed in acceptability judgments. The analyses also revealed that contextual factors related to device, to use, and to user influenced the weighting assigned to perceived impacts and thus contributed to variability in interviewees’ judgments of acceptability. Giving consideration to researchers’ perspective could accompany first steps of implementation and development of nanomedicine by producing a first, but wide, picture of the acceptability of nanocarrier-based TDD. Susan Edwards-McKie interviews Professor Dr B. F. McGuinness on the occasion of his 90th birthday.

Drawing on accounts from interviews with white women, this article explores the production of narratives of the self. It suggests that the story produced of the self is not inevitable and may revolve around notions of sameness and difference that, in turn, depend on the positionality of individuals in terms of normative discourses of `race', class and gender. Sally can be seen to be reciting the process of subjection in the way she creates herself as the subject of a (...) narrative, using tropes of difference and sameness to explain who she is and who she is not. However, for the others, the norms and conventions of lifestory do not conform with their experiences of subjection. This is because, in the case of Madeleine, she does not experience an easily retold sense of herself, while Deborah appears to want to present herself as so inevitable and conforming to dominant norms that there is no story to tell.
