The annual UK potential donor audit captures families’ reasons for not consenting to donation of their deceased family members’ organs . Given that many families’ refusals and vetoes are based on false beliefs, cognitive bias and misunderstanding, it is incumbent upon doctors, nurses and transplant coordinators to invest sufficient time to facilitate informed consent or authorization. While such families are distressed, organ donation rates could be substantially improved if they were made aware of any mistaken beliefs, using recently suggested criteria (...) for the ethical use of persuasion . This article examines some of the reasons for refusal of donation and suggests ways to help families make better decisions. It emerges that the use of persuasion is ethically essential in order to prevent families making decisions that they may come to regret. (shrink)

This article argues that some reviewers contribute more to research than many authors, and suggests that reviewers meet the ICMJE criteria for authorship in many cases.

This project tested a two-way model of communication between lay groups and experts about genetic medicine in Perth, Western Australia. Focus group discussion with community group participants was followed by a communication workshop between community group participants and experts. Four groups of concerns or themes emerged from discussion: clinical considerations; legislative concerns; research priorities, and ethical and wider considerations. Community group concerns are not always met by the actions of "experts". This is, in part, because of the differing life-worlds of (...) each group. However, the communication workshop showed the potential of two-way communication for both lay and expert members in understanding the others' viewpoint. Further, the approach developed here offers one possible way for community groups to participate in a substantial way in policy formulation processes. (shrink)

The role of editorial staff in shaping early climate change narratives has been underexplored and deserves more attention. During the 1970s, the epistemological underpinnings of the production of knowledge on climate change were contested between scientists who favoured computer-based atmospheric simulations and those who were more interested in investigating the long-term history of climatic changes. Although the former group later became predominant in the Intergovernmental Panel on Climate Change during the 1980s, the latter had a sizable influence over climate discourse (...) during the 1970s. Of these, one of the key popularizers of climate discourse during the 1970s was the British climatologist Hubert Lamb (1913–97). The correspondence between Lamb and journal editors who gatekept and curated different audiences helped craft resonant messages about climate change and its potential effects, and we explore Lamb's interactions with editors of Nature, the UNESCO Courier, The Ecologist and Development Forum in the 1973–4 period. Through understanding how climate change discussion was influenced by editors, we gain an insight into how such narratives had to be adjusted to fit into pre-existing discourses before their importance was more widely established, and how these adjustments helped shape conceptualizations of climate change as a global, human-caused phenomenon and a source of universal threat. European Union citizenship is increasingly relevant in the context of both the refugee crisis and Brexit, yet the issue of citizenship is neither new nor unique to the EU. Using historical, political and sociological perspectives, the authors explore varied experiences of combining multiple identities into a single sense of citizenship.Cases are taken from Canada, Croatia, Czechia, Estonia, Spain, Switzerland and Turkey to assess the various experiences of communities being incorporated into one entity. The studies show that the EU has a (...) comparatively large degree of diversity and complexity, with levels of integration achieved in a relatively short timeframe. Advisory models based on Canada and Switzerland allow for the EU integration processes to continue while protecting diversity and upholding common institutions. Citizenship in Segmented Societies will appeal to academics and students in the field of European and federalist studies with a focus on multiculturalism and linguistic pluralism, minority rights, and citizenship issues. An analysis of the Delos capital makes it possible to outline a method of argumentation (based on the traces preserved on the surface of the eye) in favour of hypotheses about the methods of tracing the external and internal spirals. Based on these methods, an attempt has also been made to identify the Vitruvian indications. We have analysed the integration of the spiral in the composition of the whole facade of several Hellenistic Ionic capitals (from Halicarnassus, Priene, Magnesia on the (...) Meander, Didymes, Sardis and Delos) and a fortiori of the capital described by Vitruvius, having in view the width (D), the height of the volute (G), the height of the interior tangent of the volute (N) and the secondary diagonal (Ds). The constant, so to speak, utilisation was established of pentagons as important elements of the geometrical support in the architectural composition. Thus, the presence of a "common denominator" in the internal geometry of these capitals argues strongly in favour of a research procedure on the internal — geometrical — support of the composition of Ionic capitals. Se pretende dilucidar el carácter particular de algunos de los fenómenos culturales, sociales, emocionales y comportamentales que constituyen la experiencia contemporánea del mundo (consumismo, debilitamiento de los lazos sociales y afectivos, desregulación (...) de las instituciones que sustentan la sociedad occidental, hedonismo, relativismo moral y religioso, etc). Dichos fenómenos son abordados desde un metáfora óptica –la de Saramago– y una metáfora física –la de Bauman–. Background The early phase of the COVID-19 pandemic affected nursing homes and their residents heavily. Guidelines on how to mitigate the virus’s spread and ensuring safe healthcare delivery were continually evolving. Concurrently, nursing homes faced intense media scrutiny. This challenging environment severely impacted registered nurses and physicians employed within these facilities. Aim To understand the ethical challenges experienced by registered nurses and physicians working in nursing homes during the COVID-19 pandemic. Research design Qualitative descriptive research using thematic analysis. Participants and (...) research context Individual online interviews with four registered nurses and eight physicians clinically active at nursing homes in Sweden. Ethical considerations The study was approved by the Swedish Ethical Review Authority. All participants provided written consent. Results Registered nurses and physicians working in nursing homes perceived ethical challenges stemming from early COVID-19 pandemic guidelines and media coverage. The main theme generated was ‘Struggling to maintain professional and ethical standards under pressure’ incorporating two subthemes: ‘Guidelines developed without the profession put pressure on staff’ and ‘Media’s biased reporting was perceived as unethical and undermined care’. Guidelines from the authorities were considered as developed without professional involvement. It made them difficult to adhere to without deviating from professional and ethical compasses. Media coverage adversely influenced relatives’ perceptions, resulting in mistrust towards physicians’ and registered nurses’ in delivering optimal care for the residents. Conclusions Resilient care in nursing homes necessitates the collaborative development of guidelines involving registered nurses and physicians, particularly amidst crises. Moreover, it is vital to provide support to registered nurses navigating ethical dilemmas, especially during pandemics. Guidelines and principles for care during a crisis should be development with professional involvement, be transparent, and be available to the public, to promote neutral media coverage. Future research is crucial to enhance ethical standards and tackle challenges in this context. (shrink)

In this essay, I examine Diagne’s claim that the fundamental intuition of Léopold Sédar Senghor’s thought is this: African art is philosophy. Diagne argues that it is from an experience of African art and an encounter with Bergson’s philosophy that Senghor comes to formulate his philosophical thought, which is better understood as vitalist rather than essentialist. I conclude by arguing that Senghor’s vitalism is a philosophy of becoming which nevertheless lacks an account of radical political change.

There is growing interest in the role of new urban agriculture models to increase local food production capacity in cities of the Global North. Urban rooftop greenhouses and hydroponics are examples of such models receiving increasing attention as a technological approach to year-round local food production in cities. Yet, little research has addressed the unintended consequences of new modes of urban farming and food distribution, such as increased competition with existing peri-urban and rural farmers. We examine how small-scale farmers perceive (...) and have responded to a recently established rooftop greenhouse and online marketplace enterprise in Montréal, Canada. Drawing on interviews with key informants and small-scale farmers, we find that peri-urban and rural producers have been affected in three key ways that represent tensions, adaptations, and synergies arising from this new urban agriculture and food distribution enterprise. First, many farmers are concerned about increased competition and value conflation with the ideals of community supported agriculture and organic farming. Second, some farmers have adapted by developing novel marketing strategies and working with local bridge organizations to collectively market their produce to urban consumers. Third, a few farmers have decided to wholesale their produce to this new enterprise, allowing them to specialize production and avoid marketing their produce directly to urban consumers. Our study suggests that the emergence of a new form of alternative food network in Montréal has created both positive and negative disruptions for existing small-scale producers. Advocates for the expansion of new urban food production and distribution models should therefore give greater consideration to the effects on other actors in the local food system. (shrink)

Through cultural transmission, repeated learning by new individuals transforms cultural information, which tends to become increasingly compressible . Existing diffusion chain studies include in their design two processes that could be responsible for this tendency: learning and reproducing . This paper manipulates the presence of learning in a simple iterated drawing design experiment. We find that learning seems to be the causal factor behind the increase in compressibility observed in the transmitted information, while reproducing is a source of random heritable (...) innovations. Only a theory invoking these two aspects of cultural learning will be able to explain human culture's fundamental balance between stability and innovation. (shrink)

We investigate the emergence of iconicity, specifically a bouba-kiki effect in miniature artificial languages under different functional constraints: when the languages are reproduced and when they are used communicatively. We ran transmission chains of participant dyads who played an interactive communicative game and individual participants who played a matched learning game. An analysis of the languages over six generations in an iterated learning experiment revealed that in the Communication condition, but not in the Reproduction condition, words for spiky shapes tend (...) to be rated by naive judges as more spiky than the words for round shapes. This suggests that iconicity may not only be the outcome of innovations introduced by individuals, but, crucially, the result of interlocutor negotiation of new communicative conventions. We interpret our results as an illustration of cultural evolution by random mutation and selection. (shrink)

BackgroundMoral distress (MD) is an emotional and psychological response to morally challenging dilemmas. Moral distress is experienced frequently by nurses in the intensive care unit (ICU) and can result in emotional anguish, work dissatisfaction, poor patient outcomes, and high levels of nurse turnover. Opportunities to discuss ethically challenging situations may lessen MD and its associated sequela.ObjectiveThe purpose of this project was to develop, implement, and evaluate the impact of nursing ethics huddles on participants’ MD, clinical ethics knowledge, work satisfaction, and (...) patient care among ICU nurses.Sample and SettingThe sample, 32 nurses from three ICU settings in an 800-bed tertiary academic medical center, participated in six nursing ethics huddles over a two-month period.MethodsAlvita K. Nathaniel’s Theory of Moral Reckoning guided development of the nursing ethics huddle process.1 The Moral Distress Thermometer was administered at three data points: baseline level of MD, and pre- and post-huddle to determine changes in the subjects’ level of MD.2 Focused content analysis was used to analyze qualitative responses from questionnaires about the subjects’ perception of the effect of the huddles on work satisfaction and patient care. Knowledge attainment was evaluated via open-ended short-answer questions.ResultsOverall, use of nurse-ethicist-led nursing ethics huddles was associated with improved quality of work life, patient care, and clinical ethics knowledge. The change in pre- and post-nursing ethics huddles MD scores was statistically significant (p < 0.0001). (shrink)

The promise of genomic medicine lies in the opportunity to improve health outcomes via a personalized approach to management, grounded in genetic and genomic variation unique to an individual. However, disparities and inequities mar this remarkable landscape of genomic innovation. Prior efforts to understand these inequities have focused on populations for which genetic testing is relatively protocolized or where test utility varies greatly by ancestry groups, where equitable outcomes are more clearly defined. We therefore consider the current landscape of rare (...) disease genomics, in which diagnostic approaches vary widely and utility remains to be fully understood, and suggest a path forward: how ecosocial theory may be used to guide novel equity‐focused initiatives that incorporate illness narratives to improve population health. We present examples of narrative medicine in rare disease and reimagine the role this discipline may play in genomic sequencing studies, toward incorporation of the unique illness narrative into clinical genetics and genomics practice. Approaches that broaden the definitions of disease and of outcomes of interest will force the field to grapple with its racist history and begin to advance health equity and promote justice so that genomic medicine may truly deliver on its promise. ABSTRACT In recent decades poverty has been defined as a relative rather than absolute notion. Those in poverty have been seen as poor relative to a level of income, or social condition, accepted as average or normal for a society. Poverty has been redefined as ‘relative deprivation’. This paper argues, first, that the redefinition of poverty as relative to social norms is a radical departure from the traditional notion of poverty. Secondly, it considers whether such a redefinition gives support to (...) the view that the claims of those in poverty (relative) may be considered as morally equivalent to the claims of those in poverty (absolute). The paper concludes by arguing that the notion of relative poverty is advanced to give support to social and economic policies whose primary aim is equality rather than the relief of poverty. The results in this paper are in a context of abstract elementary classes identified by Shelah and Villaveces in which the amalgamation property is not assumed. The long-term goal is to solve Shelah’s Categoricity Conjecture in this context. Here we tackle a problem of Shelah and Villaveces by proving that in their context, the uniqueness of limit models follows from categoricity under the assumption that the subclass of amalgamation bases is closed under unions of bounded, -increasing chains.

One of the first philosophers to relate Indian philosophical thought to Western analytic philosophy, Jaysankar Lal Shaw has been reflecting on analytic themes from Indian philosophy for over 40 years. This collection of his most important writings, introduces his work and presents new ways of using Indian classical thought to approach and understand Western philosophy. By expanding, reinterpreting and reclassifying concepts and views of Indian philosophers, Shaw applies them to the main issues and theories discussed in contemporary philosophy (...) of language and epistemology. Cet article vise à situer les notions de sujet, de corps et de vie chez Foucault à partir du passage d’un cadre conceptuel basé sur les notions de corps-pouvoir à un centré sur celles de vie-gouvernement. Mon hypothèse est que ce mouvement caractérise le développement de ses recherches entre les années 1970 et les années 1980. Le problème central est la difficulté de penser le statut ontologique des rapports de pouvoir (au sens physique) sans attribuer au corps une sorte de (...) qualité ontologique du même type (comme dans tous les formes de vitalisme), ce qui conduit Foucault à privilégier la thèmatique de la vie à celle du corps dans le développement de sa « question du sujet ».
