Fetal alcohol spectrum disorder is a multifaceted disability, characterized not only by brain- and body-based challenges, but also high rates of environmental adversity, lifelong difficulties with daily living, and distinct sociocultural considerations. FASD is one of the most common neurodevelopmental disabilities in the Western world and associated with significant social and economic costs. It is important to understand the complexities of FASD and the ways in which FASD requires unique consideration in research, practice, and policy. In this article, we discuss (...) our perspectives on factors that distinguish FASD from other disabilities in terms of complexity, co-occurrence, and magnitude. We provide an overview of select literature related to FASD as a socially rooted disability with intergenerational impacts and multiple layers of stigma. These social issues are intertwined with notable experiences of adversity across the lifespan and high rates of co-occurring health concerns for individuals with FASD, all of which present unique challenges for individuals, caregivers, families, service providers, and policy makers. Understanding these factors is the first step in developing and implementing specialized initiatives in support of positive outcomes for individuals with FASD and their families. Future directions are proposed for advancing research, practice, and policy, and responding to the unique complexities of FASD. (shrink)

The pioneering analysis of synchronicity was given by Jung, yet despite the concept's momentous significance in Jung's work, and despite the widespread dissemination of the term 'synchronicity' even within pop culture, synchronicity is often badly misconstrued and remains "perhaps the least understood of Jung's theories". Synchronicity, Science, and Soul-Making has already been hailed as the most important analysis of synchronicity since Jung himself.

It is a common opinion that chance events cannot be understood in causal terms. Conversely, according to a causal view of chance, intersections between independent causal chains originate accidental events, called “coincidences”. Firstly, this book explores this causal conception of chance and tries to shed new light on it. Such a view has been defended by authors like Antoine Augustine Cournot and Jacques Monod. Second, a relevant alternative is provided by those accounts that, instead of acknowledging an intersection among causal (...) lines, claim to track coincidences back to some common cause. Third, starting from Herbert Hart and Anthony Honoré’s view of coincidences (Causation in the Law. Clarendon Press, Oxford, 1959). This book provides a more detailed account of coincidences, according to which coincidental events are hybrids constituted by ontic (physical) components, which is the intersection between independent causal chains, plus epistemic aspects, including but not limited to, access to information, expectations, relevance, significance, desires, which in turn are psychological aspects. -/- The main target of the present work is to show that the epistemic aspects of coincidences are, together with the independence between the intersecting causal chains, a constitutive part of coincidental phenomena. This book aims to introduce and discuss recent work in psychology concerning one’s judgment about coincidences; this data offers further materials and reasons to reflect upon our understanding of coincidences and to refine our hybrid conception. In particular, this paper investigates the role of the epistemic aspects in our understanding of what coincidences are. In fact, although the independence between the causal lines involved plays a crucial role in understanding coincidental events, that condition results to be insufficient to give a satisfactory definition of coincidences. The main target of the present work is to show that the epistemic aspects of coincidences are, together with the independence between the intersecting causal chains, a constitutive part of coincidental phenomena. Many examples are offered throughout this paper to enforce this idea. According to Antoine Augustine Cournot, chance events are the result of the intersection between independent causal chains. This coincidental notion of chance is not a new one, but—as Cournot remarks—it comes from Saint Thomas Aquinas, Boethius, and more probably from Jean de La Placette. Such a conception of chance phenomena seems to be very important, not only because it is closely related to the Principle of Causality, but also since it grounds Cournot’s theory of objective probability. Starting from Martin’s work, (...) the main attempt of this survey is to endorse the idea that Cournot’s coincidental notion of hasard is objective, that is it is ontic and it does not depend—in some sense—on our degree of knowledge. In order to do that, a central role in the discussion will be given to the meaning of the independence between the intersecting causal chains and to Cournot’s conception of causation. (shrink)

Objectives To investigate empirically the motivations for not consenting to DNA biobanking in a Swedish population-based study and to discuss the implications. Design Structured questionnaires and semistructured interviews. Setting A longitudinal epidemiological project (PART) ongoing since 1998 in Stockholm, Sweden. The DNA-collection wave took place during 2006–7. Participants 903 individuals completed the questionnaire (participation rate 36%) and 23 were interviewed. All individuals had participated in both non-genetic waves of the project, but refused to contribute saliva samples during the DNA-collection wave. (...) Main outcome measures Motivations behind refusing to consent to DNA biobanking, with subsequent focus on participants' explanations regarding this unwillingness. Results Public refusal to consent to DNA biobanking, as revealed by the questionnaire, was mainly explained by a lack of personal relevance of DNA contribution and feelings of discomfort related to the DNA being used for purposes other than the respective study. Interviews of individuals representing the second motivation, revealed a significant mistrust of DNA biobank studies. The underlying beliefs and attitudes were associated with concerns about integrity, privacy, suspiciousness and insecurity. However, most interviewees were supportive of genetic research per se and interpreted their mistrust in the light of distressing environmental influences. Conclusion The results suggest a need for guidelines on benefit sharing, as well as trustworthy and stable measures to maintain privacy, as a means for increasing personal relevance and trust among potential participants in genetic research. Measures taken from biobanks seem insufficient in maintaining and increasing trust, suggesting that broader societal measures should be taken. (shrink)

Background Pediatric critical care physician attitudes about withdrawal of ventricular assist devices (VAD) and extracorporeal membrane oxygenation (ECMO) in cases of medical futility are poorly defined. Our aim was to define current attitudes regarding the withdrawal of these devices. Methods IRB-approved, cross-sectional observational survey conducted among pediatric critical care attending physicians and fellow physicians in the United States between 2016 and 2017. Data was collected anonymously and statistically analyzed. Results A total of 158 physicians responded with 67% being attending physicians. (...) Compared to a VAD, a higher percentage had taken care of a patient on ECMO where the device was turned off because care was believed to be futile (99% vs. 84%), including currently (95% vs. 57%). Nearly all reported that it can be ethically permissible to withdraw support from a patient with a VAD and on ECMO (97% vs. 99%), but varied opinions existed as to who should ultimately make this decision if the patient/their family disagrees. More respondents agreed that a patient/their family should agree to withdrawal of VAD or ECMO support prior to initiation if futility is later determined (60% vs. 58%) and that protocols should be created for VAD and ECMO withdrawal (77% vs. 76%). Conclusion Most pediatric critical care physicians felt that it can be ethically permissible to withdraw VAD and ECMO support. Our study indicates that pediatric VAD and ECMO withdrawal protocols are desired, but further investigation is needed to determine how to best design protocols that would incorporate multiple stakeholders. Machiavelli's New Modes and Orders is the only full-length interpretive study on Machiavelli's controversial and ambiguous work, Discourses on Livy. These discourses, considered by some to be Machiavelli's most important work, are thoroughly explained in a chapter-by-chapter commentary by Harvey C. Mansfield, one of the world's foremost interpreters of this remarkable philosopher. Mansfield's aim is to discern Machiavelli's intention in writing the book: he argues that Machiavelli wanted to introduce new modes and orders in political philosophy in order (...) to make himself the founder of modern politics. Mansfield maintains that Machiavelli deliberately concealed part of his intentions so that only the most perceptive reader could see beneath the surface of the text and understand the whole of his book. Previously out of print, Mansfield's penetrating study brings to light the hidden thoughts lurking in the details of the Discourses on Livy to inform and challenge its readers at every step along the way. Selective reporting is prevalent in the medical literature, particularly in industry-sponsored research. In this paper, we expose selective reporting that is not evident without access to internal company documents. The published report of study 329 of paroxetine in adolescents sponsored by GlaxoSmithKline claims that “paroxetine is generally well tolerated and effective for major depression in adolescents”. By contrast, documents obtained during litigation reveal that study 329 was negative for efficacy on all 8 protocol specified outcomes and positive for harm.

Derrida's work on hospitality presents particular local conventions of hospitality as in a necessary but impossible relationship with an absolute hospitality, the obligation to welcome the other without conditions. Although this absolute hospitality is commonly read as the aspiration to which all of our practices of hospitality should tend, Derrida proposes a series of examples that show the dangers implicit in an automatic or limitless welcoming. The most famous of these is that of the Old Testament patriarch, Lot. The aim (...) of this paper is to show, however, that the Genesis story is not primarily a parable about correct and incorrect practices of hospitality. In fact, what is at stake in the visit of the angels to Lot is the covenant between Abraham's line and the divine and the coming into the world of God's absolute sovereign violence. Derrida's account of hospitality is thus part of his discussion of sovereignty, its limitlessness, force and danger. (shrink)

This study applied psychographic segmentation theory to explore the psychological type profile of 1082 visitors to four cathedrals (three in England and one in Wales) and to set this profile alongside the published national normative data. Data provided by the Francis Psychological Type Scales demonstrated that among cathedral visitors there were more introverts (60%), sensing types (72%) and judging types (80%), with a balance between thinking types (49%) and feeling types (51%). Comparisons with the population norms demonstrated that extraverts and (...) perceiving types were significantly underrepresented among visitors to these four cathedrals. The implications of these findings are discussed for enhancing the visitor experience of those currently visiting and for attracting those psychological types currently less likely to visit. CONTRIBUTION: Situated within the science of cathedral studies, this article demonstrates (by means of applying psychographic segmentation theory and gathering data from four cathedrals) that extraverts and perceiving types were significantly under-represented among cathedral visitors. These data are important for understanding limitations on the reach of cathedrals within the wider community. (shrink)

His principal work is the one which establishes that the labor theory of value holds not only in the state of nature, as for Locke, nor only in primitive society, as for Adam Smith, but also in the highest civilized society and, as against Ricardo, in such a way as to prove that (...) the proletariat is and must be exploited but will and can never exploit. With the analysis and reduction of capital into labor, the possibility of freedom in this new meaning, which is merely asserted in the "humanism" of his earlier writings, is proven and secured with the necessity of science. Drug promotion should be evaluated according to its impact on health, access to information, informed consent, and wealth. Drug promotion currently does more harm than good to each of these objectives because it is usually misleading. This is a systemic problem. Whilst improved regulation and education will address it to some degree, major reforms to payment systems for drug companies and doctors are also required. Until all these systemic reforms can be put in place, the best policy option is to (...) ban the promotion of drugs to doctors and the public. Consequently, pending major reforms, it is appropriate for governments to restrict drug promotion as much as is politically achievable. (shrink)

Plimpton 322 is one of the most sophisticated and interesting mathematical objects from antiquity. It is often regarded as teacher’s list of school problems, however new analysis suggests that it relates to a particular geometric problem in contemporary surveying.

People with dementia have commonly been excluded from research. The adverse impacts of this exclusion are now being recognized and research literature, position statements, and ethics guidelines increasingly call for inclusion of people with dementia in research. However, few published studies investigate the views of potential participants on taking part in research should they experience dementia-related cognitive impairment. This cross-sectional survey examined the views of people aged sixty and older attending hospital outpatient clinics about clinical research participation if they had (...) dementia and impaired decision-making ability. Over 90 percent of respondents were agreeable to participating in a wide range of research activities, such as cognitive testing, physical measurements, imaging procedures, and blood draws. For drug studies, however, agreement dropped to 60 percent. Altruism was a strong motivator for research participation. In regard to who should be involved in decisions about their participation in research during periods of incapacity, respondents mostly preferred the person they appoint as their substitute decision-maker for healthcare matters or a doctor or health professional on the research team. Over three-quarters expressed interest in making an advance research directive. The study findings are discussed in relation to law reforms in Australia that aim to strengthen respect and inclusion for people with impaired decision-making capacity, especially by providing frameworks for advance planning for research participation.
