Use of humanities content in American medical education has been debated for well over 60 years. While many respected scholars and medical educators have purported the value of humanities content in medical training, its inclusion remains unstandardized, and the undergraduate medical curriculum continues to be focused on scientific and technical content. Cited barriers to the integration of humanities include time and space in an already overburdened curriculum, and a lack of consensus on the exact content, pedagogy and instruction. Edmund Pellegrino, (...) physician and scholar of the latter twentieth century, spent much of his professional life promoting the value and importance of the humanities in medical education, seeking the best way to incorporate and teach this content in clinically relevant ways. His efforts included the founding of multiple enterprises starting in the 1960s and 1970s to promote human values in medical education, including the Society for Health and Human Values and its Institute on Human Values in Medicine. Regardless of his efforts and those of many others into the current century, the medical humanities remains a curricular orphan, unable to find a lasting home in medical education and training. Gyngell and colleagues consider that the recent Nuffield Council report does not go far enough: heritable genome editing is not just justifiable in a few rare cases; instead, there is a moral imperative to undertake it. We agree that there is a moral argument for this, but in the real world it is mitigated by the fact that it is not usually possible to ensure a better life. We suggest that a moral imperative for HGE can currently only be concluded (...) if one first buys into an overly deterministic view of a genome sequence, and the role of variation within in it, in the aetiology of the disease: most diseases cannot simply be attributed to specific genetic variants that we could edit away. Multiple, poorly understood genetic and environmental factors interact to influence the expression of diseases with a genetic component, even well understood ‘monogenic’ disorders. Population-level genome analyses are now demonstrating that many genetic ’mutations' are much less predictive than previously thought 1. Furthermore, HGE might introduce new risks just as it reduces old ones; or remove protections not yet clearly delineated. (shrink)

Some scientists suggest that it might be possible to reflect a portion of incoming sunlight back into space to reduce climate change and its impacts. Others argue that such solar radiation management (SRM) geoengineering is inherently incompatible with democracy. In this article, we reject this incompatibility argument. First, we counterargue that technologies such as SRM lack innate political characteristics and predetermined social effects, and that democracy need not be deliberative to serve as a standard for governance. We then rebut each (...) of the argument’s core claims, countering that (1) democratic institutions are sufficiently resilient to manage SRM, (2) opting out of governance decisions is not a fundamental democratic right, (3) SRM may not require an undue degree of technocracy, and (4) its implementation may not concentrate power and promote authoritarianism. Although we reject the incompatibility argument, we do not argue that SRM is necessarily, or even likely to be, democratic in practice. (shrink)

How does violence shape citizens’ preferences for conflict termination? The existing literature has argued that violence either begets sympathy for more violence or drives support for making peace. Focusing on the 2016 Colombian Peace Agreement, this article finds that victimhood dissociation strongly shapes these preferences. With victimhood dissociation, a discrepancy exists between objective and subjective victimization, and the effect of violence on peace attitudes depends on citizens’ subjective interpretations of their personal experiences of violence. Citizens who do not experience violence (...) often see themselves as victims of the conflict and vice versa. Victimhood dissociation is linked to cultural stereotypes of victimhood as portrayed in elite narratives concerning the conflict. In Colombia, political leaders framed victimhood around rurality, business entrepreneurship, kidnapping, extortion, and disappearance. In 2016, this widespread narrative was instrumental for politicians opposing the peace agreement. Citizens who disassociated their personal experiences of violence from their self-understanding as victims were instrumental to the failure of the peace plebiscite. (shrink)

Previous studies have shown a robust bias to express the goal path over the source path when describing events. Motivated by linguistic theory, this study manipulated the causal structure of events and measured the extent to which adults and 3.5- to 4-year-old English-speaking children included the goal and source in their descriptions. We found that both children's and adults’ encoding of the source increased for events in which the source caused the motion of the figure compared to nearly identical events (...) in which the source played no such causal role. However, a goal bias persisted overall for both causal and noncausal motion events. These findings suggest that although the goal bias in language is highly robust, properties of the source influence its likelihood of being encoded in language, thus shedding light on how properties of an event can influence the mapping of event components into language. Siminoff (bio), and Renie Schapiro (bio)This issue of the Kennedy Institute of Ethics Journal (KIEJ) centers on a piece of empirical research. The motivation behind the study of Laura Siminoff, Christopher Burant, and Stuart Youngner (2004) was to find out more about what the general public understands and believes about when a person is dead. More specifically, the study tried to (...) determine how members of the public define death, especially their acceptance and interpretation of the medical concept of "brain death," and their attitudes toward the so-called "dead donor rule," an informal but powerful social and legal norm insisting that the taking of organs for transplantation not cause the death of the source of those organs.As the demand for organs increases and the waiting list grows, the United States has explored multiple ways to expand the pool of potential donors. One approach has been to gerrymander (coherently or not) the line between life and death to shift patients who are extremely compromised but traditionally considered to be in the living category into the dead category. In this way, the dead donor rule remains technically intact. This reasoning is explicit in the report of the Harvard Medical School Ad Hoc Committee (1968) that "introduced" "brain death" to the American culture. Since then, other efforts to redraw the line have been proposed and rejected.One alternative to gerrymandering is to consider under what conditions, if any, it is morally acceptable to violate the dead donor rule. Some would argue that this alternative is the more "honest" approach. Rather than sidestepping moral problems by calling people dead, it brings the moral problems to the surface where the necessity of sticking to the rule can be considered on its own merits. However, when the Journal of the American Medical Association printed a position paper that proposed making anencepahlic newborns an exception to the dead donor rule, along with arguments about why this was acceptable, the outcry was so great that the proposal was retracted (AMA 1995).Clearly, our society has identified organ transplantation as a priority, although two papers in this volume (those by Courtney Campbell and by Megan Crowley-Matoka and Robert Arnold) take issue with that prioritization. The fact that the donor pool has remained relatively stable (and inadequate) despite massive efforts [End Page 211] to increase it speaks, at least in part, to the cultural and legal resistance to "new" ways to procure more organs. Voluntarism seems to have peaked. Financial incentives clash with the heavily endorsed notion of the "gift of life" and seem to many to violate the dignity of the human body. Presumed consent or eminent domain, in which the community has a greater claim to a person's organs than that person or his/her family, go against the grain of American individual freedom and the ethos of patient autonomy.Abandoning the dead donor rule generally is regarded as an even tougher sell to the American public and its legislators. The study by Siminoff and colleagues attempts, among other things, to gauge just how tough this sell would be. What the data actually demonstrate is a matter of interpretation, and this issue of the KIEJ is devoted to some of those interpretations. The authors, with the exception of Daniel Hausman, came together for a meeting in Cleveland, Ohio, during which the study's methods and data were presented with time for questions, clarifications, and general discussion. The reporting of clinical trial data is necessary not only for doctors to determine treatment efficacy, but also to explore new questions without unnecessarily repeating trials, and to protect patients and the public from dangers when data are withheld. This issue is particularly salient in those trials involving invasive neurosurgical interventions, such as deep brain stimulation (DBS), for ‘treatment refractory’ psychiatric disorders. Using the federal database ClinicalTrials.gov, it was discovered that out of the completed or unknown‐status trials related to psychiatric (...) DBS up to November 2018, only two had submitted results to ClinicalTrials.gov. These results suggest that, despite federal requirements to report clinical trial data, reporting on psychiatric DBS trials is problematically minimal. It is argued that a human rights approach to this problem establishes a legal and ethical foundation for the need to report clinical trial results in this area. (shrink)

ABSTRACTUsing medical literature citations, Congressional hearings, and declassified documents this paper examines the uses of pharmaceuticals in the interrogation of vulnerable populations. From the use of IV relaxants on criminal suspects during the 1920s to the Global War on Terror, the nexus of drugs, testing, and interrogations will be explored in both the domestic and international contexts.

The studies on Nietzsche and Buddhism in the Nietzsche literature are rather recent. The first English monograph on the subject was Freny Mistry’s Nietzsche and Buddhism: Prolegomenon to a Comparative Study, followed by Robert G. Morrison’s Nietzsche and Buddhism: A Study in Nihilism and Ironic Affinities. While Mistry’s study focuses on the Buddhist and the Nietzschean theories of eternal recurrence, Morrison’s compares Nietzsche’s concepts with Buddhist tenets. Many families worldwide have at least one member with a behavioral or mental disorder, and yet the majority of the public fails to correctly recognize symptoms of mental illness. Previous research has found that Mental Health Literacy (MHL)—the knowledge and positive beliefs about mental disorders—tends to be higher in European and North American cultures, compared to Asian and African cultures. Nonetheless quantitative research examining the variables that explain this cultural difference remains limited. The purpose of our study was fourfold: (a) (...) to validate measures of MHL cross-culturally, (b) to examine the MHL model quantitatively, (c) to investigate cultural differences in the MHL model, and (d) to examine collectivism as a predictor of MHL. We validated measures of MHL in European American and Indian samples. The results lend strong quantitative support to the MHL model. Recognition of symptoms of mental illness was a central variable: greater recognition predicted greater endorsement of social causes of mental illness and endorsement of professional help-seeking as well as lesser endorsement of lay help-seeking. The MHL model also showed an overwhelming cultural difference; namely, lay help-seeking beliefs played a central role in the Indian sample, and a negligible role in the European American sample. Further, collectivism was positively associated with causal beliefs of mental illness in the European American sample, and with lay help-seeking beliefs in the Indian sample. These findings demonstrate the importance of understanding cultural differences in beliefs about mental illness, particularly in relation to help-seeking beliefs. (shrink)

En el caso venezolano, tales principios se encuentran expresamente plasmados en nuestra Carta Magna y son: El principio de competencia administrativa, el principio de legalidad, el principio de separación y colaboración entre los poderes y el principio de responsabilidad (...) del Estado y sus funcionarios. La intención es definir y desarrollar brevemente estos principios, a fin de poder establecer las características del Estado Venezolano, desde el punto de vista Constitucional (positivo) y desde el punto de vista real. In Venezuela’s case such principles are explicitly presented in our Constitution. They are: the principle of administrative competence, the principle of legality, the principle of separation and cooperation between powers and the principle of responsibility of the State and its officials. The aim is to define and briefly unfold these principles, so as to establish the characteristics of the Venezuelan State, both from the Constitutional (positive) and reality standpoints. Le droit culturel international a été renforcé grâce à l'adoption par l'Unesco de la Convention sur la protection et la promotion de la diversité des expressions culturelles. Ce texte relie la culture à la coopération internationale, au développement et aux droits de l'homme. Sa mise en oeuvre devient un véritable défi, et son suivi se révèle une nécessité. La diversité culturelle, déjà objet d'étude de l'anthropologie, doit à présent devenir un concept politique et être mis en pratique. La culture doit (...) être envisagée comme vecteur de la vie politique, économique et sociale, tant au niveau national qu'au niveau des rapports entre les différentes aires culturelles. Réfléchir à la problématique de l'Autre, gérer l'acculturation et la cohabitation culturelle devient une question politique, et la gestion des rapports entre culture, communication, société et politique requiert la construction de théories spécifiques et adaptées.The international cultural right has been strengthened with the adoption by the Unesco Convention on the Protection and Promotion of the Diversity of Cultural Expressions. This text connects culture international cooperation, development and human rights. Its implementation is a challenge, and its follow-up reveals a necessity. Cultural diversity, already an object of study of anthropology, has now become a political concept and be put into practice. Culture should be considered as a vector of political, economic and social, both nationally and relationships between different cultural areas. Consider the problem of the Other, managing acculturation and cultural cohabitation becomes a political issue, and the management of the relationship between culture and communication, society and politics requires the construction of specific theories and adapted. In the „Cena Trimalchionis“ Encolpius is continually faced with Trimalchio’s oddities (res novae, sat. 27, 2). The paper aims to highlight the occurrence of a ‚scheme of the res novae‘, a narrative pattern that repeats itself during the „Cena“ and in which the only variation is constituted by Encolpius’ reactions to Trimalchio’s jokes: while the host is never tired of testing his guests’ intelligence with (mostly gastronomic) enigmas, Encolpius moves from amazement to skepticism to the desire of competing with Trimalchio (...) in foreseeing his plans. Encolpius’ suppositions mainly link Trimalchio’s stratagems to the world of the theatre and of the Saturnalia: following his footsteps modern critics often used the same references to explain Trimalchio’s eccentric ideas. The present essay will show that the point of view of the narrator, Encolpius, is not to be confused with the strategy of Trimalchio, who takes over the imaginary of the theatre and of the Saturnalia only to overthrow them in search of a totally new and ‚not foreseeable‘ transgression. In the Port-Royal Logic, Arnauld and Nicole argue that eloquence plays a crucial role in the cultivation of the art of thinking. In this essay, we demonstrate that Arnauld and Nicole's reflections on eloquence exemplify the need to reconceive the larger framework in which Cartesian theories of ideas operate. Instead of understanding epistemic agents as solitary thinkers who pursue their intellectual goals without the influence of others, our analysis shows that for Arnauld and Nicole thinking well was an intersubjective discursive (...) activity that unfolds between complexly organized persons. Central to this activity is the ability to gauge the affective and associative tendencies of interlocutors and to communicate accordingly. This ability is required to enable speakers to deal constructively with problems arising from the context sensitivity of language, the influences of the passions, and the audience's capacity to decipher meaning in the communication of ideas that facilitate understanding and knowledge. By drawing attention to communication, affect, and association in the Port-Royal Logic, we show that there is a significant connection between thinking and expressing oneself well in the early modern period. This paper reports on the author’s experiences as manager of a capacity-building project in Latin America. The project aimed to strengthen planning, monitoring, and evaluation (PM&E) in agricultural research. Nine lessons are drawn: (1) Project design is much more than a technical process; it is essentially one of negotiation. (2) In capacity-building projects, design activities cannot end when implementation begins. (3) Capacity-building efforts should prepare managers to deal with complexity, uncertainty and change. (4) In capacity-building efforts, it is essential to (...) collaborate rather than patronize. (5) Organizational assessment is a complex social process, intertwined with organizational politics. (6) In designing capacity-building projects, it is essential to involve managers and staff members in assessing needs and opportunities. (7) Action-learning strategies offer great potential for capacity building. (8) In the context of strategic management and organizational learning, PM&E take on new meanings. (9) Training is most effective when it is designed to serve a purpose within an organizational change process. It is concluded that capacity building is more a process of social experimentation than of social engineering. Management systems cannot be imported, but need to be developed within organizations. Development agencies should play catalytic, facilitating roles, rather than take responsibility for organizational change. To support genuine capacity development, donors and funding agencies need to ensure that their planning and accountability procedures foster flexibility, innovation, and learning. (shrink)

Before September 11, 2001, a mass-casualty terrorist attack on American soil was generally considered a remote possibility. Similarly, before October 4, 2001—the first confirmed case of anthrax caused by intentional release — widespread bioterrorism seemed implausible. Among the arguments that such a biological artack was unlikely included: the lack of a historical precedent; the technological and organizational challenges to acquiring and weaponizing a biological agent; and the almost universal moral opprobrium that would certainly accompany the use by terrorists of such (...) a weapon. In the wake of September 11th and October 4th, however, many are reconsidering the likelihood of a large-scale bioterrorist attack against civilians.The Centers for Disease Control and Prevention defines bioterrorism as the intentional release of viruses, bacteria, or toxins for the purpose of harming or killing civilian. One measurement of the public health system's level of bioterrorism preparedness is the quality and distribution of laws mandating the reporting of diseases caused by certain biological agents.
