BackgroundIn gender-affirming medical care (GAMC), ethical challenges in decision-making are ubiquitous. These challenges are becoming more pressing due to exponentially increasing referrals, politico-legal contestation, and divergent normative views regarding decisional roles and models. Little is known, however, about what ethical challenges related to decision-making healthcare professionals (HCPs) themselves face in their daily work in GAMC and how these relate to, for example, the subjective nature of Gender Incongruence (GI), the multidisciplinary character of GAMC and the role HCPs play in assessing (...) GI and eligibility for interventions. Given the relevance and urgency of these questions, we conducted a qualitative study among HCPs providing GAMC to transgender adults in the Netherlands.MethodsIn this qualitative research, we conducted 11 semi-structured interviews between May 2020 and February 2021 with HCPs (six mental health professionals, two HCPs in endocrinology, two in plastic surgery, and one in nursing) working in two distinct GAMC settings. We purposively sampled for professional background and years of experience in GAMC. We analyzed our interview data using thematic analysis. As some respondents were more inclined to speak about what should or ought to be done to arrive at good or right decision-making, we identified both ethical challenges and norms. Furthermore, in our analysis, we differentiated between respondents’ explicit and implicit ethical challenges and norms and ascertained the specific context in which these challenges emerged.ResultsRespondents’ ethical challenges and norms centered on (1) dividing and defining decisional roles and bounds, (2) negotiating decision-making in a (multidisciplinary) team, and (3) navigating various decision-making temporalities. These themes arose in the context of uncertainties regarding (1) GAMC’s guidelines, evidence, and outcomes, as well as (2) the boundaries and assessment of GI.ConclusionsThis interview study provides detailed empirical insight into both the explicit and implicit ethical challenges that HCPs experience and their ethical norms regarding decision-making. It also describes how uncertainties and (implicit) normativities concerning GAMC and GI pre-structure the moral environment in which these challenges and norms manifest. We provide normative reflections and recommendations on handling these ethical challenges in a way that is sensitive to the context in which they arise. (shrink)

This paper argues that a guaranteed income is not only consistent with the principle of reciprocity but is required for reciprocity. This conclusion follows from a three-part argument. First, if a guaranteed income is in place, all individuals have the same opportunity to live without working. Therefore, those who choose not to work do not take advantage of a privilege that is unavailable to everyone else. Second, in the absence of an unconditional income, society is, in effect, applying the principle, (...) “(S)he who does not work, will not eat.” If the application of this principle is to be consistent with reciprocity, it must be applied to everyone. Most modern industrial societies exempt many citizens from that choice. For example, the owners of external assets do not face the work-or-starve choice and do take advantage of a privilege that is not available to others. An unconditional guaranteed income is one way to eliminate that violation of reciprocity. Third, this paper addresses the criticism that the guaranteed income exploits middle-class workers by demonstrating that a basic income will have a positive effect on wages, which will at least partially counteract the effect of the taxes needed to pay for it. This paper is a very early and very preliminary report of some of the findings from the research project, "Prehistoric Myths in Modern Political Philosophy." The project eventually lead to two books: "Prehistoric Myths in Modern Political Philosophy" and "the Prehistory of Private Property" (both coauthored by Grant S. McCall). The basic argument of the project is that influential, modern political theories often rely on dubious claims about prehistory. It examines the political philosophy literature to show how these claims are (...) used as essential premises in influential arguments. It then examines evidence from anthropology, archaeology, and history to show that these claims are dubious. This paper previews some of the early findings of the project that became those books. After reviewing the papers in this special issue, I must conclude that brains are not syntactic engines, but control systems that orient to biological, interindividual, and cultural norms. By themselves, syntactic constraints both underdetermine and overdetermine cognitive operations. So, rather than serving as the basis for general cognition, they are just another kind of empirically acquired constraint. In humans, symbols emerge from a particular sensorimotor activity through a process of contextual broadening that depends on the coordination of conscious and nonconscious (...) processing. This process provides the representational freedom and stability that constitute the human brain’s solution to the frame problem and symbol grounding problem. Symbol formation and grounding is an ongoing process of generalising constraints from particular contexts, selectively enlisting their use, and re-automating them. This process is central to the self-creation of a language-using person with beliefs, agency, and identity. (shrink)

This programmatic statement of a lecture series presented by intellectuals close to Georg Lukács in 1917 concerns the paradoxical stress, influentially expounded by Georg Simmel, between the subjective culture of the self and its growing alienation from the overwhelming mass of the objectified culture that surrounds it. The theme is the reflection and analytic grasp of that division and of the insights it yields into the nature of culture.

Many studies have revealed the top-down modulation on unconscious processing. However, there is little research about how category-selective attention could modulate the unconscious processing. In the present study, using functional magnetic resonance imaging , the results showed that category-selective attention modulated unconscious face/tool processing in the middle occipital gyrus . Interestingly, MOG effects were of opposed direction for face and tool processes. During unconscious face processing, activation in MOG decreased under the face-selective attention compared with tool-selective attention. This result was (...) in line with the predictive coding theory. During unconscious tool processing, however, activation in MOG increased under the tool-selective attention compared with face-selective attention. The different effects might be ascribed to an interaction between top-down category-selective processes and bottom-up processes in the partial awareness level as proposed by Kouider, De Gardelle, Sackur, and Dupoux . Can desires and actions be evaluated as responsive or unresponsive to reasons, in ways that extend beyond the instrumental implications of one's (other) desires? And does there exist any form of inference or reasoning that is practical in nature? Hume is generally supposed to have given an unambiguously negative reply to both of these questions. In particular, he is often taken to have held that no desire, passion, or action may ever be said to be opposed to reasons, except (perhaps) (...) in so far as it is based on a false belief or confused piece of means-ends reasoning. Joseph Addison’s (1672--1719) essays in The Spectator occupy contradictory positions in the history of aesthetics. While they are generally considered central to the institution of aesthetics as a scholarly discipline, their reception has throughout history entailed a strong questioning of their philosophical and scholarly importance. In the following paper, I consider this dual feature as regards reception, and set out to clarify how this has come about. A re-examination of the arguments advanced by Addison makes clear that his role is (...) not that of a philosopher, but that of a public educator. As such he aims to raise the standard of general education of the British ‘middling orders’ in the early eighteenth century, and by using art for didactic purposes he seeks to contribute to the shaping of morally accomplished individuals.
