This article offers a critical analysis of John Broome's conception of practical reasoning. I first introduce his main claims and then point out some of the difficulties raised by the notion of “double expression” and by some aspects of the cognitivism which he explicitly endorses. I then emphasize two consequences of these criticisms: one concerning the link he sees between belief and intention, and the other concerning the idea that our practical reasonings are inextricably linked to our theoretical reasonings. I argue that the problem Broome seems to be facing has its source in the difficulty of distinguishing clearly between reasoning (a relation between mental states) and the description of its content (a relation between propositions). Feminist scholars have long demonstrated how women are constrained through dieting discourse. Today’s scholars wrestle with similar themes, but confront a thornier question: how do we make sense of a food discourse that frames food choices through a lens of empowerment and health, rather than vanity and restriction? This article addresses this question, drawing from interviews and focus groups with women (N = 100), as well as health-focused food writing. These data allow us to document a postfeminist food discourse that (...) we term the do-diet. The do-diet reframes dietary restrictions as positive choices, while maintaining an emphasis on body discipline, expert knowledge, and self-control. Our analysis demonstrates how the do-diet remediates a tension at the heart of neoliberal consumer culture: namely, the tension between embodying discipline through dietary control and expressing freedom through consumer choice. With respect to theory, our analysis demonstrates how the embodied dimensions of neoliberalism find gendered expression through postfeminism. We conclude that the do-diet heightens the challenge of developing feminist critiques of gendered body ideals and corporeal surveillance, as it promises a way of eating that is both morally responsible and personally empowering. The concept of scale is useful in analyzing both the strengths and limitations of community food security programs that attempt to link issues of ecological sustainability with social justice. One scalar issue that is particularly important but under-theorized is the scale of social reproduction, which is often neglected in production-focused studies of globalization. FoodShare Toronto's good food box (GFB) program, engages people in the politics of their everyday lives, empowering them to make connections between consumption patterns and broader political-economic, cultural, (...) and political-ecological issues. Community food security (CFS) projects such as the GFB are currently limited in their scope and reach and have been criticized for their inability to deliver food to a larger segment of marginalized, hungry people. A central dilemma for CFS projects is how to engage the majority of urban consumers who still eat “inside the box” of the industrial food system. We argue that the concept of scale helps clarify how CFS projects must “scale out” to other localities, as well as “scale up” to address structural concerns like state capacity, industrial agriculture, and unequal distribution of wealth. This requires the state and the third sector to recognize the importance of multi-scaled food politics as well as a long-term pedagogical project promoting ecological sustainability, social responsibility, and the pleasures of eating locally. This paper focuses on the centrality of permaculture design courses as the principal sociopolitical strategy of the permaculture community in Canada to transform local food production practices. Building on the work of Antonio Gramsci and political agroecology as a framework of analysis, we argue that permaculture instruction remains deeply embedded within market and colonial relations, which orients the pedagogy of permaculture trainings in such a way as to reproduce the basic elements of the colonial capitalist economy among its practitioners. In (...) the specific case of eastern Ontario, this embeddedness had the effect of diluting the transformative capacity of permaculture practitioners who were unable to create its own social movement organization. The paper then highlights key elements of the agroecological pedagogy used by the Brazilian Landless Rural Workers Movement and the Escola Latinoamericana de Agroecología in Paraná, Brazil. The objective is to draw lessons from these inspiring experiences, in a rather unique context of struggles that can help to critically assess the pedagogical practices and principles presently informing permaculture communities in Canada and in advanced industrialized countries more generally. We then conclude by reiterating the key arguments and lessons drawn from the Brazilian pedagogical experiences, pointing out the importance of engagement and coalition-building with established rural and urban movements, as well as progressive farmer, Indigenous, and rural associations to foster a just and sustainable transformation of agri-food systems, starting at the local and regional levels. In rehabilitation, there is a growing interest in ethics. That said, few meta-ethical reflections have been conducted to date. Therefore, a review and critical analysis of the use of the concept of “ethical issue” is warranted. To this end, a systematic and critical review of the literature discussing ethical issues in rehabilitation was conducted. This review, based on the method developed by McCullough and colleagues, identified and analyzed 80 articles. Several characteristics and gaps in the conceptualization of the ethical issue (...) concept were noted. Three main findings emerge from the study: 1) the corpus of texts studied is dominated by descriptive ethics; 2) ethical conceptualisation is marginal, in that ethical concepts are generally stated without being defined; and 3) the corpus is characterized by the use of amalgams and by the multiplication of imprecise ethical concepts that are often used as synonyms while referring to distinct phenomena. These conceptual gaps create conceptual confusion and imprecision that can undermine ethics research, teaching and clinical ethics, and discredit the discipline of ethics. (shrink)

Knowledge is a presumed motivator for changed consumption practices in ethical eating discourse: the consumer learns more about where their food comes from and makes different consumption choices. Despite intuitive appeal, scholars are beginning to illuminate the limits of knowledge-focused praxis for ethical eating. In this paper, we draw from qualitative interviews and focus groups with Toronto mothers to explore the role of knowledge in conceptions of ethical foodwork. While the goal of educating children about their food has become central (...) to Canadian and American discourses of “good” mothering, we identify a paradoxical maternal expectation surrounding meat consumption: to raise informed child consumers who know where their food comes from, and to protect children from the harsh realities of animal slaughter. Rather than revealing the story behind the meat on a child’s plate, mothers seek to shield children from knowledge of meat production. Our analysis of the child consumer contributes to ethical eating scholarship and illuminates a larger paradox surrounding knowledge of meat in an industrialized food system. In the practice of feeding children, mothers confront the visceral discomforts of meat consumption; their reactions speak to discordant feelings involved with eating meat in a setting far-removed from the lives and deaths of animals. Ultimately, the paper illustrates the limits of consumer-focused strategies for food-system change that call on individual mothers to educate young consumers and protect childhood innocence, all while getting ethically-sourced meals on the table. J’examine d’abord quelle fonction il reconnaît aux notions d’implication et d’inconsistance, et montre ensuite que si le processus dans lequel s’engage (...) celui qui raisonne peut être conçu à la manière d’Harman, c’est-à-dire à la 3e personne, il doit aussi être conçu du point de vue de l’agent qui raisonne, c’est-à-dire à la 1re personne. Après avoir discuté des limites inhérentes au fait de concevoir l’aspect processuel du raisonnement d’un point de vue essentiellement externe, à la 3e personne, j’identifie quelques conséquences que fait apparaître la perspective de la 1re personne, dans le but de montrer pourquoi une théorie adéquate du raisonnement ne peut en faire l’économie.Assuming that an adequate account of reasoning should be able to explain how the processual aspect of a piece of reasoning relates to its inferential aspect, this article seeks to determine how Gilbert Harman’s conceives of this processual aspect, and whether his theory allows for a correct explanation of its relation to the inferential aspect. I consider first which role he attributes to the notions of implication and consistency, and then argue that even though the process in which a reasoning subject engages can be thought of in the way suggested by Harman, i.e., from the 3rd person perspective, it must also be conceived from the 1st person perspective, from the point of view of the reasoning subject. I discuss some of the unavoidable limitations of conceiving the processual aspect of reasoning from such an essentially external point of view, and draw attention to some of the advantages of adopting the 1st person perspective, with the aim of showing why no adequate theory of reasoning could completely dispense with it. Ethical consumer discourse is organized around the idea that shopping, and particularly food shopping, is a way to create progressive social change. A key component of this discourse is the “citizen-consumer” hybrid, found in both activist and academic writing on ethical consumption. The hybrid concept implies a social practice – “voting with your dollar” – that can satisfy competing ideologies of consumerism (an idea rooted in individual self-interest) and citizenship (an ideal rooted in collective responsibility to a social and ecological (...) commons). While a hopeful sign, this hybrid concept needs to be theoretically unpacked, and empirically explored. This article has two purposes. First, it is a theory-building project that unpacks the citizen-consumer concept, and investigates underlying ideological tensions and contradictions. The second purpose of the paper is to relate theory to an empirical case-study of the citizen-consumer in practice. Using the case-study of Whole Foods Market (WFM), a corporation frequently touted as an ethical market actor, I ask: (1) how does WFM frame the citizen-consumer hybrid, and (2) what ideological tensions between consumer and citizen ideals are present in the framing? Are both ideals coexisting and balanced in the citizen-consumer hybrid, or is this construct used to disguise underlying ideological inconsistencies? Rather than meeting the requirements of consumerism and citizenship equally, the case of WFM suggests that the citizen-consumer hybrid provides superficial attention to citizenship goals in order to serve three consumerist interests better: consumer choice, status distinction, and ecological cornucopianism. I argue that a true “citizen-consumer” hybrid is not only difficult to achieve, but may be internally inconsistent in a growth-oriented corporate setting. (shrink)

In the context of academic research, a diversity of ethical issues, conditioned by the different roles of members within these institutions, arise. Previous studies on this topic addressed mainly the perceptions of researchers. However, to our knowledge, no studies have explored the transversal ethical issues from a wider spectrum, including other members of academic institutions as the research ethics board (REB) members, and the research ethics experts. The present study used a descriptive phenomenological approach to document the ethical issues experienced (...) by a heterogeneous group of Canadian researchers, REB members, and research ethics experts. Data collection involved socio-demographic questionnaires and individual semi-structured interviews. Following the triangulation of different perspectives (researchers, REB members and ethics experts), emerging ethical issues were synthesized in ten units of meaning: (1) research integrity, (2) conflicts of interest, (3) respect for research participants, (4) lack of supervision and power imbalances, (5) individualism and performance, (6) inadequate ethical guidance, (7) social injustices, (8) distributive injustices, (9) epistemic injustices, and (10) ethical distress. This study highlighted several problematic elements that can support the identification of future solutions to resolve transversal ethical issues in research that affect the heterogeneous members of the academic community. This article draws on interviews with “foodies”—people with a passion for eating and learning about food—to explore questions of gender and foodie culture. The analysis suggests that while this culture is by no means gender-neutral, foodies are enacting gender in ways that warrant closer inspection. This article puts forward new empirical findings about gender and food and employs the concept of “doing gender” to explore how masculinities and femininities are negotiated in foodie culture. Our focus on doing gender generates two (...) insights into gender and food work. First, we find that doing gender has different implications for men and women within foodie culture. Alongside evidence that foodies are contesting particular gendered relations within the food world, we explore how broader gender inequities persist. Second, we contend that opportunities for doing gender in foodie culture cannot be considered apart from class privilege. (shrink)

Background There has been significant discussion about the need to manage conflict of interest (COI) in medical journals. This has lead many journals to implement policies to manage COI for authors and reviewers; however, surprisingly little attention has been focused on the COI of journal editors. Objective The goal of this exploratory study was to determine whether the policies were accessible to the public and to researchers, and to discuss the potential impact on public transparency. Design The authors conducted an (...) internet search of editor COI policy instruments that have been developed, implemented and communicated by the top 10 peer-reviewed medical journals (2010 ISI Web of Knowledge Impact Factor), and assessed their general accessibility by gauging the level of difficulty in navigating the journal's website (number of clicks to find the policy instruments). Results Only four of the 10 medical journals (40%) in this study have accessible COI policy directives that include editors (JIM, PLoS Medicine, AIM, CMAJ). One journal (NEJM) had an editorial on the subject, and another (The Lancet) mentioned editor COI in their general guidelines. These documents are not readily accessible; starting from the journal's main website at least four clicks are needed to access these documents. Conclusion These results suggest that there is a general lack of accessible editor COI policy instruments among leading medical journals, something that may consequently have a negative impact on the trust accorded to these journals. En plus de présenter la genèse de cette vision occupationnelle de la justice climatique, la définition et l’explication de cette valeur sont mises en contraste avec d’autres concepts apparentés en ergothérapie et dans la science de l’occupation.

The aim of our article is to identify what, in the thinking of the libertarian and egalitarian philosopher Ruwen Ogien, enables us to unmask and interrogate the latent paternalism that still persists in the practices of healthcare professionals. Although recent advances in models of care have made more room for the voice of those being cared for and for their free self-determination, they have not put an end to paternalism in healthcare. We present here the various key points of Ogien’s (...) argument aimed at criticising paternalism in healthcare, in order to be aware of the latent paternalism and recognize that it gives rise to epistemic and social injustices that must be reversed. In the current literature, the impacts of the transformation undergone by the Quebec health and social services network (RSSS) in 2015 deal mainly with issues of accessibility, efficiency and performance. The impacts of this reform on the place of ethics in the RSSS are to date little documented. In this article, we are interested in the place occupied by clinical and organizational ethics in the institutions created by this reform, namely the integrated health and social services centres (CISSS) and the (...) integrated university health and social services centres (CIUSSS). How is ethics regulated? In a self-regulatory manner, that is to say based on rational reflection based on values? Or is it rather in a hetero-regulatory manner, based on obedience to norms dictated by an external authority? The aim of our study was to gain a better understanding of how clinical and organizational ethics are regulated in the CISSS and CIUSSS of the RSSS. To this end, an analysis of ethics-related documents developed by these establishment and made available to the public on their websites was carried out, using the conceptual framework that guided this study. In doing so, we were able to observe the predominance of hetero-regulatory regulation of ethics. Thus, although ethics seems to have a greater place than in the past within RSSS establishments, the regulation of clinical and organizational ethics remains largely and predominantly hetero-regulatory. There has been widespread academic and popular debate about the transformative potential of consumption choices, particularly food shopping. While popular food media is optimistic about “shopping for change,” food scholars are more critical, drawing attention to fetishist approaches to “local” or “organic,” and suggesting the need for reflexive engagement with food politics. We argue that reflexivity is central to understanding the potential and limitations of consumer-focused food politics, but argue that this concept is often relatively unspecified. The first objective of (...) this paper is to operationalize reflexivity and advance understanding of reflexivity as an important tool for understanding the lived experience of food shopping. Our second objective is to explore the range of reflexivity observed in a mainstream “shopping for change” market sector. To do this, we draw from in-depth interviews with shoppers at Whole Foods Market (WFM)—a retail venue with the stated goal of making consumers “feel good about where [they] shop.” This group is chosen because of our interest in investigating the reflexivity of consumer engagement with the corporatized arm of ethical consumption—a realm of concern to food scholars as alternative agricultural initiatives are absorbed (both materially and symbolically) into corporate institutions. Our analysis suggests that shopping at venues like WFM is primarily motivated by traditional consumer pleasures, even for politicized consumers, a finding that poses serious limitations for a consumer-regulated food system. The article aims to take a stand in the debates surrounding the potential contribution of the theoreticians of the first generation of the Frankfurt School to postcolonial/decolonial theory, by showing that Herbert Marcuse, in his work, has outlined coloniality as later authors have defined it. Marcuse denounced the neocolonialism and neoimperialism of which the Global South populations were prey at the time of decolonizations. He showed that the welfare state and the affluent society in contemporary Western societies largely fed themselves (...) on the continued economic exploitation of the Global South. Marcuse decried the uneven development bred by the capitalist system and criticized the ideology and mechanisms of development imposed on the newly independent countries, which also violated the subjectivity of their peoples, ignoring their values and aspirations. He condemned the racial-based violence and war imposed on the ‘underprivileged’ in the Global South and the Global North alike. These trains of thought describe different aspects of coloniality. This article will analyze these insights through a close reading of Marcuse’s work and comparisons with the writings of Frantz Fanon, Kwame Nkrumah and Samir Amin. (shrink)

ABSTRACT: The concept of the “banality of evil,” put forward by Hannah Arendt to describe the psychological profile of the Nazi criminal in Eichmann in Jerusalem, is intimately tied to her reading of Plato. In Arendt’s examination of the question of evil, she found some support in Kant’s philosophy. However, the problem of guilt under Nazism ultimately goes back to an inability to think. The two-in-one, a concept which describes the activity of thinking, is based on Plato’s dialogues. An examination (...) of these philosophical sources contributes to the understanding of the possibilities and the limits of Arendt’s reflection on evil. RÉSUMÉ : Le concept de banalité du mal, proposé par Hannah Arendt pour décrire le profil psychologique du criminel nazi dans Eichmann in Jerusalem, est intimement lié à ses lectures de Platon. Pour aborder la question du mal, elle a découvert des appuis chez Kant, mais le problème de la culpabilité sous le régime nazi renvoie ultimement au défaut de penser. Le deux-en-un, concept qui décrit la pensée, a été défini sur la base de l’oeuvre platonicienne. L’examen de ces sources philosophiques permet de mieux comprendre les possibilités et les limites de la réflexion arendtienne sur le mal. (shrink)

This article presents results of a critical review of the literature discussing the ethical issues arising in humanitarian work, following the method proposed by McCullough, Coverdale and Chervenak. Our aim was primarily to focus on how the ethical issues arising in humanitarian work are conceptualized within the literature we reviewed. We think that properly conceptualizing the ethical issues which humanitarian workers may face can provide avenues to better respond to them. We analysed 61 documents, as part of a literature review, (...) which revealed that there truly is a need, amongst the authors and in humanitarian work, to discuss ethics. Indeed, even if only a small number of authors define explicitly the words they use to discuss ethics, the great quantity that we have uncovered in the documents seem to suggest vast and rich grounds upon which to address ethical issues. We believe it to be important that the ethical issues of humanitarian work are increasingly addressed in the literature and argue that it would be helpful for the vocabulary used by authors to be employed and developed even more rigorously, so that their discussions show more precision, coherence, relevance, exhaustiveness, and sufficiency. The review of the literature, as well as the resulting analysis in this article, is part of a broader project to suggest a way to conceptualize the ethical issues of humanitarian work based on the strengths and innovations of this and other studies. This study aims to address how (through which mechanisms) and when (under which conditions) retailers’ sustainability efforts translate into positive consumer responses. Hypotheses are developed and tested through a scenario-based experiment among 672 consumers. Retailers’ assortment sustainability and distribution sustainability are manipulated. Retailers’ sustainability efforts lead to positive consumer responses (e.g., improved store evaluations) via two underlying mechanisms: consumers’ identification with the store (personal route) and store legitimacy (social route). The effects of sustainability efforts are strengthened if consumers have personal (...) norms favoring shopping at environmentally friendly stores. Remarkably, when controlling for moderation by personal norms, social norms weaken the effects. The findings show that traditional marketing mix elements provide opportunities for retailers to improve their organizations’ bottom line and positively affect consumer (and societal) well-being. This study helps retailers decide whether or not to invest in and communicate about sustainability. Past research has shown the clear potential for positive consumer responses to firms’ sustainability efforts, but little is known about the underlying mechanisms and the conditions under which such responses take place. This study advances theory by examining personal and social factors as mediators and moderators of the retailers’ sustainability efforts–consumer responses relationship. (shrink)

At a time when Quebec’s public health system is facing various challenges in a post-pandemic context, it is time to find solutions to lighten the load on the system and on the people working in it. In order to further reflection and identify solutions to promote sustainable health, the first Sustainable Health Summit was organized by the Association de la santé publique du Québec (ASPQ). It took place from January 25 to 27, 2023, at the Musée de la civilisation de (...) Québec. Cet article questionne la pertinence de soutenir la valeur qu’est l’écoresponsabilité et, plus largement, la justice occupationnelle intergénérationnelle dans le contexte de la pratique clinique de l’ergothérapeute. Au moment d’adopter des pratiques professionnelles respectueuses de ces valeurs, l’ergothérapeute peut être amené à vivre un dilemme éthique opposant celles-ci à l’approche centrée sur le client qui est grandement valorisée au sein de la profession. Cet article développe une réflexion éthique sur un des résultats d’une recherche qui a été menée sur les (...) pratiques durables ou écoresponsables en ergothérapie. Plus précisément, nous développons une réflexion éthique sur la légitimité pour l’ergothérapeute de défendre les valeurs que sont l’écoresponsabilité et la justice occupationnelle intergénérationnelle dans un contexte clinique. Pour ce faire, le Cadre éthique quadripartite (CÉQ) – un cadre d’analyse éthique qui a été développé pour soutenir l’ergothérapeute dans ses réflexions éthiques – est mobilisé. Sans apporter une réponse claire ni définitive quant à la pertinence éthique de défendre ces valeurs en clinique, la réflexion ici développée met en lumière des éléments qui pourraient être considérés par l’ergothérapeute qui vit ce dilemme éthique. The ethical issues of health research are well documented. Ethical issues in occupational therapy research are beginning to attract the interest of researchers. However, no research has documented the ethical issues experienced by occupational therapists conducting research in academic settings in Quebec. This is revealed by the literature review that was the basis for this qualitative research, the results of which are presented here. This article also presents the results of a qualitative research conducted with eleven women occupational therapy researchers (...) in Quebec. A phenomenologically inspired qualitative approach was used. Semi-structured interviews were conducted to discuss the ethical issues experienced in occupational therapy research. Three units of meaning emerge from the narrative data, namely that ethical issues are related to: 1) the research environment; 2) people and their interactions; or 3) the occupation that is research. Overall, the results of the study and the literature review agree that health researchers, including occupational therapy researchers, operate in an environment that is not conducive to scientific discovery and responsible conduct of research. Ethical issues in research are primarily systemic in nature, contributing to the creation of unhealthy work environments that not only harm interpersonal relationships, but also adversely affect researchers’ occupational health and well-being and negatively affect researchers’ personal and family lives. Four new findings emerge from the study: 1) the academic world values individualism; 2) occupational therapists are rebellious towards research ethics boards; 3) clinical and academic environments are too disconnected; and 4) quantitative approaches can contribute to the exclusion of vulnerable populations. While the Battle of Seattle immortalized a certain image of anti-globalization resistance, processes and agents of contestation remain sociologically underdeveloped. Even with the time-space compression afforded by new information technologies, how can a global civil society emerge among multi-cultured, multi-tongued peoples divided by miles of space and oceans of inequality? This article examines two cases that confronted the U.S. model of global corporate rule: the defeat of the Multilateral Agreement on Investment (MAI), and the Zapatista challenge to the North American (...) Free Trade Agreement (NAFTA). Evaluating cross-border solidarity in these cases encourages critical evaluation of claims about global civil society, the role of the Internet, and the eclipse of traditional politics in a supposedly post-national age. Contrary to orthodox globalization narratives, our analysis suggests that states, nations, and nationalisms remain key elements in contestation processes, at least in the kinds of cases examined. At the same time, transnational networks played an important role in bypassing unfavorable political opportunity structures at the domestic level, and nurtured incipient processes of framing resistance to neo-liberal globalism across national boundaries. In the past, several researchers in the field of physiotherapy have asserted that physiotherapy clinicians rarely use ethical knowledge to solve ethical issues raised by their practice. Does this assertion still hold true? Do the theoretical frameworks used by researchers and clinicians allow them to analyze thoroughly the ethical issues they encounter in their everyday practice? In our quest for answers, we conducted a literature review and analyzed the ethical theoretical frameworks used by physiotherapy researchers and clinicians to discuss the (...) ethical issues raised by private physiotherapy practice. Our final analysis corpus consisted of thirty-nine texts. Our main finding is that researchers and clinicians in physiotherapy rarely use ethical knowledge to analyze the ethical issues raised in their practice and that gaps exist in the theoretical frameworks currently used to analyze these issues. Consequently, we developed, for ethical analysis, a four-part prism which we have called the Quadripartite Ethical Tool (QET). This tool can be incorporated into existing theoretical frameworks to enable professionals to integrate ethical knowledge into their ethical analyses. The innovative particularity of the QET is that it encompasses three ethical theories (utilitarism, deontologism, and virtue ethics) and axiological ontology (professional values) and also draws on both deductive and inductive approaches. It is our hope that this new tool will help researchers and clinicians integrate ethical knowledge into their analysis of ethical issues and contribute to fostering ethical analyses that are grounded in relevant philosophical and axiological foundations. Research is an occupation that is conducive to the emergence of ethical issues, particularly because it is linked to conflicts of interest that are sometimes difficult to manage and is carried out in a highly competitive context that values performance. While the ethical issues in the clinical practice of occupational therapy are beginning to be documented, the ethical issues experienced by occupational therapy researchers and the means they use to manage these issues are virtually undocumented. This article presents the results (...) of a qualitative study that documented the means (macro, meso and micro) that are proposed by occupational therapist-researchers to address ethical issues in research. Eleven occupational therapists (n=11) participated in the research and share the means they are using or consider to deal with the ethical issues they experience as researchers. The means proposed by occupational therapists echo many of the means suggested in the literature, although some of those proposed in the literature regarding the relationship to be established with research participants are little discussed by our participants, which is surprising given the humanistic values that underlie the profession of occupational therapy. In the end, this article, which presents a synthesis of both the means identified in the literature to resolve ethical issues in research and those discussed by participants, aims to empower occupational therapist-researchers to resolve the issues raised by academic research while remaining true to the values of the profession. The passage from a posture of clinician to that of clinician–bioethicist poses significant challenges for health professionals, most notably with regards to theoretical or epistemological views of complex ethical impasses encountered in clinical settings. Apprehending these situations from the only clinical perspective of the nurse or the doctor, for example, can be very unproductive to help solve this kind of situation and certainly poses great limits to the role of the clinician–bioethicist. Drawing on my own experience as a former nurse (...) who, following graduate studies in bioethics has begun providing ethics consultation services, I argue that clinicians must undergo an epistemological transformation in order to become clinician–bioethicists. A source of inspiration or framework for would-be clinician–bioethicists is, I suggest, the Petite éthique developed by the contemporary French philosopher Paul Ricoeur. Specifically, clinician–bioethicists should develop specific core ethical competencies (in line with the conclusions of the American Society for Bioethics and Humanities (Core competencies for health care ethics consultation, 1998); namely: savoir or knowing, savoir faire or knowing how to do, and savoir être or knowing how to be. Il permet ainsi d’initier les ergothérapeutes à une perspective de bioéthique globale susceptible de les soutenir dans l’actualisation d’une approche écosystémique respectueuse de l’environnement.

This article presents the second part of the results of a study our team conducted on the ethical issues in occupational therapy education and their solutions. While the first article in 2020 highlighted these issues, this article focuses on ways to address them. The occupational therapist who teaches occupational therapy students is sooner or later confronted with ethical issues. However, these issues are not well documented in the literature, nor are the solutions used or considered by occupational therapists educators. In (...) this qualitative study, eleven occupational therapist educators from four francophone universities in Quebec who are preparing the next generation of occupational therapists shared their success stories. Ten units of meaning related to these solutions emerge from the results, which have been grouped according to Glaser’s three domains of ethics (micro, meso and macro). The four micro-environmental solutions include: a) developing ethical skills; b) peer support; c) developing task-related skills; and d) self-care. The four meso-environmental solutions are: a) creating spaces for discussion; b) providing ethics training; c) doing meso advocacy; and d) changing the academic culture. The two macro-environmental solutions are: a) macro-level advocacy; and b) working in partnership with clinical settings. Although some of the solutions documented in the literature were not discussed by the participants, those discussed by the participants were consistent with the solutions discussed in the literature. The results indicate that ethics has a limited place in the preparation and continuing education of occupational therapy educators and that the organizational context, i.e., the culture of academia in general and research in particular, is likely to create ethical issues of concern. This article discusses the process of restructuring experience during the first year of teaching in elementary school. Inspired by interactionism, the study espouses a theory of experiential learning and adapts the principles of continuity and transaction to the field of teaching. The methodology relies on mentoring to serve both development support and research in accordance with a collaborative approach. The results of a grounded theory analysis are illustrated starting from an «experiential frame» rebuilt with one of the participants transacting the (...) experience through three transactional ways of understanding. An examination of this restructuration process suggests a dual identity development where both an operative and a unified self come into play. (shrink)

BackgroundThe overwhelming scarcity of organs within renal transplantation forces researchers and transplantation teams to seek new ways to increase efficacy. One of the possibilities is the use of personalized medicine, an approach based on quantifiable and scientific factors that determine the global immunological risk of rejection for each patient. Although this approach can improve the efficacy of transplantations, it also poses a number of ethical questions.MethodsThe qualitative research involved 22 semi-structured interviews with nephrologists involved in renal transplantation, with the goal (...) of determining the professionals' views about calculating the global immunological risk and the attendant ethical issues.ResultsThe results demonstrate a general acceptance of this approach amongst the participants in the study. Knowledge of each patient's immunological risk could improve treatment and the post-graft follow-up. On the other hand, the possibility that patients might be excluded from transplantation poses a significant ethical issue. This approach is not seen as something entirely new, given the fact that medicine is increasingly scientific and evidence-based. Although renal transplantation incorporates scientific data, these physicians believe that there should always be a place for clinical judgment and the physician-patient relationship.ConclusionsThe participants see the benefits of including the calculation of the global immunological risk within transplantation. Such data, being more precise and rigorous, could be of help in their clinical work. However, in spite of the use of such scientific data, a place must be retained for the clinical judgment that allows a physician to make decisions based on medical data, professional expertise and knowledge of the patient. To act in the best interests of the patient is key to whether the calculation of the global immunological risk is employed. (shrink)

In order to support the professional development of high school preservice teachers during their initial university training as reflective practitioners an innovative educational design was placed in a classroom management course. With this design, inspired by the principles of the learning community (Brown, 1994), has assisted students to develop a collective discourse on their professional practice through their own issues in the secondary classroom management and their concerns during a five weeks internship. We make the assumption that the participation in (...) the development of a collective discourse on professional practice allows students to not only reflect on their practice, but also to gain significant professional learning. Thus, our study focuses on student engagement in the development of collective discourse and actions enabling them to advance it. (shrink)

On January 30 and 31, 2024, the second edition of the Sustainable Health Summit, organized by the Quebec Public Health Association, took place simultaneously in Quebec City and Montreal. The aim of this text is to describe the event and take an ethical look, highlighting the Summit’s strengths and points to consider for a possible third edition.

Launched in 2007, the American Medical Students Association PharmFree Scorecard is an annual ranking of conflict of interest policies at American medical centres; it focuses on COIs that may occur when medical education seems likely to be influenced by university-industry relationships, especially those with the pharmaceutical and medical device industries. The PharmFree Scorecard has proven influential in stimulating changes in policy regarding the management of COI at American medical institutions, thus it provides a useful jumping off point for reflection on (...) how and why medical education institutions in other countries – and for our purposes, Canada – should pay more attention to the appropriate identification and management of COI. The PharmFree Scorecard methodology examines a diversity of factors and interests that could influence medical education; as such, it is an interesting approach to analysing the COI policies of medical schools. To test its utility or applicability outside the US, we decided to apply the PharmFree Scorecard to the COI policies of the 16 Canadian universities hosting medical schools. Overall, Canadian institutions rank very poorly, especially in ensuring that education and training tools are provided to staff, students and faculty members to enable the identification and management of COI. However, differences between the US and Canadian medical education contexts, e.g., with regards to the governance and funding of universities, limit to some extent the direct applicability of the AMSA ranking. Canadian medical schools – and their host universities – nonetheless have much to learn from insights provided by the AMSA PharmFree Scorecard ranking, although they can and should go further in developing their own COI policies and procedures.
