BackgroundCommunity engagement within biomedical research is broadly defined as a collaborative relationship between a research team and a group of individuals targeted for research. A Community Advisory Board is one mechanism of engaging the community. Within genomics research CABs may be particularly relevant due to the potential implications of research findings drawn from individual participants on the larger communities they represent. Within such research, CABs seek to meet instrumental goals such as protecting research participants and their community from research-related risks, (...) as well as intrinsic goals such as promoting the respect of participants and their community. However, successful community engagement depends on the degree to which CABs legitimately represent and engage with communities targeted for research. Currently, there is little literature describing the use of CABs in genomics research taking place in developing countries, and even less in the field of genomics research relating to mental illness. The aim of this article is to describe and consider the contributions made by a researcher-driven, population-specific CAB in a genomics of schizophrenia research project taking place in South Africa, from the perspective of the research team.DiscussionFour broad discussion topics emerged during the CAB meetings namely: 1) informed consent procedures, 2) recruitment strategies, 3) patient illness beliefs and stigma experiences, and 4) specific ethical concerns relating to the project. The authors consider these discussions in terms of their contributions to instrumental and intrinsic goals of community engagement.SummaryThe CAB gave valuable input on the consent processes and materials, recruitment strategies and suggested ways of minimizing the potential for stigma and discrimination. All of these contributions were of an instrumental nature, and helped improve the way in which the research took place. In addition, and perhaps more importantly, the CAB made a unique and important contribution relating to intrinsic functions such as promoting the respect and dignity of research participants and their community. This was particularly evident in ensuring sensitivity and respect of the community’s traditional beliefs about schizophrenia and its treatment, and in this way promoting a respectful relationship between the research team and the participants. Cell line immortalisation is a growing component of African genomics research and biobanking. However, little is known about the factors influencing consent to cell line creation and immortalisation in African research settings. We contribute to addressing this gap by exploring three questions in a sample of Xhosa participants recruited for a South African psychiatric genomics study: First, what proportion of participants consented to cell line storage? Second, what were predictors of this consent? Third, what questions were raised by participants during (...) this consent process? 760 Xhose people with schizophrenia and 760 controls were matched to sex, age, level of education and recruitment region. We used descriptive statistics to determine the proportion of participants who consented to cell line creation and immortalization. Logistic regression methods were used to examine the predictors of consent. Reflections from study recruiters were elicited and discussed to identify key questions raised by participants about consent. Approximately 40% of participants consented to cell line storage. The recruiter who sought consent was a strong predictor of participant’s consent. Participants recruited from the South African Eastern Cape, and older participants, were more likely to consent; both these groups were more likely to hold traditional Xhosa values. Neither illness nor education were significant predictors of consent. Key questions raised by participants included two broad themes: clarification of what cell immortalisation means, and issues around individual and community benefit. These findings provide guidance on the proportion of participants likely to consent to cell line immortalisation in genomics research in Africa, and reinforce the important and influential role that study recruiters play during seeking of this consent. Our results reinforce the cultural and contextual factors underpinning consent choices, particularly around sharing and reciprocity. Finally, these results provide support for the growing literature challenging the stigmatizing perception that people with severe mental illness are overly vulnerable as a target group for heath research and specifically genomics studies. BackgroundMulti-centre studies generally cost more than single-centre studies because of larger sample sizes and the need for multiple ethical approvals. Multi-centre studies include clinical trials, clinical quality registries, observational studies and implementation studies. We examined the costs of two large Australian multi-centre studies in obtaining ethical and site-specific approvals.MethodsWe collected data on staff time spent on approvals and expressed the overall cost as a percent of the total budget.ResultsThe total costs of gaining approval were 38 % of the budget for (...) a study of 50 centres (mean cost AUD $6960 per site) and 2 % for a study of 11 centres (mean cost AUD $2300 per site). Seventy-five and 90 % of time was spent on repeated tasks, respectively, and many time-consuming tasks, such as reformatting documents, did nothing to improve the study design or participant safety.ConclusionsImprovements have been made to the ethical approval application system, but more gains could be made without increasing risks of harm to research participants. We propose that ethical review bodies and individual sites publish statistics on how long they take to process approvals which could then be nationally benchmarked. (shrink)

Prefrontal cortex is dysregulated in women with restricting anorexia nervosa . It is not known whether appetitive non-conscious stimuli bias cognitive responses in those with RAN. Thirteen women with RAN and 20 healthy controls completed a dorsolateral PFC working memory task and an anterior cingulate cortex conflict task, while masked subliminal food, aversive and neutral images were presented. During the DLPFC task, accuracy was higher in the RAN compared to the HC group, but superior performance was compromised when subliminal food (...) stimuli were presented: errors positively correlated with self-reported trait anxiety in the RAN group. These effects were not observed in the ACC task. Appetitive activation is intact and anxiogenic in women with RAN, and non-consciously interacts with working memory processes associated with the DLPFC. This interaction mechanism may underlie cognitive inhibition of appetitive processes that are anxiety inducing, in people with AN. (shrink)

Drug Safety Communications are used by the Food and Drug Administration to inform health care providers, patients, caregivers, and the general public about safety issues related to FDA-approved drugs. To assess patient knowledge of the messaging contained in DSCs related to the sleep aids zolpidem and eszopiclone, we conducted a large, cross-sectional patient survey of 1,982 commercially insured patients selected by stratified random sampling from the Optum Research Database who had filled at least two prescriptions for either zolpidem or eszopiclone (...) between July 1, 2012 and June 30, 2013. Among the 594 respondents, two-thirds reported hearing generally about drug safety information prior to starting a new drug, with the remaining one-third “rarely” or “never” hearing such information. Providers and pharmacists were primary sources of drug safety information. Two-thirds of zolpidem users and half of eszopiclone users reported having heard about the related DSC messages, ability to accurately identify the major factual messages was limited. Respondents reacted to new drug safety information about their sleep aids by reporting that they would want to learn about alternative ways to help them sleep and seek out more information about the safety of their specific sleeping pill. Opportunities may exist for the FDA to work with providers and pharmacies to help ensure the DSC information is more widely received and is more fully understood by those taking the affected medications. As explained in the Preface, this book connects two sets of goals, one historical and the other social. The historical aim is to "recover a fuller understanding" of the American intellectual past, and the social aims concern the "complexities of building a better future." The chief thesis is that "these two sets of goals should be connected." Among others, gratitude is expressed for the work of John J. McDermott.

The greatest philosopher of the twentieth century may not have been Wittgenstein, or Russell, or Quine (and he certainly wasn’t Heidegger), but he may have been a somewhat obscure and conservative Australian named David Stove (1927-94). If he wasn’t the greatest philosopher of the century, Stove was certainly the funniest and most dazzling defender of common sense to be numbered among the ranks of last century’s thinkers, better even—by far—than G. E. Moore and J. L. Austin. The twentieth century was (...) not a period in which philosophers distinguished themselves as essayists, or even as capable of writing interestingly on any subject outside their speciality (or even within it). Stove, though, was an essayist, polemicist, and wit of the highest order, rather like a super-intelligent H. L. Mencken. A heavyweight admirer was once led to write that “Reading Stove is like watching Fred Astaire dance. Nursing Inquiry 2012; 19: 116–127 [Epub ahead of print]Finding middle ground: negotiating university and tribal community interests in community‐based participatory researchCommunity‐based participatory research (CBPR) has been hailed as an alternative approach to one‐sided research endeavors that have traditionally been conducted on communities as opposed to with them. Although CBPR engenders numerous relationship strengths, through its emphasis on co‐sharing, mutual benefit, and community capacity building, it is often challenging as (...) well. In this article, we describe some of the challenges of implementing CBPR in a research project designed to prevent cardiovascular disease among an indigenous community in the Pacific Northwest of the United States and how we addressed them. Specifically, we highlight the process of collaboratively constructing a Research Protocol/Data Sharing Agreement and qualitative interview guide that addressed the concerns of both university and tribal community constituents. Establishing these two items was a process of negotiation that required: (i) balancing of individual, occupational, research, and community interests; (ii) definition of terminology (e.g., ownership of data); and (iii) extensive consideration of how to best protect research participants. Finding middle ground in CBPR requires research partners to examine and articulate their own assumptions and expectations, and nurture a relationship based on compromise to effectively meet the needs of each group. This essay deals with the problem of the status of colours, traditionally considered as the paradigmatic case of secondary qualities: do colours exist only as aspects of experience or are they real properties of objects, existing independently of human and animal perception? Recently, John Campbell has argued in favour of the simple view of colours, according to which colours are real properties of objects. I discuss the place of Campbell's position in a debated which was started by John (...) Mackie and continued by John McDowell, and defend it from a criticism due to Michael Smith. I conclude that the simple view is a philosophically credible position. Subsequently, I consider an alleged contradiction between the simple view and semantic externalism pointed out by Jim Edwards. I suggest that a supporter of the simple view may consistently maintain semantic externalism, if she also accepts epistemological externalism about the canonical warrant of perceptual judgements. Edward Hackett (bio)In the following article, I aim to discuss three separate linkages in William James’s overall philosophy of religion. James’s philosophy of religion is based thoroughly on his radical empiricism, and this is the uniting thread often missed in contemporary scholarship. Radical empiricism makes it possible to link 1) his criticism of both representational metaphysics and theology (...) and that philosophy through James must take to heart the lack of access both representative metaphysics and theology conventionally claimed, and 2) the affective ground on which both philosophy and religion have operated for James, especially in his “Will to Believe” argument, and 3) how understanding the affective ground informs a thoroughly empiricist philosophy of religion that moves through his treatment of religious themes (e.g. his mysticism in The Varieties of Religious Experience) and how this affective ground emerges in the body and action.In showing the accuracy of James’s affective grounds of religion, many reading this essay will note that I am not directing my comments in any particular spiritual path. Instead, my Jamesian commitments are about the individual experience of religion, and for James, he affirmed the widest possible conception of religion to remain neutral to the power such traditions have. In The Varieties of Religious Experience, he writes, “Religion, whatever it is, is man’s total reaction upon life.” To get at and underneath these total reactions is not to regard these reactions as causal. Instead, to discover them, James writes, “you must go behind the foreground of existence and reach down to that curious sense of the whole residual cosmos.”1 This residualness is the affective dimension of experience and its unfolding relations that do not regularly become thematized in philosophy of religion. By contrast, many versions of philosophy of religion attempt to justify the epistemic beliefs about religion through theoretical reason. By contrast, our total reactions make us feel the marrow of our existence because the sense of the world’s presence causes us to feel existence. These religious feeling acts come to form our most completest [End Page 67] answer to the question: “What is the character of the universe in which we dwell?”2 In feeling existence, we experience an ongoing relation with particulars in the world. These feeling acts consist in a whole range of possible religious experiences. By looking at the character of the universe in which we dwell, James is asking: What is the place of feeling that comes to characterize the shared character of the universe in which we all dwell? Reason always comes last. Feeling is always constitutive of all later and built-up cognition.My essay is about the Jamesian-experiential-scaffolding surrounding the religious traditions as ways to encounter the meaning of suffering and uncertainty in these ongoing felt relations. Following the example of James Campbell, I am not committed to drawing a distinction between psychological and metaphysical interpretations of affectivity. Suffering and uncertainty are ways we encounter the feeling of existence. Feeling of existence is, then, akin to a nearly phenomenological-like ontology in which the psychological that inspired the metaphysical are not entirely separate.3 The experiential-scaffolding is regarded as the very same starting place for every single religious person. In this essay and what I hope to make clear in James is that philosophy starts in the affective ground where existence is felt and philosophized about rather than assuming the whole of reality is complete, determined, and somehow reflected in our contemplation of it.4 In essence, the lack of access to reality in-itself means that our beliefs are risky and must start in the marrow where existence is felt. [End Page 68] We risk everything in our spiritual practices. As James reminds, “On pragmatic principles we cannot reject any [religious] hypothesis if consequences useful to life flow from it.”5 That is what makes them so necessary on an existential and practical level. Our passional nature (what James calls the feeling sense of religious feeling in The Varieties of Religious Experience), thus, fuels what we need, and philosophical reflection turns into proposing concepts and beliefs... Although much has been written about the vigorous debates over science and religion in the Victorian era, little attention has been paid to their continuing importance in early twentieth-century Britain. Reconciling Science and Religion provides a comprehensive survey of the interplay between British science and religion from the late nineteenth century to World War II. Peter J. Bowler argues that unlike the United States, where a strong fundamentalist opposition to evolutionism developed in the 1920s (most famously expressed in the Scopes (...) "monkey trial" of 1925), in Britain there was a concerted effort to reconcile science and religion. Intellectually conservative scientists championed the reconciliation and were supported by liberal theologians in the Free Churches and the Church of England, especially the Anglican "Modernists." Popular writers such as Julian Huxley and George Bernard Shaw sought to create a non-Christian religion similar in some respects to the Modernist position. Younger scientists and secularists—including Rationalists such as H. G. Wells and the Marxists—tended to oppose these efforts, as did conservative Christians, who saw the liberal position as a betrayal of the true spirit of their religion. With the increased social tensions of the 1930s, as the churches moved toward a neo-orthodoxy unfriendly to natural theology and biologists adopted the "Modern Synthesis" of genetics and evolutionary theory, the proposed reconciliation fell apart. Because the tensions between science and religion—and efforts at reconciling the two—are still very much with us today, Bowler's book will be important for everyone interested in these issues. This article presents evidence supporting the claim that ethical reasoning is a skill that can be taught and assessed. We propose a working definition of ethical reasoning as 1) the ability to identify, analyze, and weigh moral aspects of a particular situation, and 2) to make decisions that are informed and warranted by the moral investigation. The evidence consists of a description of an ethical reasoning education program—Ethical Reasoning in Action —designed to increase ethical reasoning skills in a variety of (...) situations and areas of life. ERiA is housed at a public, major comprehensive U.S. university—James Madison University—and assessment of the program focuses on interventions delivered prior to and during orientation for incoming first-year students. Findings indicate that the interventions measurably enhance the ability of undergraduate students to reason ethically. ERiA’s competency-targeted program and positive student learning outcomes offers a promising model for higher education ethics programs seeking to connect classroom learning in ethics to decision-making in everyday life. In some sense, survival seems to be an intrinsic matter. Whether or not you survive some event seems to depend on what goes on with you yourself —what happens in the environment shouldn’t make a difference. Likewise, being a person at a time seems intrinsic. The principle that survival seems intrinsic is one factor which makes personal fission puzzles so awkward. Fission scenarios present cases where if survival is an intrinsic matter, it appears that an individual could survive twice over. (...) But it’s well known that standard notions of “intrinsicality” won’t do to articulate the sense in which survival is intrinsic, since ‘personhood’ appears to be a maximal property. We formulate a sense in which survival and personhood (and perhaps other maximal properties) may be almost intrinsic—a sense that would suffice, for example, to ground fission arguments. It turns out that this notion of almost-intrinsicality allows us to formulate a new version of the problem of the many. (shrink)

The texts collected in this volume, which was originally published in 1969, contain Herder's most original and stimulating ideas on politics, history and language. They had for the most part not been previously available in English. In his introduction, Professor Barnard analyses the basic premises of Herder's political thought against the background of the Enlightenment. He examines Herder's concepts of language, community and culture, his theory of historical interaction, and his approach to the problem of change and progress. Finally, he (...) provides a brief comparative analysis of traditionalist thought following the French Revolution, showing how substantive writers like Burke differed from Herder despite the close similarity of political vocabulary. The reception history of the French Revolution in France and England is well documented among Anglophone scholars; however, the debate over the Revolution in Germany is much less well known. Fichte's Contribution played an important role in this debate. Presented here for the first time in English, Fichte's work provides a distinctive synthesis of Locke's "possessive individualism," Rousseau's general will, and Kant's moral philosophy. This eclectic blend results in an unusual rights theory that at times veers close to a form (...) of anarchism. Written in 1792–93, just before Fichte moved to Jena to develop his philosophical system in a series of works—above all the Wissenschaftslehre of 1794—the Contribution provides invaluable insight into Fichte's early development. In addition, Fichte's work predates much of Kant's political philosophy, and can shed light on the rich dialogue in German political thought in the 1790s. (shrink)

J.G.A. Pocock has been a dominant force in the history of political thought since his first major work, The Ancient Constitution and the Feudal Law, was published in 1957. This article is focused on the contribution he has made to the study of the revolutions of seventeenth-century England and the extraordinary body of political discourse to which they gave rise. It begins with an examination of the ways in which ideas about continuity, innovation, institutions and historiography have shaped his approach (...) to the history of political thought and their application to seventeenth-century conditions. Central to a fundamental continuity in his ideas over the last five decades have been notions about the interface of 'paradigms' with both language and socio-political circumstance in the construction and deconstruction of both historiographies and political theories.
