Se presenta un estudio cuyo propósito es analizar la pertenencia, tendencia y desafíos de los estudios generales en currículos de programas académicos de Universidades peruanas. El estudio, realizado según el diseño fenomenológico, estuvo conformada por docentes universitarios conocedores del currículo elegidos hasta lograr la saturación. Se aplicó un cuestionario de preguntas abiertas enfocado en la apreciación de los informantes en el objeto de la investigación y como resultados se encontró que los estudios generales contribuyen: al desarrollo de habilidades blandas en (...) la formación integral del estudiante, a la identificación de fortalezas e intereses sobre la profesión, a la vida ciudadana, al descubrimiento y la reafirmación de la vocación; de ahí su pertinencia curricular. Este trabajo tiene como objetivo caracterizar el proceso de evaluación del aprendizaje del contenido estadístico en la carrera de Medicina. Los resultados obtenidos evidencian que el nivel de asimilación alcanzado por los estudiantes en muchas ocasiones es el reproductivo, lo que refleja la necesidad de seguir profundizando en los estudios de esta problemática. Issues related to the learning evaluation have been present throughout the teaching learning process history. Results demonstrate that the assimilation level reached by students, on many occasions, is the reproductive one, as a need to deepen on methodology of the design of the system of evaluation of the statistical content learning in medicine. This document describes the technique and technology currently used to apply cryosurgery, a technic based on low temperatures for medical purposes that allows the controlled destruction of pathological cells and tissues. Cryosurgery is an effective and innocuous process that is painless, safe and economical, applied to multiple specialities in which it provides alternatives to its traditional methods. A search was carried out for articles and bibliographic reviews referring to Cryosurgery and from whose information this technical note was obtained. Research on (...) the application of Cryosurgery presents solid evidence of its benefits. Therefore, it has been used in several countries and health campaigns, showing excellent results, so it is advisable to implement it in health systems. (shrink)

BackgroundCarefully planned research is critical to developing policies and interventions that counter physical, psychological and social challenges faced by young people living with HIV/aids, without increasing burdens. Such studies, however, must navigate a ‘vulnerability paradox’, since including potentially vulnerable groups also risks unintentionally worsening their situation. Through embedded social science research, linked to a cohort study involving Adolescents Living with HIV/aids (ALH) in Kenya, we develop an account of researchers’ responsibilities towards young people, incorporating concepts of vulnerability, resilience, and agency (...) as ‘interacting layers’.MethodsUsing a qualitative, iterative approach across three linked data collection phases including interviews, group discussions, observations and a participatory workshop, we explored stakeholders’ perspectives on vulnerability and resilience of young people living with HIV/aids, in relation to home and community, school, health care and health research participation. A total of 62 policy, provider, research, and community-based stakeholders were involved, including 27 ALH participating in a longitudinal cohort study. Data analysis drew on a Framework Analysis approach; ethical analysis adapts Luna’s layered account of vulnerability.ResultsALH experienced forms of vulnerability and resilience in their daily lives in which socioeconomic context, institutional policies, organisational systems and interpersonal relations were key, interrelated influences. Anticipated and experienced forms of stigma and discrimination in schools, health clinics and communities were linked to actions undermining ART adherence, worsening physical and mental health, and poor educational outcomes, indicating cascading forms of vulnerability, resulting in worsened vulnerabilities. Positive inputs within and across sectors could build resilience, improve outcomes, and support positive research experiences.ConclusionsThe most serious forms of vulnerability faced by ALH in the cohort study were related to structural, inter-sectoral influences, unrelated to study participation and underscored by constraints to their agency. Vulnerabilities, including cascading forms, were potentially responsive to policy-based and interpersonal actions. Stakeholder engagement supported cohort design and implementation, building privacy, stakeholder understanding, interpersonal relations and ancillary care policies. Structural forms of vulnerability underscore researchers’ responsibilities to work within multi-sectoral partnerships to plan and implement studies involving ALH, share findings in a timely way and contribute to policies addressing known causes of vulnerabilities. Background Respect is essential to providing high quality healthcare, particularly for groups that are historically marginalized and stigmatized. While ethical principles taught to health professionals focus on patient autonomy as the object of respect for persons, limited studies explore patients’ views of respect. The purpose of this study was to explore the perspectives of a multiculturally diverse group of low-income women living with HIV (WLH) regarding their experience of respect from their medical physicians. Methods We analyzed 57 semi-structured interviews conducted (...) at HIV case management sites in South Florida as part of a larger qualitative study that explored practices facilitating retention and adherence in care. Women were eligible to participate if they identified as African American (n = 28), Hispanic/Latina (n = 22), or Haitian (n = 7). They were asked to describe instances when they were treated with respect by their medical physicians. Interviews were conducted by a fluent research interviewer in either English, Spanish, or Haitian Creole, depending on participant’s language preference. Transcripts were translated, back-translated and reviewed in entirety for any statements or comments about “respect.” After independent coding by 3 investigators, we used a consensual thematic analysis approach to determine themes. Results Results from this study grouped into two overarching classifications: respect manifested in physicians’ orientation towards the patient (i.e., interpersonal behaviors in interactions) and respect in medical professionalism (i.e., clinic procedures and practices). Four main themes emerged regarding respect in provider’s orientation towards the patient: being treated as a person, treated as an equal, treated without blame or prejudice, and treated with concern/emotional support. Two main themes emerged regarding respect as evidenced in medical professionalism: physician availability and considerations of privacy. Conclusions Findings suggest a more robust conception of what ‘respect for persons’ entails in medical ethics for a diverse group of low-income women living with HIV. Findings have implications for broadening areas of focus of future bioethics education, training, and research to include components of interpersonal relationship development, communication, and clinic procedures. We suggest these areas of training may increase respectful medical care experiences and potentially serve to influence persistent and known social and structural determinants of health through provider interactions and health care delivery. Background: Ethical competence in nursing practice helps clinical nurses to think critically, analyse issues, make ethical decisions, solve ethical problems and behave ethically in their daily work. Thus, ethical competence contributes to the promotion of high-quality care. However, studies on ethical competence in Malawi are scanty. Objectives: The aim of this study was to explore ethical competence among clinical nurses in selected hospitals in Malawi. Methodology: A cross-sectional survey was conducted in four selected hospitals in Malawi with a sample of (...) 271 clinical nurses. Data were collected using self-administered questionnaires, which included a Moral Competence Scale for Home Care Nurses. Descriptive statistics and logistic regression were computed for the dataset using STATA version 12.0. Ethical consideration: The study protocol complied with all ethical requirements and was approved by the College of Medicine Research Ethics Committee under the University of Malawi. Results: The clinical nurses in Malawi are ethically competent. However, there is a significantly high number (p 0.05). Three determinants of high ethical competence level (strong will, judgement skills and recognition of discrepancy of intention) were identified through a reduced model after stepwise logistic regression analysis. Furthermore, results show that indicators of ethical competence include caring, confidentiality and observance of nurses dressing code. The study has also confirmed that the Moral Competence Scale for Home Care Nurses is a reliable tool to assess ethical competence in low-resource settings. Conclusion: The majority of nurses who completed the survey had low ethical competence. However, clinical nurses with high ethical competence level are required to competently manage complex ethical challenges in health facilities. Strategies for enhancing ethical competence such as continuing ethics education, establishment of ethics committees and provision of supportive supervision are recommended to enable nurses in Malawi attain a high level of ethical competence. Recent ideological positioning on the world stage has born a startling resemblance to a form of positioning within nursing theory – that of taking complex ideas, reducing them to a simplistic binary form, and uncritically adopting one half of that form. In some cases, this adoption of a binary position has led to a passionately held form of ‘othering’ that prohibits a healthy and critical engagement with ideas. As alluring as settling for the binary form may be – we argue (...) for holding binaries in tension as a catalyst for stimulating dialogue – reasoning and exploration of new ways of wrestling with the social and moral complexity of nursing. Esta investigación está contextualizada en torno a la influencia que ejercen las tecnologías de la información y de la comunicación, en la educación informal. En concreto, nuestro objetivo es reflexionar sobre la televisión. Para ello, partimos de un análisis teórico, que nos permite entender cómo ha cambiado el significado de algunos conceptos claves, como educación y comunicación, en la postmodernidad globalizada. Con los datos derivados de diversas investigaciones, determinamos cuál es el impacto de la televisión en los niños y adolescentes, (...) valoramos sus efectos educacionales y su influencia en diversos aspectos de la persona, fundamentalmente en su conducta. No obstante los resultados negativos que reflejan estos trabajos, debemos tener en cuenta los efectos positivos de la televisión, que resultan menos subrayados y que pueden ser más abundantes si son cabalmente planeados, desde el ámbito educativo. Finalmente, esta investigación nos permite constatar que hay una asignatura pendiente: dar un nuevo impulso a nuestro desarrollo humano y ético, con ayuda de las tecnologías. Partiendo de la importancia de la “escuela paralela”, creemos que es posible elaborar un nuevo modelo educativo integral, que responda a los retos que nos plantea la sociedad moderna. (shrink)

This response to the articles in this issue of ‘Analyse & Kritik’ begins with some general remarks on the ‘Singer-Affair’ in which I suggest that while the rational discussion of the ethical issue of euthanasia poses no threat of a return to Nazism, there is a real danger in the creation of a climate in which people are ready to use force to suppress ideas with which they disagree. I then state and criticise two popular theses about t he wrongness (...) of killing: that there is a crucial moral distinction between an act and an omission, and that all human beings possess an intrinsic right to life that no nonhuman beings have. This serves as a background to the section that follows, in which I take up the detailed criticisms of my views made by Professors Lenzen, Birnbacher, J.C. Wolf and Hoerster. (shrink)

This study explores how the integration of environmental practices in gastronomic tourism can increase the competitiveness of tourist destinations in Latin America. Through the analysis of cases in Mexico, Peru and Colombia, sustainable strategies are identified, such as the use of local products and the reduction of waste, which not only contribute to sustainable tourism development, but also improve the image and attractiveness of destinations. The results show that destinations that implement sustainable environmental practices are perceived as more attractive and (...) competitive in a tourism market that is increasingly aware of ecological impacts. Higher education, a key driver of women’s empowerment, is still segregated by gender across the world. Agricultural higher education is a field that is male-dominated, even though internationally women play a large role in agricultural production. The purpose of this study was to understand the experience, including challenges and coping strategies, of women from 10 Latin American countries attending an agricultural university in Latin America. The participants were 28 women students with a mean age of 20.9 ± 1.8 years. Following (...) informed consent and assurance of confidentiality, four focus group sessions (one for each year of study with a mean duration of 81 min) were conducted in Spanish. The central question was, “what has been your experience at the university?” Sessions were recorded and transcribed. Thematic coding was performed independently by two teams of researchers (from Latin America and North America), with the resulting schemas combined through mutual discussion. Member checking, auditing, and reflexivity contributed to trustworthiness of the process. Students reported that the personal qualities needed for success included determination, persistence, and self-efficacy. Many described an empowerment process, including increased discipline and self-efficacy from the first to fourth year of study. University life encompassed six themes: university structure and discipline (part of the exosystem), two supportive microsystems (friends and classmates and institutional support) as well as three challenges (academics, peers, and machismo). Cultural influences instantiated in students’ daily experiences included familism, machismo, and religious faith. Students anticipated futures involving further education and contributions to society. We conclude that higher education in agriculture can serve as an effective means of empowering women to feed the world. (shrink)

In the specialized area of pediatric endocrinology, the use of human growth hormone (hGH) both for children who have a growth hormone abnormality and for the treatment of non-hGH-deficient children who are short is a current clinical reality that raises important ethical questions. Generally speaking, the use of hGH for those children who are clearly lacking it is an efficacious intervention based upon established clinical criteria. The use of hGH for children who are short, but have no growth hormone abnormality (...) is ethically and clinically more controversial. The moral conundrum of how to gain knowledge about new medical treatments that may be beneficial to children while, at the same time, ethically enrolling them in clinical trials with placebo arms in order to gain such knowledge will continue to be a contentious issue in the conduct of research and in the delivery of health care. In addition, there are questions about what ought to be studied and whether a physical characteristic such as short stature ought to be viewed as a circumstance of less than optimal health. (shrink)

How the insane asylum became a laboratory of democracy is revealed in this provocative look at the treatment of the mentally ill in nineteenth-century France. Political thinkers reasoned that if government was to rest in the hands of individuals, then measures should be taken to understand the deepest reaches of the self, including the state of madness. Marcel Gauchet and Gladys Swain maintain that the asylum originally embodied the revolutionary hope of curing all the insane by saving the glimmer (...) of sanity left in them. Their analysis of why this utopian vision failed ultimately constitutes both a powerful argument for liberalism and a direct challenge to Michel Foucault's indictment of liberal institutions. The creation of an artificial environment was meant to encourage the mentally ill to live as social beings, in conditions that resembled as much as possible those prevailing in real life. The asylum was therefore the first instance of a modern utopian community in which a scientifically designed environment was supposed to achieve complete control over the minds of a whole category of human beings. Gauchet and Swain argue that the social domination of the inner self, far from being the hidden truth of emancipation, represented the failure of its overly optimistic beginnings. Madness and Democracy combines rich details of nineteenth-century asylum life with reflections on the crucial role of subjectivity and difference within modernism. Its final achievement is to show that the lessons learned from the failure of the asylum led to the rise of psychoanalysis, an endeavor focused on individual care and on the cooperation between psychiatrist and patient. By linking the rise of liberalism to a chapter in the history of psychiatry, Gauchet and Swain offer a fascinating reassessment of political modernity. (shrink)
