Twelve retired nurses were asked to narrate a care situation in which it had been difficult for them as nurses to know what was the right and good thing to do. The transcribed interviews were examined by content analyses. Physicians were the central coactors in the nurses’ stories. Colleagues were seldom mentioned. Other ward staff were mainly called ‘the girls’. The patient was central and referred to with respect. All the nurses focused on experiential learning. Guiding ethical principles are listed.

This study is part of a comprehensive investigation of ethical thinking among male and female physicians and nurses. Nine women physicians with different levels of expertise, working in various wards in paediatric clinics at two of the university hospitals in Norway, narrated 37 stories about their experience of being in ethically difficult care situations. All of the interviewees’ narrations were concerned with problems relating to both action ethics and relation ethics. The main focus was on problems in a relation ethics (...) perspective. The most common themes in an action ethics perspective were overtreatment and withholding treatment. The more experienced physicians reasoned differently from the group of less experienced physicians and they coped with pressure in different ways. The less experienced physicians disclosed their professional experience yet seemed uncertain, while putting on an air of certainty, but the more experienced physicians disclosed both their professional and personal experience of caregiving and they seemed to allow themselves to feel uncertain in their certainty. Both groups emphasized a need for deep discussion between colleagues about their being in ethically difficult care situations. (shrink)

In this paper we will show you a part of a course “Clinical Psychology of Religion” that has been developed in the Netherlands for introducing mental health professionals in the field of clinical psychology of religion. Clinical psychology of religion applies insights from general psychology of religion to the field of the clinical psychologist. Clinical psychology of religion can be defined as that part of the psychology of religion dealing with the relation between religion, worldview and mental health. Like the (...) clinical psychologist, the clinical psychologist of religion deals with psychodiagnostics and psychotherapy, but concentrates on the role religion or worldview plays in mental health problems. The relation between religion and mental health has been a subject for study since the start of the psychology of religion at the end of the last century. A number of authors have elaborated on the ways in which religion can be beneficial or detrimental to psychological health. In recent research we have found that there is a great need among psychotherapists to become better equipped in this field. (shrink)

This narrative symposium brings to light the extreme difficulties faced by parents of children diagnosed with brain tumors. NIB editorial staff and narrative symposium editors, Gigi McMillan and Christy A. Rentmeester, developed a call for stories that was distributed on several list serves and posted on Narrative Inquiry in Bioethics’ website. The call asks parents to share their personal experience of diagnosis, treatment, long–term effects of treatment, social issues and the doctor–patient–parent dynamic that develops during this process. Thirteen stories (...) are found in the print version of the journal and an additional six supplemental stories are published online only through Project MUSE. One change readers may notice is that the story authors are not listed in alphabetical order. The symposium editors had a vision for this issue that included leading readers through the timeline of this topic: diagnosis–treatment–acute recovery–recurrence–treatment (again)– acute recovery (again)– long-term quality of life– (possibly) end of life. Stories are arranged to help lead the reader through this timeline. Gigi McMillan is a patient and research subject advocate, co-founder of We Can, Pediatric Brain Tumor Network, as well as, the mother of a child who suffered from a pediatric brain tumor. She also authored the introduction for this symposium. Christy Rentmeester is an Associate Professor of Health Policy and Ethics in the Creighton University School of Medicine. She served as a commentator for this issue. In considering how to weigh the risks and benefits of synthetic biology, Kaebnick, Gusmano, and Murray pose the question of whether there is scientific re­search that should not be funded or performed, or if there are potentially dangerous results that should not be wide­ly disseminated. Such questions, they propose, require a new set of rules and norms for knowledge generation—an “ethics of knowledge.” They identify two examples of research that might fall into a nonpermissible category, including “research that is aimed (...) at producing and dis­seminating knowledge of... how to produce more dan­gerous forms of H5N1 and smallpox.” There are already rules and norms to guide the funding and generation of scientific knowledge, however, including research on in­fluenza and smallpox. Even if more rules and guidance were added to the practice of science, potentially prob­lematic, “dual‐use” research would still occur, and as a practical matter, it is unlikely that results from those stud­ies can be contained, particularly if the research is of wide interest. In the Netherlands religion on the one hand and mental health care on the other, have grown apart in the process of secularisation. Nowadays however one can witness a growing need amongst counselors and psychotherapists to be educated in the field of religion, meaning giving and world views (Van der Lans, Pieper, Van Uden, 1993; Raaijmakers, 1994). That religion is back in the focus of attention can also be concluded from the fact that in the latest version of the diagnostic (...) bible, the DSM IV, a separate code (code: V 62.89) has been included for religious or spiritual problems (Lukoff, Lu, Turner, 1992). The case study of a patient with an obsessive-compulsive disorder, which we will present is meant as a contribution to this educational process. We will show how important it is to look at the religious frame of reference of a patient and how this can be handled in the psychotherapeutic treatment. From where I sit, as I listen to the loudspeaker on the council truck telling me to stay indoors, it is impossible to know what direction the covid-19 crisis will take. Certainly, aspects of the virus’s social impact have mirrored the trajectory of previous pandemics. Back in February, people in (...) France began circulating the phrase Je ne suis pas un virus, protesting acts of violence against Asian communities and resisting the well-documented tendency for societies to target ethnic minorities as scapegoats for epidemic disease. The President’s labeling of covid-19 as the “Chinese virus” aroused similar protest in the United States.1 At an armed rally at the state house in Lansing, MI, armed protesters demanded the reopening of businesses despite public health advice to the contrary, an act that raised familiar questions about the freedom of the individual versus the health of the collective in times of health crisis. Here in Boston, anti-lockdown protesters assembled at the Massachusetts State House on May 4, chanting “no more dictatorship”—language congenial to Boston’s history of colonial rebellion, though not especially fitting to the crisis at hand. Just one day earlier, a banner had flown over the buildings of Manhattan with the words CAPITALISM IS THE PANDEMIC. Daniel Defoe’s journalistic account of plague in London in 1665, A Journal of the Plague Year (1722), has catapulted to an unlikely relevance, promoted on the publisher’s website as the “surprise ‘must-read’ for people facing the covid-19 epidemic.” Thucydides ’ description of Athens’s experience of the plague in 430 bc has made news in The Economist, The Washington Post and The Guardian. Historical accounts of plagues are regularly being called upon to offer advice to the present. Yet not all plagues are alike. Historians have rightly protested about parallels being made between covid-19 and the aids crisis, since the current emergency response contrasts damningly with the Reagan government’s protracted silence and willful ignorance throughout the 1980s.3 One problem is the word “plague” itself, a term too suggestive of sin to represent the devastating impartiality of viral disease. “Plagues,” Susan Sontag wrote in aids and its Metaphors (1988), are “inflicted, not just endured.” She argued that the term imposed additional burdens on patients, who were portrayed as suffering some punishment, and could not simply be ill.4 To speak of “plagues” in the plural also suggests a common experience across time, a regular and shared event. Literary accounts of the plague do indeed share many common tropes. Yet there are important differences in the way communities have responded to outbreaks in different eras, and significant divergences in the meaning of disease. Accounts of plagues understandably arouse interest during a pandemic, but they are apt to mislead us, because the plagues so often look the same. One unshakable constant is the analogy between the body and the state, the “body politic.” As has long been recognized, the symbol surfaces across many, unrelated world cultures.5 In seeking to explain a unity composed of many parts, human beings have tended to look to the most immediate parallel— themselves. Physicians, registered nurses (RNs) and enrolled nurses (ENs) engaged in geriatric (n = 49) and surgical (n = 59) care at a large hospital in Sweden gave 180 accounts of morally difficult care episodes. In total, the ENs (n = 40) gave 78, the RNs (n = 38) 55 and the physicians (n = 30) 47 accounts; there were 83 from geriatric care and 97 from surgical care. Forty-nine participants were male, and 59 were female; there were no differences in (...) gender in the form and content of the moral reasoning disclosed in either morally difficult care episodes or in the complete interviews when coded as mainly narrative or propositional, or as showing a care or a justice orientation. The ENs showed a care orientation, while RNs and physicians, to an equal degree, showed both a justice and a care orientation. The ENs also used narrative reasoning more often than RNs and physicians. No differences were seen between the two health care specialties. (shrink)

This paper presents the results of an empirical study among two groups of psychotherapists in the Netherlands. One group works in a mental health institution based on religious principles (the Gliagg), and the other group in a general mental health institution (the Riagg). The investigation starts with an overview of the religious background and religious practices of the therapists. Some equations with the average Dutch citizen are made. The next part of the study deals with the perception by psychotherapists of (...) religious issues connected with psycho-social problems. How often in their opinion is there a relation and what kind of relation is at issue? The third part of the study considers the way in which psychotherapists treat these religious issues. Do they attend to them and what kind of (religious) interventions are used? Special attention is given to the question of whether there are contacts with the clergy. We conclude that religion should be given a more pronounced place in general mental health care and that Riagg psychotherapists, in particular, should become better equipped to deal with religious issues. Whenever Catullus is sexually aggressive or brutally frank in his poetry, modern commentators will often call him "Priapic," an adjective that tends to obscure rather than elucidate the various ways in which Catullus uses the figure of Priapus in crafting his poetic persona. This article attempts to read poems 47, 56, and, in particular, 16, as Catullus' experiments in the Greek and Roman subgenre of Priapic poetry. Once we see that these poems are focalized through the generic perspective of Priapus, (...) Catullus' impersonation of Priapus becomes less an assumption of hyperphallic masculinity and more a witty way in which to lampoon a world-view dominated by an obsessive focus on penetration. Impersonating Priapus meant, in fact, exposing the garden god and his hopeless rusticity to urbane critique. (shrink)

This article reports on a study of the relationship between narcissism, as an important personality trait in individualistic societies, and religious styles of coping. We distinguish between two dimensions of narcissism: overt and covert narcissism, and four different styles of religious coping: self-directing, collaborative, deferring and receptive. The study was carried out by inviting 116 students to complete questionnaires about narcissism and religious coping. It revealed a positive correlation between covert narcissism and the collaborative, deferring and receptive styles of religious (...) coping, and a negative correlation between covert narcissism and the self-directing style. Overt narcissism only had a positive correlation with the receptive style. The article discusses in detail the significance of the relationships between narcissism and styles of religious coping. Should a debate of the choice between metaphorical investigation and epistemological realism in organizational research be prioritized as Willy McCourt called for in Organization Studies? We argue here against doing any such thing — a ‘realism’ debate in organizational theory would merely be a ‘red herring’. Theoretical investigation from Ricoeur to Derrida has liberated us from the need to re-visit the theme, but examination of Gareth Morgan's intellectual development, as begun by McCourt, is of interest because it reveals two very (...) different ‘realisms’. What is of interest about ‘realism’ is not an eitherlor of either ‘realism’ or ‘constructivism’, but a polyphony of the many voices of research. (shrink)

In recent years, researchers have tried to unpack the meaning of the term flexibility and test how different constructs of flexibility are associated with various psychopathologies. For example, it is apparent that high levels of flexibility allow individuals to adaptively cope and avoid psychopathology following traumatic events, but the precise nature of this flexibility is ambiguous. In this study we focus on two central constructs: cognitive flexibility – the ability to recognize and implement possible responses to a situation– and regulatory (...) flexibility – the ability to modulate emotional expression and experience across situations. We aim to explore the connection between cognitive and regulatory flexibility and evaluate their relative effect on PTSD symptoms. Trauma-exposed college students were assessed for cognitive and regulatory flexibility and current and lifetime PTSD symptoms. We predicted and found a relatively weak, yet significant, overlap between participants’ cognitive and regulatory flexibility. Importantly, while both cognitive and regulatory flexibility were associated with lifetime PTSD symptoms, only cognitive flexibility was associated with current PTSD symptoms. The findings illuminate the possible value of differentiating between constructs of flexibility in predicting short and long-term effects of traumatic exposure and may pave the ground for developing personalized intervention methods. Purpose This paper aims to analyze the ways socio-economic issues are represented in mainstream news media and how it is consumed, understood and interpreted by Israeli young adults. It examines how mainstream media uses neo-liberal discourse, and the ways YAs internalize this ethic, while simultaneously finding ways to overcome its limitations. Design/methodology/approach This was a mixed methods study. First, it undertook content analysis of the most popular Israeli mainstream news media among YAs: the online news site Ynet and the TV (...) Channel 2 news. Second, the authors undertook semi-structured in-depth interviews with 29 Israeli YAs. The analysis is based on an online survey of 600 young Israelis, aged 18–35 years. Findings Most YAs did not perceive mainstream media as enabling a reliable understanding of the issues important to them. The content analysis revealed that self-representation of YAs is rare, and that their issues were explained, and even resolved, by older adults. Furthermore, most of YAs' problems in mainstream news media were presented using a neo-liberal perspective. Finally, from the interviews, the authors learned that YAs did not find information that could help them deal with their most pressing economic and social issue, in the content offered by mainstream media. For most of them, social media overcomes these shortcomings. Originality/value Contrary to research that has explored YAs’ consumerism of new media outlets, this article explores how YAs in Israel are constructed in the media, as well as the way in which YAs understand mainstream and new social media coverage of the issues most important to them. Using media content analysis and interviews, the authors found that Young Adults tend to be ambivalent toward media coverage. They understand the lack of media information: most of them know that they do not learn enough from the media. This acknowledgment accompanies their tendency to internalize the neo-liberal logic and conservative Israeli national culture, in which class and economic redistribution are largely overlooked. Mainstream news media uses neo-liberal discourse, and young adults internalize this logic, while simultaneously finding ways to overcome the limitations this discourse offers. They do so by turning to social media, mainly Facebook. Consequently, their behavior maintains the logic of the market, while also developing new social relations, enabled by social media. (shrink)

In this paper, we will present some results of a study among patients in a forensic psychiatric hospital in The Netherlands. We will focus on the following issues: the patients' general religious beliefs and activities; the patients' religious coping activities; the patients' well-being; the relationship between general religious beliefs and activities, religious coping activities and well-being. We will compare the results among this population with the results of our earlier research in various other psychiatric settings.

Background Caring practice begins with awareness of the suffering of patients in a given context. Understanding the interrelationship between the perceived ethical climate of the clinical environment and the ethical sensitivity and caring efficacy of nurses is crucial for strengthening the caring competency of nurses. Research aim This study aimed to examine the associations between the ethical climate of the clinical environment and the ethical sensitivity and caring efficacy of nurses and to investigate the mediating effect of ethical sensitivity on (...) the association between ethical climate and caring efficacy. Research design This was a quantitative study with a cross-sectional descriptive correlational design. The participants completed an online survey that measured the ethical climate, ethical sensitivity and caring efficacy using the Hospital Ethical Climate Survey, Moral Sensitivity Questionnaire-Revised and Caring Efficacy Scale, respectively. Participants and research context The study recruited 293 nurses from two general hospitals that provided acute in-patient and extended care in Hong Kong. Ethical considerations Ethical approval was obtained from the ethics committee of the university and the hospitals involved. Written consent was obtained from the participants. Results Ethical climate was associated with caring efficacy ( β = 0.340, p <.001) and ethical sensitivity ( β = 0.197, p <.001). After adjusting for ethical climate, ethical sensitivity was associated with caring efficacy ( β = 0.860, p <.001). Ethical sensitivity showed a significant mediating effect on the association between ethical climate and caring efficacy (indirect effect = 0.169, 95% confidence interval: 0.097 to 0.261), which accounted for 50% of the total effect. Conclusions The study reveals the complex and interwoven relationship between contextual and personal factors that affect nurses’ caring efficacy from an ethical perspective. It provides insights into the significant roles of ethical climate and ethical sensitivity in strengthening caring efficacy. The results suggest theoretical and clinical implications for professionalisation. (shrink)

Background Protecting a person’s right to privacy and confidentiality is important in healthcare services. As future health professionals, nursing students should bear the same responsibility as qualified health professionals in protecting patient privacy. Objectives To investigate nursing students’ practices of patient privacy protection and to identify factors associated with their practices. Research design A cross-sectional study design was adopted. A two-part survey was used to collect two types of data on nursing students: (1) personal characteristics, including demographics, clinical experience and (...) use of information and communication technology and social media and (2) practice of patient privacy protection, collected using the Patient Privacy Scale. Participants and research context: A total of 319 nursing students aged 18 or above, studying pre-registration nursing programmes and who had attended at least one block of clinical placement, were recruited from a university in Hong Kong. Ethical considerations The study received ethical approval from the Survey and Behavioural Research Ethics Committee of the Chinese University of Hong Kong. The participants were informed of the study aim and written consent was obtained before completing the survey. Findings: The mean total score on the Patient Privacy Scale was 119.7 out of 135. Nursing students who were regular users of Instagram and those who had never taken photographs with patients and hence did not need to obtain patient consent were associated with better practices of patient privacy protection (higher total scores on the Patient Privacy Scale). Conclusions The findings improve our understanding of nursing students’ practice of patient privacy protection and the associated factors. This will inform the development and revision of current strategies to enhance nursing students’ practice of patient privacy protection, especially their use of social media. (shrink)
