Deep brain stimulation interventions are novel devices being investigated for the management of severe treatment-resistant psychiatric illnesses. These interventions require the invasive implantation of high-frequency neurostimulatory probes intracranially aiming to provide symptom relief in treatment-resistant disorders including obsessive-compulsive disorder and anorexia nervosa. In the scientific literature, these neurostimulatory interventions are commonly described as reversible and to be used as a last resort option for psychiatric patients. However, the ‘last resort’ status of these interventions is rarely expanded upon. Contrastingly, usages of (...) DBS devices for neurological symptoms have paved the way for established safety and efficacy standards when used earlier in a disease’s timeline. As DBS treatments for these neurological diseases progress to have earlier indications, there is a parallel ethical concern that early implementation may one day become prescribed for psychiatric illnesses. The purpose of this article is to build off contemporary understandings of reversible neurostimulatory interventions to examine and provide clarifications on the ‘last resort’ status of DBS to better address its ethically charged use in psychiatric neurosurgery. To do this, evaluative differences between DBS treatments will be discussed to demonstrate how patient autonomy would be a paramount guiding principle when one day implementing these devices at various points along a psychiatric disease’s timeline. In presenting the clarification of ‘last resort’ status, the ethical tensions of early DBS interventions will be better understood to assist in providing psychiatric patients with more quality of life years in line with their values. A première vue, Friedrich Nietzsche et Cesare Pavese n'ont rien en commun. Et pourtant! Tous deux sont orphelins de père. Tous deux ont grandi dans un entourage exclusivement féminin. Tous deux n'ont jamais su se faire aimer d'une femme. Tous deux sont également poètes. Tous deux ont eu une vie brève, solitaire et émouvante. Et tous deux ont amplement écrit sur Turin, sur son atmosphère si parfaitement " psychologique ". De cette ville, qui fut un temps la capitale de l'Italie, (...) Nietzsche n'en a dit que l'abord exaltant, aristocratique et baroque, tandis que Pavese en a recraché toute la tristesse, avec ses vastes quartiers industriels, ses usines qui noircissent le ciel, ses ouvriers exténués. C'est à Turin que Nietzsche perd définitivement la raison. Il a quarante-quatre ans. Et c'est à Turin que Pavese se suicide dans une chambre d'hôtel. Il a quarante-deux ans. Le philosophe allemand meurt le 25 août 1900. L'écrivain piémontais meurt le 26 août 1950 - un demi-siècle plus tard, à un jour près. En cherchant des rapprochements entre ces deux artistes exceptionnels, l'auteur pénètre dans leur drame intime, dans les blessures inguérissables de leur enfance ; et l'on revit les derniers instants tragiques qui les conduisirent à la folie et à la mort. Ce livre, s'il est empreint de gravité, est avant tout une rêverie faite de détours et de coïncidences. Les murs de Turin y transpirent. Ils parlent. Geron recently announced that it had begun enrolling patients in the world's first-in-human clinical trial involving cells derived from human embryonic stem cells (hESCs). This trial raises important questions regarding the future of hESC-based therapies, especially in spinal cord injury (SCI) patients. We address some safety and efficacy concerns with this research, as well as the ethics of fair subject selection. We consider other populations that might be better for this research: chronic complete SCI patients for a safety trial, subacute (...) incomplete SCI patients for an efficacy trial, and perhaps primary progressive multiple sclerosis (MS) patients for a combined safety and efficacy trial. The mystery surrounding consciousness as subjectivity dissipates dramatically when understood in its biological context. The core characteristics of consciousness can be seen to derive from its functionality, and the fundamental function of cognition, given the equivalence of mental activity and brain process, is to advance the survival and thus the self-regulative capacity of the organism of which the brain is a part. These core elements of consciousness are comprised of a self-locational data structure which serves to configure ongoing experience in (...) terms of controllable spatial and temporal parameters, and a processing regime for this orientational schema which has evolved from the feedback architecture necessary for regulating behavior in relation to homeostatic needs. These two self-regulative constituents yield a primitive form of consciousness as subjectivity — simple reflexive awareness — which provides the basis for the subsequent development of metacognitive mechanisms which monitor and control the cognitive processes which regulate behavior in relation to metabolic requirements. The prominent Russian philosopher Nikolai Lossky and his ex-student Nicolai Hartmann shared many metaphysical and epistemological views, and Lossky is likely to have influenced Hartmann in adopting several of them. But, in the case of axiological issues, it appears that Lossky also borrowed from the axiologies of Hartmann and the latter's Cologne colleague, Max Scheler. The links between the theories of values of Scheler and Hartmann have been studied abundantly, but never in relation to Lossky. In this paper, I examine (...) the manifold relationships – similarities, differences, borrowings, criticisms, and possible influences – between Lossky's axiology and those of Scheler and Hartmann on four key interweaving issues: (1) their ontological realism with regards to the objectivity of values, (2) their epistemological theories of the intuition of values, (3) their ontological definitions of "value", i.e., whether values are relations, qualities, essences, powers, meanings, etc., and (4) their theories of the stratification of values. This book contributes to the environmental humanities field by offering an analysis of the Anthropocene fantasy: the idea that the Anthropocene is an opportunity to remake our terrestrial environment thanks to the power of technology. The author argues that the earth always escapes the human desire to remake and master it.

Michel Henry’s critique of barbarism,1 understood as a flight from life, almost immediately raises the question of how life’s tendency to negate itself is then to be overcome. Undoubtedly, such a question refers to ethics. Although Henry not only provides an analysis of civilization and its malaise, but also targets the level of the individual through the concept of despair inspired by Kierkegaard, there is no systematic treatment of ethics to be found in his phenomenology of life.2,3 In light of (...) the diagnosis of barbarism, it is therefore necessary to investigate what ethics would be contained in, or follow from, Henry’s phenomenology. And if the essence of life is to be found in immanent affectivity, the questions thus become: is there an “ethics of affectivity” and, if so, which are its main aspects? The purpose of this article is to give an overview of ethics from the standpoint of Henry’s radical phenomenology and to discuss some of the main problems it implies. The opposition of barbarism and culture is essential in order to understand Henry’s distinction between ethos and normative ethics, but it is only intelligible if one refers to immanent affectivity as the key-concept of his phenomenology. As we will see, it is the lack of recognition of immanent life as fundamental phenomenality that makes barbarism possible, a recognition that is therefore central to phenomenology of life as ethics and to the concept of “second birth.” However, since phenomenology is, as theory, tied to intentionality, how can it become indicative of that which escapes intentionality, and how can it provide a truth-criterion for propositions that refer to affectivity as invisible? As the access to the transcendental becomes both an ethical and a theoretical problem, it is necessary to investigate how a radicalised reduction can be performed with regard to three interrelated aspects that are central to human life and therefore to ethics: community, personhood, and action. The analysis of action in particular shows Henry’s reductive move at work, while also facing the problem of articulating transcendence with immanence. His radicalised concept of personhood reveals the same features as those contained in his approach to action: they both combine affectivity and intentionality, but limit the reality of personhood and action to their affective core. Despite growing evidence that a significant number of patients living with Parkison’s disease experience neuropsychiatric changes following Deep Brain Stimulation treatment, the phenomenon remains poorly understood and largely unexplored in the literature. To shed new light on this phenomenon, we used qualitative methods grounded in phenomenology to conduct in-depth, semi-structured interviews with 17 patients living with Parkinson’s Disease who had undergone DBS. Our study found that patients appear to experience postoperative DBS-induced changes in the form of self-estrangement. Using the insights (...) from patients’ subjective perceptions of postoperative self-change provides a potent explanation of potential DBS-induced self-estrangement. (shrink)
