BackgroundObtaining informed consent for intravenous thrombolysis in acute ischemic stroke can be challenging, and little is known about if and how the informed consent procedure is performed by neurologists in clinical practice. This study examines the procedure of informed consent for intravenous thrombolysis in acute ischemic stroke in high-volume stroke centers in the Netherlands.MethodsIn four high volume stroke centers, neurology residents and attending neurologists received an online questionnaire concerning informed consent for thrombolysis with tissue-type plasminogen activator (tPA). The respondents were (...) asked to report their usual informed consent practice for tPA treatment and their considerations on whether informed consent should be obtained.ResultsFrom the 203 invited clinicians, 50% (n = 101) completed the questionnaire. One-third of the neurology residents (n = 21) and 21% of the neurologists (n = 8) reported that they always obtain informed consent for tPA treatment. If a patient is not capable of providing informed consent, 30% of the residents (n = 19) reported that they start tPA treatment without informed consent. In these circumstances, 53% of the neurologists (n = 20) reported that the resident under their supervision would start tPA treatment without informed consent. Most neurologists (n = 21; 55%) and neurology residents (n = 45; 72%) obtained informed consent within one minute. None of the respondents used more than five minutes for informed consent. Important themes regarding obtaining informed consent for treatment were patients’ capacity, and medical, ethical and legal considerations.ConclusionThe current practice of informed consent for thrombolysis in acute ischemic stroke varies among neurologists and neurology residents. If informed consent is obtained, most clinicians stated to obtain informed consent within one minute. In the future, a shortened information provision process may be applied, making a shift from informed consent to informed refusal, while still considering the patient’s capacity, stroke severity, and possible treatment delays. Kochen has suggested an interpretation of quantum mechanics in which he denies that wavepackets ever collapse, while affirming that measurements have definite results. In this paper I attempt to show that his interpretation is untenable. I then suggest ways in which to construct similar, but more satisfactory, hidden variable interpretations.

Time travel has been a staple of science fiction. With the advent of general relativity it has been entertained by serious physicists. But, especially in the philosophy literature, there have been arguments that time travel is inherently paradoxical. The most famous paradox is the grandfather paradox: you travel back in time and kill your grandfather, thereby preventing your own existence. To avoid inconsistency some circumstance will have to occur which makes you fail in this attempt to kill your grandfather. Doesn't (...) this require some implausible constraint on otherwise unrelated circumstances? We examine such worries in the context of modern physics. (shrink)

We present empirical evidence from social psychological research which suggests that standard methods employed when teaching the heuristics and biases program in the context of critical thinking instruction are likelier to facilitate the discernment and correction of biases in others’ reasoning than to have a similar effect in the self-monitoring case. Exemplified by the social phenomenon of false polarization, we suggest that CT instruction may be improved by fostering student’s abilities at counterfactual meta-cognition, and present a corresponding teaching and learning (...) activity. The purpose of this paper is to investigate some problems of using finite (or *finite) computational arguments and of the nonstandard notion of an infinitesimal. We will begin by looking at the canonical example illustrating the distinction between classical and constructive analysis, the Intermediate Value Theorem.

In this paper, I derive a syntactic procedure for revising theories in propositional logic from considerations of indifference and informational economy (minimality). The procedure is very flexible. It allows us to make use of information about the relative epistemic merit (entrenchment) of the sentences in the theories whenever such information is available, and, unlike other procedures proposed in the literature, yields plausible results even for very simple entrenchment orderings.

Increasing complexities facing physicians negotiating the bedside decision continue to fuel the debate over who is the appropriate party to offer ethics consults, should one be needed, during the decision-making process. Some very good arguments have been put forth on behalf of clinical ethicists as being the proper and best party to engage in ethics consultations. However, serious questions remain about the role of the clinical ethicist and his ability to provide the necessary level of objectivity called for in an (...) ethics consult.I argue that the clinician's professional psyche, or mode of thinking as a professional, leaves him little room to maneuver as an objective and detached third party ethics consultant. Several factors are cited and discussed that greatly influence the analyses applied to a case problem by physicians. The most formidable of these factors are habits and the practice of defensive medicine. I conclude that clinical ethicists are less suited for the overall tasks required of an objective consultant in medical cases that appear to involve insurmountable ethical issues. (shrink)

Among modern philosophers, Immanuel Kant has few rivals for his influence over the development of contemporary philosophy as a whole. While the issue of language has become a key fulcrum of continental philosophy since the twentieth century, Kant has been overlooked as a thinker whose breadth of insight has helped to spearhead this advance. The Linguistic Dimension of Kant’s Thought remedies this historical gap by gathering new essays by distinguished Kant scholars. There is supposed to be deep agreement among the work of Wilfrid Sellars, Robert Brandom, and John McDowell in regard to normativity. As a result, according to Robert Brandom (2008), and echoed by Chauncey Maher (2012), “normative functionalism” (NF) may refer to a position held by Sellars, Brandom, and McDowell, i.e., “The Pittsburgh School” of philosophy. The standard criticism of the various forms of this normative functionalist position points out the inconsistency in the commitment of normative functionalists to both metaphysical (...) realism and psychological nominalism. Yet, the inconsistency between metaphysical realism and psychological nominalism may be difficult to see until the relation between normativity and perception is clarified. To this end, in this article I discuss the role of habit in perception. Normative functionalists aspire for a sort of pragmatism between the horns of psychologism and pan-logicism. However, once a discussion of habit in perception reveals a kind of relation between an agent and its environment that exceeds the inferential capacity of normativity, the normative functionalist position seems tautological. Put more specifically, the NF thesis may merely be claiming that the inferential sort of normativity which governs rational synthetic processing of experience is an inferential sort of normativity governing rational synthetic processing. The revelation of such a tautological grounding should be sufficient evidence for the Pittsburgh School to consider re-working its understanding of the functionality of normativity; for example, regarding claims such as: “In an important sense there is no such boundary [between the discursive and non-discursive], and so nothing outside the realm of the conceptual” (Brandom 2000, 357). This discussion should be, at least, valuable as a supplement to the standard criticism of NF or in regard to the Pittsburgh School’s avowed relation to G.W.F. Hegel. (shrink)
