We have previously found that attention to internal somatic sensations during a heart beat perception task increases the misperception of external touch on a somatic signal detection task , during which healthy participants erroneously report feeling near-threshold vibrations presented to their fingertip in the absence of a stimulus. However, it has been suggested that mindful interoceptive attention should result in more accurate somatic perception, due to its non-evaluative and controlled nature. To investigate this possibility, 62 participants completed the SSDT before (...) and after a period of brief body-scan mindfulness meditation training, or a control intervention . The meditation intervention reduced tactile misperception and increased sensitivity during the SSDT. This finding suggests that the perceptual effects of interoceptive attention depend on its particular nature, and raises the possibility that body-scan meditation could reduce the misperception of physical symptoms in individuals with medically unexplained symptoms. Over the past two decades, the competency movement has been gathering momentum internationally within the ranks of professional nursing. It can be argued that this momentum is in response to government initiatives aimed at improving consistency in workforce training and accreditation, and fostering national and international portability of qualifications. At the same time, the competency movement has provided the opportunity for regulators, service providers and government to develop mechanisms to reconstitute competent nurses as accountable, self‐regulating subjects and to monitor this (...) transformation in particular ways. The ways in which competencies have been taken up to do this transforming work in Canada are unique and deserving of some detailed description and analysis.We argue that Canadian nursing regulatory authorities have chosen to view competence as the rehearsal of ethical attributes consonant with the professional role,as opposed to the enactment of skilled conduct. As a result, reflective practices rather than skill and knowledge have become the cornerstone of nursing competency formulation and review. We argue that this distinctly Canadian approach to competency serves three ends. First, it privileges the ‘attribute’ element of the nursing competency model and side steps the ever‐problematic ‘nursing knowledge’ issue; second, self‐surveillance by nurses shifts the onus for professional development from industry to the individual; and third, it concedes responsibility for skill assessment by the regulatory authorities to industry control, leaving employers free to determine skill base and skill mix requirements for practice. Finally, we argue that the reflective component of the regulatory framework for Canadian nurses radically fails as a tool for auditing quality and assessing competency. Rather, it functions as a governmental practice that furthers the deregulatory and economic rationalist aims of the Canadian health reform agenda. (shrink)

The present research isolates the fairness assessment of the process used by the retailer to set a price, as well as the distributive fairness of the price compared to the price that others are offered, and examines the combined effect of procedural fairness and distributive fairness on overall price fairness. Two experimental studies examine procedural and distributive fairness effects on overall price fairness. In study 1, procedural fairness and distributive fairness are manipulated and found to interact to bring about overall (...) price fairness. This article describes the ethical conflicts that a sample of US nurse utilization reviewers faced in their work, and also each nurse’s self-reported ethical orientation that was used to resolve the dilemmas. Data were collected from a sample of 97 registered nurses who were working at least 20 hours per week as utilization reviewers. Respondents were recruited from three managed care organizations that conduct utilization reviews in a large midwestern city. A cross-sectional survey design was used to collect demographic data (...) and to ask closed-response, short-answer and open-ended questions. Ethical conflicts reported by nurses were similar across utilization review settings and many were justice orientated. Self-reported ethical orientations were similar across organizations, with beneficence dominating. Implications of these findings are discussed. (shrink)

Disfluencies include editing terms such as uh and um as well as repeats and revisions. Little is known about how disfluencies are processed, and there has been next to no research focused on the way that disfluencies affect structure-building operations during comprehension. We review major findings from both computational linguistics and psycholinguistics, and then we summarize the results of our own work which centers on how the parser behaves when it encounters a disfluency. We describe some new research showing that (...) information associated with misarticulated verbs lingers, and which adds to the large body of data on the critical influence of verb argument structures on sentence comprehension. The paper also presents a model of disfluency processing. The parser uses a Tree Adjoining Grammar to build phrase structure. In this approach, filled and unfilled pauses affect the timing of Substitution operations. Repairs and corrections are handled by a mechanism we term “Overlay,” which allows the parser to overwrite an undesired tree with the appropriate, correct tree. This model of disfluency processing highlights the need for the parser to sometimes coordinate the mechanisms that perform garden-path reanalysis with those that do disfluency repair. The research program as a whole demonstrates that it is possible to study disfluencies systematically and to learn how the parser handles filler material and mistakes. It also showcases the power of Tree Adjoining Grammars, a formalism developed by Aravind Joshi which has yielded results in many different areas of linguistics and cognitive science. (shrink)

Many new genetic tests are used in clinical practice, and the number of available tests is growing. Two important health policy questions arise as these genetic tests become available. The first question, whether a new test should be made available, has been the focus of much recent discussion. The second question concerns defining the appropriate standards surrounding the use of these tests, including patient selection, education, informed consent, test interpretation and counseling.Genetic tests currently move from the research arena, where strategies (...) are put in place to minimize risks,into the clinical context where the practice of medicine is more variable and the risks to patients less understood. In the research setting, risks can be minimized through safeguards including education, consent, and counseling. The frequency and magnitude of psychosocial and clinical risks canbe assessed. In recent years, many studies have highlighted the changing nature of scholarly research, reflecting the new digital tools and techniques that have been developed. But researcher uptake of these tools is strongly influenced by existing information behaviour, itself affected by a number of factors, particularly discipline. This article outlines findings from a recent study which used six case studies to look at the information behaviours of researchers working in different disciplinary fields or academic departments, or using specific tools. The study (...) suggested that researchers’ uses of, and attitudes towards, digital technologies are affected by existing disciplinary habits and preconceptions. Professor Solmsen's interpretation is orthodox; his comprehensive account builds on recent more specialized studies, including his own, and those of Jaeger, Ross, and Cherniss. If in some ways the book contains no large surprises, it nevertheless makes a major contribution by its treatment of Plato. The author has skillfully disengaged Plato's observations about nature from the customary ethical, epistemic, or, as the case may be, metaphysical contexts. He demonstrates that Plato was toward the end of his career a more serious (...) investigator of physical phenomena than is commonly supposed. Moreover, at almost every turn he finds Platonic precedents for Aristotle's thinking in this field. He does not detract from Aristotle's originality, but illumines it, and Plato's as well. (shrink)

I develop a phenomenological hermeneutics of four poems by Sylvia Plath: 'Mystic,' 'Ariel,' 'The Moon and the Yew Tree,' and 'The Arrival of the Bee Box.' Inspired by the work of Maurice Merleau-Ponty, I illustrate how we can experience individual poems through the multiple aspects of our embodiment. Importantly, single artworks are treated here with the same respect as single philosophical texts. Heidegger treats poems similarly in his "later" philosophy, which also influenced this dissertation. This emphasis on embodiment does not (...) mean to privilege the body over the cognitive. Instead, the cognitive is revealed as one aspect of our bodily existence. ;Plath's poetic themes intertwine with philosophical questions, such as the difference between instituted and creative language, the nature of gender, and the ethical status of the non-human. For example, Plath's work offers new ways of addressing feminist concerns over essentialism, identity, and spirituality. Plath's emerging notion of a feminine divine resonates with Luce Irigaray's call for a philosophical revisioning of the God of Western metaphysics. Plath's articulations of the body as it lives through pain contribute to the existentialist task of drawing us closer to an authentic experience of death. I show how pain possesses a spiritual dimension for Plath and connect this phenomenon with the act of writing poetry. ;The poetic dimension of language has been suppressed by reductive accounts that theorize language and the body's involvement in language in terms of representation rather than embodiment. I reveal the non-linguistic meanings within the lines and silences of poetic space that impact and alter our bodily being. Poetry allows us to experience, among other things, the flow of lived time, the colors of sounds, and our body's fusion with the world. Significantly, phenomenology bends with these issues in a chiasmic gathering where we can experience the affinity between these two discourses. Phenomenological hermeneutics uncovers the depth of language that is usually suppressed in calculative-rational discourses. ;Through these poetic phenomenological meditations, I open up ways of understanding spiritual and poetic language that impact our ethical encounters with one another, the environment, and the divine. In this paper, I submit that it is the controlled part of skilled action, that is, that part of an action that accounts for the exact, nuanced ways in which a skilled performer modifies, adjusts and guides her performance for which an adequate, philosophical theory of skill must account. I will argue that neither Jason Stanley nor Hubert Dreyfus have an adequate account of control. Further, and perhaps surprisingly, I will argue that both Stanley and Dreyfus relinquish an account of (...) control for precisely the same reason: each reduce control to a passive, mechanistic, automatic process, which then prevents them from producing a substantive account of how controlled processes can be characterized by seemingly intelligent features and integrated with personal-level states. I will end by introducing three different kinds of control, which are constitutive of skilled action: strategic control, selective, top–down, automatic attention, and motor control. It will become clear that Dreyfus cannot account for any of these three kinds of control while Stanley has difficulty tackling the two latter kinds. (shrink)

Hilary of Poitiers is an anomaly in the standard scholarly classification of Patristic Greek and Latin soteriology, for, though he is Latin, his soteriology shows such resemblance to Greek mystical theory that he is considered one of its major proponents. Since Harnack, the Greek mystical model is said to depend upon Platonism. However, this paper argues that Hilary teaches a "Greek" mystical model of redemption based on Christ‘s assumption of all humanity without recourse to Platonism. Hilary‘s soteriology is instead a (...) development of Latin Stoicism and a literal exegetical understanding of the Pauline Adam-Christ parallel. (shrink)

The vast visual coverage of the Boxer Uprising in northern China that ended with foreigners traipsing through the Forbidden City in Beijing reflects both the richly innovative image technology at the turn of the century and the international interest in events that involved many of the world’s most powerful nations. Historians and students who seek to understand the incident through the innovative technology of today can, for the first time, access primary source images in large numbers malleably displayed in the (...) digital environment. The sheer volume of images collected in one place offers powerful new insights into a history already broadly covered in analytical texts and first-person accounts. One qualitative difference from previous histories is the emergence of subjective points of view that can be conveyed in multidimensional models that juxtapose experiential recordings of the day. Harnessing the tension of contrasting viewpoints – who knew what when – enables today’s learners to see through the lenses of past participants whose understanding was limited by location, communications and, one must remember, by not knowing the outcome. Those who were besieged in the Foreign Legations did not know if they would survive; soldiers and rebels on the road to Beijing knew only the difficulties of the march; illustrations in newspaper reports overseas were coloured by politics and both national and international rivalries. Digital visual representations have the possibility of recreating the collision of experiences that allow users to move between personal records and public reporting from the time to gain a knowledge that is unmitigated by an author’s text-based presentation and conclusions. The result is as much a revolution in perception of historical events – perception-based learning – as it is in information about these events. (shrink)

The Contracts of Fiction invites readers to consider the advantages of describing fictions as governed by a set of social contracts, teaching us how to think about the stuff of daily life, animate and inanimate, as abstractions.

Context: Although ethics consultation is commonplace in United States (U.S.) hospitals, descriptive data about this health service are lacking. Objective: To describe the prevalence, practitioners, and processes of ethics consultation in U.S. hospitals. Design: A 56-item phone or questionnaire survey of the "best informant" within each hospital. Participants: Random sample of 600 U.S. general hospitals, stratified by bed size. Results: The response rate was 87.4%. Ethics consultation services (ECSs) were found in 81% of all general hospitals in the U.S., and (...) in 100% of hospitals with more than 400 beds. The median number of consults performed by ECSs in the year prior to survey was 3. Most individuals performing ethics consultation were physicians (34%), nurses (31%), social workers (11%), or chaplains (10%). Only 41% had formal supervised training in ethics consultation. Consultation practices varied widely both within and between ECSs. For example, 65% of ECSs always made recommendations, whereas 6% never did. These findings highlight a need to clarify standards for ethics consultation practices. (shrink)

Some health care organizations allow physicians to withhold cardiopulmonary resuscitation from a patient, despite patient or surrogate requests that it be provided, when they believe it will be more harmful than beneficial. Such cases usually involve patients with terminal diagnoses whose medical teams argue that aggressive treatments are medically inappropriate or likely to be harmful. Although there is state-to-state variability and a considerable judicial gray area about the conditions and mechanisms for refusals to perform CPR, medical teams typically follow a (...) set of clearly defined procedures for these decisions. The procedures are based on the principle of nonmaleficence and typically include consultation with hospital ethics committees, reflecting the guidelines of relevant professional associations. Ethical debates about when CPR can and should be limited tend to rely more on discussions of theory, principles, and case studies than systematic empirical study of the situations in which such limitations are applied. Sociologists of bioethics call for empirical study, arguing that what ethicists and health professionals believe they are doing when they draft policies or invoke principles does not always mirror what is happening on the ground. In this article, we begin the task of modeling the empirical analyses sociologists call for, focusing on a cohort at Massachusetts General Hospital. We inductively analyzed ethics committee notes and medical records of nineteen patients whose surrogates did not accept the decision to withhold CPR. (shrink)
