The covert administration of medication occurs with incapacitated patients without their knowledge, involving some form of deliberate deception in disguising or hiding the medication. Covert medication in food is a relatively common practice globally, including in institutional and homecare contexts. Until recently, it has received little attention in the bioethics literature, and there are few laws or rules governing the practice. In this paper, we discuss significant, but often overlooked, ethical issues related to covert medication in food. We emphasise the (...) variety of ways in which eating has ethical importance, highlighting what is at risk if covert administration of medication in food is discovered. For example, losing trust in feeders and food due to covert medication may risk important opportunities for identity maintenance in contexts where identity is already unstable. Since therapeutic relationships may be jeopardised by a patient’s discovery that caregivers had secretly put medications in their food, this practice can result in an ongoing deception loop. While there may be circumstances in which covert medication is ethically justified, given a lack of suitable alternatives, we argue that in any particular case this practice should be continually re-evaluated in light of the building moral costs to the relational agent over time. In this discussion paper we consider the influence of ethnicity, religiosity, spirituality and health literacy on Advance Care Planning for older people. Older people from cultural and ethnic minorities have low access to palliative or end-of-life care and there is poor uptake of advance care planning by this group across a number of countries where advance care planning is promoted. For many, religiosity, spirituality and health literacy are significant factors that influence how they make end-of-life decisions. Health literacy issues have (...) been identified as one of the main reasons for a communication gaps between physicians and their patients in discussing end-of-life care, where poor health literacy, particularly specific difficulty with written and oral communication often limits their understanding of clinical terms such as diagnoses and prognoses. This then contributes to health inequalities given it impacts on their ability to use their moral agency to make appropriate decisions about end-of-life care and complete their Advance Care Plans. Currently, strategies to promote advance care planning seem to overlook engagement with religious communities. Consequently, policy makers, nurses, medical professions, social workers and even educators continue to shape advance care planning programmes within the context of a medical model. The ethical principle of justice is a useful approach to responding to inequities and to promote older peoples’ ability to enact moral agency in making such decisions. (shrink)

In this article the public-theological motives in the film As it is in heaven is analised to demonstrate the film producer Kay Pollack's ideal to communicate through the film that people should live their lives here and now authentically without seeking excuses for being happy. In this article the principles of narratology is applied in the analysis of the film's plot, characterisation, plotted time and narrated spaces. It is also argued that the protagonist in the film can be regarded as (...) a 'Christ-figure' and that the film conveys a 'Christ narrative' in a secularised context. Societal issues such as emotional abuse and violence against children, women, people with disability and animals constitute building blocks of the narrative. Ecclesiastical hypocrisy and outdated sexual values endorsed by the institutional church are replaced by a choir consisting of common people which leads a whole world to sing in harmony. (shrink)

This article examines the contribution that poetry written over the last fifty years might make to the established and burgeoning field of Medical Humanities. It takes poems by women about cancer and depression as a case study of how they can offer insight into the impact of these conditions on the sufferer. Collectively, the poems document and effect shifts in knowledge about, and the associated stigmas concerning, illnesses that carry secrecy and shame, specifically cancer and depression. Additionally, drawing on Virginia (...) Woolf’s remarkable essay “On Being Ill”, we see how the non-verbal features of poems, particularly metaphors and symbols, mark the space of what a patient cannot fully articulate. Accordingly, the poems discussed illuminate how literature can express human interiority but at the same time remind the reader – or health professional – that interiority can never be fully presented. Thus, poetry both contributes to and also checks the ways in which literature is appropriated by the biomedical sciences. The poets included are Eavan Boland, Julia Darling, U.A. Fanthorpe, Jackie Kay, Gwyneth Lewis, Carol Satyamurti and Jo Shapcott. Some poems specify a female subjectivity, some express agency as autonomous individuals, while others presume to speak of common human experience. (shrink)

This paper argues that current iterations of lesbian, gay, bisexual, transgender and intersex rights are limited by an overreliance on particular representations of sexuality, in which homosexuality is defined negatively through a binary of homosexual/heterosexual. The limits of these representations are explored in order to unpick the possibility of engaging in a form of sexuality politics that is grounded in difference rather than in sameness or opposition. The paper seeks to respond to Braidotti’s call for an “affirmative politics” that is (...) open to forms of creative, future-oriented action and that might serve to answer some of the more common criticisms of current LGBTI rights activism. Subjectivists about practical normativity hold that an agent’s favoring and disfavoring attitudes give rise to practical reasons. On this view, an agent’s normative reason to choose vanilla over chocolate ice cream ultimately turns on facts about what appeals to her rather than facts about what her options are like attitude-independently. Objectivists—who ground reasons in the attitude-independent features of the things we aim at—owe us an explanation of why it is rational to choose what we favor, if not because favoring is (...) a source of reasons. My aim in this paper is to supply such a story. The proposal is roughly that when an agent cannot base her choices on her judgments about what she has most reason to do, structural rationality extends to her a license to choose something just because she favors it, without imbuing favoring with the authority of a normative reason. The fact that I am a committed gardener has some practical upshot or other. But what, exactly, is the upshot of the fact that I am committed to some project, person, or principle? According to a standard view, my commitment to gardening directly effects a change in the normative facts by giving me a further reason to garden. I argue that we should reject this view and its attendant psychological story involving a normatively significant will. According to the view I (...) develop here, a commitment to X just is a decision to take seriously the reasons to which X gives rise, and where necessary, to give those reasons an elevated place in one’s practical thinking. In virtue of being decisions to conduct one’s deliberation in a certain way, commitments alter what it is rational for us to do without giving us further reasons to act. (shrink)

The ArgumentThis paper focuses on the opening of a discursive space: the emergence of informational and scriptural representations of life and their self-negating consequences for the construction of biological meaning. It probes the notion of writing and the book of life and shows how molecular biology's claims to a status of language and texuality undermines its own objective of control. These textual significations were historically contingent. The informational representations of heredity and life were not an outcome of the internal cognitive (...) momentum of molecular biology; they were not a logical necessity of the unravelling of the base-pairing of the DNA double-helix. They were transported into molecular biology still within the protein paradigm of the gene in the 1940s and permeated nearly every discipline in the life and social sciences. These information-based models, metaphors, linguistic, and semiotic tools which were central to the formulation of the genetic code were transported into molecular biology from cybernetics, information theory, electronic computing, and control and communication systems — technosciences that were deeply embedded with the military experiences of world war II and the Cold War. The information discourse thus became fixed in molecular biology not because it worked in the narrow epistemic sense (it did not), but because it positioned molecular biology within postwar discourse and culture, perhaps within the transition to a post-modern information-based society. “You Have Seen Their Faces”: Gisèle Freund, Walter Benjamin and Margaret Bourke-White as Headhunters of the Thirties -/- Abstract This paper concentrates on one work by each of three authors: Walter Benjamin’s Deutsche Menschen (Germans), an anthology of twenty-five 18th and 19th century German personal letters; Margaret Bourke-White and Erskine Caldwell’s You Have Seen Their Faces; and Gisèle Freund’s collection of photographic portraits of writers and artists, compiled between 1936 and 1939. The purpose of this paper is to compare the (...) politics of representation of these three figures and to examine the challenge they addressed to the dominant iconography of the 1930s. At the heart of each one’s work was a desire to bring about a shift in contemporary cultural politics by reimagining the body politic. Benjamin’s and Freund’s aim was to displace the monumentalizing, distancing, ‘heroic’ forms of fascist art and writing. They construct and project an intimate, or ‘domestic’ image of humanism as a form of social practice, past and present. Bourke-White’s aim was to bring to visibility an America concealed beneath the glossy homogenized images of white American advertising. All three authors aligned themselves with a documentary tradition that believed that representation should function as a constructed record of the traces of lived experience. We expect schools to be a safe haven, but after more than a decade of targeted school violence prevention laws and safety plans, students are still marginalized and bullied to the point of despondence, retaliation, and even suicide. This thoughtful exploration of what makes a school a safe place is based on the understanding that violence is a continuum of acts and attitudes–subtle to overt–that have a negative effect on how students feel and learn.

Environmentalists often argue that, in order to address fundamentally the harmful impact of their activities on the environment, western industrial societies need to change their attitude to nature. Specifically, they need to see nature as sacred, and to acknowledge that humanity is a part of nature rather than separate from it. In this paper, I seek to show that these tow ideas are incompatible in the context of western culture. Drawing particularly on ideas expressed by western conservationists, I argue that (...) nature is already seen as sacred, and that its sacredness depends on it being seen as separate from humanity, an idea which effectively contradicts the scientific knowledge on which many conservationists base their actions. Goodin's green theory of value is used as a source of ideas about why non-human nature is experienced as sacred, and can be extended to suggest that other values, such as 'development' and progress', are also seen as sacred. I defend Spinoza's claim that extension is an attribute that an indivisible substance, such as God, could have. However, in order to explain why, we must abandon two long held orthodoxies in Spinoza scholarship. First, Spinoza acknowledges only parts that do not depend on their whole. Second, God, considered as natura naturans, has no parts of any kind. Against these orthodoxies, I show that having parts which depend on their whole, for Spinoza, does not entail divisibility and that God, considered (...) as natura naturans, must have such parts in order to be extended. Along the way, we will have a closer look at Spinoza’s mereology and address apparently conflicting statements that Spinoza makes about the relation of part and whole that have long vexed commentators. This article provides an analysis of theories of metaphor, tracing how far those which have dominated Western thought until the past few decades are reflective of the definitions within which writers from Classical Greece were working. It is shown how, during the Middle Ages and beyond, in particular since the seventeenth century, definitions of metaphor which emphasised ‘literal’ and ‘figurative’ levels of meaning have led to serious misconceptions concerning its nature and function in the attempts of human beings to conceptualise (...) and make sense of their experiences. A discussion of these misconceptions is provided in some detail and traditional definitions are shown to have distorted the deeper insights held by classical writers, such as Aristotle, Demetrius and Longinus. The article continues with descriptive analyses of the mistaken nature of definitions of metaphor which confine it to creative, poetic forms of discussion. It is shown that conventional speech utterances comprise simple metaphors to a large extent and how these provide an immensely important base from which education could help pupils extend and enrich both their thought and language. This could be done by teaching them ways of interpreting the novel, poetic metaphors of others and how to create their own. (shrink)
