Although there are notable exceptions, examination of nursing's participation in colonizing processes and practices has not taken hold in nursing's consciousness or political agenda. Critical analyses, based on the examination of politics and power of the structural determinants of health, continue to be marginalized in the profession. The goals of this discussion article are to underscore the urgent need to further articulate postcolonial theory in nursing and to contribute to nursing knowledge about paths to work toward decolonizing the profession. The (...) authors begin with a description of unifying themes in postcolonial theory, with an emphasis on colonized subjectivities and imperialism; the application of a critical social science perspective, including postcolonial feminist theory; and the project of working toward decolonization. Processes involved in the colonization of nursing are described in detail, including colonization of nursing's intellectual development and the white privilege and racism that sustain colonizing thinking and action in nursing. The authors conclude with strategies to increase the counter‐narrative to continued colonization, with a focus on critical social justice, human rights and the structural determinants of health. ArgumentMontpellier vitalists upheld a medical perspective akin to modern “holism” in positing the functional unity of creatures imbued with life. While early vitalists focused on the human organism, Jean-Charles-Marguerite-Guillaume Grimaud investigated digestion, growth, and other physiological processes that human beings shared with simpler organisms. Eschewing modern investigative methods, Grimaud promoted a medically-grounded “metaphysics.” His influential doctrine of the “two lives” broke with Montpellier holism, classifying some vital phenomena as “higher” and others as “lower” and attributing the “nobility” of the human (...) species to the predominance of the former. In place of Montpellier teaching that attributed health to the holistic equilibration of vital activities, Grimaud embraced spiritualist dualisms of soul and body, Creator and created. Celebrating the divinely-ordained “wisdom” evident in involuntary physiological processes, he argued that such life functions were incomprehensible to human investigators. While Grimaud's work encouraged inquiry into the division between the central and “vegetative” nervous systems that became paradigmatic in nineteenth-century neuroscience, it also opened Montpellier vitalism to charges of conservatism and obscurantism that are still lodged against it to the present day. This paper presents and advocates an approach to the semantics of opinion statements, including matters of personal taste and moral claims. In this framework, ‘outlook-based semantics’, the circumstances of evaluation are not composed of a possible world and a judge ; rather, outlooks replace possible worlds in the role of circumstance of evaluation. Outlooks are refinements of worlds that settle not only matters of fact but also matters of opinion. Several virtues of the framework and advantages over existing implementations of (...) world-judge relativism are demonstrated in this paper. First, world-judge relativism does not actually explain the ‘disagreement’ of ‘faultless disagreement’, while a straightforward explanation suggests itself in outlook-based semantics. Second, outlook-based semantics provides an account of subjective attitude verbs that can capture lack of opinionatedness. Third, outlook-based semantics unproblematically explains the connection-building role of aesthetic discourse and the group-relevance of discretionary assertions, while capturing the same effects in world-judge relativism obviates the purpose of the judge parameter. Finally, because the proposed circumstances of evaluation are entirely analogous to possible worlds, the framework is easy to use and extend. (shrink)

It is designed to introduce the newcomer to the main ideas that form the subject area with a view to equipping students with all the major arguments and standpoints required to understand this burgeoning area of study. ArgumentSociologists and historians have long favored externalist over internalist accounts of practices in the clinical disciplines. This has been particularly true in the case of the so-called new patient or borderline personality, which a range of commentators have located in culturally resonant narratives of decline. I argue here that these narratives, while pleasing, do not hold up as history; most problematic is their assumption that the appearance of the borderline portends the emergence of altogether novel forms of modal personhood. Internalist (...) accounts of the category's consolidation are equally problematic in asserting a coherence and a stable referent that repeatedly proves elusive. In the end, I suggest that a complex account of disciplinary practice that attends to knowledge production in the clinical encounter supports the conclusion that the new patient was new to psychoanalysis. Whether or not he was altogether new cannot be established, undermining the critics' certitude and prophecies of decline. (shrink)

Iannis Xenakis (1922–2001), composer, architect, engineer and media artist, designed together with Le Corbusier the Philips-pavilion for the 1958 Brussels World Fair. This pavilion is an early example of (“hybrid”) combined media and architectural space as it contained a Poème Électronique, an electronic synthesis of visual projections (conceived by Le Corbusier ) and acoustic events (composed by Varèse). The pavilion's architecture with its hyperbolic-paraboloid shells had a dynamic expression. Xenakis continued this research into complex material architectural forms. He also worked (...) on the complex ephemeral architectures of light and sound events. What is specific to Xenakis is the way he used forms in different fields and transferred them from one field to another, from engineering to music, from music to architecture and visual events. This experience of working simultaneously and applying the same (mental) structures in different fields opened him the way (partly supported by the universal computer instrument) for the practice of transferring mathematical-scientific structures into artistic production. In this context Xenakis pleads for the development of a “General Morphology”, a research concerned with the understanding of form and its generation. Xenakis's ‘material’ architectural work is to be seen in continuity with his architectures of music and light. (shrink)

In ‘Ethics of sharing medical knowledge with the community: is the physician responsible for medical outreach during a pandemic?’ Strous and Karni note that the revised physician’s pledge in the World Medical Association Declaration of Geneva obligates individual physicians to share medical knowledge, which they interpret to mean a requirement to share knowledge publicly and through outreach. In the context of the COVID-19 pandemic, Strous and Karni defend a form of medical paternalism insofar as the individual physician must reach out (...) to communities who may not want, or know to seek out, medical advice, for reasons of public health and health equity. Strous and Karni offer a novel defence of why physicians ought to intervene even in insular communities, and they offer suggestions for how this could be done in culturally sensitive ways. Yet their view rests on an unfounded interpretation of the Geneva Declaration language. More problematically, their paper confuses shared and collective responsibility, misattributing the scope of individual physician obligations in potentially harmful ways. In response, this reply delineates between shared and collective responsibility, and suggests that to defend the obligation of medical outreach Strous and Karni propose, it is better conceptualised as a collective responsibility of the medical profession, rather than a shared responsibility of individual physicians. This interpretation rejects paternalism on the part of individual providers in favour of a more sensitive and collaborative practice of knowledge sharing between physicians and communities, and in the service of collective responsibility. Although a second-person neuroscience has high ecological validity, the extent to which a second- versus third-person neuroscience approach fundamentally alters neural patterns of activation requires more careful investigation. Nonetheless, we are hopeful that this new avenue will prove fruitful in significantly advancing our understanding of typical and atypical social cognition.

This paper discusses Jacques Derrida's Death Penalty Seminars (two consecutive seminars he gave at the École des hautes études en sciences sociales in 1999–2000 and 2000–2001), as well as his 2000 Paris address to the States General of Psychoanalysis entitled “Psychoanalysis Searches the States of Its Soul.” The paper is magnetized by two questions: what does it mean to say, as Derrida says in his provocative statement at the end of his 1999 seminar, “even when the death penalty will have (...) been abolished . . . it will survive, there will still be some death penalty [même quand la peine de mort sera abolie . . . elle survivra, il y en aura encore]”? And why is psychoanalysis in a privileged position to address “this irreducible thing in the life of the animate being” that is the possibility of cruelty? (shrink)

Not only is deception commonplace in medical encounters, according to Christopher Meyers (2021), but the clinical ethicist might have moral obligations to support and even enact deception. Descriptively Meyers is right that there are “opportunistic, self-interested and benevolent reasons” for deception through omission and commission in clinical medicine. But it is possible to retain this premise while rejecting the normative conclusion that the clinical ethicist “should sometimes be an active participant in the deception of patients and families.” One reason to (...) reject the normative conclusion is its incompatibility with providing trauma informed ethics consultation (TIEC). In the TIEC framework I developed with Uchenna Anani, we defend the integration of trauma informed care (TIC) into ethics consultation (Lanphier and Anani 2021). While ethicists narrowly attend to the question and scope of the ethics consults made to them, doing so in alignment with trauma informed principles is tantamount to providing good ethics consultation. According to Meyers, the clinical ethicist is an “advocate for the best ethical choices.” The best ethical choices are also trauma informed ones. (shrink)

A growing frustration with “spin doctors,” doublespeak, and outright lying by public officials has resulted in a deep public cynicism regarding politics today. It has also led many voters to seek out politicians who engage in “straight talk,” out of a hope that sincerity signifies a dedication to the truth. While this is an understandable reaction to the degradation of public discourse inflicted by political hype, Elizabeth Markovits argues that the search for sincerity in the public arena actually constitutes (...) a dangerous distraction from more important concerns, including factual truth and the ethical import of political statements. Her argument takes her back to an examination of the Greek notion of _parrhesia_, and she draws from her study of the Platonic dialogues a nuanced understanding of this ancient analogue of “straight talk.” She shows Plato to have an appreciation for rhetoric rather than a desire to purge it from public life, providing insights into the ways it can contribute to a fruitful form of deliberative democracy today. Until recently, raising a young child as transgender was culturally unintelligible. Most scholarship on transgender identity refers to adults’ experiences and perspectives. Now, the increasing visibility of gender-variant children, as they are identified by the parents who raise them, presents new opportunities to examine how individuals confront the gender binary and imagine more gender-inclusive possibilities. Drawing on Foucault’s notion of “truth regime” to conceptualize the regulatory forces of the gender binary in everyday life, this work examines the strategies of 24 (...) such parents, who represent 16 cases of childhood gender variance. Specifically, I analyze three practices—“gender hedging,” “gender literacy,” and “playing along”—through which these parents develop a critical consciousness about gender binary ideology and work to accommodate their children’s nonconformity in diverse discursive interactions. Taken together, their newfound strategies and perspectives subvert traditional conceptions of “gender-neutral” or “feminist” parenting, and reveal new modes of resistance to the normative transmission and regulation of gender practices. Tigard (2019) suggests that the medical community would benefit from continuing to promote notions of individual responsibility and blame in healthcare settings. In particular, he contends that blame will promote systematic improvement, both on the individual and institutional levels, by increasing the likelihood that the blameworthy party will ‘own up’ to his or her mistake and apologise. While we agree that communicating regret and offering a genuine apology are critical steps to take when addressing patient harm, the idea that medical (...) professionals should continue to ‘take the blame’ for medical errors flies in the face of existing science and threatens to do more harm than good. We contrast Dr Tigard’s approach with the current literature on blame to promote an alternative strategy that may help to create lasting change in the face of unfortunate error. (shrink)

Moral disengagement is a process whereby the self-regulatory mechanisms that would otherwise sanction unethical conduct can be selectively disabled. The present research proposed that moral disengagement might be adopted in the endorsement of asylum seeker policies in Australia, and in order to test this, a scale was developed and was validated in two studies. Factor analysis demonstrated that a 2-factor, 16-item structure had the best fit, and the construct validity of the scale was supported. Results provide evidence for the use (...) of moral disengagement in the context of asylum seekers as a means of rationalizing conduct that may otherwise be sanctioned. (shrink)

In “Obligations of the Gift” Sandra Lee (2021) suggests that social norms of reciprocity and the expectations and obligations associated with gift-giving afford a framework for addressing social justice considerations in precision medicine. Lee is particularly concerned with obligations to marginalized or oppressed racial and ethnic groups, which are also historically under-represented populations in precision medicine. Obligations arise, Lee argues, through the “gift” that research participants make when they contribute their data or biospecimens to precision medicine research. This conceptualization of (...) research participation is distinct from an altruistic donation model that explicitly precludes obligations of reciprocity. I agree with Lee’s overall project to support social justice aims by better recognizing responsibilities of researchers to their research participants, and increasing diverse participation in biomedical research. However, I am not convinced that the account of “the gift” she deploys can achieve the social justice goals she intends because it risks replicating rather than resisting problematic conceptions of labor, property, and ownership of selves and others. However, an alternate conceptualization of “the gift” found in the work of 20th century French philosopher Jacques Derrida recasts ethical obligations on the part of precision medicine to those who contribute their data and biospecimens for precision medicine research.
