To analyze which ethically relevant biases have been identified by academic literature in artificial intelligence algorithms developed either for patient risk prediction and triage, or for contact tracing to deal with the COVID-19 pandemic. Additionally, to specifically investigate whether the role of social determinants of health have been considered in these AI developments or not. We conducted a scoping review of the literature, which covered publications from March 2020 to April 2021. ​Studies mentioning biases on AI algorithms developed for contact (...) tracing and medical triage or risk prediction regarding COVID-19 were included. From 1054 identified articles, 20 studies were finally included. We propose a typology of biases identified in the literature based on bias, limitations and other ethical issues in both areas of analysis. Results on health disparities and SDOH were classified into five categories: racial disparities, biased data, socio-economic disparities, unequal accessibility and workforce, and information communication. SDOH needs to be considered in the clinical context, where they still seem underestimated. Epidemiological conditions depend on geographic location, so the use of local data in studies to develop international solutions may increase some biases. Gender bias was not specifically addressed in the articles included. The main biases are related to data collection and management. Ethical problems related to privacy, consent, and lack of regulation have been identified in contact tracing while some bias-related health inequalities have been highlighted. There is a need for further research focusing on SDOH and these specific AI apps. (shrink)

Este ensayo postula que la noción de dignidad humana formulada filosófica y moralmente desde la civilización occidental moderna, demanda hoy una re-lectura de sentido, a la luz de las perplejidades que anidan en la disonancia entre los principios morales y el derecho legal justiciable de los Derechos Humanos en las democracias contemporáneas. Si bien, la noción de Derechos Humanos deriva de la pertenencia a la familia humana en la que se asienta idealmente la universalidad de sus titulares y por el (...) principio de autonomía, el resultado de esta mixtura tiene su traducción en dos concepciones problemáticas: una abstracta del ser humano sobre la que se instrumenta una supuesta universalidad que derivó en otra individualista que convierte a los hombres en sujetos aislados y egoístas. The increasing application of artificial intelligence (AI) to healthcare raises both hope and ethical concerns. Some advanced machine learning methods provide accurate clinical predictions at the expense of a significant lack of explainability. Alex John London has defended that accuracy is a more important value than explainability in AI medicine. In this article, we locate the trade-off between accurate performance and explainable algorithms in the context of distributive justice. We acknowledge that accuracy is cardinal from outcome-oriented justice because it helps (...) to maximize patients’ benefits and optimizes limited resources. However, we claim that the opaqueness of the algorithmic black box and its absence of explainability threatens core commitments of procedural fairness such as accountability, avoidance of bias, and transparency. To illustrate this, we discuss liver transplantation as a case of critical medical resources in which the lack of explainability in AI-based allocation algorithms is procedurally unfair. Finally, we provide a number of ethical recommendations for when considering the use of unexplainable algorithms in the distribution of health-related resources. (shrink)

The main aim of this article is to reflect on the impact of biases related to artificial intelligence (AI) systems developed to tackle issues arising from the COVID-19 pandemic, with special focus on those developed for triage and risk prediction. A secondary aim is to review assessment tools that have been developed to prevent biases in AI systems. In addition, we provide a conceptual clarification for some terms related to biases in this particular context. We focus mainly on nonracial biases (...) that may be less considered when addressing biases in AI systems in the existing literature. In the manuscript, we found that the existence of bias in AI systems used for COVID-19 can result in algorithmic justice and that the legal frameworks and strategies developed to prevent the apparition of bias have failed to adequately consider social determinants of health. Finally, we make some recommendations on how to include more diverse professional profiles in order to develop AI systems that increase the epistemic diversity needed to tackle AI biases during the COVID-19 pandemic and beyond. This paper proposes communities of practice (CoP) as a process to build moral resilience in healthcare settings. We introduce the starting point of moral distress that arises from ethical challenges when actions of the healthcare professional are constrained. We examine how situations such as the current COVID-19 pandemic can exponentially increase moral distress in healthcare professionals. Then, we explore how moral resilience can help cope with moral distress. We propose the term collective moral resilience to capture the shared capacity arising (...) from mutual engagement and dialogue in group settings, towards responding to individual moral distress and towards building an ethical practice environment. Background In 2021, Spain became the first Southern European country to grant and provide the right to euthanasia and medically assisted suicide. According to the law, the State has the obligation to ensure its access through the health services, which means that healthcare professionals’ participation is crucial. Nevertheless, its implementation has been uneven. Our research focuses on understanding possible ethical conflicts that shape different positions towards the practice of Medical Assistance in Dying, on identifying which core ideas may be underlying (...) them, and on suggesting possible reasons for this disparity. The knowledge acquired contributes to understanding its complexity, shedding light into ambivalent profiles and creating strategies to increase their participation. Methods We conducted an exploratory qualitative research study by means of semi-structured interviews (1 h) with 25 physicians and nurses from primary care (12), hospital care (7), and palliative care (6), 17 women and 8 men, recruited from Madrid, Catalonia, and Andalusia between March and May 2023. Interviews were recorded, transcribed, and coded in Atlas.ti software by means of thematic and interpretative methods to develop a conceptual model. Results We identified four approaches to MAiD: Full Support (FS), Conditioned Support (CS), Conditioned Rejection (CR), and Full Rejection (FR). Full Support and Full Rejection fitted the traditional for and against positions on MAiD. Nevertheless, there was a gray area in between represented by conditioned profiles, whose participation cannot be predicted beforehand. The profiles were differentiated considering their different interpretations of four core ideas: end-of-life care, religion, professional duty/deontology, and patient autonomy. These ideas can intersect, which means that participants' positions are multicausal and complex. Divergences between profiles can be explained by different sources of moral authority used in their moral reasoning and their individualistic or relational approach to autonomy. Conclusions There is ultimately no agreement but rather a coexistence of plural moral perspectives regarding MAiD among healthcare professionals. Comprehending which cases are especially difficult to evaluate or which aspects of the law are not easy to interpret will help in developing new strategies, clarifying the legal framework, or guiding moral reasoning and education with the aim of reducing unpredictable non-participations in MAID. This paper examines the influence of internal barriers on the relationship between the organizational capability of stakeholder integration and proactive environmental strategies. We adopt a moderate hierarchical regression model to test the hypotheses using data from a sample of 73 managers in the business education industry. The paper contributes to stakeholder theory by showing that stakeholder integration positively influences the development of proactive environmental strategies when managers perceive internal barriers to the development of such strategies. In this article, I argue that the main contribution of Hegel's philosophy of right to the contemporary political debate is that it opens a window on the idea that liberalism and individual freedom are incompatible. My main thesis is that the liberal conception of the State and law, structured from a nonrelational account of singularity, ends up denying the individual freedom that it claims to defend. I begin by reconstructing the Hegelian concept of freedom from its most general lines, showing (...) that being free consists in wanting to become someone and not so much in wanting to do something. After showing that the purpose of becoming someone can only be fulfilled in a social medium, I focus on Hegel's critique of the liberal state (as expounded in the section on “civil society”). I argue that civil society structurally impedes individuals from realizing their purpose of becoming someone: the economical interactions of civil society give rise to impersonal and inscrutable mechanisms, which take the social conditions of people's individuation beyond their control. Finally, I claim that (rational) individual freedom consists in being able to thematize and democratically control the very social conditions of individuation through the State and law. (shrink)

ABSTRACT The thought and social practices of Athenians during the Classical period were hierarchized in favor of males, who were considered naturally superior and endowed with the capacity and right to rule the polis and the home. The article shows how Aeschylus' Agamemnon questions the naturalization of that superiority through the character of Clytemnestra, who acts, thinks, and speaks like a man, and shows that killing a family member, changing partners, and fighting for power were justified if Agamemnon did those things. El presente artículo aborda de manera breve la obra Pasado Negro de Rubem Fonseca, escritor y guionista del cine brasileño; partiendo del análisis semiótico y lingüístico de su obra, sin dejar de lado el matiz que ofrece en la narrativa clásica del siglo XX.

El artículo sostiene una relación fundacional entre la visión bioética potteriana con el cometido de la sostenibilidad. Teniendo en cuenta que V. R. Potter no desarrolló conceptualmente cómo y desde qué enfoque tendría que entenderse la sostenibilidad de la humanidad, se hace necesario mostrar tal desarrollo conceptual. Así, la visión bioética potteriana podría ser repensada y actualizada para enfrentar el desafío de la humanidad en la época actual. En este orden de ideas, se propone analizar descriptivamente los enfoques teóricos y (...) las ampliaciones conceptuales del concepto de sostenibilidad para reconocer en qué sentido contribuyen a la construcción de una supervivencia aceptable para la humanidad, en términos de la bioética global. Para esto, se presenta el eje teórico del desarrollo sostenible, del cual se resalta su componente sistémico e intergeneracional, sin dejar de exponer las críticas a su estructura de crecimiento económico y la incompatibilidad de esto con la visión bioética potteriana. Seguidamente, se expone la propuesta alternativa del ecodesarrollo y de la racionalidad ambiental, como elementos importantes para ampliar el paradigma de la sostenibilidad. BackgroundUmbilical cord banks are a central component, as umbilical cord tissue providers, in both medical treatment and scientific research with stem cells. But, whereas the creation of umbilical cord banks is seen as successful practice, it is perceived as a risky style of play by others. This article examines and discusses the ethical, medical and legal considerations that arise from the operation of umbilical cord banks in Mexico.DiscussionA number of experts have stated that the use of umbilical cord goes beyond (...) the mere utilization of human tissues for the purpose of treatment. This tissue is also used in research studies: genetic studies, studies to evaluate the effectiveness of new antibiotics, studies to identify new proteins, etc. Meanwhile, others claim that the law and other norms for the functioning of cord banks are not consistent and are poorly defined. Some of these critics point out that the confidentiality of donor information is handled differently in different places. The fact that private cord banks offer their services as "biological insurance" in order to obtain informed consent by promising the parents that the tissue that will be stored insures the health of their child in the future raises the issue of whether the consent is freely given or given under coercion. Another consideration that must be made in relation to privately owned cord banks has to do with the ownership of the stored umbilical cord.SummaryConflicts between moral principles and economic interests (non-moral principles) cause dilemmas in the clinical practice of umbilical cord blood storage and use especially in privately owned banks. This article presents a reflection and some of the guidelines that must be followed by umbilical cord banks in order to deal with these conflicts. This essay explores an overlooked dimension of Slavoj Žižek's thought: his perspective on, and engagement with, a concrete philosophy of Trans-History. Specifically, it examines the philosophy of Trans-History presented in Surplus Enjoyment. While I do not claim that Žižek constructs a new philosophy of Trans-History in this book, I demonstrate that he directly and consistently engages with this philosophical domain and adopts several premises from Hegelian, Marxist, and Christian philosophies of Trans-History. Building on this analysis, I elucidate Žižek's stance on (...) historicism, his advocacy for the use of theological concepts to understand and safeguard contemporary history, and his interpretation of Todd Phillips’ Joker (2019) as a figure that should not be considered a sociopolitical paradigm for achieving the telos of contemporary history—namely, the unification and cooperation of global societies to avert a looming ecological catastrophe. Additionally, I challenge Žižek's praise of Joker for its portrayal of a profound and ambiguous character symbolizing "self-destructive political nihilism." I argue that this depiction of the Joker lacks originality and merit within the context of the film, as it borrows heavily from the Joker's extensive history in comics, a legacy dating back to 1940 and shaped by numerous writers. This article develops a hermeneutic study of Heidegger’s text The Word of Nietzsche: “God is Dead”. We attempt to read Heidegger’s remarks in the context of the “period of transition” that, according to Nietzsche, is occurring in the history of western thought and culture. This essay unfolds in the following manner: beginning with Heidegger’s contention that Nietzsche’s philosophy is the “fulfilment” of Platonism, we go over the problem of nihilism in relation to the metaphysics of the will to power, which (...) for Heidegger requires revising Cartesian subjectivity in search of a new ontology. Heidegger’s critique of modernity encompasses a narrative that goes from “Plato” to “Nietzsche”, leading to a reconsideration of the notions of art and truth. Finally, we attempt to interpret the meaning of the “madman’s lament” voicing the passing of God. (shrink)

Although the view that sees proper names as referential singular terms is widely considered orthodoxy, there is a growing popularity to the view that proper names are predicates. This is partly because the orthodoxy faces two anomalies that Predicativism can solve: on the one hand, proper names can have multiple bearers. But multiple bearerhood is a problem to the idea that proper names have just one individual as referent. On the other hand, as Burge noted, proper names can have predicative (...) uses. But the view that proper names are singular terms arguably does not have the resources to deal with Burge’s cases. In this paper I argue that the Predicate View of proper names is mistaken. I first argue against the syntactic evidence used to support the view and against the predicativist’s methodology of inferring a semantic account for proper names based on incomplete syntactic data. I also show that Predicativism can neither explain the behaviour of proper names in full generality, nor claim the fundamentality of predicative names. In developing my own view, however, I accept the insight that proper names in some sense express generality. Hence I propose that proper names—albeit fundamentally singular referential terms—express generality in two senses. First, by being used as predicates, since then they are true of many individuals; and second, by being referentially related to many individuals. I respond to the problem of multiple bearerhood by proposing that proper names are polyreferential, and also explain the behaviour of proper names in light of the wider phenomenon I called category change, and show how Polyreferentialism can account for all uses of proper names. Por ese motivo, el objetivo de este trabajo es que el artista revele qué hizo asequible que la transmisión de su ideal fuera bien recibida por la sociedad, sin (...) que tenga que enfrentarse a la censura o al silencio político. Más allá de ese contenido, debe destacarse que la melodía de sus canciones también hizo posible que la recepción fuera grata. However, this concept has not been sufficiently developed, neither in the area of moral philosophy nor in bioethics. For this reason, it is necessary to define and analyze the conceptual framework in which the notion of vulnerability has been developed within the scope of bioethics. Thus, the purpose of (...) this paper is to indicate how the concept of vulnerability in the field of bioethics has been developed and what the main problems that derive from this approach are. Secondly, the paper analyzes some of the main philosophical approaches that I consider should be articulated in the expansion of a critical bioethics approach focused on the notion of vulnerability. To address this issue, I analyze the reflections that we find in the work of Martha Fineman, emphasizing the main relevant aspects for bioethics. Finally, I highlight the main implications of a theory of vulnerability for bioethics. | : Le concept de vulnérabilité est au coeur des développements actuels de la bioéthique, non seulement en raison de sa nature analytique, mais aussi en raison de son potentiel critique. Cependant, ce concept n’a pas été suffisamment développé, que ce soit dans le domaine de la philosophie morale ou de la bioéthique. Pour cette raison, il est nécessaire de définir et d’analyser le cadre conceptuel dans lequel la notion de vulnérabilité a été développée en bioéthique. Ainsi, le but de cet article est d’indiquer comment le concept de vulnérabilité a été développé dans le domaine de la bioéthique et quels sont les principaux problèmes découlant de cette approche. Deuxièmement, l’article analyse certaines des principales approches philosophiques qui devraient selon moi être articulées à une approche bioéthique critique centrée sur la notion de vulnérabilité. Although AI is expected to improve efficiency and productivity, some tasks and jobs are also expected to be damaged by automation. It is necessary that companies in Colombia adopt policies and strategies that allow a fair transition towards the implementation of AI, to minimize the negative impact on employability and to take advantage of the benefits that this technology can offer.
