ObjectiveTo assess the impact of the closure of group-based cardiac rehabilitation training during the first COVID-19 lockdown in spring 2020 on patients’ physical activity, cardiorespiratory fitness, and cardiovascular risk, and to describe the patient experience of lockdown and home-based exercise training during lockdown.DesignMixed methods study. Prospectively collected post-lockdown measurements were compared to pre-lockdown medical record data. Quantitative measurements were supplemented with qualitative interviews about the patient experience during lockdown.SettingOutpatient CR centre in Salzburg, Austria.ParticipantsTwenty-seven patients [six female, mean age 69 years] (...) who attended weekly CR training sessions until the first COVID-19 lockdown in March 2020.Outcome MeasureQuantitative: exercise capacity, cardiovascular risk. Qualitative: individual semi-structured interviews.ResultsExercise capacity had significantly reduced from pre- to post-lockdown: mean power in maximal ergometry 165 vs. 151, p < 0.001; submaximal ergometer training 99 vs. 97, p = 0.038. There was no significant difference in Framingham risk score and other cardiovascular risk factors. Qualitative data showed that almost all patients had kept physically active during lockdown, but 17 said they had been unable to maintain their exercise levels, and 15 felt their cardiorespiratory fitness had deteriorated. Many patients missed the weekly CR training and the motivation and sense of community from training together with others. Several patients stated that without professional supervision they had felt less confident to carry out home-based exercise training at high intensity.ConclusionThis study highlights the importance of group-based supervised exercise training for patients who engage well in such a setting, and the detrimental impact of disruption to this type of CR service on physical activity levels and exercise capacity. Additionally, learning from the COVID-19 pandemic may inform the development and implementation of remote CR modalities going forward. (shrink)

Background: Ethical and legal issues are increasingly being reported by health caregivers; however, little is known about the nature of these issues in geriatric care. These issues can improve work and care conditions in healthcare, and consequently, the health and welfare of older people. Aim: This literature review aims to identify research focusing on ethical and legal issues in geriatric care, in order to give nurses and other health care workers an overview of existing grievances and possible solutions to take (...) care of old patients in a both ethical and legally correct way. Methods: Using a systematic approach based on Aveyard, a search of the PubMed, CINAHL, and Ethicshare databases was conducted to find out the articles published on ethical and legal issues in geriatric care. Ethical considerations: The approval for the study was obtained from UMIT-The Health and Life Sciences University, Austria. Results: Only 50 articles were included for systematic analysis reporting ethical and legal issues in the geriatric care. The results presented in this article showed that the main ethical issues were related to the older people’s autonomy, respect for their needs, wishes and values, and respect for their decision-making. The main legal issues were related to patients’ rights, advance directives, elderly rights, treatment nutrition dilemma, and autonomy. Conclusion: Further education for professional caregivers, elderly people, and their families is needed on following topics: care planning, directive and living wills, and caregiver-family member relationships to guide and support the elderly people within their decision-making processes and during the end-of-life care. Background The ethical governance of Artificial Intelligence (AI) in health care and public health continues to be an urgent issue for attention in policy, research, and practice. In this paper we report on central themes related to challenges and strategies for promoting ethics in research involving AI in global health, arising from the Global Forum on Bioethics in Research (GFBR), held in Cape Town, South Africa in November 2022. Methods The GFBR is an annual meeting organized by the World Health (...) Organization and supported by the Wellcome Trust, the US National Institutes of Health, the UK Medical Research Council (MRC) and the South African MRC. The forum aims to bring together ethicists, researchers, policymakers, research ethics committee members and other actors to engage with challenges and opportunities specifically related to research ethics. In 2022 the focus of the GFBR was “Ethics of AI in Global Health Research”. The forum consisted of 6 case study presentations, 16 governance presentations, and a series of small group and large group discussions. A total of 87 participants attended the forum from 31 countries around the world, representing disciplines of bioethics, AI, health policy, health professional practice, research funding, and bioinformatics. In this paper, we highlight central insights arising from GFBR 2022. Results We describe the significance of four thematic insights arising from the forum: (1) Appropriateness of building AI, (2) Transferability of AI systems, (3) Accountability for AI decision-making and outcomes, and (4) Individual consent. We then describe eight recommendations for governance leaders to enhance the ethical governance of AI in global health research, addressing issues such as AI impact assessments, environmental values, and fair partnerships. Conclusions The 2022 Global Forum on Bioethics in Research illustrated several innovations in ethical governance of AI for global health research, as well as several areas in need of urgent attention internationally. This summary is intended to inform international and domestic efforts to strengthen research ethics and support the evolution of governance leadership to meet the demands of AI in global health research. Se propone revisar la elaboración del concepto de piñen examinando la relación entre lenguaje y colonialidad, para, de ese modo, subrayar el potencial político y reivindicativo que comporta en la novela del mismo nombre, Piñen (2019). Dicha hipótesis articulará el ejercicio de lectura aquí propuesto, atendiendo principalmente las estrategias narrativas empleadas –servirse de un polilingüismo que rompe estructuras gramaticales– para descentrar el lenguaje de su determinismo colonial. Scientists have used models for hundreds of years as a means of describing phenomena and as a basis for further analogy. In Scientific Models in Philosophy of Science, Daniela Bailer-Jones assembles an original and comprehensive philosophical analysis of how models have been used and interpreted in both historical and contemporary contexts. Bailer-Jones delineates the many forms models can take (ranging from equations to animals; from physical objects to theoretical constructs), and how they are put to use. She examines early (...) mechanical models employed by nineteenth-century physicists such as Kelvin and Maxwell, describes their roots in the mathematical principles of Newton and others, and compares them to contemporary mechanistic approaches. Bailer-Jones then views the use of analogy in the late nineteenth century as a means of understanding models and to link different branches of science. She reveals how analogies can also be models themselves, or can help to create them. The first half of the twentieth century saw little mention of models in the literature of logical empiricism. Focusing primarily on theory, logical empiricists believed that models were of temporary importance, flawed, and awaiting correction. The later contesting of logical empiricism, particularly the hypothetico-deductive account of theories, by philosophers such as Mary Hesse, sparked a renewed interest in the importance of models during the 1950s that continues to this day. Bailer-Jones analyzes subsequent propositions of: models as metaphors; Kuhn's concept of a paradigm; the Semantic View of theories; and the case study approaches of Cartwright and Morrison, among others. She then engages current debates on topics such as phenomena versus data, the distinctions between models and theories, the concepts of representation and realism, and the discerning of falsities in models. ZusammenfassungDie finanziellen Ressourcen im deutschen Gesundheitssystem sind begrenzt. Diese Mittelknappheit führt im Rahmen der ärztlichen Tätigkeit zu medizinischen, ökonomischen, juristischen und ethischen Problemen, welche sich in den kommenden Jahren weiter verschärfen dürften. Aus ethischer Perspektive sind die Probleme einer gerechten Verteilung knapper Ressourcen sowie mögliche Rollen- oder Gewissenskonflikte der ärztlichen Profession besonders relevant. Mit Hilfe von qualitativen Interviewstudien lässt sich der aktuelle ärztliche Umgang mit der Mittelknappheit in der klinischen Versorgung in seiner Komplexität und seinen ethisch relevanten Aspekten untersuchen. An (...) fünf Kliniken mit unterschiedlichem Versorgungsauftrag wurden Tiefeninterviews mit fünfzehn leitenden Ärzten in den Bereichen Intensivmedizin und Kardiologie durchgeführt. Die auf Tonband aufgezeichneten Interviews wurden nach dem Verfahren der Grounded Theory ausgewertet. Die Ärzte berichten ein heterogenes Spektrum von Einflussfaktoren, Konsequenzen und Reaktionen im Zusammenhang mit ihrem Handeln bei Mittelknappheit. Zuteilungskriterien wie der medizinische Nutzen oder die Kosteneffektivität werden von den befragten Ärzten sehr unterschiedlich interpretiert und führen damit möglicherweise zu inkonsistenten Allokationsentscheidungen. Gleichzeitig bestätigen die Untersuchungen aber auch, dass die Ärzte nicht leichtfertig mit den Allokationsentscheidungen umgehen, im Gegenteil: Die Mittelknappheit und die dadurch hervorgerufenen Verteilungsprobleme können zu Gewissenskonflikten, emotionalem Stress und Gefühlen der Überforderung führen. Um Lösungskonzepte für ethische Probleme im Umgang mit der Mittelknappheit in der klinischen Praxis entwickeln und erfolgreich implementieren zu können, müssen die in dieser Arbeit systematisch zusammengestellten Verständnisweisen, Einflussfaktoren und Konsequenzen im Zusammenhang mit dem Kostendruck berücksichtigt werden. Emotional stimuli (e.g. words, images) are often remembered better than neutral stimuli. However, little is known about how memory is affected by an environmentally induced emotional state (without any overtly emotional occurrences) – the focus of this study. Participants were randomly assigned to discovery (n = 305) and replication (n = 306) subsamples and viewed a desktop virtual environment before rating their emotions and completing objective (i.e. item, temporal-order, duration) and subjective (e.g. vividness, sensory detail, coherence) memory measures. In both (...) samples, a Partial Least Squares Correlation analysis showed that an emotional state characterised by high negative emotion (i.e. threat, fear, anxiety) and arousal was reliably associated with better memory in both objective (i.e. item) and subjective (i.e. vividness and sensory detail) domains. No reliable associations were observed for any temporal memory measures (objective or subjective). Thus, an environmentally induced state of negative emotion corresponds with enhanced memory for indices of episodic memory pertaining to “what” happened, but not necessarily “when” it happened. In this article we propose to return to the notions of public and universality in the so-called Critical Theory, in order to rethink the relation between politics, affects and women. For these purposes, we will analyze the famous The Structural Transformation of the Public Sphere of J. Habermas, the first systematization of the notion of public sphere, understood as the scope of rational and universal debate which excludes the private-affective. Later, we will focus on the criticism of this study made (...) by the feminist revisionism of the eighties. These authors found a connection between the theoretical exclusion of the affects and the political expulsion of women. We will see that Habermas’ own Theory of Communicative Action opens the possibility of thinking the rationality present in the expression of affections, desires and feelings. The main aim of this work is to start a tradition of reflections for a truly universal public sphere theory. (shrink)

Nature connectedness is determined by the representation individuals have about themselves within nature. This concept is often studied in relation to the direct contact individuals have with natural environment, which according to some studies have demonstrated to generate positive effects by fostering a feeling of connecting and bonding with nature, as well as improving their wellbeing. The main focus of this study was to calculate and assess the relation between Nature Connectedness and wellbeing of participants. The methodological approach of this (...) research reaches quantitative data comparing results obtained from both samples, as well as correlations between the variables. The sample for this study was composed by two groups of university students. Both contrast group and experience group filled the questionnaire in two separate moments and in different environments. First data collection moment for both groups was held inside a university classroom. A second moment of data collection was carried out after a month from the first application, having the contrast group answer the questionnaire on a classroom again whilst the experience group responded it during an excursion to Isla Del Tiburon in Northwestern Mexico after performing some recreational activities being totally immersed in a local desertic environment. Questionnaire was composed by a 6 point Likert type scale measuring Nature Connectedness through concepts such as Nature relatedness and Love and care for the natural, as well as Subjective and Psychological Wellbeing of participants. Results show that both wellbeing and Nature Connectedness are positively influenced by performing activities out in the natural environment. This work was also conducted in response to the need to understand the full extent of Contact and Connectedness to nature, carrying out an exploratory study in desertic settings when much of the early work centers around the study of these variables in green nature environments. Background: As a non–European Union member state, Albania is increasingly orienting itself on Western models regarding human rights, patient rights, and legal regulations for healthcare. Due to its limited fiscal and legal power, enforcing legal and ethical regulations poses a major problem. Aim: The aim of this study is to investigate nurse’s knowledge and experiences regarding ethical and legal issues in Albanian elder care in state-funded and privately run institutions. Research design: The study was conducted using an inductive and qualitative (...) design, utilizing a focused ethnographic approach, based on Roper and Shapira’s framework. Method: Data were collected between June 2017 and September 2018 using participant observation, field notes, and semi-structured interviews with 15 nurses in seven different elder care institutions. In total, 100 h of observation and 15 interviews were performed. Data analysis was based on Mayring’s qualitative content analysis. Ethical consideration: The approval for the study was obtained from UMIT—The Health and Life Sciences University, Austria. Findings: The findings of the study fell into the following main categories: “Everyday care issues,” “End-of-life issues,” “Legal issues,” and “Ethical-legal education and conflicts.” Discussion: The participants reported many ethical and legal issues when describing their everyday challenges and displayed a strong lack of ethical and legal education. Despite a wide spectrum in the quality of care between private and state-funded nursing homes, older people mostly do not know their own diagnosis. Conclusion: This study indicates that further ethical and legal education is needed. Furthermore, nurses need to be better prepared for ethical conflicts with families, as strengthening patient rights could come into conflict with traditional rights of the Albanian family. (shrink)

Purpose: To explore the characteristics of the use of laughs and jokes during doctor-couple assisted reproductive technology visits.Methods: 75 videotaped doctor-couple ART visits were analyzed and transcribed in order to: quantify laugh and jokes, describing the contribution of doctors and couples and identifying the timing of appearance; explore the topic of laughs and jokes with qualitative thematic analysis.Results: On average, each visit contained 17.1 utterances of laughs and jokes. Patients contributed for 64.7% of utterances recorded. Doctor and women introduced the (...) majority of laughs and jokes. Visits with female physicians had significantly more laughs and jokes than visits with male doctors; no differences were found considering physicians’ age and years of experience, cause of infertility, and prognosis. Laughs and jokes were mainly recorded during history taking and information giving. Four core themes were identified, regarding the topic of laughs and jokes: health status, infertility treatment, organizational aspects, and doctor-patient interaction.Conclusion: Laughs and jokes are common in doctor-couple ART visits and are frequently used during the dialogue, covering a wide range of topics. Results seem to show that laughs and jokes are related to doctor’s personal characteristics, while are not associated with infertility aspects. Given the complexity of this communicative category, further studies are needed to explore the functions and the effects of laugh and jokes. (shrink)

Primary progressive multiple sclerosis (PPMS) shows a highly variable disease progression with poor prognosis and a characteristic accumulation of disabilities in patients. These hallmarks of PPMS make it difficult to diagnose and currently impossible to efficiently treat. This study aimed to identify plasma metabolite profiles that allow diagnosis of PPMS and its differentiation from the relapsing-remitting subtype (RRMS), primary neurodegenerative disease (Parkinson’s disease, PD), and healthy controls (HCs) and that significantly change during the disease course and could serve as surrogate (...) markers of multiple sclerosis (MS)-associated neurodegeneration over time. We applied untargeted high-resolution metabolomics to plasma samples to identify PPMS-specific signatures, validated our findings in independent sex- and age-matched PPMS and HC cohorts and built discriminatory models by partial least square discriminant analysis (PLS-DA). This signature was compared to sex- and age-matched RRMS patients, to patients with PD and HC. Finally, we investigated these metabolites in a longitudinal cohort of PPMS patients over a 24-month period. PLS-DA yielded predictive models for classification along with a set of 20 PPMS-specific informative metabolite markers. These metabolites suggest disease-specific alterations in glycerophospholipid and linoleic acid pathways. Notably, the glycerophospholipid LysoPC(20:0) significantly decreased during the observation period. These findings show potential for diagnosis and disease course monitoring, and might serve as biomarkers to assess treatment efficacy in future clinical trials for neuroprotective MS therapies. The “extreme consequence” is a very common pattern in advertising messages that presents an odd, even negative, situation resulting from the use of the advertised product as a good reason to buy it. By analyzing selected advertisements employing this pattern using the conceptual integration theory and the Argumentum Model of Topics, we aim to understand how “extreme consequence” works at the rhetorical and argumentative levels. The analyses allow us to detect the typical, generic, cognitive, and argumentative structure underlying the pattern (...) and to identify the rhetorical and argumentative role played by its main components, namely exaggeration and cause-effect relation. The teaching of religion in public schools – whether the subject should or should not be included in the school curricula, what the content structure should be and which approach the teacher should adopt – led to various ethical dilemmas and conflicts in many regions of the world. Our article aims at reviewing, from the perspectives of numerous authors, the different topics as well as the ways in which aspects related to the impact of religious teaching and to specific approaches (...) could be taught to economics students in a democratic society. At the same time, we underline different dilemmas and preoccupations resulting from religious values in the organizational management and in marketing, but also the synergies that could be capitalized from this standpoint, in order to obtain a competitive advantage in a context where diversity, and religious diversity in particular, is a reality that gets more and more obvious, while a good capitalization of it can bring forth loyalty in organizations and competitive advantages on the market. There are few doubts about the significance of science and technology for modern human culture and society. But as historians, we are still struggling to find appropriate descriptive terms to capture the broad processes of transformation brought about by “techno-science,” the merging of technical production and modern institutionalized science. This dissertation argues that the term “experience” may serve as such an analytic lens in the specific historical setting of German aviation research from the 1920s through 1945. I reconstruct, on the (...) one hand, the theorization of experience as a concept by the technical physicist Paul von Handel, influenced by the British astronomer Sir Arthur Eddington’s distinction between "scientific" and "everyday" experience. On the other hand, I use the term as a historian’s analytic concept to investigate practices in the context of flight experiments that I take to be constitutive of my historical actors’ experiences. These are recordings of experimental pilots’ cognitive judgements and bodily actions, some of them—such as in-flight note taking—continuous with older cultural technologies. On both of these levels of analysis, I explore the different resonances of “experience” as a term with a legacy as a central epistemological concept in the modern sciences, and as capturing the changing everyday reality in an increasingly technicized environment. My analysis of the textual theorization and simultaneous practical constitution of "techno-scientific experience" serves to read in a new light the story of the pilot and physicist Melitta Schiller-Stauffenberg. Of Jewish descent, Schiller chose to work for the Luftwaffe, the German air force, until her death in 1945 on a flight searching for her husband, Count Alexander Stauffenberg, who was imprisoned after his brother’s failed attempt to assassinate Hitler. The concept and practical reality of “experience” are key to understanding the two striking choices Schiller made as intrinsically connected: the professional choice of working simultaneously as a pilot and a physicist, and the political choice of supporting the Reich’s war effort. Schiller’s story may be understood as exemplifying the fragile identity of the experiencing and the knowing self in 20th-century techno-scientific modernity. (shrink)

This paper intends to make a revision of the academic literature that focuses social responsibility from a strategic view. In line with the previous ideas, the aim of this paper is to add itself to the group of researches that conceive CSR as an integral part of the business strategy. For this purpose it focuses on studying those relationships that may exist between the firm’s Business Strategy (BS) and the Social Strategy (SS). The paper examines two possible analyses of fictional names within Pavel Tichý’s Transparent Intensional Logic. The first of them is the analysis actually proposed by Tichý in his (1988) book The Foundations of Frege’s Logic. He analysed fictional names in terms of free variables. I will introduce, explain, and assess this analysis. Subsequently, I will explain Tichý’s notion of individual role (office, thing-to-be). On the basis of this notion, I will outline and defend the second analysis of fictional names. Kulicki and Trypuz (2016) introduced three systems of multivalued deontic action logic to handle normative conflicts. The first system suggests a pessimistic view on normative conflicts, according to which any conflicting option represents something forbidden; the second system suggests an optimistic view, according to which any conflicting option represents something obligatory; finally, the third system suggests a neutral view, according to which any conflicting option represents something that is neither obligatory nor forbidden. The aim of the present paper is to (...) propose a fourth system in this family, which comes with a realistic view on normative conflicts: a normative conflict remains unsolved unless it is generated by two or more normative sources that can be compared. In accordance with this, we will provide a more refined formal framework for the family of systems at issue, which allows for explicit reference to sources of norms. Voi ilustra schimbările sociale şi legislative în ce priveşte acceptarea statutului moral al copiilor şi a egalității morale între copii şi adulți, folosind exemple din România şi Suedia. Voi examina dinamica dintre aspirațiile legale şi morale la care aderă România în ce priveşte statutul moral al copiilor şi normele sociale despre copii, copilărie, şi familie, care nu sunt întotdeauna în acord cu aceste aspirații. Die Medizin der Gegenwart ist durch eine Reihe von Tendenzen charakterisiert, die dazu fuhren, dass sich das traditionelle Verstandnis der Medizin als blosser Heilkunde nicht mehr von selbst versteht. Zu nennen sind hier etwa die oft beklagte Okonomisierung der Medizin oder der Trend zur wunscherfullenden Medizin. Angesichts dieser Veranderungen gewinnt die theoretische Frage nach dem Wesen der Medizin an praktischer Bedeutung. Der Band "Was ist Medizin?" beantwortet die Frage aus verschiedenen Perspektiven. Beitrage aus der Geschichte der Medizin informieren uber Konstanten (...) und Wandlungen im Verstandnis der Medizin. Medizin- und wissenschaftstheoretische Auseinandersetzungen beschaftigen sich mit der Frage, was fur eine Art von Wissenschaft die Medizin ist. Dabei gilt ihr Augenmerk u. a. der Spannung zwischen der wissenschaftlichen Forschung und der klinischen Praxis. Ausserdem werden aus der Sicht der Ethik die vieldiskutierten Fragen erortert, ob die Medizin ein inharentes Ethos aufweist, worin dieses bestehen konnte und ob es einen Beitrag zur Klarung medizinethischer Probleme leisten kann. In this book, Gobetti combines political theory with the history of political thought to question the conceptual conventions and tacit assumptions which surround the concepts of private and public. In seeking the foundations of the modern liberal conception of private and public, she traces it to modern Natural Law thinkers, in particular Locke and Hutcheson. By developing a revised interpretation of 17th century natural jurisprudence, which recognizes that every adult controls an individual or private domain, as well as engaging in (...) political, community or public interaction, Gobetti raises interesting questions about the politics of participation in modern society. Die Frage, ob eine emphatische Bezugnahme auf nachhaltige Entwicklung in kritisch-emanzipatorischer Absicht uberhaupt noch moglich ist oder ob es nicht besser ware, sich von der Leerformel Nachhaltigkeit zu verabschieden, bildet den Ausgangspunkt des Bandes. Die Autorin analysiert die Gerechtigkeits-, Politik- und Okonomieverstandnisse verschiedener Strange im Nachhaltigkeitsdiskurs (politisch-institutionelle, feministische, herrschaftskritische und integrative) und zeigt, dass in ihnen ein emanzipatorisches Potenzial steckt. Dieses arbeitet sie mit Hilfe eines feministisch-gepragten, diskursanalytischen Ansatzes heraus. Die Verfasserin leistet damit einen Beitrag, Nachhaltigkeit neu und anders zu (...) denken und zieht dabei die Erkenntnisse der kritischen Debatten, die in den vergangenen zwanzig Jahren uber dieses Neu-, Anders- und Weiterdenken gefuhrt worden sind, kenntnisreich mit ein. Die Arbeit wurde mit dem Christiane Busch-Luty Forderpreis der Vereinigung fur Okologische Okonomie ausgezeichnet. (shrink)

"Theory of mind� describes the ability to impute mental states, such as beliefs, desires and intentions, to oneself and to other people. Usually we want to know why people did what they did and we wonder what they are going to do next. That is, everyday we try to predict and explain human behaviour. In order to do this we refer to a person"s beliefs, desires, emotions, intentions etc. Behaviour is the product of belief and desire: People do things because (...) they desire something and believe some act will achieve it. For adults it is clear that our assumptions about reality do not necessarily match the real world; but we know, regardless of whether our beliefs are true or not, our beliefs direct our actions. In contrast, it is not until the age of 4 years that children understand that one can be mistaken about the world and that actions can be based on this false belief. (shrink)
