The allocation of any scarce health care resource, especially a lifesaving resource, can create profound ethical and legal challenges. Liver transplant allocation currently is based upon urgency, a sickest-first approach, and does not utilize capacity to benefit. While urgency can be described reasonably well with the MELD system, benefit encompasses multiple dimensions of patients’ well-being. Currently, the balance between both principles is ill-defined. This survey with 502 participants examines how urgency and benefit are weighted by different stakeholders. Liver transplant patients (...) favored the sickest-first allocation, although all other groups tended to favor benefit. Criteria of a successful transplantation were a minimum survival of at least 1 year and recovery of functional status to being ambulatory and capable of all self-care. An individual delisting decision was accepted when the 1-year survival probability would fall below 50%. Benefit was found to be a critical variable that may also trigger the willingness to donate organs. The strong interest of stakeholder for successful liver transplants is inadequately translated into current allocation rules. (shrink)

Residências Integradas Multiprofissionais em Saúde são programas de pós-graduação caracterizados por ensino em serviço. A ULBRA oferece o programa com ênfase em Saúde do Adulto e do Idoso e Saúde Comunitária. Objetiva-se descrever as atividades realizadas em ambiente hospitalar por enfermeiras. Trata-se de um relato de experiência de enfermeiras residentes em relação ao primeiro ano de formação especializada. O primeiro ano de residência abrangeu atividades em unidades de internação, centro cirúrgico e unidade de tratamento intensivo. Diversas foram as atividades desenvolvidas (...) pelas residentes, abrangendo aperfeiçoamento técnico, produção de conhecimento científico e atuação em gestão e gerenciamento. A presença de tutores e preceptores é fundamental no sentido de discutir, orientar e facilitar a inserção e o desenvolvimento profissional do residente. O programa de residência permitiu aperfeiçoar habilidades nos três campos de atuação da categoria: assistência, gerenciamento e pesquisa. (shrink)

Primary progressive multiple sclerosis (PPMS) shows a highly variable disease progression with poor prognosis and a characteristic accumulation of disabilities in patients. These hallmarks of PPMS make it difficult to diagnose and currently impossible to efficiently treat. This study aimed to identify plasma metabolite profiles that allow diagnosis of PPMS and its differentiation from the relapsing-remitting subtype (RRMS), primary neurodegenerative disease (Parkinson’s disease, PD), and healthy controls (HCs) and that significantly change during the disease course and could serve as surrogate (...) markers of multiple sclerosis (MS)-associated neurodegeneration over time. We applied untargeted high-resolution metabolomics to plasma samples to identify PPMS-specific signatures, validated our findings in independent sex- and age-matched PPMS and HC cohorts and built discriminatory models by partial least square discriminant analysis (PLS-DA). This signature was compared to sex- and age-matched RRMS patients, to patients with PD and HC. Finally, we investigated these metabolites in a longitudinal cohort of PPMS patients over a 24-month period. PLS-DA yielded predictive models for classification along with a set of 20 PPMS-specific informative metabolite markers. These metabolites suggest disease-specific alterations in glycerophospholipid and linoleic acid pathways. Notably, the glycerophospholipid LysoPC(20:0) significantly decreased during the observation period. These findings show potential for diagnosis and disease course monitoring, and might serve as biomarkers to assess treatment efficacy in future clinical trials for neuroprotective MS therapies. Resumen: Dada la multiplicidad de planos que acompañan a las observaciones de la actual pandemia, importa identificar aquellas que alcanzan una mayor resonancia. Así, observamos cómo los ciudadanos y los agentes de decisión han incrementado sus demandas de informaciones validadas por medio de la aplicación de la racionalidad, los métodos y los procedimientos de la ciencia de manera exponencial. En este trabajo desarrollamos en forma sintética un conjunto seleccionado de lineamientos sobre cómo se aborda la actual pandemia desde las ciencias (...) sociales sistémicas, para luego exponer algunas de sus propuestas respecto a las condiciones, a nivel organizacional, las cuales podrían permitir alcanzar los niveles de diálogo y cooperación requeridos para enfrentar la pandemia actual.: Given the multiplicity of perspectives that accompany the observation of the current pandemic, becomes relevant to identify those that achieve the greatest resonance. Thus, it is observed how citizens and decision makers have increased their demands for valid information by applying rationality, methods, and procedures of science in an exponential manner. In this paper we develop a synthetic set of selected guidelines on how the current pandemic is approached from systemic social sciences, and then present some of their proposals regarding the conditions, at the organizational level, which could enable the achievement of the levels of dialogue and cooperation required to address the current pandemic. The changes and general alarm of the current COVID-19 pandemic have amplified the sense of precariousness and vulnerability for family members who, in addition to the emotional trauma of the cancer diagnosis, add the distress and fear of the risks associated with infection. The primary objectives of the present study were to investigate the psychological impact of the COVID-19 pandemic on the parents of pediatric cancer patients, and the level of stress, anxiety, and the child’s quality of life perceived by (...) the parents during the COVID-19 epidemic. The parents of 45 consecutive children with solid and hematological tumors were enrolled. Four questionnaires were administered to the parents at the beginning of the pandemic lockdown. A 75% of parents exhibited remarkable levels of anxiety, with 60 subjects in state scale and 45 subjects in trait scale having scores that reached and exceeded the STAI-Y cut off. The bivariate matrix of correlation found a significant positive correlation between the IES-R and PSS scores. There was a positive correlation between the PSS and PedsQL scale and a negative correlation between IES-R and STAI-Y. The results confirm that parents of pediatric cancer patients have a high psychological risk for post-traumatic symptoms, high stress levels, and the presence of clinically significant levels of anxiety. Jewish law has a history stretching from the early period to the modern State of Israel, encompassing the Talmud, Geonic and later codifications, the Spanish Golden Age, medieval and modern responsa, the Holocaust and modern reforms. Fifteen distinct periods are separately studied in this volume, each one by a leading specialist, and the emphasis throughout is on the development of the institutions and sources of the law.

We provide results allowing to state, by the simple inspection of suitable classes of posets , that the corresponding intermediate propositional logics are maximal among the ones which satisfy the disjunction property. Starting from these results, we directly exhibit, without using the axiom of choice, the Kripke frames semantics of 2No maximal intermediate propositional logics with the disjunction property. This improves previous evaluations, giving rise to the same conclusion but made with an essential use of the axiom of choice, of (...) the cardinality of the set of the maximal intermediate propositional logics with the disjunction property. Objectives: While previous research has mainly focused on the impact of the first acute phase of the COVID-19 pandemic on mental health, little empirical knowledge exists about depression, anxiety, and somatic symptom levels and possible predictors of symptom levels in the pandemic’s recovery phase. The present study aimed to analyze the mental burden of a convenience ample of the general German population during the first recovery phase of the pandemic and to identify significant predictors of symptom levels.Methods: Standardized measures of (...) anxiety, depression, somatic symptoms, and health anxiety, as well as measures of COVID-19 fears and possible vulnerability factors, were administered through a national, cross-sectional online survey, asking participants for their current symptom levels and their symptom levels prior to the COVID-19 pandemic.Results: Our findings show significantly elevated levels of depression, anxiety, somatic symptoms, and health anxiety in the recovery period compared to before the pandemic. The current prevalence rates based on self-reporting were 26.7% for depression, 24.5% for anxiety, and 29% for somatization. The strongest predictors of these symptom reports included domain-specific pre-existing symptom levels, neuroticism, biological COVID-19 risk factors, avoidance of illness information, and younger age. The most important predictors of COVID-19 fears were subjective COVID-19 risk perception, followed by pre-existing health anxiety, the number of biological COVID-19 risk factors, older age, neuroticism, avoidance of illness information and female gender.Discussion: These findings indicate the need for specific psychological programs to help individuals with enhanced psychological and biological vulnerability to cope better with the mental distress experienced during all phases of the ongoing COVID-19 crisis. We propose to present the clinical case of a woman diagnosed with Borderline Personality Disorder. The descriptive and comprehensive analysis of his biography and the psychotherapeutic relationship is carried out against the background of existential psychoanalysis and the progressive-regressive method formulated by Jean-Paul Sartre. The need for the psychotherapist to have a reflective and critical awareness of how the relationship with the client is established is the fundamental point, considering that it is a dialectical relationship and being under the same (...) aegis of the ambivalence reported by the philosopher to explain how human reality is, in phenomenological-hermeneutic terms, constructed. The detailing of the case promotes reflections, understanding, in practice, the existential and phenomenological project of being. Finally, the study leads us to reflect that the procedures with these psychopathological cases require much more than the professional role of a psychotherapist ordinarily fulfills, because it is only when one manages to be aware of their experience in the relationship, that one can hermeneutically situate how the client apprehends herself originally and existentially. It is widely believed that we ought not to criticize others for wrongs that we ourselves have committed. The author draws out and challenges some of the background assumptions about the practice of criticism that underlie our attraction to this claim, such as the tendency to think of criticism either as a social sanction or as a didactic intervention. The author goes on to offer a taxonomy of cases in which the moral legitimacy of criticism is challenged on the grounds (...) that the critic him- or herself engages in the behavior that he or she criticizes in others. The author argues that, in each type of case, the would-be critics should not constrain their participation in moral discourse on the grounds that they are not themselves innocent of the wrongdoing they criticize in others. This study problematizes English language teaching departments in Turkey that have ignored the importance of radical pedagogy, the history of British colonialism and neoliberalism in the curriculum because Orientalist, Occidentalist and neoliberal discourses have led to the exclusion of critical discourses in ELT in Turkey. Therefore, the possible reasons for the absence of some curricular topics present a complicated structural problem. Exclusionary practices of ELT departments can be ascribed to Turkey’s political regimes that have reinforced both nation-state ideology and Anglo-American (...) neoliberal policies. The English language has also been reified as a gateway to westernization and globalization under the feeling of urgency and fear of falling behind. Therefore, the spread of English has been allowed in almost all spheres of life even if neglecting local, national and international languages has violated linguistic human rights. This paper proposes that the dominance of English can be deconstructed by including radical pedagogy and the history of British colonialism and neoliberalism into the curriculum and by prioritizing linguistic human rights to allow more space for the survival of other languages. The study's goal was to examine the socially responsible power use in the context of ethical leadership as an explanatory mechanism of the ethical leadership-follower outcomes link. Drawing on the attachment theory (Bowlby, 1969/1982 ), we explored a power-based process model, which assumes that a leader's personal power is an intervening variable in the relationship between ethical leadership and follower outcomes, while incorporating the moderating role of followers' moral identity in this transformation process. The results of a two-wave field study (...) ( N = 235) that surveyed employees and a scenario experiment ( N = 169) fully supported the proposed (moderated) mediation models, as personal power mediated the positive relationship between ethical leadership and a broad range of tested follower outcomes (i.e., leader effectiveness, follower extra effort, organizational commitment, job satisfaction, and work engagement), as well as the interactive effects of ethical leadership and follower moral identity on these follower outcomes. Al-Mu'jam Al-muḫtaṣ of Murtadā Al-Zabīdī (d. 1205/1791) in which a scholar recorded his teachers, the books that he obtained the right to narrate during his education life and the chain of transmission from his teachers to the author, is one of the important representatives of the mu'jam genres in 12th/18th century. It can be said that the mu'jam that Zabīdī wrote to record the important people in his life, goes beyond the standard mu'jam with its compilation style, sources, diversity of (...) biographers and anecdotes about the writing activity of the period. Based on Zabīdī's teachers, students and contemporaries, their origins, specialization and profession, the dates of their deaths, the dates of his encounters with them, the extent of his friendships, the dated rihle route, the lecture places and the books he taught, a panorama about the mu'jam will be drawn. The mu’jam, which includes 635 biographies, has a wide range of written and oral sources, especially ijazat and samā sources. Zabīdī's correspondence with scholars from different regions, taqrīz/appreciation written for his famous books and poems belonging to scholars of the period are among these sources. Numerous taqrīzs on his Tāj al-ʿArūs and Sharḥ Al-Iḥyā's point to their fame at that time, while Mu’jam, which has the appearance of an independent poetry anthology due to the poems that reach tens of pages, reveals his unknown poetic side. On the other hand, some couplets recorded in Persian and Turkish show his knowledge of these languages. Zabīdī is a versatile personality who lived in different geographies such as India, Yemen and Egypt and met many scholars there. Rihles in different regions of Yemen such as Zabid, Moravıa and Mocha, as well as inside Egypt, including Damietta, Raşıt, Fuwwah and Asyut, show that Zabīdī traveled frequently for the sake of knowledge. Egypt, where he went in 1167 (1753) with the advice of his teacher al-Aydarūs and spent thirty-eight years of his life, has an important place in his scientific adventure. Recording sufis, merchants and janitors as well as famous people from different fields such as poets, jurists and muhaddis in biographies reflects Zabīdī's extensive contact reaching various segments of society through the aforementioned rihles. While Zabīdī was obtaining knowledge through both these journeys and ijazat, he also gave ratification/ijazat to the students who came to him. While he prefers ijazat of al-musalsal bil awwaliyyah and Al-Awail Ṣaḥīḥ/al-Kutub al-Sittah for those who come to him for short periods of time on the occasion of pilgrimage, but on the other hand he gaves ijazat of Ṣaḥīḥ al-Bukhari, Shamail Tirmidhi, Sharḥ Al-Iḥyā and Al-Amali for his long-term students. Lessons, both open to the public in mosques and with more private groups at home, were held in informal rather than systematic/formal style like in the madrasah. The books that Zabīdī received the right to transmit through his rihles are listed below under the "textbooks" heading. When a book is mentioned in whichever biography, it can be implicitly considered the person has the elevated chain of transmission of the book. Similarly, it can be revealed in which region and by whom tranmits the short chain of a book. This reveals the unknown aspects of regional and temporal writing style. On the other hand, it is possible to find other traces of the background of writing and representation/taqrir through the mu'jam. For instance, a book was sometimes typed as a result of the students' compilation of lecture notes of their masters, sometimes when a person obtained a book he needed and he abbreviated important parts and copied a special copy for himself or a student could only get permission for giving lectures after he had written a book. The mu’jam includes besides of Zabīdī's books, the books written by the biographers in the form of "he has many books" or by mentioning the name directly. Even the recording of details such as the reason for writing, date and content information reveals the unknown aspects of the book writing style of the period. Ultimately, the importance of mu’jam, which was written as the result of a life spent with learning, although it is not well known in terms of genre; is to mention the biographical information collected from many different secondary sources, compiled as a result of long effort, such as history and tabaqat-tarajim about the scientific life of the author together with their evidences. Therefore, these kinds of books, that are mostly written by the author himself or his close student, in which the teacher constitutes its main element, is a primary source in the determination of the biographical information of the author, as well as the book and isnad, deserve to be examined separately because of their potential to shed light on the unknown aspects of the hadith history and literature of the later period. This paper explores the perceptual space of Thomas A. Clark’s poetry and its links with the long and influential Western literary and artistic traditions of walking in the landscape, from Romanticism to Land Art. Particular attention will be given to the relations that Clark establishes in his writing between walking as a bodily practice and the multi-sensory engagement with the landscape it provides. It will be shown that Clark’s most significant contribution to the literature of walking lies in the balance (...) he creates between movement and stillness. In this dynamics, walking is envisaged as means of contemplation and communion to be found within the body, but which is at the same time directed towards the world, and the landscape appears as an infinite variety of perspectival views and as a dynamic process of discovery. Physicians working in the world of competitive sports face unique ethical challenges, many of which center around conflicts of interest. Team-employed physicians have obligations to act in the club's best interest while caring for the individual athlete. As such, they must balance issues like protecting versus sharing health information, as well as issues regarding autonomous informed consent versus paternalistic decision making in determining whether an athlete may compete safely. Moreover, the physician has to deal with an athlete's decisions about performance (...) enhancement and return to play, pursuit of which may not be in the athlete's long-term best interests but may benefit the athlete and team in the short term. These difficult tasks are complicated by the lack of evidence-based standards in a field influenced by the lure of financial gains for multiple parties involved. It has been argued that, when there is a probable imminent risk of loss of children’s fertility, their parents should take active steps to preserve their reproductive potential if possible – or even that children have a right to such interventions being undertaken on them on their behalf, as an expression of their right to an open future. In this chapter, I explore these proposals and some of their implications. I place the discussion of fertility preservation for children into the (...) more general context of the choices that parents might have to help keep their children’s future reproductive (and parenting) choices open. I discuss the role of desert and fairness in arguments for fertility preservation and their relevance for framings of infertility in general, as well as the relation between having a (slight) possibility to reproduce and becoming a parent. An extensive literature has focused on the impact of new public management oriented structural changes on academics’ practice and identity. These critical studies have been resolute in concluding that NPM inevitably leads to a degeneration of academics’ ethos and values. Drawing from the moral philosophy of Alasdair MacIntyre, we argue that these previous analyses have overlooked the moral agency of the academics and their role in ‘moralizing’ and consequently shaping the ethical nature of their practices. The paper provides a new (...) theoretical understanding of NPM-oriented reforms in light of the virtue ethics approach, thereby directing the attention to the moral character and moral agency of academics. Our analysis of interviews collected in the business department of a Danish university provides an example of how individuals have divergent ethical understandings of these structural changes and enact/resist pre-defined social roles in different ways. While in some cases the NPM agenda of the institutions has triggered internal moral conflict and a crisis of moral character, in other cases the new logic resonates with academics’ values and evaluative standards. Partially departing from the theoretical ground of MacIntyre, we conclude that academics can play a crucial role in shaping the morality of NPM-oriented institutions and in transforming these settings into suitable contexts for the cultivation of virtues. Deleuze's theory of time set out in Difference and Repetition is a complex structure of three different syntheses of time – the passive synthesis of the living present, the passive synthesis of the pure past and the static synthesis of the future. This article focuses on Deleuze's third synthesis of time, which seems to be the most obscure part of his tripartite theory, as Deleuze mixes different theoretical concepts drawn from philosophy, Greek drama theory and mathematics. Of central importance is (...) the notion of the cut, which is constitutive of the third synthesis of time defined as an a priori ordered temporal series separated unequally into a before and an after. This article argues that Deleuze develops his ordinal definition of time with recourse to Kant's definition of time as pure and empty form, Hölderlin's notion of ‘caesura’ drawn from his ‘Remarks on Oedipus’ (1803) and Dedekind's method of cuts as developed in his pioneering essay ‘Continuity and Irrational Numbers’ (1872). Deleuze then ties together the conceptions of the Kantian empty form of time and the Nietzschean eternal return, both of which are essentially related to a fractured I or dissolved self. This article aims to assemble the different heterogeneous elements that Deleuze picks up on and to show how the third synthesis of time emerges from this differential multiplicity. (shrink)

Seed libraries are institutions that support the creation of semi-formal seed systems, but are often intended to address larger issues that are part of the “food movement” in the global north. Over 100 SLs are reported present in California. I describe a functional framework for studying and comparing seed systems, and use that to investigate the social and biological characteristics of California SLs in 2016 and how they are contributing to alternative seed systems based on interviews with 45 SL managers. (...) At a minimum, SLs function as new seed distribution institutions founded and overseen by dedicated, values-driven individuals and groups with goals including education, seed access, local adaptation, biodiversity conservation, community-building, and human health. Annually about 4776 people borrow seeds from, and 238 people return seeds to the SLs in this study, that operate through over 17,000 hours of work/year. These SLs distribute approximately 6456 packets of seed annually, mostly of commercial seeds from small seed companies, but some SLs emphasize local and culturally meaningful seeds. The significance of a 6% seed return rate depends on SL goals and can be investigated once appropriate indicators for those goals are identified and documented. Beyond distribution, the seed system functions accomplished by SLs differ, and all can have consequences for the processes shaping the diversity and adaptation of their crops. The SLs engaged in seed system functions beyond distribution are new forms of socially-motivated community science, poised to develop biological and social innovations reflecting their values and interests. Leading philosophical models of curiosity represent it as a desiderative attitude whose content is a question, and which is satisfied by knowledge of the answer to that question. I argue that these models do not capture the distinctive character of a form of curiosity that I call 'erotic curiosity'. Killing in war as a matter of course may be inferred from the fact that, as stated by Thomas Hobbes, “all laws are silent in the time of war”. Although this traditional law-suspending power of war has been restricted to a certain degree by modern humanitarian international law, it is still commonly assumed that killing in war, unless and as long as not explicitly forbidden, is per se permitted and thus does not require any further legitimisation. This is in fundamental (...) contrast to a “normal” homicide, which requires special justification to be considered lawful. This commonly unquestioned license to kill, by the mere reason of being at war, is all the more astonishing given that no legal norm can be found that positively and explicitly declares killing in war to be lawful. However, on what ground can such an unwritten privilege be based? How can it be morally justified and sufficiently legitimised? This question shall be pursued in four steps: by, first, asking whether and why killing in war, as long as it does not cross the threshold of a war crime, shall stay outside the rubric of criminal law. After not having found a satisfactory answer, it must, second, be asked whether and to what degree constitutional law could provide empowerments and restrictions of killing in war. As neither will give full answers, third, final refuge may be taken in international law, with particular attention to state sovereignty. Since even on this level, however, killing in war has not yet found convincing legitimisation, fourth, further challenges are to be met. (shrink)

This article is articulated in three voices of scholars who have worked on questions of war, visual culture, and contemporary political aesthetics that also relates to art and film practices. Media theorist Jussi Parikka, literary scholar Anders Engberg-Pedersen, and visual culture researcher Daniela Agostinho address the relations between images, aesthetics and operations through the lens of two books published concomitantly, Parikka’s Operational Images: From the Visual to the Invisual and Engberg-Pedersen’s Martial Aesthetics: How War Became an Art Form. Both (...) books expand the scope of what Czechoslovakian-born filmmaker Harun Farocki termed “operational images” in his experimental documentaries and theoretical writings from the early 2000s. Through his analyses of the politics of imagery in the military-industrial context, Farocki notably defined “operational images” as images that do not depict or represent but rather perform tasks such as tracking, surveilling, detecting, and targeting. For both Parikka and Engberg-Pedersen, Farocki’s central concept of operational images forms a point of departure for writing media archaeologies of the present. In a three-voiced dialogue, the authors unfold operations as a “machine of articulation,” a conceptual and analytical device that reveals surprising linkages and frictions across different themes, techniques, scales, and historical periods. Justified belief is a core concept in epistemology and there has been an increasing interest in its logic over the last years. While many logical investigations consider justified belief as an operator, in this paper, we propose a logic for justified belief in which the relevant notion is treated as a predicate instead. Although this gives rise to the possibility of liar-like paradoxes, a predicate treatment allows for a rich and highly expressive framework, which lives up to the universal ambitions (...) of investigating epistemological concepts. We start with a base theory for justified belief, and then systematically present putative additional axioms for justified belief. We provide an overview of consistency results when the additional principles are added to the base theory, and discuss their philosophical plausibility. (shrink)

This paper explores a distinctive form of social interaction—interpersonal inquiry—in which two or more people attempt to understand one another by engaging in conversation. Like many modes of inquiry into human beings, interpersonal inquiry partly shapes its own objects. How we conduct it thus affects who we become. I present an ethical ideal of conversation to which, I argue, at least some of our interpersonal inquiry ought to aspire. Este artículo estudia el uso de documentos de archivo en el arte chileno contemporáneo en un contexto social de demandas de acceso a los documentos de la dictadura en ese país. Partiendo del libro de poesía documental Chile Project: [Re-classified] (2013) que Carlos Soto Román realiza con los archivos desclasificados de la CIA, y poniéndolo en relación con la obra de Voluspa Jarpa y de Francisco Tapia Salinas, el artículo examina demandas artísticas y sociales de colectivización y desclasificación de archivos. (...) Mediante el análisis de los modos en que, en contra de la prohibición estatal que clasifica esos documentos hasta el año 2054, artistas y organizaciones independientes intentan acceder “desde abajo” e intervenir los archivos, se propone que estas iniciativas ponen en cuestión la monopolización de información pública por el Estado y la construcción de una “verdad de archivo”, planteando así el problema de la formación, acceso y destrucción de información y documentos históricos. Finalmente, el artículo plantea que el arte y la literatura muestran un nuevo poder de crítica y de intervención y, junto con propuestas sociales para un archivo colectivo descentralizado, disputan el poder monopólico del Estado sobre los archivos y la información pública.
