Background Despite continuous performance improvements, especially in clinical contexts, a major challenge of Artificial Intelligence based Decision Support Systems (AI-DSS) remains their degree of epistemic opacity. The conditions of and the solutions for the justified use of the occasionally unexplainable technology in healthcare are an active field of research. In March 2024, the European Union agreed upon the Artificial Intelligence Act (AIA), requiring medical AI-DSS to be ad-hoc explainable or to use post-hoc explainability methods. The ethical debate does not seem (...) to settle on this requirement yet. This systematic review aims to outline and categorize the positions and arguments in the ethical debate. Methods We conducted a literature search on PubMed, BASE, and Scopus for English-speaking scientific peer-reviewed publications from 2016 to 2024. The inclusion criterion was to give explicit requirements of explainability for AI-DSS in healthcare and reason for it. Non-domain-specific documents, as well as surveys, reviews, and meta-analyses were excluded. The ethical requirements for explainability outlined in the documents were qualitatively analyzed with respect to arguments for the requirement of explainability and the required level of explainability. Results The literature search resulted in 1662 documents; 44 documents were included in the review after eligibility screening of the remaining full texts. Our analysis showed that 17 records argue in favor of the requirement of explainable AI methods (xAI) or ad-hoc explainable models, providing 9 categories of arguments. The other 27 records argued against a general requirement, providing 11 categories of arguments. Also, we found that 14 works advocate the need for context-dependent levels of explainability, as opposed to 30 documents, arguing for context-independent, absolute standards. Conclusions The systematic review of reasons shows no clear agreement on the requirement of post-hoc explainability methods or ad-hoc explainable models for AI-DSS in healthcare. The arguments found in the debate were referenced and responded to from different perspectives, demonstrating an interactive discourse. Policymakers and researchers should watch the development of the debate closely. Conversely, ethicists should be well informed by empirical and technical research, given the frequency of advancements in the field. (shrink)

Abstract— This study aimed to identify the degree of use of the capabilities of decision-support systems in Palestinian institutions higher education, Aqsa University in Gaza - a case study. The study used a analytical descriptive approach, and the researchers used the of questionnaire tool to collect the data, the researchers using stratified random sample distributed (150) questioners to the study population and (126) was obtained back with rate of 84%. The study showed that the most important results are: that senior (...) management supports the existence of decision support systems and that there is approval by the respondents on the paragraphs of the use of the capabilities of decision support systems in general. And that there are no significant differences between the averages of the answers of respondents differences about the degree of use of decision support systems capabilities attributed to personal data. The study also concluded a series of recommendations including: increasing the adoption of the senior management decision support in their decision-making systems. And increased regulatory attention to the potential available to decision support systems directly to the senior management in the Palestinian universities in the Gaza Strip. This paper explores the role and resolution of disagreements between physicians and their diagnostic AI-based decision support systems. With an ever-growing number of applications for these independently operating diagnostic tools, it becomes less and less clear what a physician ought to do in case their diagnosis is in faultless conflict with the results of the DSS. The consequences of such uncertainty can ultimately lead to effects detrimental to the intended purpose of such machines, e.g. by shifting the burden of proof (...) towards a physician. Thus, we require normative clarity for integrating these machines without affecting established, trusted, and relied upon workflows. In reconstructing different causes of conflicts between physicians and their AI-based tools—inspired by the approach of “meaningful human control” over autonomous systems and the challenges to resolve them—we will delineate normative conditions for “meaningful disagreements”. These incorporate the potential of DSS to take on more tasks and outline how the moral responsibility of a physician can be preserved in an increasingly automated clinical work environment. (shrink)

Making good decisions in extremely complex and difficult processes and situations has always been both a key task as well as a challenge in the clinic and has led to a large amount of clinical, legal and ethical routines, protocols and reflections in order to guarantee fair, participatory and up-to-date pathways for clinical decision-making. Nevertheless, the complexity of processes and physical phenomena, time as well as economic constraints and not least further endeavours as well as achievements in medicine and healthcare (...) continuously raise the need to evaluate and to improve clinical decision-making. This article scrutinises if and how clinical decision-making processes are challenged by the rise of so-called artificial intelligence-driven decision support systems (AI-DSS). In a first step, this article analyses how the rise of AI-DSS will affect and transform the modes of interaction between different agents in the clinic. In a second step, we point out how these changing modes of interaction also imply shifts in the conditions of trustworthiness, epistemic challenges regarding transparency, the underlying normative concepts of agency and its embedding into concrete contexts of deployment and, finally, the consequences for (possible) ascriptions of responsibility. In their article, ‘Responsibility, Second Opinions, and Peer-Disagreement—Ethical and Epistemological Challenges of Using AI in Clinical Diagnostic Contexts,’ Kempt and Nagel argue for a ‘rule of disagreement’ for the integration of diagnostic AI in healthcare contexts. The type of AI in question is a ‘decision support system’, the purpose of which is to augment human judgement and decision-making in the clinical context by automating or supplementing parts of the cognitive labor. Under the authors’ proposal, artificial decision support systems which produce (...) automated diagnoses should serve chiefly as confirmatory tools; so long as the physician and AI agree, the matter is settled, and the physician’s initial judgement is considered epistemically justified. If, however, the AI-DSS and physician disagree, then a second physician’s opinion is called on to resolve the dispute. While the cognitive labour of the decision is shared between the physicians and AI, the final decision remains at the discretion of the first physician, and with it the moral and legal culpability. The putative benefits of this approach are twofold: healthcare administration can improve diagnostic performance by introducing AI-DSS without the unintended byproduct of a responsibility gap, and assuming the physician and AI disagree less than the general rate of requested second opinions, and the AI’s diagnostic accuracy supersedes or at least …. Providing healthcare services frequently involves cognitively demanding tasks, including diagnoses and analyses as well as complex decisions about treatments and therapy. From a global perspective, ethically significant inequalities exist between regions where the expert knowledge required for these tasks is scarce or abundant. One possible strategy to diminish such inequalities and increase healthcare opportunities in expert-scarce settings is to provide healthcare solutions involving digital technologies that do not necessarily require the presence of a human expert, e.g., in the form of (...) artificial intelligent decision-support systems (AI-DSS). Such algorithmic decision-making, however, is mostly developed in resource- and expert-abundant settings to support healthcare experts in their work. As a practical consequence, the normative standards and requirements for such algorithmic decision-making in healthcare require the technology to be at least as explainable as the decisions made by the experts themselves. The goal of providing healthcare in settings where resources and expertise are scarce might come with a normative pull to lower the normative standards of using digital technologies in order to provide at least some healthcare in the first place. We scrutinize this tendency to lower standards in particular settings from a normative perspective, distinguish between different types of absolute and relative, local and global standards of explainability, and conclude by defending an ambitious and practicable standard of local relative explainability. Dialogue systems, from Virtual Personal Assistants such as Siri, Cortana, and Alexa to state-of-the-art systems such as BlenderBot3 and ChatGPT, are already widely available, used in a variety of applications, and are increasingly part of many people’s lives. However, the task of enabling them to use empathetic language more convincingly is still an emerging research topic. Such systems generally make use of complex neural networks to learn the patterns of typical human language use, and the interactions in which the systems (...) participate are usually mediated either via interactive text-based or speech-based interfaces. In human–human interaction, empathy has been shown to promote prosocial behaviour and improve interaction. In the context of dialogue systems, to advance the understanding of how perceptions of empathy affect interactions, it is necessary to bring greater clarity to how empathy is measured and assessed. Assessing the way dialogue systems create perceptions of empathy brings together a range of technological, psychological, and ethical considerations that merit greater scrutiny than they have received so far. However, there is currently no widely accepted evaluation method for determining the degree of empathy that any given system possesses (or, at least, appears to possess). Currently, different research teams use a variety of automated metrics, alongside different forms of subjective human assessment such as questionnaires, self-assessment measures and narrative engagement scales. This diversity of evaluation practice means that, given two DSs, it is usually impossible to determine which of them conveys the greater degree of empathy in its dialogic exchanges with human users. Acknowledging this problem, the present article provides an overview of how empathy is measured in human–human interactions and considers some of the ways it is currently measured in human–DS interactions. Finally, it introduces a novel third-person analytical framework, called the Empathy Scale for Human–Computer Communication (ESHCC), to support greater uniformity in how perceived empathy is measured during interactions with state-of-the-art DSs. The authors present an approach to conceptualising and predicting environmental conflicts in which conflicts are analysed as a continuum of disagreement over values and options. They also operationalise this approach using an online values-centred survey tool, the ‘public-to-public decision support system’ (P2P-DSS). The authors put values and conflict in environmental management into perspective. Next, they review how values are defined in scholarship and operationalised for decision support. The relevance of values research to con-flict management is presented. With reference to a (...) real-world aggregate-mining conflict, the authors demonstrate how P2P-DSS can be used to collect data and categorise conflicts to enhance environmental management decision-making. The authors argue that P2P-DSS has potential to support values-sensitive thinking for environmental conflict management. They then set out research priorities to investigate the theoretical and practical implications of this approach. Background Given that AI-driven decision support systems (AI-DSS) are intended to assist in medical decision making, it is essential that clinicians are willing to incorporate AI-DSS into their practice. This study takes as a case study the use of AI-driven cardiotography (CTG), a type of AI-DSS, in the context of intrapartum care. Focusing on the perspectives of obstetricians and midwives regarding the ethical and trust-related issues of incorporating AI-driven tools in their practice, this paper explores the conditions that AI-driven CTG (...) must fulfill for clinicians to feel justified in incorporating this assistive technology into their decision-making processes regarding interventions in labor. Methods This study is based on semi-structured interviews conducted online with eight obstetricians and five midwives based in England. Participants were asked about their current decision-making processes about when to intervene in labor, how AI-driven CTG might enhance or disrupt this process, and what it would take for them to trust this kind of technology. Interviews were transcribed verbatim and analyzed with thematic analysis. NVivo software was used to organize thematic codes that recurred in interviews to identify the issues that mattered most to participants. Topics and themes that were repeated across interviews were identified to form the basis of the analysis and conclusions of this paper. Results There were four major themes that emerged from our interviews with obstetricians and midwives regarding the conditions that AI-driven CTG must fulfill: (1) the importance of accurate and efficient risk assessments; (2) the capacity for personalization and individualized medicine; (3) the lack of significance regarding the type of institution that develops technology; and (4) the need for transparency in the development process. Conclusions Accuracy, efficiency, personalization abilities, transparency, and clear evidence that it can improve outcomes are conditions that clinicians deem necessary for AI-DSS to meet in order to be considered reliable and therefore worthy of being incorporated into the decision-making process. Importantly, healthcare professionals considered themselves as the epistemic authorities in the clinical context and the bearers of responsibility for delivering appropriate care. Therefore, what mattered to them was being able to evaluate the reliability of AI-DSS on their own terms, and have confidence in implementing them in their practice. (shrink)

Human decisions are increasingly supported by decision support systems (DSS). Humans are required to remain “on the loop,” by monitoring and approving/rejecting machine recommendations. However, use of DSS can lead to overreliance on machines, reducing human oversight. This paper proposes “reflection machines” (RM) to increase meaningful human control. An RM provides a medical expert not with suggestions for a decision, but with questions that stimulate reflection about decisions. It can refer to data points or suggest counterarguments that are less compatible (...) with the planned decision. RMs think against the proposed decision in order to increase human resistance against automation complacency. Building on preliminary research, this paper will (1) make a case for deriving a set of design requirements for RMs from EU regulations, (2) suggest a way how RMs could support decision-making, (3) describe the possibility of how a prototype of an RM could apply to the medical domain of chronic low back pain, and (4) highlight the importance of exploring an RM’s functionality and the experiences of users working with it. The Argumentative Adversariality debate is over the question of whether argument must be adversarial. A particular locus of this debate is on skeptical challenges in critical dialogue. The Default Skeptical Stance in argument is a practical manifestation of argumentative adversariality. Views about the on-the-ground value of the DSS vary. On one hand, in “The Social & Political Limitations of Philosophy”, Phyllis Rooney argues that the DSS leads to epistemic injustice. On the other, Allan Hazlett in his recent piece “Critical Injustice” (...) argues for the virtues of the skeptical stance in terms of epistemic justice. Both Rooney and Hazlett are concerned with the role skeptical engagement plays in argument, but they assign opposite values to it. In this essay, I review Rooney and Hazlett’s examples and show that the epistemic dysfunction in the two scholar’s going cases is one and the same, and argue that the cause of both is a lack of proper skeptical engagement. Skeptical engagement is a requirement for epistemic justice. Together and constitute an initial defense of the Adversarialist position against objections regarding the social epistemic risk of the skeptical stance. (shrink)

The WTO Dispute Settlement System (DSS) has been the object of many studies in politics, law, and economics focusing on institutional design problems. This paper contributes to such studies by accounting for the argumentative nature and sophisticated features of the DSS through a philosophical analysis of the procedures through which it is articulated. Jürgen Habermas's discourse theory is used as a hermeneutic device to disentangle the types of ‘orientations’ (compromise, consensus, and mutual understanding) pertaining to DSS procedures. We show that (...) these latter are oriented primarily to put the parties in a position to reach mutual understanding. Such an orientation is no mere idiosyncrasy of the DSS but is the only one consistently conducive to the WTO's general aims, in response to the various types of disputes that may arise between its Members. Otherwise than Being is preferable to Totality and Infinity is that the ethics in the former does not rest on a failed narrative of family life and woman’s role in it as ethics does in the latter,2 to Claire Katz’s that there is a rich, positive ethical component to women’s role for Levinas since he uses maternity as an example of being-for-the-other.3This essay complements those accounts by giving a detailed analysis of the discussion of women in Levinas’s talmudic commentary, “And God Created Woman.”4 Specifically, I ask whether Levinas is right, in his commentary, that, according to Talmud Berakhot 61a, women are not ultimately inferior to men because both men and women are human and their humanity precedes the division of humanity into male [End Page 137] and female. I pursue the question by reading the talmudic passage the way Levinas, in his commentary, tells us that we should read Talmud, namely, by situating texts quoted in it in their broader textual context in Torah rather than treating them as proof texts.When I put quotations cited in Berakhot 61a in their textual context, I find that the male figures treated in the texts from Torah are dismayingly dismissive of women in distress, suggesting that women are treated as inferior, but then learn their mistake and treat the women as equal or even superior, suggesting that Talmud does see them as having a humanity that precedes sexual difference. Nonetheless, I go on to argue, though the talmudic passage does not see women as ultimately inferior to men, it nonetheless treats them as inferior on the day-to-day level and such treatment belies the attempt to treat them as human.I go on to ask about Levinas’s stance toward this problem in the talmudic text, Does he accept the treatment of women as inferior? I argue, to the contrary, that Levinas sees the problem and attempts to overcome it by criticizing two ways the talmudic passage treats women as inferior on the day-to-day level: he rejects the passages’ prohibitions on certain types of male contact with women—such as looking at them!—and he reinterprets its association of women with makeup and deception. His stance is more complicated, though, because he goes on to find a kernel of truth to preserve in each of the problematic talmudic positions: in the first, the fundamental ethical ambiguity of sexuality, since sexuality points toward the other as other but also toward one’s own satisfaction; in the second, what we might anachronistically call women’s ways of knowing that, for Levinas, have a certain ethical superiority over the ways men know. The two resolutions have their own problems, I maintain, since if Levinas is to avoid gender norming and gender hierarchy, he needs to make it clear that both the problematic ethical ambiguity of sexuality and also the ways of knowing he thinks are ethically superior are not essentially associated more with women than with men. Crisis management is a complex problem raised by the scientific community currently. Decision support systems are a suitable solution for such issues, they are indeed able to help emergency managers to prevent and to manage crisis in emergency situations. However, they should be enough flexible and adaptive in order to be efficient to solve complex problems that are plunged in dynamic and unpredictable environments. The approach we propose in this paper addresses this challenge. First, we expose a modelling of information (...) for an emergency environment. Then, we describe an architecture of a multiagent DSS that deals with these information in order to prevent and to manage crisis situations. We focus here on the first level of the system mechanism which intends to perceive and to reflect the evolution of the current situation. The temple schematics in the Dead Sea Scrolls (DSS), that is, New Jerusalem and Temple Scroll, has often been comparatively examined with the sanctuary structures in the Hebrew Bible (HB) (Ezk 40–48 and Num 2). Typically, in scholarship, the irreconcilable differences between all accounts (regarding the size, shape, name-gate ordering, etc.) is underscored, thus rendering a literary conundrum. This article argues that New Jerusalem and Temple Scroll drew from both Ezekiel 40–48 and Numbers 2 in different ways, purporting the sect(s)’s (...) theologies and ideologies which accords, further, with the life setting of the Qumran communities; the influence of Numbers in the DSS is underscored. These aspects include (1) the eastern orientation of sacred structures and the compound at Khirbet Qumran, (2) the precise locale of the communities at the Dead Sea vis-à-vis Ezekiel 47 and (3) the desert encampment configuration together with its militaristic overtones in Numbers, which corresponds to the DSS sect(s)’s apocalyptic expectations as indicated in the War Scroll. Consequently, the Qumran sect(s) truly saw itself as an alternative priesthood of the forthcoming restored temple of God, even as in the interim they functioned as an alternative sanctuary (4QFlor; 4QMMT; 1QS). The import of Numbers upon the DSS sect(s)’s temple ideologies and priestly theologies is, therefore, equivalent to that of Ezekiel. Contribution: This article traces theological themes of temple and priestly ideologies between and among the Qumran literature and Hebrew Scriptures; both the respective library or canon and methodological approach are core to the historical thought’s aim and scope of HTS Teologiese Studies/Theological Studies. The important contribution that the Dead Sea Scrolls (DSS) hold for New Testament studies is probably most evident in Ad Hebraeos. This contribution seeks to present an overview of relevant extant DSS fragments available for an investigation of the Old Testament explicit quotations and motifs in the book of Hebrews. A large number of the explicit quotations in Hebrews were already alluded to, or even quoted, in some of the DSS. The DSS are of great importance for the study of (...) the explicit quotations in Ad Hebraeos in at least four areas, namely in terms of its text-critical value, the hermeneutical methods employed in both the DSS and Hebrews, theological themes and motifs that surface in both works, and the socio-religious background in which these quotations are embedded. After these four areas are briefly explored, this contribution concludes, among others, that one can cautiously imagine a similar Jewish sectarian matrix from which certain Christian converts might have come – such as the author of Hebrews himself. Based on applied linguistics, this study looked at the decision support system for emphasizing self-assurance in academic writing. From a generic perspective, academic writing has been considered both a process and a product. It has highlighted the planning composite processes, editing, composing, revising, and assessment, which depend upon the familiarity of someone with confidence in their capability for engagement in these activities. As a product, it has focused on the writing results through the product’s characteristics. These contain specific content areas (...) in acceptable depth and well-structured technical vocabulary. Higher education aims to support students in optimizing their potential for achieving satisfactory outcomes. For example, the assessment of grades involves academic writing, contributing to the degree course classification. Students have differences in many respects, such as expectations, background knowledge, and study and learning approaches. There were varying students’ beliefs about what academic writing is for evaluation. Modern-day motivations and theories highlight the significance of students’ confidence in their studies. The role of high confidence can support students to apply more effort toward setting challenging goals. Students may find it more difficult to succeed in higher education if they lack confidence in their academic writing abilities. A DSS has many applications in diverse areas and can play a significant role in the ranking and prioritization process. The current study has considered the DSS for prioritizing self-assurance in academic writing based on applied linguistics. Various criteria were considered for the evaluation of the research. Supervisory styles Are Key predictors of graduate students’ innovation performance, but the mediating and moderating mechanisms underlying this relationship require further exploration. Based on the job demands-resources model and conservation of resources theory, this study analyzed the influence of supervisory styles on GSIP, including the mediating role of psychological capital and the moderating role of harmonious academic passion. Questionnaires were completed by 400 graduate students from a Chinese university. The results indicated that both supportive and directive supervisory styles were positively (...) related To GSIP, PsyCap fully mediated the relationship between SSS and GSIP, and HAP significantly moderated the effect of DSS but exhibited no moderating influence on the effect of SSS. These findings contribute to a deeper understanding of why, how and when supervisory styles influence GSIP. Physicians and other healthcare professionals are increasingly finding ways to use artificial intelligent decision support systems in their work. IBM Watson Health, for example, is a commercially available technology that is providing AI-DDS services in genomics, oncology, healthcare management and more.1 AI’s ability to scan massive amounts of data, detect patterns, and derive solutions from data is vastly more superior than that of humans. AI technology is undeniably integral to the future of healthcare and public health, and thoughtful consideration of (...) the legal, ethical and moral issues surrounding this technology is a must. The authors of the article, Responsibility, Second Opinions, and Peer-Disagreement—Ethical and Epistemological Challenges of Using AI in Clinical Diagnostic Contexts, provide an informed discussion of how AI-DSS may be used, both practically and ethically, to assist healthcare professionals in cooperative diagnostic processes2. The authors propose a process, whereby AI-DSS would provide a physician a second opinion, and when there is a mismatch between opinions, another physician would provide a third opinion. This approach maintains a ‘physician-in-charge’ perspective, and suggests that decision-making must ultimately be made by a person. This approach is also consistent with the ‘physician in-the-loop’ concept, such that even when increasingly autonomous AI-DSSs are put to use, physicians will still be providing checks and overseeing clinical decisions. This article reports on the development of an instrument to measure the perceived stress that results from the use and ubiquity of digital technology in the workplace. Based upon a contemporary understanding of stress and a set of stressors that is a substantial update to existing scales, the Digital Stressors Scale (DSS) advances the measurement of digital stress. Initially, 138 items were constructed for the instrument and grouped into a set of 15 digital stressors. Based on a sample ofN= 1,998 (...) online questionnaires completed by individuals representative of the US employed population, the scale was refined using exploratory factor analyses (EFA) and PLS-SEM. The resulting and final scale consists of ten stressor categories reflective of one higher-order construct and a total of 50 items. Through a nomological network that includes important outcome variables of digital stress (emotional exhaustion, innovation climate, job satisfaction, user satisfaction) it was then demonstrated that the DSS provides substantial explanatory power, particularly related to emotional exhaustion and user satisfaction. Thus, the DSS constitutes a state-of-the-art self-report instrument to measure the extent of distress appraisal related to digital technologies in the workplace and helps to explain further how and why information and communication technologies can lead to adverse outcomes in individuals, thereby providing the starting point for job related organizational interventions. In this research work, a user-friendly decision support framework is developed to analyze the behavior of Pakistani students in academics. The purpose of this article is to analyze the performance of the Pakistani students using an intelligent decision support system (DSS) based on the three-level machine learning (ML) technique. The neural network used a three-level classifier approach for the prediction of Pakistani student achievement. A self-recorded dataset of 1,011 respondents of graduate students of English and Physics courses are used. The (...) ten interviews along with ten questions were conducted to determine the perception of the individual student. The chi-squared ( χ ) \left(\chi ) test was applied to test statistical significancy of the questionnaire. The statistical calculations and computation of data were performed by using the statistical package of IBMM SPSS version 21.0. The seven different algorithms were tested to improve the data classification. The Java-based environment was used for the development of numerous prediction classifiers. C4.5 algorithm shows the finest accuracy, whereas Naïve Bayes (NB) algorithm shows the least. The results depict that the classifier’s efficiency was improved by using a three-level proposed scheme from 83.2% to 88.8%. This prediction has shown remarkable results when compared with the individual level classifier technique of ML. This improvement in the accuracy of DSSs is used to identify more efficiently the gray areas in the education stratum of Pakistan. This will pave a path for making policies in the higher education system of Pakistan. The presented framework can be deployed on different platforms under numerous operating systems. (shrink)
