Pulmonary rehabilitation has demonstrated patients’ physiological and psychosocial improvements, symptoms reduction and health-economic benefits whilst enhances the ability of the whole family to adjust to illness. However, PR remains highly inaccessible due to lack of awareness of its benefits, poor referral and availability mostly in hospitals. Novel models of PR delivery are needed to enhance its implementation while maintaining cost-efficiency. We aim to implement an innovative community-based PR programme and assess its cost-benefit. A 12-week community-based PR will be implemented in (...) primary healthcare centres where programmes are not available. Healthcare professionals will be trained. 73 patients with CRD and their caregivers will compose the experimental group. The control group will include dyads age- and disease-matched willing to collaborate in data collection but not in PR. Patients/family-centred outcomes will be dyspnoea, fatigue, cough and sputum, impact of the disease, emotional state, number of exacerbations, healthcare utilisation, health-related quality of life and family adaptability/cohesion. Other clinical outcomes will be peripheral and respiratory muscle strength, muscle thickness and cross sectional area, exercise capacity, balance and physical activity. Data will be collected at baseline, at 12 weeks, at 3- and 6-months post-PR. Changes in the outcome measures will be compared between groups, after multivariate adjustment for possible confounders, and effect sizes will be calculated. A cost-benefit analysis will be conducted. This study will enhance patients access to PR, by training healthcare professionals in the local primary healthcare centres to conduct such programmes and actively involving caregivers. The cost-benefit analysis of this intervention will provide an evidence-based insight into the economic benefit of community-based PR in chronic respiratory diseases. This article presents an overview of research on gender in Spanish society. Six areas of literature are examined including families, education, work, politics, sexuality, and men. The author argues that political factors have shaped the development of sociology of gender in Spain and that there are still important gaps in coverage in this area of sociological inquiry.

The author argues that the pragmatically oriented historiography of science that recently has been so strongly recommended has fallen into the mistake of focusing on scientists' circumstantial attempts to fix beliefs without discussing the scientific importance of the beliefs in the first place. This mistake has led historians of science to engage in pointless exercises, made them mute about crucial aspects of the development of science, and, above all, prevented them from avoiding, in a satisfactory way, the ghost of "triumphalism." Postula-se que nesse trabalho de 1954, Deleuze exponha a noção de 'diferença', que está presente na obra de Henri Bergson, por meio da relação entre três dimensões: metodológica, cosmológica e ontológica. Para tanto, caracteriza-se o conjunto do pensamento de Deleuze em relação à História da Filosofia. Em seguida, identifica-se o percurso das pesquisas (...) de Deleuze sobre a obra de Bergson no contexto de sua versão para a História da Filosofia. Por fim, explicita-se o itinerário de Deleuze no artigo de 1954: "A concepção da diferença em Bergson". ABSTRACT The purpose of this paper is to explain the itinerary or argumentative line of Gilles Deleuze in the article named "The conception of difference in Bergson" of 1954. It is postulated that in this work of 1954, Deleuze exposes the notion of "difference", which is present in the work of Henri Bergson, through the relation between three dimensions: the methodological, cosmological and ontological ones. For that purpose, the whole of Deleuze's thought in relation to the History of Philosophy is characterized. Then, we identify the course of Deleuze's research on Bergson's work in the context of his version for the History of Philosophy. Although there is clearly a need for evidenced‐based behavioral or biomedical prevention or treatment programs for suicide, substance abuse, and sexual health targeted to members of the LGBT population under the age of eighteen, few such programs exist, due in substantial part to limited research knowledge. Ambiguities in regulations that govern human subjects protections and the related inconsistencies in institutional review board (IRB) interpretations of regulatory language are the key reason for the lack of rigorous clinical trial evidence to support (...) treatment choices and prevention approaches to reducing health disparities for this population. Given the socially sensitive nature of suicide, substance abuse, and HIV and STI research in general and LGBT research specifically, in the absence of empirical data to guide their decisions, IRBs must often rely on subjective judgments of minimal risk, which can lead to overestimation of the magnitude and probability of psychological, social, and informational harms that might arise from LGBT youth participation in clinical trials. In addition, more than other youth, LGBT adolescents whose families are unaware of their sexual orientation or gender identity or whose families have victimized them on account of it may be reluctant to participate in studies that require guardian permission. This, in turn, intensifies problems of recruitment and unbiased sampling. However, many IRBs are reluctant to apply federal regulations permitting waiver of guardian permission under conditions in which such permission is clearly not “feasible” or “reasonable” to require. Consequently, many investigators have excluded LGBT individuals under eighteen years of age in health intervention research proposals because of anticipated or actual difficulties obtaining IRB approval. This situation is in conflict with current ethical discourse focusing on the right of youths to participate in trials that will protect them from receiving developmentally untested, inappropriate, and unsafe treatments. In this article, we describe these barriers and recommendations for providing LGBT youth safe and fair access to health research. (shrink)

Lucid dreams are dreams in which a person becomes aware that they are dreaming. They are different from ordinary dreams, not just because of the dreamer's awareness that they are dreaming, but because lucid dreams are often strikingly realistic and may be emotionally charged to the point of elation. Celia Green and Charles McCreery have written a unique introduction to lucid dreams that will appeal to the specialist and general reader alike. The authors explore the experience of lucid dreaming, relate (...) it to other experiences such as out-of-the-body experiences and apparitions, and look at how lucid dreams can be induced and controlled. The HIV/aids pandemic has brought global attention to the ethical challenges of conducting research involving socially vulnerable participants. Such challenges require not only ethical deliberation but also an empirical evidentiary basis for research ethics policies and practices. This need has been addressed through the Fordham University HIV and Drug Abuse Prevention Research Ethics Institute, a National Institute on Drug Abuse–funded program that trains and funds early career scientists in conducting research on HIV/drug abuse research ethics. People sometimes explain behavior by appealing to an essentialist concept of the self, often referred to as the true self. Existing studies suggest that people tend to believe that the true self is morally virtuous; that is deep inside, every person is motivated to behave in morally good ways. Is this belief particular to individuals with optimistic beliefs or people from Western cultures, or does it reflect a widely held cognitive bias in how people understand the self? To address this (...) question, we tested the good true self theory against two potential boundary conditions that are known to elicit different beliefs about the self as a whole. Study 1 tested whether individual differences in misanthropy—the tendency to view humans negatively—predict beliefs about the good true self in an American sample. The results indicate a consistent belief in a good true self, even among individuals who have an explicitly pessimistic view of others. Study 2 compared true self-attributions across cultural groups, by comparing samples from an independent country and a diverse set of interdependent countries. Results indicated that the direction and magnitude of the effect are comparable across all groups we tested. The belief in a good true self appears robust across groups varying in cultural orientation or misanthropy, suggesting a consistent psychological tendency to view the true self as morally good. A growing body of research has examined how people judge the persistence of identity over time—that is, how they decide that a particular individual is the same entity from one time to the next. While a great deal of progress has been made in understanding the types of features that people typically consider when making such judgments, to date, existing work has not explored how these judgments may be shaped by normative considerations. The present studies demonstrate that normative beliefs do (...) appear to play an important role in people's beliefs about persistence. Specifically, people are more likely to judge that the identity of a given entity remains the same when its features improve than when its features deteriorate. Study 1 provides a basic demonstration of this effect. Study 2 shows that this effect is moderated by individual differences in normative beliefs. Study 3 examines the underlying mechanism, which is the belief that, in general, various entities are essentially good. Study 4 directly manipulates beliefs about essence to show that the positivity bias regarding essences is causally responsible for the effect. (shrink)

A major conceptual dynamic in all major religious traditions is the need for purification and transformation of the individual in order to effect integration and maturation of the personality in the divine. Although the means by which this purification takes place differs according to the cultural and religious configurations of any given tradition, nevertheless a recurring image is that of an inner and outer odyssey. A major example is the threefold path of John of the Cross, which presents a psycho-spiritual (...) journey by which 'divine osmosis' can be realised, passing through the 'dark night of the soul', and culminating in 'spiritual marriage'. Although not accepted by many theoreticians and practitioners of mysticism, nevertheless the value of the Sanjuanist schemata still holds sway in contemporary society. (shrink)

Given the plethora of research conducted in the field of spirituality and mysticism over the last 30 years, it is almost a superhuman feat to keep up with the explosion of information. Of necessity, in a limited article of this nature, it is possible to discuss only a few salient aspects of the spirituality and mysticism phenomenon and by so doing contribute to ongoing research in this important domain. Contemporary spiritualties encompass the whole range of human experience and new variants (...) are emerging; for example, the relatively recent Contemplative Studies, a cognate and close companion to Spirituality. Crossing inter-religious boundaries enhances studies in Mysticism; natural mysticism is clearly in the foreground; and breaking research in neurotheology sheds light on the nature of the ‘mystical mind’. Discussion of the value or otherwise of techniques and methods of the mystical journey continues unabated. Of great value for today’s frenetic, Internet-crazy world is the path of mystical silence. By contributing to a discussion of these issues, it is hoped that the threads of spirituality and mysticism will continue to share their colour in a world desperate for beauty and peace. (shrink)

This paper explores Albert Bandura's concept of moral disengagement in the context of organizational corruption. First, the construct of moral disengagement is defined and elaborated. Moral disengagement is then hypothesized to play a role in the initiation of corruption by both easing and expediting individual unethical decision-making that advances organizational interests. It is hypothesized to be a factor in the facilitation of organizational corruption through dampening individuals’ awareness of the ethical content of the decisions they make. Finally, it is hypothesized (...) to contribute to the perpetuation of corruption in organizations, because if individuals who have a greater propensity to morally disengage are more likely to make decisions that advance organizational interests regardless of the ethicality of those decisions, they may also be rewarded for those decisions in terms of organizational advancement. Together these studies form an argument that moral disengagement plays an important role in processes of organizational corruption. (shrink)

Neste artigo, pretendemos abordar os elementos educacionais da Grécia antiga, fundamentais tanto para a formação e o desenvolvimento da cultura ocidental como para a constituição da paideia cristã da antiguidade. Destacamos como a idealização do épos deu lugar à formação do cidadão a partir do ideal de uma areté política que, no período helenístico, tendo seu apogeu na pólis grega, vai ser alternado por outros valores mais ligados à individualidade. Nesse sentido, consideramos que a paideia grega, em seus diferentes momentos, (...) vai ser elemento chave para a construção de uma paideia cristã. Esta, por sua vez além de sintetizar elementos universalistas do pensamento grego, se nutre de um tempo oportuno, de um kairós para expandir a mensagem cristã oriunda das promessas da religião judaica de um messiah para a universalidade dos homens, cuja expressão vai ser fundamental para a construção dos valores da sociedade ocidental. Biogerontologist Aubrey de Grey has suggested that one of the reasons we as a society invest so little in research on combating aging is because we are in an intellectual trance. We think the effort will be futile: aging is immutable, so why try? A healthy skepticism can be a good thing but it is a major mistake to bet against the irresistible force of inexorable technological progress. Over the next few decades, nanotechnology will come to play a pivotal role (...) in the solution to the problem of human aging. Medical nanorobotics, if it can be made to work, can unquestionably offer convenient solutions to all known causes of age-related damage and most likely can also successfully address any new causes of senescence that remain undiscovered today. In this article, the hypothesis is developed that Descartes does not clearly articulate the knowledge of the thinking substance in the Meditations on First Philosophy and in Objections and Replies. It is argued that such exposition is necessary for a comprehensive understanding of the status of Cartesian philosophy at the time of writing the Meditations, particularly to grasp Descartes' conception of the knowledge of the thinking substance in the years 1641 and 1642. As known, knowledge of the thinking substance is (...) a fundamental element in Descartes' philosophy. Firstly, Descartes' two modes of presenting knowledge of the thinking substance in the Meditations are examined, highlighting the issues each entails. Secondly, drawing from the Objections and Replies: especially the fifth and seventh, criticisms from Pierre Gassendi and Pierre Bourdin regarding the knowledge of the thinking substance are presented. Both Gassendi and Bourdin emphasize that the text of the Meditations does not sufficiently clarify how the thinking substance is known. They question the possibility of such knowledge, pointing out the ambiguity and obscurity of Cartesian responses. It is concluded that in the Meditations, Descartes confronts an insurmountable gap between the ontology of substance and its knowledge, which prompts him to further develop and clarify his theory in later writings. In Spain, public preschool programs have continuously expanded in the past three decades. However, this education policy has done little to support increases in the proportion of women in the paid workforce. Preschool is not child care because the former does not address the care needed by children younger than three years old and offers programs with short hours and long holidays.

Theories of autobiographical memory emphasise effortful, generative search processes in memory retrieval. However recent research suggests that memories are often retrieved directly, without effortful search. We investigated whether direct and generative retrieval differed in the characteristics of memories recalled, or only in terms of retrieval latency. Participants recalled autobiographical memories in response to cue words. For each memory, they reported whether it was retrieved directly or generatively, rated its visuo-spatial perspective, and judged its accompanying recollective experience. Our results indicated that (...) direct retrieval was commonly reported and was faster than generative retrieval, replicating recent findings. The characteristics of directly retrieved memories differed from generatively retrieved memories: directly retrieved memories had higher field perspective ratings and lower observer perspective ratings. However, retrieval mode did not influence recollective experience. We discuss our findings in terms of cue generation and content construction, and the implication for reconstructive models of autobiographical memory. (shrink)

Harris, Barnier, Sutton and Savage examine the communication styles that boost the mnemonic consequences associated with conversations for long‐term married couples and the circumstances under which the couples form a TMS. Harris and colleagues demonstrated that specific communication styles (e.g., cueing each other) promote group memory success whereas others (e.g., correcting each other) did not enhance group recall performance. These results showed that even in well‐established and enduring distributed cognitive systems such as long‐term intimate couples (Harris, Barnier, Sutton & Keil, (...) 2014), memory benefits depend on effective communication and the specificities of the task performed (e.g., remembering lists of words vs. remembering shared, autobiographical experiences). The primary aim of this article is to bring the work of Deleuze and Guattari to bear on the question ofcommunication in the classroom. I focus on the mathematics classroom, where agency and subjectivity are highly regulated by the rituals of the discipline, and where neoliberal psychological frameworks continue to dominate theories of teaching and learning. Moreover, the nature ofcommunication in mathematics classrooms remains highlyelusive and problematic, due in part to the distinct relationship the discipline has with verbal language and (...) thought. I first discuss current attempts to better address the embodied nature ofcommunication in mathematics classrooms, and argue that these remain overly logo-centric and language-centric in their conception of thinking. I then show how the work of Deleuze and Guattari on thought as a radical disruptive event can be used effectively to critique current pedagogical practices that privilege a narrow conception of communication in the classroom. I examine a set of exemplary classroom videos used in mathematics teacher education to argue that the current approach fails to honor the highly creative and disruptive nature ofthinking. (shrink)

Withdrawal of artificially delivered nutrition and hydration (ANH) from patients in a permanent vegetative state (PVS) requires judicial approval in England and Wales, even when families and healthcare professionals agree that withdrawal is in the patient9s best interests. Part of the rationale underpinning the original recommendation for such court approval was the reassurance of patients’ families, but there has been no research as to whether or not family members are reassured by the requirement for court proceedings or how they experience (...) the process. The research reported here draws on in-depth narrative interviews with 10 family members (from five different families) of PVS patients who have been the subject of court proceedings for ANH-withdrawal. We analyse the empirical evidence to understand how family members perceive and experience the process of applying to the courts for ANH-withdrawal and consider the ethical and practice implications of our findings. Our analysis of family experience supports arguments grounded in economic and legal analysis that court approval should no longer be required. We conclude with some suggestions for how we might develop other more efficient, just and humane mechanisms for reviewing best interests decisions about ANH-withdrawal from these patients. (shrink)

Transactive memory theory describes the processes by which benefits for memory can occur when remembering is shared in dyads or groups. In contrast, cognitive psychology experiments demonstrate that social influences on memory disrupt and inhibit individual recall. However, most research in cognitive psychology has focused on groups of strangers recalling relatively meaningless stimuli. In the current study, we examined social influences on memory in groups with a shared history, who were recalling a range of stimuli, from word lists to personal, (...) shared memories. We focused in detail on the products and processes of remembering during in-depth interviews with 12 older married couples. These interviews consisted of three recall tasks: (1) word list recall; (2) personal list recall, where stimuli were relevant to the couples’ shared past; and (3) an open-ended autobiographical interview. We conducted these tasks individually and then collaboratively two weeks later. Across each of the tasks, although some couples demonstrated collaborative inhibition, others demonstrated collaborative facilitation. We identified a number of factors that predicted collaborative success, in particular, group-level strategy use. Our results show that collaboration may help or hinder memory, and certain interactions are more likely to produce collaborative benefits. (shrink)

United States federal regulations for pediatric research with no prospect of direct benefit restrict institutional review board (IRB) approval to procedures presenting: 1) no more than "minimal risk" (§ 45CFR46.404); or 2) no more than a "minor increase over minimal risk" if the research is commensurate with the subjects' previous or expected experiences and intended to gain vitally important information about the child's disorder or condition (§ 45CFR46.406) (DHHS 2001). During the 25 years since their adoption, these regulations have helped (...) IRBs balance subject protections with the pursuit of scientific knowledge to advance children's welfare. At the same time, inconsistency in IRB application of these regulations to pediatric protocols has been widespread, in part because of the ambiguity of the regulatory language. During the past decade, three federally-charged committees have addressed these ambiguities: 1) the National Human Research Protections Advisory Committee (NHRPAC) (Washington, DC), 2) the Institute of Medicine (IOM) Committee on the Ethical Conduct of Clinical Research Involving Children (Washington, DC); and 3) the United States Department of Health and Human Services Secretary's Advisory Committee for Human Research Protections (SACHRP) (Washington, DC). The committees have reached similar conclusions on interpretation of language within regulations § § 45CFR46.404 and 406; these conclusions are remarkably consistent with recent international recommendations and those of the original National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research (1977) report from which current regulations are based. Drawing on the committees' public reports, this article identifies the ethical issues posed by ambiguities in regulatory language, summarizes the committees' deliberations, and calls for a national consensus on recommended criteria.
