Background There is ongoing debate regarding whether and under which circumstances secondary findings (SF) should be offered in the pediatric context. Although studies have examined patient perspectives on receiving SF, little research has been conducted examining the experiences of clinicians offering SF to parents of newborns receiving genomic sequencing for a recently diagnosed medical condition.Methods To address this, we conducted qualitative interviews exploring the views and experiences of 12 clinicians who offered SF to parents of infants who had diagnostic exome (...) sequencing (ES) to identify the cause of their hearing loss. Interviews explored clinicians’ accounts of parents’ choices and decision-making about receiving SF, their views on whether and when to offer SF, their experiences returning SF, and any ethical challenges they encountered. Interviews were audio-recorded, transcribed and analyzed using inductive content analysis.Results Clinicians reported parents who declined all SF often felt finding out about future conditions unrelated to their child’s hearing loss may be unhelpful, or even harmful, or were overwhelmed by their child’s diagnosis. Clinicians also reported that some parents chose SF because they felt obliged to, even if they did not want to receive them. They explained that while some parents experienced decision-making regarding SF as positive, for others, this process was challenging or distressing. While clinicians generally agreed SF should be offered, mainly to promote parental choice, most felt SF should be offered after disclosing diagnostic results, primarily to avoid overwhelming parents. Clinicians encountered several ethical challenges, including balancing parental autonomy with non-maleficence, wanting to report or not report certain SF, and questioning whether parents can make an autonomous choice regarding SF.Conclusions Our findings, which are novel as they relate to parents of young infants with a recent diagnosis of hearing loss, add new insights into clinicians’ and parents’ decision-making regarding SF in pediatrics. Background: Personal genomic testing (PGT) offers individuals genetic information about relationships, wellness, sporting ability, and health. PGT is increasingly accessible online, including in emerging markets such as Australia. Little is known about what consumers expect from these tests and whether their reflections on testing resonate with bioethics concepts such as autonomy. Methods: We report findings from focus groups and semi-structured interviews that explored attitudes to and experiences of PGT. Focus group participants had little experience with PGT, while interview participants had (...) undergone testing. Recordings were transcribed and analyzed using thematic analysis. Findings were critically interpreted with reference to bioethics scholarship on autonomy. Results: Fifty-six members of the public participated in seven focus groups, and 40 individuals were interviewed separately. Both groups valued the choice of PGT, and believed that it could motivate relevant actions. Focus group themes centered on the perceived value of choices, knowledge enabling action and knowledge about the self. Interview themes suggest that participants reflexively engage with their PGT information to make meaning, and that some appreciate its shortcomings. Critical interpretation of findings shows that while consumers of PGT are able to exercise a degree of autonomy in choosing, they may not be able to achieve a substantive conceptualization of autonomy, one that promotes alignment with higher-order desires. Conclusions: PGT consumers can critically reason about testing. However, they may uncritically accept test results, may not appreciate drawbacks of increased choice, or may overestimate the potential for information to motivate behavioral change. While consumers appear to be capable of substantive autonomy, they do so without ongoing support from companies. PGT companies promote a problematic (“default”) account of autonomy, reliant on empowerment rhetoric. This leaves consumers vulnerable to making decisions inconsistent with their higher-order desires. As PGT expands, claims about its power and value need to be carefully drawn. (shrink)

Schizophrenia as a pathology of self-awareness has attracted much attention from philosophical theorists and empirical scientists alike. I view schizophrenia as a basic self-disturbance leading to a lifeworld of solipsism adopted by the sufferer and explain how this adoption takes place, which then manifests in ways such as first-rank psychotic symptoms. I then discuss the relationships between these symptoms, not as isolated mental events, but as end-products of a loss of agency and ownership, and argue that symptoms like thought insertion (...) and other ego-boundary disorders are by nature a multitude of paradoxes created by a fragmented awareness. I argue that such fragmentation does not always require or lead to a delusional elaboration as the definitive feature of its phenomenology, and present reasons for the role of the first-person pronoun as a mere metaphor used to represent the patient’s bizarre experiences where sensory perception and thinking processes converge. COVID-19 is compromising all aspects of society, with devastating impacts on health, political, social, economic and educational spheres. A premium is being placed on scientific research as the source of possible solutions, with a situational imperative to carry out investigations at an accelerated rate. There is a major challenge not to neglect ethical standards, in a context where doing so may mean the difference between life and death. In this paper we offer a rubric for considering the ethical challenges in (...) COVID-19 related research, in the form of an ethics toolkit for global research developed at the University of Edinburgh in collaboration with more than 200 global researchers from around the world. This toolkit provides a framework to support confrontation of ethical conflicts through the integrated and iterative analysis of Place, People, Principles and Precedents, throughout the research journey. Two case analyses are offered to exemplify the utility of the toolkit as a flexible and dynamic tool to promote ethical research in the context of COVID-19. (shrink)

We argue that several apparently distinct responses to the hole argument, all invoking formal or mathematical considerations, should be viewed as a unified “mathematical response.” We then consider and rebut two prominent critiques of the mathematical response before reflecting on what is ultimately at issue in this literature.

Different areas of child welfare work call for psychometric measurement to replace professionals’ judgements with objective numbers. Using data from a national Swedish evaluation of interventions for abused children, the present article investigates child interviewees’ resistance to constraints in psychometric questions. The article contributes to studies of how psychology operates in institutional settings; it looks into the discursive production of the interviewee’s position in the struggle between the principle of recordability and ‘sensitive’ interviewing. The findings suggest that interviewees resist questions’ (...) structural restrictions in the service of telling the interviewer about their lives. Thus, the function of their misalignment is affiliative; it cooperates both with the institutional goal of eliciting information and with the interviewer’s interest in them. However, the interviewer’s solutions to children’s problem presentations remain focused on generating recordable answers. The article concludes that the narrow focus on recordability neglects important opportunities to talk about children’s experiences. (shrink)

Como mecanismos emocionales más relevantes se encuentranla implicación —del enseñante y el aprendiz— la resonancia y la proximidad emocional. In the present work, we will analyze the concept of intuition mainly in relation to the epistemological and the metaphysical theses of Schopenhauerian theory. In the first section, we will discuss the central axes of Schopenhauer’s metaphysical system, especially regarding the concept of will (Wille) and the relationship that this entails with his theory of knowledge. Then, we will examine the difference that the German philosopher establishes between representative —or mediated— rational knowledge and direct —or immediate— intuitive knowledge. Likewise, we (...) will trace at first the theses and the fundamental problems of the dualism of representation and we will furtherly establish the problem of the intuition of one’s own body. Finally, we will consider the scope and the limits of intuition, as well as its different variants: mainly aesthetic intuition and its culmination in mystical intuition. (shrink)

In this article, I give a counterexample to a claim made in that empirically equivalent theories can often be regarded as theoretically equivalent by treating one as having surplus structure, thereby overcoming the problem of underdetermination of theory choice. The case I present is that of Lorentz's ether theory and Einstein's theory of special relativity. I argue that Norton's suggestion that surplus structure is present in Lorentz's theory in the form of the ether state of rest is based on a (...) misunderstanding of the role that the ether plays in Lorentz's theory, and that in general, consideration of the conceptual framework in which a theory is embedded is vital to understanding the relationship between different theories. Universities increasingly play an important role in entrepreneurship, which has contributed to gender equality in the business world. The aim of this study is to establish a causal model of entrepreneurial intentions and explore it by gender, based on the dimensions of the Theory of Planned Behavior, and how these are mediated by the individuals’ resilience and psychological well-being. The previous work experience was considered as one of the control variables, in order to analyze whether this influence the entrepreneurial intention. (...) With a convenience sample of 644 Portuguese students, Structural Equation Modeling was used. For a better understanding, multivariate analyses were performed and each one was individually reported, and for appropriate comparisons by gender, the t-student test was used. The comparison of means, between genders, showed that there are differences only between perceived behavioral control, subjective norm, and entrepreneurial intention, with women scoring the highest values, and psychological well-being, with men scoring the higher. A serial mediation path was performed, and psychological resilience was found to mediate a significant effect between perceived behavioral control and intention in females, but not in males. It also mediates a significant effect between attitude and intention in females, but not in males. These results show that attitude is a determining factor for females to become entrepreneurs. Finally, after discussing the results, theoretical and practical contributions are analyzed, with regard to the field of entrepreneurship in Portugal, and alternatives are pointed out for a more entrepreneurial future, reinforcing the role of higher education institutions. Embodiment theories have proposed a reciprocal relationship between emotional state and bodily reactions. Besides large body postures, recent studies have found emotions to affect rather subtle bodily expressions, such as slumped or upright sitting posture. This study investigated back muscle activity as an indication of an effect of positive and negative emotions on the sitting position. The electromyography activity of six back muscles was recorded in 31 healthy subjects during exposure to positive and negative affective pictures. A resting period was (...) used as a control condition. Increased muscle activity patterns in the back were found during the exposure to negative emotional stimuli, which was mainly measured in the lumbar and thorax regions. The positive emotion condition caused no elevated activity. The findings show that negative emotions lead to increased differential muscle activity in the back and thus corroborate those of previous research that emotion affects subtle bodily expressions. En este artículo buscamos mostrar que, tanto en la filosofía de Schopenhauer como también en la de Bergson, la experiencia estética se constituye como el paradigma del conocimiento intuitivo. A diferencia de lo que establece la teoría kantiana, para Schopenhauer y Bergson la intuición no sólo es la única forma de conocimiento inmediato, sino que no necesita de conceptos para aprehender aquello dado en la sensibilidad. Asimismo, puesto que actualmente no encontramos demasiada literatura sobre la influencia de Schopenhauer en la (...) filosofía de Bergson, dicho análisis pretende contribuir al establecimiento de los posibles vínculos entre ambas teorías. An in-depth analysis of the recent reform in Italian psychiatry reveals that the relevance of these changes transcends national borders. However, these changes are, from the scientific point of view, worth much more than mere biased pragmatic interest. The reshaping of theory made possible by the transformation of Italian psychiatry in fact opens up new prospects for a scientifically founded form of psychiatric care. Thanks to the new Mental Health Act (No. 180 of 1978),2 it has been possible to set (...) up a type of administrative and institutional organization in which the psychiatric hospital no longer constitutes a functional part of the structures of psychiatric care. This reform thus provides the launching path for a real alternative where current psychiatric ideas and working methods can no longer be accepted as the gospel truth. This paper discusses how a correct problem-definition of psychiatric admission can provide a basis for a correct approach to psychopathological behaviour in a setting where the psychiatric hospital has no role to play and for the consolidation of this new practice. We address a recent proposal concerning ‘surplus structure’ due to Nguyen et al.. We argue that the sense of ‘surplus structure’ captured by their formal criterion is importantly different from—and in a sense, opposite to—another sense of ‘surplus structure’ used by philosophers. We argue that minimizing structure in one sense is generally incompatible with minimizing structure in the other sense. We then show how these distinctions bear on Nguyen et al.’s arguments about Yang-Mills theory and on the hole argument.

In 2018, Irish citizens voted overwhelmingly to repeal the Eighth Amendment to the Constitution to allow for the introduction of a more liberal abortion law. In this article, I develop a retrospective reading of the stubborn persistence of the denial of reproductive rights to women in Ireland over the decades. I argue that the ban’s severity and longevity is rooted in deep-seated, affective attachments that formed part of processes of postcolonial nation-building and relied on shame and the construction of the (...) Irish nation as a particular, gendered place. The article develops the notion of ‘gendered displacement’ to conceptualise abortion travel in the context of the history of women’s coercive confinement, and provides an affective, feminist reading of the interlinkages between place and nationhood. It also draws on three cases—the X, Y and Z cases—to illustrate the centrality of place and women’s occupation of space to the analysis of Ireland’s abortion ban, which should be read in the wider context of the legacy of what I term the ‘affective politics of place’. This paper develops and defends a paradigm-based explanation of respect. Paradigm-based explanations propose to illuminate subject matter that are basically disunified, by identifying a form of them (“the paradigm”) that is then shown to be explanatorily basic to the subject as a whole. This explanatory strategy is well-suited to the subject of respect, which is widely agreed to encompass two distinct kinds, appraisal respect and recognition respect. Accordingly, the paper sets out to determine which of these two kinds is paradigmatic (...) and which, derivative. It begins by advancing a novel account of appraisal respect (“perspectival respect”), which it also hypothesizes is respect in its paradigmatic form. On this hypothesis, perspectival respect is paradigmatic in virtue of its apparent “point” or function, which is that it constitutively aims at understanding another person’s perspective on things. The paper then tests the viability of this hypothesis by showing how one central form of recognition respect (“respect for persons”) can reasonably be understood to derive from respect in its paradigmatic form, by showing how it is illuminated by the aim of interpersonal understanding. Patients with psychotic disorders experience a range of reality distortions. These often include auditory-verbal hallucinations, and thought insertion to a lesser degree; however, their mechanisms and relationships between each other remain largely elusive. Here we attempt to establish a integrative model drawing from the phenomenology of both AVHs and TI and argue that they in fact can be seen as ‘spectra’ of experiences with varying degrees of agency and ownership, with ‘silent and internal own thoughts’ on one extreme and ‘fully (...) external and clearly audible voices’ in the absence of a speaker on the other. We believe a spectral model will add emphasis to the continuity of experience and help to better understand how one type of psychotic symptom may interact with another, and put forward the argument that the experience of TI itself is not sufficient to classify as a delusion. In addition we aim to discuss some of the conceptual issues surrounding AVHs and TI with first-person accounts and current philosophical and neuropsychological theories in mind. We propose that the mechanisms behind AVHs and TI are more complex than source-monitoring deficits; indeed, to understand such phenomena one must appreciate that their very ‘existence’ and ‘reality’ as experienced by the individual have much deeper implications and meaning, both philosophically and clinically. Les refuges des migrants expulsés à la frontière du Mali et de l’Algérie montrent comment, dans une situation extrême d’abandon et d’isolement, ceux-ci s’organisent pour survivre, mais mettent également en place des stratégies de dépassement de la coercition (...) exercée à leur endroit par les pouvoirs étatiques. Les murs de leurs abris précaires marquent l’amorce d’une opposition organisée contre les murs des États.Any thinking on the moment that follows expulsion has to cross the bounds of political sovereignty and the territories it governs, to move into the social and political effects of expulsion that arise in migrants’ countries of origin and/or transit. The shelters erected by migrants expelled at the frontier between Mali and Algeria show how, in a situation of extreme abandonment and isolation, migrants not only organise their own survival but also develop strategies to overcome the coercion that the State apparatus exerts upon them. This communication wants to deal with the relations between the Philosophy of Plato and the cult of Dionysus in Ancient Greece. This makes necessary to distinguish the cult of Dionysus with its feasts and secret rituals, especially the maenadic rites of women, from orphism. The Maenads practiced rites closely associated with mania and those rites included bloody sacrifice (sparagmos) and the consommation of raw flesh (omophagy). As we assumed in our book“Dionysos em transe: la voix des femmes”, this cult was (...) an iniciatic feminine rite, connected to biological transformations particular to women, as pregnance and birth. At the other side, orphism is an asceptic and vegetarian sect, which clearly influenced some platonic theories, as exposed in Cratyle or Gorgias, concerning “the body as the tomb of the soul”. What we want to show here is the dionysiac character of some other theories, as those about the mania in Phaedrus or even the so called “platonic theory of Love” in the Symposium. We are dealing than with the definition of Philosophy itself in platonic dialogs: is Philosophy a study about death as orphic influence demands or is it a work of birth, as dionysiac influence suggests? Pragmatist philosopher John Dewey famously stated that man is a creature of habit, and not of reason or instinct. In this paper, I will assess Dewey’s explication of the habituated self and the potential it holds for radical transformative processes. In particular, I will examine the process of coming to feminist consciousness, and will show that a feminist-pragmatist reading of change can accommodate a view of the self as responsible agent. Following the elucidation of the changing self, I will appraise (...) key pragmatist concepts of inquiry, such as doubt and self-reflexivity, with regard to their treatment of deep-seated internalisations of oppressive norms and the initiation of change. Ultimately, I will argue that a feminist-pragmatist understanding of transformation is conducive not only to the project of personal transformation, but also to social and political change more generally. Ce livre est centré sur les rapports entre religion et philosophie en Grèce antique. Il s'agit en particulier de dégager les fondements philosophiques du rituel féminin des bacchantes et de montrer comment celui-ci trouve un écho dans la vie quotidienne des Grecs, théâtre compris, et un prolongement dans les doctrines philosophiques, notamment dans les textes des philosophes pythagoriciens, dans le platonisme par la voix de Diotime et dans le stoïcisme.

En este trabajo se analizan ciertos aspectos de la sociedad nazarí de finales del siglo XV. Para hacerlo, se emplean las informaciones contenidas en un pleito de hidalguía de principios del siglo XVI. Este pleito afectó a los descendientes de un converso granadino hijo de un alcaide de Iznalloz. La particular perspectiva que proporcionan estos testimonios, que realizan una equiparación entre hidalguía castellana y parte de la oligarquía granadina, plantean numerosas preguntas sobre la sociedad del Reino de Granada a finales (...) de la Edad Media.
