BackgroundHIV prevention research in resource-limited countries is associated with a variety of ethical dilemmas. Key amongst these is the question of what constitutes an appropriate standard of health care (SoC) for participants in HIV prevention trials. This paper describes a community-focused approach to develop a locally-appropriate SoC in the context of a phase III vaginal microbicide trial in Mwanza City, northwest Tanzania.MethodsA mobile community-based sexual and reproductive health service for women working as informal food vendors or in traditional and modern (...) bars, restaurants, hotels and guesthouses has been established in 10 city wards. Wards were divided into geographical clusters and community representatives elected at cluster and ward level. A city-level Community Advisory Committee (CAC) with representatives from each ward has been established. Workshops and community meetings at ward and city-level have explored project-related concerns using tools adapted from participatory learning and action techniques e.g. chapati diagrams, pair-wise ranking. Secondary stakeholders representing local public-sector and non-governmental health and social care providers have formed a trial Stakeholders' Advisory Group (SAG), which includes two CAC representatives.ResultsKey recommendations from participatory community workshops, CAC and SAG meetings conducted in the first year of the trial relate to the quality and range of clinic services provided at study clinics as well as broader standard of care issues. Recommendations have included streamlining clinic services to reduce waiting times, expanding services to include the children and spouses of participants and providing care for common local conditions such as malaria. Participants, community representatives and stakeholders felt there was an ethical obligation to ensure effective access to antiretroviral drugs and to provide supportive community-based care for women identified as HIV positive during the trial. This obligation includes ensuring sustainable, post-trial access to these services. Post-trial access to an effective vaginal microbicide was also felt to be a moral imperative.ConclusionParticipatory methodologies enabled effective partnerships between researchers, participant representatives and community stakeholders to be developed and facilitated local dialogue and consensus on what constitutes a locally-appropriate standard of care in the context of a vaginal microbicide trial in this setting.Trial registrationCurrent Controlled Trials ISRCTN64716212. The well-known experiments of Nisbett and Wilson lead to the conclusion that we have no introspective access to our decision-making processes. Johansson et al. have recently developed an original protocol consisting in manipulating covertly the relationship between the subjects’ intended choice and the outcome they were presented with: in 79.6% of cases, they do not detect the manipulation and provide an explanation of the choice they did not make, confirming the findings of Nisbett and Wilson. We have reproduced this protocol, (...) while introducing for some choices an expert guidance to the description of this choice. The subjects who were assisted detected the manipulation in 80% of cases. Our experiment confirms Nisbett and Wilson’s findings that we are usually unaware of our decision processes, but goes further by showing that we can access them through specific mental acts. The implicit activities of the living body in sports (such as heart rate, involuntary gestures, stress, reflex, emotional regulation and interaction expressions) emerge in the consciousness of the lived body without our voluntary control. We demonstrate physiological emersion, and how, including in dramaturgical perception, physiological flows and processes collide with the image of a whole body. In this paper, we introduce corporeal non-property as the missing (?) link between phenomenology and neuroscience, renewed by research on the cerebral unconscious and the (...) perception of their lived body. Rather than mindfulness, physical activity obliges the subject to learn the unconscious and his motor skills from his living body: a new psychoanalysis of sport is thus established by studying what emerges from the activity of the living body up to the level of the conscious description of motor action. In this paper, we explain why that awareness occurs, and why it matters for the understanding of unconscious activity of the athlete’s living body. This is potentially interesting, although it needs, as a theory, to overcome the obvious reliance of the body on unconscious or habitual/automated physiological processes. But the incorporation of body techniques forms a habitus. (shrink)

As an institution, the ?postmodern university? is central to the canon of today's research on higher education policy. Yet in this essay I argue that the postmodern university is a fiction that frames and inhibits our thinking about the future university. To understand why the postmodern university is a fiction, I first turn to grand theory and ask whether we can make sense of the notion of ?post?-postmodernity. Second, I turn to the UK higher education sector and show that the (...) postmodern university is a chimera, a modern artefact of competing instrumentalist, gothic, and postmodernist discourses. Third, I discuss competing visions of the future university and find that the progressive (yet modernist) agendas that re-imagine the public value of knowledge production, transmission, and contestation, are those that can move us beyond the palliative and panacea of the postmodern university. Individual differences in children's talk about inner states are striking, but how should they be interpreted? This study used transcripts of preschoolers’ conversations with siblings and best friends to address this question in two ways. Our first aim was to elucidate the exact nature of individual differences by contrasting categories (emotion/desire vs. cognitive state) and referents (own vs. other/shared) of inner state talk. Our second aim was to compare performance vs. competence views of inner state talk by exploring (i) the (...) stability of individual differences in inner state talk across different relationships and (ii) the cognitive correlates of inner state talk. A sample of 44 children (mean age = 4 years 3 months) was observed for 20 minutes at home playing with a sib and for 20 minutes at school playing with a best friend. Videos were transcribed and coded for the frequency and form of inner state talk (e.g., talk about different categories of inner state, or about own vs. others’ inner states). During the school visits, children completed a set of tasks tapping theory-of-mind skills and verbal ability. Individual differences in inner state talk (especially reference to others’ inner states) were stable across relationships. There was no association between individual differences in talk about cognitive vs. emotion/desire states, although both categories of inner state talk were significantly associated with individual differences in children's theory-of-mind skills. Si l’on s’accorde à voir dans l’ouvrage du clergyman mélancolique Robert Burton paru en 1621 une sorte d’aboutissement et de consécration de la mode mélancolique, l’on a toutefois tendance à négliger le fait que l’anatomiste utilise le discours médical et la tradition mélancolique pour attirer l’attention de ses contemporains sur l’existence d’un désordre qui se manifeste, au niveau de la collectivité, par une crise religieuse, politique, sociale et économique. C’est sous le patronage de l’un des premiers représentants du courant mercantiliste (...) que Burton se place pour justifier l’emploi de la notion de « mélancolie politique » : celui de Giovanni Botero. La lecture de la « Préface » de L’Anatomie de la Mélancolie permet de constater l’existence d’une forte convergence entre les méthodes et les analyses des premiers mercantilistes anglais et le diagnostic de Robert Burton sur la crise économique que traverse l’Angleterre de Jacques Ier. Comment expliquer alors la coexistence dans le même texte de ce discours économique qui propose des réformes concrètes pour remédier à une situation de crise et d’un discours utopique qui préconise une forme autoritaire et disciplinaire de pouvoir comme remède à la mélancolie politique tout en affirmant le caractère chimérique et irréalisable de tout changement radical? Ben Rich, J.D., Ph.D., presents a scholarly, passionate view of the ethics of the “barriers to effective pain management.” His manuscript is detailed, analytical, and compassionate. No reasonable sensitive person, especially a physician committed to caring for patients, can disagree with the proposal that human beings should have their physical, emotional, and spiritual pain tended to aggressively, meticulously, and compassionately. Similarly, the same individuals advocating for such pain management would agree that no one should go to jail unless he or (...) she is guilty of a serious crime, that decent people should not be robbed or murdered, that children should not be hungry or homeless, and that all citizens of the United States deserve healthcare. Our society attempts to achieve these goals. Laws are written, discussed, and approved by state and federal congresses, voted on by citizens, and theoretically upheld by the courts, churches, and decent individuals. But, unless the world suddenly becomes inhabited by virtuous, ethical humans who can unfailingly differentiate “good” from “bad,” then, in spite of an abundance of laws and lawyers, doctors, and nurses, this world will continue to have pain and suffering. And, although we want to hold our doctors, politicians, educators, champion athletes, and others to “higher standards” than the average citizen, it is best to remind ourselves frequently that all humans can be weak and are bound to make imprecise judgments, that there is not a homogenous definition of “good,” that values and religious beliefs are variable. There is limited consensus about what constitutes humanly sensitive care, or how it can be sustained in care settings. A new humanised care assessment tool may point to caring practices that are up to the task of meeting persons as humans within busy healthcare environments. This paper describes qualitative development of a tool that is conceptually sensitive to human dimensions of care informed by a life‐world philosophical orientation. Items were generated to reflect eight theoretical dimensions that constitute what makes care (...) feel humanly focused. An action research group process in 2014–2015 with researchers, service users, healthcare professionals in two diverse clinical settings (stroke rehabilitation and dermatology) was used. Feedback on conceptual content, transparency of meaning and readability was then gained from a panel in Sweden and third‐year student nurses in the UK. The tool can be applied to attune staff to human dimensions of care, offering items which point to concrete examples of humanising and dehumanising features of practice in ways that have not yet been fully captured in the caring literature. Based on theoretically led experiential items, with dedicated focus on what makes people feel more, or less than human, it may offer improvement on available assessments of care. (shrink)

In the 1970s and 1980s a strong opposition and anxiety towards biological and naturalizing knowledges was the norm in feminist discourse. In the past decades the certainties of that ‘anti-biologism’ have been challenged, in part because of a new recognition of the role of contingency in both biological determination and biological science. What seems to have survived the shift is a set of normative assumptions concerning the role of determinacy and contingency (or being-born and becoming) in the political implications of (...) ontological claims: an assumed political valorization of contingency. This article challenges those assumptions. It draws attention to the embrace of contingency and processuality on the part of supremacist biopolitical discourse, and suggests the need to think again about the politics of contingency and becoming (in constructivist as well as biologistic discourses). Focusing on the issue of racism and supremacist-specification, the article takes a genealogical look at ‘second-wave’ feminist anti-biologism. Monique Wittig’s materialist feminist attack on naturalizing ideology and ‘the myth of woman’ provides the (ideal-typical) historical example. The article draws attention to curious absences in Wittig’s (and Rosalind Rosenberg’s) anti-biologistic statements concerning early 20th-century biologistic feminism: the absence of a critique of eugenics, racism and supremacism. Arguably the condemnation of biology as a conservative ‘ideology of the status quo’ created masks for biopolitical ontology, obscuring the progressive, dynamic, processual character of biologism and of modern racism. While dislodging some powers of biologistic discourse, feminist anti-biologism might also have played a part in facilitating the revitalization of biopolitical racism within the constructivist culturalist rubric. The aim of the article is not to critique ‘second-wave’ feminism from the perspective of contemporary scholarship, but to help generate new ways of thinking and feeling about the role of ontology, contingency and temporality in the present politics of classification. Gender, Power and Sexuality is a collection of original and exciting articles by well-known feminists which makes a major contribution to our understanding of the ways in which men exercise control over girls and women in their daily lives, in the home, at school, at work and in the courts. Women are seen to resent and challenge male power, but, the institutionalisation of male power is shown to mitigate against women taking control over their own lives.

The proposed national PrEP program would serve people who are uninsured as well as those enrolled in Medicaid. In this article, the authors propose a set of recommendations for the proposed program’s implementers as well as state Medicaid agencies and Medicaid managed care organizations to ensure PrEP access for people enrolled in Medicaid, addressing gaps without undermining the important role of the Medicaid program in covering and promoting PrEP.

This article aims to characterise the visions of ecologisation found within scientific approaches embraced by different epistemic communities, and which have inspired empirical work and public action on agrifood system transitions. Based on comparative readings of works anchored in our two disciplinary fields (ecology and sociology), we identified six large ensembles of epistemic communities as well as their points of convergence and divergence. We identify six ideotypical visions of ecologisation based on the types of ‘relationships to nature’ embedded in these (...) large sets of epistemic communities: protectionism, functionalism, structuralism, post-structuralism, relational and pragmatist-experience-based. We suggest that pragmatist-experience-based approaches allow us to transcend two classical oppositions: between realism and constructivism, and between a conception of nature as passive and external as opposed to active and relational. Without claiming to offer a detailed analysis of these approaches, we hope that our work can be used as a tool to support reflection among scientists and other actors involved in agrifood system transitions. This book provides an up-to-date introduction to the study of generics and pursues the enterprise of the influential Generic Book edited by Gregory Carlson and Jeffry Pelletier, which was published in 1995. Genericity is a key notion in the study of human cognition as it reveals our capacity to organize our perceived reality into classes and to describe regularities. The generic can be expressed at the level of a word or phrase (ie the potato in The Irish economy became dependent (...) upon the potato) or an entire sentence (eg in John smokes a cigar after dinner, the generic aspect is a property of the expression, rather than any single word or phrase within it). This book gathers new work from senior and young researchers to reconsider the notion of genericity, examining the distinct contributions made by the determiner phrase (eg the notions of kind/individual) and the verbal predicate (eg the notions of permanency, disposition, ability, habituality, and plurality). Finally, in connection with the whole sentence, the analytic/synthetic distinction is discussed as well as the notion of normality. In lieu of an abstract, here is a brief excerpt of the content:Moments of Beating Addiction and Inscription in Virginia Woolf’s “A Sketch of the Past”Barbara Claire Freeman (bio)My title, which alludes to the collection of autobiographical essays authored by Virginia Woolf and entitled Moments of Being, implies that being and beating are co-constitutive and that exploring their interdependence may shed light upon the logic that binds the one to the other. In particular, I want to examine the ways (...) in which Woolf’s “A Sketch of the Past”—it is the longest essay in the collection and presumably incomplete, since Woolf wrote the last entry shortly before killing herself in 1941—sets up a complex relationship between beating, writing, and compulsion. For the text in which the phrase “moment of being” first occurs also reveals that the activity of writing, to which Woolf was addicted, has the symbolic valence of beating and functions at the level of language in the same ways that beating functions on the body. I will begin to unpack this perhaps obscure equation by focusing upon the relationship between being and beating in the experiences that define Woolf’s “moments of being.” 1In the opening pages of “A Sketch,” Woolf describes the routine, eminently forgettable aspects of daily life as “a kind of nondescript cotton wool”; the phrase is her “private shorthand” for the general and diffuse “moments of non-being” that predominate in daily life and in which, by contrast, “moments of being” are “embedded” [70]. “Every day,” Woolf notes, “includes much more non-being than being”; and although “the real novelist can somehow convey both sorts of being” [70], Woolf believes that “behind the cotton wool is hidden a pattern” [72] and that her vocation as a writer is to discover and communicate it.Though occasionally “moments of being” produce the experience of rapture or ecstasy, an aspect of beating I will presently explore, most frequently they name moments that entail negation and self-shattering. “Moments of being” are moments of extreme violence that “remain interesting” [78] not because they lead to self-transcendence or discovery, but because they provoke a sense of utter helplessness and paralysis. With one exception, Woolf’s “moments” involve memories of being beaten up by life: “a sudden violent shock” occurs “when something happened so violently that I have remembered it all my life” [71]. Woolf’s submission to her brother Thoby’s beating remains in her memory as inaugural:The first: I was fighting with Thoby on the lawn. We were pommelling each other with our fists. Just as I raised my fist to hit him, I felt: why hurt another person? I dropped my hand instantly, and stood there, and let him beat me. I remember the feeling. It was a feeling of hopeless sadness. I slunk off alone, feeling horribly depressed.[71]The shock of a beating occasions Woolf’s knowledge that rupture may function as a form of revelation; its reception initiates a drive to repeat in words the impact and imprint of the blows she has received.
