Since its introduction in the 1980s, futility as a concept has held contested meaning and applications throughout medicine. There has been little discussion within the psychiatric literature about the use of futility in the care of individuals experiencing severe and persistent mental illness (SPMI), despite some tacit acceptance that futility may apply in certain cases of psychiatric illness. In this paper, we explore the literature surrounding futility and argue that its connotation within medicine is to describe situations where patients (or (...) their substitute decision-makers) believe that interventions will almost certainly provide no meaningful benefit. We then provide two arguments in support of the use of futility within the care of individuals experiencing SPMI: that some SPMI can be considered a terminal illness, and that the risk-benefit ratio is a dynamic entity such that futility can help describe what Gillett calls the ‘risk of unacceptable badness’ when it comes to considering how an intervention might impact a patient’s quality of life. We posit that capacity should not pose an obstacle to declaring futility when caring for individuals experiencing SPMI and explain how futility is not antithetical to recovery in mental health. Finally, we describe how using futility within psychiatric practice can allow for a reorientation of care by signalling the need to shift to a palliative approach. (shrink)

The authors take on critics of science whose views range from moderate to extreme, from social constructivists to deconstructionists, from creationists and feminists to Afro-centrists. They discuss the rise of "alternative medicine" and radical environmentalism (here skewered as "ecosentimentalism"). They explain why the "uncertainty principle" does not work as a metaphor for ambiguity, and why "chaos theory" cannot be invoked without an understanding of mathematics. The aim of this paper is to trace in Leibniz’s drafts the sketched outline of a conceptual framework he organized around the key concept of ‘requisite’. We are faced with the project of a semi-formal theory of conditions, whose logical skeleton can have a lot of different interpretations. In particular, it is well suited to capture some crucial relations of ontological dependence. Firstly the area of ‘mediate requisites’ is explored - where causal and temporal relations are dealt with on the (...) basis of a general theory of ‘consequence’.Then the study of ‘immediate requisites’ is taken into account - a true sample of mereological inquiry, where Leibniz strives for a unitary treatment of part-whole relation, conceptual inclusion and inherence. Far from simply conflating these relations one with another and with causality, therefore, Leibniz tried to spell them out, while at the same time understanding them within a single conceptual framework. To engender the Jewish past is to continue to question how and what we think we already know about Jewish history and Jewish memory. In order to imagine other stories, we must risk engaging in other ways of doing Jewish study. Only by repeatedly engaging in these other practices can we begin to undo the assumptions about gender we have come to assume as normal or natural. Research on sleep loss and vigilance both focus on declines in cognitive performance, but theoretical accounts have developed largely in parallel in these two areas. In addition, computational instantiations of theoretical accounts are rare. The current work uses computational modeling to explore whether the same mechanisms can account for the effects of both sleep loss and time on task on performance. A classic task used in the sleep deprivation literature, the Psychomotor Vigilance Test, was extended from the typical 10-min duration (...) to 35 min, to make the task similar in duration to traditional vigilance tasks. A computational cognitive model demonstrated that the effects of time on task in the PVT were equivalent to those observed with sleep loss. Subsequently, the same mechanisms were applied to a more traditional vigilance task—the Mackworth Clock Task—providing a good fit to existing data. This supports the hypothesis that these different types of fatigue may produce functionally equivalent declines in performance. (shrink)

Biobanks that are run on an opt-in basis depend on people having the motivation to give and to trust in those who control their samples. Yet in the UK trust in the healthcare system has been in decline and there have been a number of health-related scandals that have received widespread media and public attention. Given this background, and the previous public consultations on UK Biobank, the paper explores the way people express their trust and mistrust in the area of (...) medical and genetic information through discussions in six focus groups held in different areas of Britain. The themes that emerged in the discussion were the special character of genetic data; the ease with which information spreads; the public/private divide in genetic research; the potential of genetic information to harm people; choice, consent and lack of control; and the difficulty of establishing trustworthy governing arrangements. The expert agenda of policy-makers and medical ethics does not address the broader concerns expressed by participants. It seems that public consultation and the language of openness and transparency may not be sufficient to establish trust in the governance of genetic databases. Donors will be asked to give freely without any return but the participants' perception of those using the samples is that they are motivated by success and profit rather than healthcare priorities. In this context altruism seems naive and even dangerous. In order to place their trust well people need evidence of a relationship with obligations and expectations on both sides. (shrink)

The organ shortage is commonly presented as having a clear solution, increase the number of organs donated and the problem will be solved. In the light of the Northern Ireland Assembly’s consultation on moving to an opt-out organ donor register this article focusses on the social factors and complexities which impact strongly on both the supply of, and demand for, transplantable organs. Judging by the experience of other countries presumed consent systems may or may not increase donations but have not (...) met demand. Donation rates have risen considerably in all parts of the UK recently but there is also an increasing demand for organs. Looking at international donation rates and attitudes, future demand for organs and education on donation, the question is whether the organ shortage could ever be met. The increase in longevity, in rates of diabetes and obesity and in alcohol related liver disease all contribute both to increased demand for transplants, and re-transplants, and a reduction in the number of usable organs. It is unlikely that demand could ever be met, since, if supply was unlimited, the focus would move to financial resources and competing demands on the health care budget in a publicly funded health system. These factors point to the need to focus on ways of reducing, or at least stabilizing, demand where lifestyle factors contribute to the underlying disease. (shrink)

The last 10 years has seen the development and deployment of new biotechnologies not just as potential treatments but also as potential enhancements. The definition and differentiation of treatment from enhancement is an ongoing clinical, ethical and social debate that ranges across a proliferating number of convergent technologies. Many of these innovations will ‘come-on-line’ as present generations of young people will be reaching adulthood and considering parenthood. This paper reports on a project that explored the possibilities for human enhancement with (...) young people in order to gather their attitudes towards enhancement and the types of arguments/reasoning they employ when thinking about the possibilities and the techniques. The project focused on if/how distinctions are made between treatment and enhancement, between the different techniques that might be used for enhancement and perceptions of risks and benefits. The young people’s viewpoints, their methods of reasoning and underlying values are compared with those of bioethicists writing on the topic. (shrink)

The Ancrene Wisse, a guide for female recluses written in the West Midlands in the early thirteenth century, and the closely related religious works of the `Katherine Group', offer a vivid insight into the religious life of the time, and their rich and varied prose style blends Latin and native English stylistic traditions with remarkable skill and assurance. Visual working memory is a construct hypothesized to store a small amount of accurate perceptual information that can be brought to bear on a task. Much research concerns the construct's capacity and the precision of the information stored. Two prominent theories of VWM representation have emerged: slot-based and continuous-resource mechanisms. Prior modeling work suggests that a continuous resource that varies over trials with variable capacity and a potential to make localization errors best accounts for the empirical data. Questions remain regarding (...) the variability in VWM capacity and precision. Using a novel eye-tracking paradigm, we demonstrate that VWM facilitates search and exhibits effects of fixation frequency and recency, particularly for prior targets. Whereas slot-based memory models cannot account for the human data, a novel continuous-resource model does capture the behavioral and eye tracking data, and identifies the relevant resource as item activation. This volume contains essays which cover a range of aspects in the debate over genetic testing. It looks at both the advantages and disadvantages involved in knowing or not knowing whether one is a carrier of certain genetic traits.

Decentralised ledgers are gaining momentum following the interest of industries and people in smart contracts. Major attention is paid to blockchain applications intended for trading assets that exploit digital cryptographic certificates called tokens. Particularly relevant tokens are the non-fungible tokens (NFTs), namely, unique and non-replicable tokens used to represent the cryptographic counterpart of assets ranging from pieces of art through to licenses and certifications. A relevant consequence of the hard-coded nature of blockchains is the hardness of probing, in particular when (...) advanced searchers involving the capabilities of the smart contracts or the assets digitised by NFTs are required. For this purpose, a formal representation for the operational semantics of smart contracts and of tokens has become particularly urgent, especially in economy and finance, where blockchains become increasingly relevant. Hence, we feel the need to tailor Semantic Web technologies to achieve that semantic representation at least for NFTS. This article reports on an ontology that leverages the Ontology for Agents, Systems, and Integration of Services (“OASIS”) towards the semantic representation of smart contracts responsible for managing ERC721-compliant NFTs and running on the Ethereum blockchain. Called Ether-OASIS, the proposed ontology adopts OASIS and tailors its behaviouristic approach to the Ethereum blockchain by conceiving smart contracts as agents running on the blockchain and, consequently, smart contract interactions as agent commitments. Smart contracts are represented in terms of their actions, purposes and tokens that they manage, thus realising a blockchain that is more usable both by users and automated applications. The ontology is evaluated using standard ontological metrics and applied on a case study concerning the minting and transferring of NFTs that digitise batches of wheat. (shrink)

Objectives—The development of predictive genetic tests provides a new area where consumers can gain knowledge of their health status and commercial opportunities. “Over-the-counter” or mail order genetic tests are most likely to provide information on carrier status or the risk of developing a multifactorial disease. The paper considers the social and ethical implications of individuals purchasing genetic tests and whether genetic information is different from other types of health information which individuals can obtain for themselves.Design—The discussion is illustrated by findings (...) from a questionnaire survey of university students as potential consumers. Topics covered included what health tests they had already used, expectations of genetic tests, willingness to pay, who should have access to the results and whether there need to be restrictions on such tests.Sample—Six hundred and fifteen first-year students in the universities of Leuven, Cardiff, Central Lancashire, Vienna and Nijmegen studying either medicine or a non-science subject.Results—Students were enthusiastic about genetic tests and had high expectations of their accuracy and usefulness but most thought they should be available through the health service and a minority thought that some tests, for example for sex selection, should not be available at all. There were few differences in responses by sex or subject of study but some by country. The paper also considers ethical and social issues outside the scope of a questionnaire survey of this type.Conclusion—To address some of these issues the sale of genetic tests to individuals can be made subject to ethical guidelines or codes of practice, for example to protect vulnerable groups, but there are fundamental social and ethical questions which such guidelines cannot address. The idea of individual responsibility for action is central to our conception of what it is to be a person. Behavioural genetic research may seem to call into question the idea of individual responsibility with possible implications for the criminal justice system. These implications will depend on the understandings of the various agencies and professional groups involved in responding to violent and anti-social behaviour, and, the result of negotiations between them over resulting practice. The paper considers two kinds of approaches (...) to the question of responsibility and ‘criminal genes’ arising from a sociological and philosophical perspective respectively. One is to consider the social context and possible practical implications of research into ‘criminal genes’ which will later be examined through interviews and discussions with a range of experts including lawyers and social workers. A second and different kind of approach is to ask whether the findings of behavioural genetics ought to have implications for attributions of responsibility. In his well-known Discourse on Metaphysics , Leibniz puts individual substance at the basis of metaphysical building. In so doing, he connects himself to a venerable tradition. His theory of individual concept, however, breaks with another idea of the same tradition, that no account of the individual as such can be given. Contrary to what has been commonly accepted, Leibniz’s intuitions are not the mere result of the transcription of subject-predicate logic, nor of the uncritical persistence of some old metaphysical (...) assumptions. They grow, instead, from an unprejudiced inquiry about our basic ontological framework, where logic of truth, linguistic analysis, and phenomenological experience of the mind’s life are tightly interwoven. Leibniz’s struggle for a concept capable of grasping concrete individuals as such is pursued in an age of great paradigm changes – from the Scholastic background to Hobbes’s nominalism to the Cartesian ‘way of ideas’ or Spinoza’s substance metaphysics – when the relationships among words, ideas and things are intensively discussed and wholly reshaped. This is the context where the genesis and significance of Leibniz’s theory of ‘complete being’ and its concept are reconstrued. The result is a fresh look at some of the most perplexing issues in Leibniz scholarship, like his ideas about individual identity and the thesis that all its properties are essential to an individual. The questions Leibniz faces, and to which his theory of individual substance aims to answer, are yet, to a large extent, those of contemporary metaphysics: how to trace a categorial framework? How to distinguish concrete and abstract items? What is the metaphysical basis of linguistic predication? How is trans-temporal sameness assured? How to make sense of essential attributions? In this ontological framework Leibniz’s further questions about the destiny of human individuals and their history are spelt out. Maybe his answers also have something to tell us. This book is aimed at all who are interested in Leibniz’s philosophy, history of early modern philosophy and metaphysical issues in their historical development. (shrink)

Cissexist perception involves a prejudicial judgment and an unmediated affective response, such as that of disgust, directed at the gendered aspects of another person. In this paper, I advance a view of how cissexist perception harms disabled people. On this view, there at least two morally problematic aspects of cissexist perception: that it has painful effects, and that it restricts bodily agency. I defend this claim through an analysis of a double bind faced by people subjected to cissexist perception: on (...) the one hand, such individuals are pressured into avoiding certain environments to dodge dangerous predicaments that can arise on behalf of looking different from others; this constitutes unfair exclusion from an environment and under some conditions causes pain. On the other hand, to avoid dangerous predicaments, targets of cissexist perception are pressured into concealing their gender non-conformity to fit into certain environments; concealing frustrates their bodily agency. I conclude with remarks about how trans-affirming artwork may have the capacity to interrupt cissexist perception. (shrink)

With the rapid development of genetic research and applications in health care there is some agreement among funding and regulatory bodies that the public(s) need to be equipped to deal with the choices that the new technologies will offer them, although this does not necessarily include a role for the public in influencing their development and regulation. This paper considers the methods and purpose of public consultations in the area of genetics including large-scale surveys of opinion, consensus conferences and focus (...) groups. Consultation has been undertaken to enable the researchers/policy makers to see what the public do not know and plan more public education to make up the deficiency, to check on areas of concern so that public education can be used to address them or to gain a public mandate for a planned policy. An alternative and more recent approach is to find out what the public(s) do know and understand and to see how experts can learn from them in order to get a fuller view of technology in use.
