Background: Research concerning relatives’ participation in the everyday care related to persons living in special care units for persons with dementia is limited. Research questions: To examine relatives’ participation in their near one’s everyday care, the level of burden experienced and important factors for participation, in this special context. Design: The study had a cross-sectional design, and data collection was carried out by means of a study-specific questionnaire. Participants and context: A total of 233 relatives from 23 different special care (...) units participated. Ethical consideration: The study was approved by the Norwegian Social Science Data Services. Results: A great majority of relatives reported that they visited weekly and were the resident’s spokesperson, but seldom really participated in decisions concerning their everyday care. Participation was seldom reported as a burden. Discussion: This study indicated that relatives were able to make a difference to their near one’s everyday life and ensure quality of care based on their biographical expertise, intimate knowledge about and emotional bond with the resident. Since knowing the resident is a prerequisite for providing individualised care that is in line with the resident’s preferences, information concerning these issues is of utmost importance. Conclusion: This study prompts reflection about what it is to be a spokesperson and whether everyday care is neglected in this role. Even though relatives were satisfied with the care provided, half of them perceived their participation as crucial for the resident’s well-being. This indicated that relatives were able to offer important extras due to their biographical expertise, intimate knowledge about and emotional bond with the resident. Good routines securing that written information about the residents’ life history and preferences is available and used should be implemented in practice. (shrink)

The aim of this study was to explore the experience of nursing personnel with respect to patient participation in special care units for persons with dementia in nursing homes, with focus on everyday life. The study has an explorative grounded theory design. Eleven nursing personnel were interviewed twice. Patient participation is regarded as being grounded in the idea that being master of one’s own life is essential to the dignity and self-esteem of all people. Patient participation was described at different (...) levels as letting the resident make their own decisions, adjusting the choices, making decisions on behalf of the residents and forcing the residents. The educational level and commitment of the nursing personnel and how often they were on duty impacted the level that each person applied, as did the ability of the residents to make decisions, and organizational conditions, such as care culture, leadership and number of personnel. Background: Person-centred care is often described as an ideal way of preserving vulnerable persons’ wellbeing and dignity and an essential component of quality-care delivery. However, the staff find that making the care dignified is the most challenging issue, often because of effectivity, everyday stress and overload. In the interests of making the care more person-centred, systematic intervention involving ‘one-to-one contact’ (resident – carer) was trialled for 30 min twice a week over 12 months in two units in a nursing home (...) in Eastern Norway. Objectives: The aim of the study was to elicit healthcare staff’s experiences of implementing ‘one-to-one contact’ between residents and carers in nursing homes. Methods: The study has a grounded-theory inspired design. Two groups of health care staff were each interviewed three times. Data were collected over an 18-month period. Ethical considerations: The study was approved by the Data Protection Official for Research under the auspices of the Norwegian Social Science Data Services. Findings: The core category is ‘One-to-one contact’ at a nursing home is possible, but requires open-mindedness. The core category indicates that open-mindedness is required, since it does not take much for scepticism to take over and cause reversion to habitual practices. The category Expectant but Sceptical describes staff thoughts and experiences before the implementation phase got underway. The category Positive but Undecided describes staff experiences 6 months into the intervention and after 12 months. Conclusions: This study has revealed that systematic ‘one-to-one contact’ between resident and carer in nursing home is achievable, and that such a simple action might be an important step towards achieving more person-centred care as the resident is seen more as a person. However, in order to make a more person-centred and dignified approach to care constant attentiveness and awareness is required, as there were ongoing factors counteracting it. (shrink)

As presidential campaigns and “Saturday Night Live” have repeatedly demonstrated, debate is an uninteresting mode of communication, imitating dialogue without engaging in it. Formally it encourages infinite regress: my misreading of your misreading of my misreading of your misreading. Intellectually its conclusions are in some ways predetermined. In the short run, the winner is whoever speaks last; in the long run, whoever has the greater power. Rather than occasion or remark on further “shifty moments” , then, I will try to (...) review some general areas of contention suggested in my exchange with Eve Kosofsky Sedgwick.Although Sedgwick and I value the personal and the theoretical both, we disagree on the lines of intersection. I am struck by her initial situation of “Van Leer” in the ranks of those who feel silenced. The complaint might just as easily have been that he has entirely too much to say. Nor, if autobiography is really the issue, do I in any way regret the notoriety of Sedgwick’s work? From what I take to be my point of view, Sedgwick’s book has opened up for me a far more visible space in the academy than has my own. And I attribute to myself at least enough self-consciousness to recognize the disingenuousness of “feeling silenced” in Critical Inquiry. The Five Senses of Haiti discusses a series of medical mission trips by physicians and medical students to the Central Plateau of Haiti delivering care in the outpatient setting. Practitioners describe their experiences through the use of their five senses to draw contrast between modern health care and medical practice in the developing world. Physicians in a resource poor setting are left without the usual diagnostic armamentarium and the safeguards and distractions of the modern hospital setting. This deficit creates an (...) opportunity to devote time and focus to individual patients. Practicing medicine in this context clinicians use a heightened sense of awareness and increase their reliance on physical exam findings. Global medicine creates an opportunity for medical students to learn and physicians out of training to practice overlooked physical exam skills in the modern era. Ralph Waldo Emerson, an important figure in the popular understanding of America has been rediscovered by scholars and critics, yet there has been no critical study of Emerson's relation to traditional nineteenth-century questions about ethics and epistemology. In Emerson's Epistemology David Van Leer turns to this unexplored area of Emerson's philosophy and especially to the problem of his relation to the central intellectual issue of his age - the transcendental idealism of Immanuel Kant. Although Emerson would throughout his life (...) try a number of vocational roles, he considered himself primarily a thinker. He saw his roles as poet and prophet as versions of the more fundamental one of philosopher. Thus an understanding of Emerson's relation to traditional problems about the theory of knowledge clarifies not only the arguments of the specific essays, but the shape of his complex career. Excessive fear generalisation is a feature characteristic of clinical anxiety and has been linked to its aetiology. Previous animal studies have shown that the mere passage of time increases fear generalisation and that brief exposure to training cues prior to long-term testing reverses this effect. The current study examined these phenomena in humans. Healthy participants learned the relationship between the presentation of a picture of a neutral male face and the delivery of a mild shock. One group was immediately tested (...) with a novel picture of a somewhat different male face (generalisation test). Another group was tested one week later. A third group was also tested one week later and was additionally exposed to the training picture prior to testing. During picture presentations, shock-expectancy ratings were obtained as a measure of fear. Fear generalisation increased from the immediate test to the 1-week follow-up test. This result could not be attributed to level of neuroticism or a general increase in fear (incubation). Furthermore, the time-dependent increase in fear generalisation vanished following brief exposure to the training picture. Results indicate that human fear generalisation is a temporally dynamic process and that memory for stimulus details can be re-established following a reminder treatment. (shrink)

The cooking show The Naked Chef (1999–2001) with Jamie Oliver has often been highlighted as an example of the cooking show genre's potential for reformulating masculine identity through cooking. Through a series of close readings of a selection of cooking shows from France, the UK and Denmark post-The Naked Chef and through a dialogue with other works on the subject, this article will attempt to identify the tendencies in the constructions and negotiations of masculinity in the cooking show genre following (...) The Naked Chef and to understand these in relation to a revision of masculine identity in contemporary culture. The article is informed by poststructural gender theory and understands ‘doing food’ and ‘doing masculinity’ as two mutually constituting practices. The analyses identify four new tendencies in the construction of masculinity in cooking shows at the beginning of the twenty-first century: 1) rechefisation, 2) the TV chef as a moral entrepreneur, 3) the TV chef and the revitalisation of the national myth and 4) cooking as masculine escapism. The article concludes that the innovation of the masculine identity that was launched in The Naked Chef has not continued; rather, the genre has become a platform for the revitalisation of traditional masculinity discourses. [Eve] Sedgwick examines from an explicitly feminist, implicitly Marxist perspective the relation of homosexuality to more general social bonds between members of the same sex . She argues that the similarity between homosocial desire and homosexuality lies at the root of much homophobia. Moreover, she sees this tension as misogynist to the extent that battles fought over patriarchy within the homosocial world automatically exclude women from that patriarchal power. Thus she places homosexuality and its attendant homophobia within a wider dynamic (...) of social relationships.1Yet even as Sedgwick invents a more sophisticated definition of “homophobia,” she may permit misreading of a more elementary sort. Her use of vocabulary is troubling. In a slangy prose that regularly juxtaposes James Hogg and Louis Lepke, Tennyson and Howard Keel, references to the “campiness” of Thackeray’s “bitchy” bachelors or the “feminized” cuckolds of Wycherley’s The Country Wife seem tame enough. Yet there is a political difference between the jokes. One can burlesque fifties musicals or organized crime with impunity; to refer to sexually embattled men with feminine adjectives, however, is to reinforce a sexual stereotype that sees in the supposed effeminacy of homosexuals a sign of their deviance. Nor are women empowered when terms of female degradation like “bitch” are turned back against men: the ironic reversal does not challenge the terms’ validity but reaffirms it, showing they have an even wider range of applicability than had been thought. 1. Throughout my analysis, I use “homosexual” and “gay” exclusively in reference to male sexuality. I do so in part to echo Sedgwick’s emphasis and in part because the logic of my own argument does not empower me to speak on female homosexuality. Background: Longitudinal studies exploring the complex interplay between family structures and residential mobility on educational achievement and failure are lacking. We investigate the interplay between the number of residential moves during late childhood, parental education level, family living situation, and the probability of completing upper secondary education. Methodology Detailed longitudinal data for a random sample of 30% of the entire Norwegian population born 1982 to 1989 ( N = 121,247) and information on all their relocations between Norwegian enumeration districts from (...) ages 10 to 18 years were extracted from the Norwegian population registries. Family structures were grouped into four intersectional family strata defined by combining categories of parental education level (distinguishing poorly educated and well-educated families) and the family’s living situation (comparing non-intact families with intact families). We applied two-level logistic regression models, which incorporated individual and family contextual factors, to estimate possible differences in completion rates of upper secondary education. Results Non-completion of secondary education (which constitutes 29% of the study sample) increases incrementally with the number of residential changes across all four family structures, but this effect was not distributed evenly between the different family strata. Individuals in “well-educated, intact families” seem to be least affected by residential moves. On the other hand, the highest disadvantage of frequent moves was among adolescents in the stratum “poorly educated, intact families.” In poorly educated families the probabilities of completing secondary school among non-intact and intact families converge toward each other as the number of moves increase. About 43% of the variation in school completion may be attributed to differences between families. The highest risk of school non-completion was found among adolescents in poorly educated families, which accounted for 74% of the non-completers. Conclusion We demonstrated underlying links between residential mobility and family structures on non-completion of upper secondary education. The adverse effect of frequent moves calls for attention in schools, public health agencies, and housing policies. The findings should be considered in a life course perspective, as the accumulation of unfavorable conditions during childhood and adolescence tends to constrict future prospects in terms of health and quality of life. In this article we explore a notion of relationship which exists between humans. This notion of relationship takes as a point of departure that differences in human relations and interaction have to be safeguarded. Starting with the Irigarayan notion of ‘two’ as a gendered difference, opposed to an understanding of humans as one and same (gender), we elaborate an understanding of otherness which opens a space where both self and other are welcomed. This relational space cannot be appropriated by either (...) one for it to exist. We continue by drawing from Harry G. Frankfurt's discussion of care in order to understand human (inter)actions in this space. Through an elaboration of how love as a special form of care represents a motivational drive, a way in which a person's will is formed, we try to show how this attentiveness towards the other is possible. Our point of departure is two statements by female head teachers that prompted these theoretical inquiries into other possibilities for interpreting human (inter)action in leadership in education. Ethical considerations The study was conducted according to the principles of Declaration of Helsinki, and was approved by the Norwegian Social Science Data Services. Objective To describe patients’ experiences of staying in multiple- and single-bed rooms. Patients and methods This qualitative study employed a descriptive and exploratory approach, and systematic text condensation was used to analyze the material. Data were collected in a hospital trust in Norway. A total of 39 in-depth interviews were performed with patients discharged from the medical, (...) surgical, and maternity departments. Results Patients had ambiguous views on whether multiple-bed rooms or single-bed rooms were to be preferred. Main results include how patients cherished “the importance of others” but at the same time valued “the importance of privacy.” Being hospitalized in multiple-bed rooms was for many patients a very positive experience in terms of social interaction. Patients in single-bed rooms reported being more dependent on nurses to maintain social contact and obtain safety. Conclusion This research provides new knowledge on how the need for privacy can be in contradiction with the need for socializing with other patients. When hospitalized, the physical structure of a hospital impacts with whom patients interact and to what extent they depend on the nursing staff to have their social needs met. (shrink)

This study assessed the International Critical Thinking Essay Test (ICTET-A) for inter-rater reliability, internal reliability, and criterion validity. A self-selecting sample of participants (N = 100) completed the ICTET-A and a comparison test online. We found the ICTET-A items to have moderate to good levels of inter-rater reliability, and overall excellent inter-rater consistency for total test scores. The test had good internal reliability. There was a strong correlation between scores on the ICTET-A and the comparison test. Factor analysis showed that (...) scores were best explained with one factor, suggesting the test measures a single construct. The ICTET-A can therefore be considered a valid measure of critical thinking. Additionally, we propose a short form of the test. (shrink)

El artículo repasa los argumentos de los entusiastas de esa idea, los que creen que leer novelas nos hace más empáticos y solidarios; de los moderados, quienes consideran que ayuda ante todo a ejercitar nuestro (...) juicio moral, y de los escépticos, que apenas encuentran ningún fin moral en ese hábito. Revisa a continuación algunas complejidades del desarrollo moral, que hacen improbable una fácil conexión, y termina distinguiendo los modos en que la lectura de novelas sí puede contribuir a cultivar la humanidad que hay en nosotros y a ahondar en el humanitarismo. El artículo es una creación en la que convergen las experiencias, indagaciones, discusiones suscitadas en los antecedentes y el desarrollo del Proyecto “Investigar, leer y escribir en la universidad:el docente investigador mediador y aprendices investigadores autónomos”, que identifica la problemática sobre la orientación de investigación formativa en pregrado, en la relación docencia-investigación; así, formula la pregunta directriz ¿Cómo orientar el proceso de investigación formativa en la formulación de proyectos de investigación? La propuesta recoge la experiencia docente e investigativa en (...) lectura, escritura, procesos de mediación y autorregulación. The beginning of Philosophy of Right with the concept of "Right" and "morality" presents the appearance of a form of treatment typical of the methodological individualism of natural right theories. Nevertheless, Hegel makes it clear that the expository plan does not reflect the genetic, temporal or ontological structure of the issue. This article will thus try to give an answer to the question about the criteria to which this problematic structure of the work plan responds. The thesis sustained by the (...) author is that the first two parts of the work have to be read mainly as a critical-negative exposition of the current theories, and only the third part about the institutions of Sittlichkeit provides a positive exposition of Hegel's thought. The treatment of Right and morality is not merely negative either, but takes up again and at the same time conforms to Kant's and Natural Rights fundamental principles, as necessary conditions for the comprehension of the institutions of modern society and modern State.El comienzo de la Filosofía del Derecho con el concepto de "Derecho abstracto" y la "moralidad" presenta la apariencia de una forma de tratamiento típica del individualismo metodológico de las teorías jusnaturalistas. Hegel aclara sin embargo que el plan de la exposición no refleja ni el orden genético, o temporal, ni la estructura ontológica de la cosa. La pregunta que tratará de responder este artículo es, entonces, a qué criterios responde esta problemática estructura del plan de la obra. La tesis que sostiene el autor es que las dos primeras partes de la obra se tienen que leer principalmente como una exposición crítico-negativo de las teorías vigentes, y que recién la tercera parte sobre las instituciones de la eticidad expone positivamente el pensamiento de Hegel. Pero el tratamiento del derecho y la moralidad no es tampoco meramente negativo, sino que retoma y hace suyos al mismo tiempo principios fundamentales del jusnaturalismo y de Kant, como condiciones necesarias para comprensión de las instituciones de la sociedad y del Estado modernos. ¿Cómo desplazar un sistema de signos? ¿Cómo expandir el alcance textual de una fuerza? ¿Cómo repensar una teoría del cambio en el marco de un discurso que por estructura se nutre del silencio como su modo otro de conformación? ¿Cómo des-leer los montajes e imágenes que el lenguaje jurídico ata y moviliza? El presente trabajo pretende abordar la pregunta por la relación entre texto jurídico, ausencia y transformación a partir de los aportes de tres autores: Julia Kristeva, Gayatri Spivak (...) y Pierre Legendre. Entendiendo que es allí en la tensión misma entre lo dicho, lo ausente y lo im-puesto donde se inscribe la posibilidad misma de la acción. Mientras que Kristeva aborda la pregunta por la transformación a partir de una crítica vuelta sobre la semiótica misma, Spivak inscribe una teoría del cambio en el marco de una relectura semiótica de la subalternidad. Legendre, por su parte, insiste en el carácter inconsciente, libidinal de la sujeción. (shrink)

En este artículo me propongo releer el llamado “fragmento de las máquinas” de los cuadernos Grundrisse a partir de la concepción de la prótesis y la producción biodrag de subjetividades desplegada por Preciado. Mientras que la mayoría de las lecturas de Preciado en clave marxiana se centran en Testo yonqui, sostendré que leer a Marx a partir del concepto de prótesis en Manifiesto contra-sexual permite reformular la noción de “individuo social” en una clave transhumanista. Esto empuja la interpretación de (...) Marx más allá de los marcos en que él mismo comprendió la crítica del capital. Al mismo tiempo, esta vuelta a Marx ilumina los marcos sociales comprehensivos en cuyo seno la producción tecnológica del género tiene lugar. La producción de géneros y la maquínica se articulan así de manera significativa. (shrink)

Este artículo busca situar la dicotomía entre leer y escribir, mediante una apropiación crítica de estos dos conceptos. Ambos participan de una doble implicación sin que uno pueda prescindir del otro y viceversa. La lectura y la escritura se aproximan como ejes de discusión a una definición intelectual de lector y de investigador, aspecto en el que constituyen un oficio de creación contingente y continua. Consecuente con esto, leer y escribir son en este caso objetos conjugados de un (...) análisis que tangencialmente recupera la exégesis sobre la teoría comunicativa, pertinente y posible para una crítica conceptual y sustancialmente creativa. Münster, Hamburg, London: LIT-Verlag, 1998. 337 pp. This is a most unusual study, pieced together out of hidden facets and neglected aspects of Buddhist and Christian studies and containing an unrivaled (...) overview of the literature. If modernWestern theology, taking its cue from Heidegger, has emphasized the temporality of existence, Münch draws our attention to its "spatiality" in what he calls "dimensional thinking." He is able to point to an astonishing number of thinkers from a variety of disciplines who have taken as their starting point our need to orient ourselves in space. In this light, the kenôsis.assage Philippians 2:5-11 is seen to be patterned on a U-shaped movement linking the dimensions "above" and "below" as distinct from S- and O-shaped movements found elsewhere in the mystical tradition.Münch goesonto establish left/right, behind/before, inside/outside, coming to be/passing away as what he calls "immanentals" (12-13) analogous to the "transcendentals" of scholastic theology. In particular, he seeks out thinkers who have tried to formulate the "fourth dimension," which would take us beyond conventional space-time. Plotinus, Eckhart, and the Kabbala are all examined for their attempts to conceptualize the transcendent in this way.What is the purpose of this exercise? Münch's leading question, to which he eventually returns, is clear: "What is lacking in our idea of God if we are ignorant of Buddhist experience?" (5). His purpose is to evaluate the relative significance of kenôsis ("he emptied himself") and tapeinosis."he humbled himself") in Philippians 2. This becomes apparent when he comes to the leading thinkers of the Kyoto School--Nishida, Nishitani, and Abe--and the debate between Takizawa and Yagi. Here Münch finds evidence of a "backwards-directed," or "dorsal," theology that corresponds to a rearwards movement in Buddhist thinking (264). This interesting hypothesis would need further verification by consulting the textual and extratextual traditions that shaped Japanese Buddhism in the context of their Indian and Chinese origins. In criticizing the work of Perry Schmidt-Leukel, Münch suggests that for Buddhists the balance between the experiences of relationship and transitoriness is not so much a matter of different directions or perspectives of transcendence as a leaving behind of all perspectivity (130-133). The search for salvation is orientated rearward in the Buddhist case, forward in the Christian, but these can be seen as complementary directions on the one road.Münch is able to agree with Ueda that Buddhist "nothingness" is best understood as a "desubstantializing dynamic" (283-284). Shûnyatâ.s the "fullness of nothingness" corresponds to the Christian concept of God. Christ is said to have "gone outside himself" rather than to have "emptied himself out" ["Christus entäusserte sich, aber er entleerte sich nicht!"(287)], to have undergone a metamorphosis. In Münch's view, only trinitarian language can encompass all the dimensions, from zero to the [End Page 139] elusive fourth, which are necessary to express what we mean by God in Buddhist terms (305-306).One of the most original things Münch does is to draw a parallel between the Zen ox-herding pictures and the Christian story. This serves to draw together pictorially, if not conceptually, the sometimes arcane researches that go to make up Münch's mosaic. It is perhaps because spatially dimensional thinking has been so rare in Western science, philosophy, and theology that he must search in such dark corners for evidence of it. The result is an at times bewildering patchwork of apparently unrelated contributions that do not blend smoothly into a unity. This sometimes puzzling heterogeneity is justified, in my view, in that it presents us with a highly diversified tool kit for getting to work on the potential offered... (shrink)

Este artículo se centra en L'Alcorano di Macometto, escrito por Giovanni Battista Castrodardo de Belluno (1517 ca.-1588 ca.) e impreso por Andrea Arrivabene en Venecia en 1547. Aunque hoy sea considerado como una torpe paráfrasis de la traducción latina del Corán de Roberto de Ketton (1143 AD), un examen atento demuestra que L¿Alcorano costituyó una práctica y exitosa enciclopedia de bolsillo sobre la historia del islam y del Imperio otomano. Siguiendo una intuición de Alessandro D¿Ancona (1889), analizo los textos históricos (...) publicados en la introducción del texto, centrándome especialmente en la biografía del profeta Muh.ammad escrita por Castrodardo. Dicha biografía presenta una larga oración renacentista pronunciada por el monje Sergio-Bahi-ra- para convencer a Muh.ammad a derrocar al emperador Heraclio. A través de un análisis lingüistico y retórico detallado de esta oración, intento demostrar la cultura literaria de Castrodardo y su interés por las obras de Maquiavelo. Este nuevo retrato de Muhammad presenta al profeta del islam como un príncipe renacentista, un legislador y un «profeta armado», revelando asimismo las ideas políticas de los círculos filoturcos y anti-imperiales vinculados a la embajada de Francia en Venecia. Se pretende en este estudio cuestionarnos sobre el significado de Leer, y desde esta pregunta encaminarnos a la cuestión tratada por Heidegger en ¿qué significa pensar? sobre el pensar y el decir. Ello en el contexto de la indagación hermenéutica de las modalidades del decir de leer, interpretar y comprender en la obra de Martin Heidegger. Esto nos obliga, en primer lugar, escuchar los sentidos arcaicos en griego de légein y noeīn y sus derivas en la lengua castellana (...) y el alemán de Heidegger. (shrink)

Leer es un acto ideológico, mucho más complejo de lo que los medios de comunicación y la Academia pretenden hacernos pensar; y los textos, resultado de múltiples “apropiaciones”. Ser conscientes de ambos hechos: de cuántas de esas decisiones que nos han llevado a leer lo que leemos, son –realmente– nuestras; y de cuál ha sido exactamente la secuencia de “apropiaciones” que ha articulado –finalmente– “aquello que leemos”, nos es capital, especialmente si deseamos ser protagonistas de la experiencia y (...) del acto mismo de leer, y no nos resignamos a ser arrastrados, acaso adonde nunca querríamos llegar, o que, en el mejor de los casos, a que la experiencia de leer resulte irrelevante o frustrada. Leer por leer, mirar un cuadro por mirarlo o escuchar a Wagner por escucharlo, no sólo no nos hace mejores, sino que pueden incluso hacernos peores de lo que somos. Leer no tiene propiedades intrínsecamente maravillosas, es cómo leemos y por qué leemos lo que hace al acto de leer productivo o pernicioso. (shrink)
