In this study the awake patient's intraoperative situation and experiences during regional anaesthetics and surgery are reflected upon by using the work of the French philosopher Maurice Merleau‐Ponty. Merleau‐Ponty's phenomenological idea of the body as being at the centre of the world highlights the patient's embodied position and bestows significance onto the body as a whole, as a lived body. A case, based on the findings from a previous interview study, is presented as a contextual starting point where a patient (...) goes from having a familiar body recognized as her own to having a partially anaesthetized body experienced as an unknown object. The intraoperative caring space is described in this context as the mutual ground where the awake patient and the nurse anaesthetist (NA) can interact to create meaning. The NA can act as the patient's bodily extension to bridge the gap between the patient's experiences and the situation. This calls for the NA's proximity and genuine presence in order to meet and understand the patient's awake experiences. Learning from the patient's situatedness gives information that is valuable for NAs to share with patients who are less experienced with this contextual situation. The challenge for the NA is not to perform routine‐based care, but to acknowledge every patient's lifeworld and uniqueness thus enabling the patient to move easily along the mind–body–world continuum. The core of intraoperative care is to provide support and promote well‐being of awake patients in the intraoperative environment. The use of a philosophical perspective is relevant for nurses who work in an intraoperative setting where patients undergo regional anaesthetics. This study shows how nursing research using phenomenological philosophy can help uncover new meanings known only to the patients living the experience. (shrink)

Background: Research concerning relatives’ participation in the everyday care related to persons living in special care units for persons with dementia is limited. Research questions: To examine relatives’ participation in their near one’s everyday care, the level of burden experienced and important factors for participation, in this special context. Design: The study had a cross-sectional design, and data collection was carried out by means of a study-specific questionnaire. Participants and context: A total of 233 relatives from 23 different special care (...) units participated. Ethical consideration: The study was approved by the Norwegian Social Science Data Services. Results: A great majority of relatives reported that they visited weekly and were the resident’s spokesperson, but seldom really participated in decisions concerning their everyday care. Participation was seldom reported as a burden. Discussion: This study indicated that relatives were able to make a difference to their near one’s everyday life and ensure quality of care based on their biographical expertise, intimate knowledge about and emotional bond with the resident. Since knowing the resident is a prerequisite for providing individualised care that is in line with the resident’s preferences, information concerning these issues is of utmost importance. Conclusion: This study prompts reflection about what it is to be a spokesperson and whether everyday care is neglected in this role. Even though relatives were satisfied with the care provided, half of them perceived their participation as crucial for the resident’s well-being. This indicated that relatives were able to offer important extras due to their biographical expertise, intimate knowledge about and emotional bond with the resident. Good routines securing that written information about the residents’ life history and preferences is available and used should be implemented in practice. (shrink)

The aim of this study was to explore the experience of nursing personnel with respect to patient participation in special care units for persons with dementia in nursing homes, with focus on everyday life. The study has an explorative grounded theory design. Eleven nursing personnel were interviewed twice. Patient participation is regarded as being grounded in the idea that being master of one’s own life is essential to the dignity and self-esteem of all people. Patient participation was described at different (...) levels as letting the resident make their own decisions, adjusting the choices, making decisions on behalf of the residents and forcing the residents. The educational level and commitment of the nursing personnel and how often they were on duty impacted the level that each person applied, as did the ability of the residents to make decisions, and organizational conditions, such as care culture, leadership and number of personnel. Background: A limited number of studies have shown that patient advocacy can be influenced by both facilitators and barriers which can encourage and discourage nurses to act as patient advocates. Objective: This study’s aim was to describe Swedish nurses’ perceptions of influencers on patient advocacy. Research design and context: Interviews with 18 registered nurses from different Swedish clinical contexts were analysed using the phenomenographic method. Ethical considerations: Ethical revisions were made in accordance with national legislation and guidelines by committees for (...) research ethics at Karlstad University. Findings: Three levels of hierarchically related influencers on patient advocacy were found in the descriptive categories. The fundamental influencer, the nurse’s character traits, was described in the perceptions that advocacy is influenced by nurse’s having a moral compass, having control over the care situation, being protective and feeling secure as a nurse. The second most vital influencer, the nurse’s bond with the patient, was expressed in the perceptions of knowing the patient and feeling empathy for the patient. The third level of influencers, the organisational conditions, was described in the perceptions that the organisational structures and organisational culture influence patient advocacy. Discussion: The results correspond with findings from earlier research but add an understanding that influencers on patient advocacy exist at three hierarchically related levels. Conclusion: The nurse’s character traits are the fundamental influencer to patient advocacy, but in order to be comfortable and secure when advocating for patients, nurses also need to be familiar with both the patient and the situation. A supposition could be that all influencers interact, which needs to be further addressed in future studies. (shrink)

In this paper, I reflect on an important article by Bob Veatch in the inaugural issue of the Hastings Center Report, entitled “Experimental Pregnancy.” It is a report and elegant analysis of the Goldzieher Study, in which nearly 400 women were randomized to receive hormonal contraception or placebo absent consent or disclosure about placebo use, resulting in several pregnancies. Noting the study’s limited notoriety, I first consider the narratives that have instead dominated bioethics’ approach to pregnancy and research: thalidomide and (...) diethylstibesterol. These narratives have facilitated a narrow focus on avoiding fetal risk, to the exclusion of other ethically relevant considerations. I then revisit “Experimental Pregnancy” and offer two ways in which Bob’s analysis serves as an important corrective, first, by foregrounding research subjects, and second, by normalizing pregnancy and thus foregrounding foundational ethical considerations that are sometimes lost amidst pregnancy’s presumed exceptionalism. Para construir una imagen y un discurso urbano diferente y duradero, y regenerar una identidad colectiva en el barrio patrimonial de Xanenetla, Puebla, un grupo de voluntarios de “Colectivo Tomate” propuso en 2010 llevar a cabo un Proyecto de muralismo urbano para detonar procesos de empoderamiento del espacio público a través de colaboración entre artistas y habitantes locales. En 2011, después de haber alcanzado visibilidad en los medios y en las redes sociales en México y en el extranjero, el Plan (...) de Desarrollo Urbano de Puebla 2011-2014 incluyó a la “Ciudad Mural” de Xanenetla en el recorrido turístico del centro histórico como un atractivo de patrimonio cultural con potencial de fortalecer el desarrollo económico de la zona histórica. Las estadísticas y los análisis urbanos revelan que el impulso municipal a la industria turística a través de “gentrificación” urbana ha puesto en riesgo la cultura popular de Xanenetla y su modo de vida barrial. Nuestra investigación explora el potencial de la cultura barrial de Xanenetla como un recurso alternativo para encaminar un desarrollo urbano socio culturalmente sustentable y sensible fundamentado en la identidad local y tomando como puntos de partida la experiencia colaborativa del Colectivo Tomate y su proyecto de muralismo urbano. La resultante ciudad moderna ignoró intencionalmente otras urbanidades y maneras subalternas de concebir espacios habitables (Bachelard, 1969: 212, 215) por contrariar el proyecto de organización económica y política que la ha sustentado. Privilegiando la estética formal, la espectacularidad tecnológica, y la distribución mecánica de las funciones urbanas a lo largo del territorio, el texto urbano-arquitectónico de la ciudad moderna excluye las dimensiones subjetivas, culturales, temporales, corpóreas y sensoriales de la vida urbana con consecuencias adversas para la vida de sus habitantes. Anne Conway was an extraordinary figure in a remarkable age. Her mastery of the intricate doctrines of the Lurianic Kabbalah, her authorship of a treatise criticising the philosophy of Descartes, Hobbes, and Spinoza, and her scandalous conversion to the despised sect of Quakers indicate a strength of character and independence of mind wholly unexpected (and unwanted) in a woman at the time. Translated for the first time into modern English, her Principles of the Most Ancient and Modern Philosophy is (...) the most interesting and original philosophical work written by a woman in the seventeenth century. Her radical and unorthodox ideas are important not only because they anticipated the more tolerant, ecumenical, and optimistic philosophy of the Enlightenment, but also because of their influence on Leibniz. This fully annotated edition includes an introduction which places Conway in her historical and philosophical contexts, together with a chronology of her life and a bibliography. (shrink)

We claim that a notion of rhetic acts can fulfil a useful function in speech act theory. Austin’s examples of rhetic acts are saying that something is so and so, telling someone to do something, and asking whether something is so or so. Though this certainly sounds as if he is talking about the illocutionary acts of asserting, giving directions, and asking questions, we explain why the acts Austin mentions are not illocutionary after all. In short, illocutionary acts are acts (...) that commit the participants in a conversation to various things. The illocutionary force of an utterance is determined by what it commits the interlocutors to. Our suggestion is that what looks like illocutionary force in the cases mentioned by Austin is really the contributions grammatical moods make to the literal meaning of utterances. We argue that this contribution is not part of what characterizes illocutionary acts as such. The semantic contribution of mood already characterizes those speech acts that Austin uses to exemplify rhetic acts. These rhetic acts are not committing in the way that illocutionary acts essentially are. So there must be a sense in which such rhetic acts are not illocutionary ones. In the final section, we suggest that the varieties of rhetic acts indicated by Austin fit neatly into a standard linguistic theoretical framework of conversational scoreboards. (shrink)

The article investigates the implications of mediatization for the legitimation strategies of humanitarian organizations. Based on a corpus of ~400 pages of brochure material from 1970 to 2007, the micro-textual processes involved in humanitarian organizations’ efforts to legitimate themselves and their moral claim were examined. A time trend analysis of the prioritization of actors in the material indicates that marked shifts in legitimation loci have taken place during the past 40 years. A discourse analysis unfolds the three dominant discourses behind (...) these shifts, namely legitimation by accountancy, legitimation by institutionalization, and legitimation by compensation. The analysis relates these changes to a problem of trust associated with mediatization through processes of mediation. (shrink)

Qualitative validation consists in showing that a model is able to mimic available observed data. In population level biological models, the available data frequently represent a group status, such as pool testing, rather than the individual statuses. They are aggregated. Our objective was to explore an approach for qualitative validation of a model with aggregated data and to apply it to validate a stochastic model simulating the bovine viral-diarrhoea virus (BVDV) spread within a dairy cattle herd. Repeated measures of the (...) level of BVDV-specific antibodies in the bulk-tank milk (total milk production of a herd) were used to summarise the BVDV herd status. First, a domain of validation was defined to ensure a comparison restricted to dynamics of pathogen spread well identified among observed aggregated data (new herd infection with a wide BVDV spread). For simulations, scenarios were defined and simulation outputs at the individual animal level were aggregated at the herd level using an aggregation function. Comparison was done only for observed data and simulated aggregated outputs that were in the domain of validation. The validity of our BVDV model was not rejected. Drawbacks and ways of improvement of the approach are discussed. (shrink)

The standard process for starting anamnesis in the follow-up cancer consultation is for the doctor to ask a ‘How are you?’ question. This question gives the patient the opportunity to give a gloss of their general condition and offer the first topic of discussion. Findings in earlier analyses of US and UK data in a broad array of medical contexts show that the question is ambiguous and hence patients may interpret it as social rather than medical. A discourse analysis of (...) a corpus of 28 video-taped consultations shows that the ‘How are you?’ question in the context of Dutch follow-up cancer consultations is consistently interpreted by both doctor and patients as a holistic medical question, making relevant a – frequently complex and nuanced – medically oriented response. We suggest that this difference may have to do with the interactional norms of hospital visits in the Netherlands, and with the specific contextual parameters of return visits, more specifically with follow-up cancer consultations, which affect the way the HAY question is placed, intended and understood. Résumé. : L’article s’interroge sur la manière dont l’institution scolaire est aujourd’hui mise en question dans son projet de formation par les activités juvéniles, dans une sphère extrascolaire où ils jouissent d’une grande autonomie. Ces activités électives (numériques, musicales, sportives…) réalisent une éducation informelle où les adolescents forgent leur caractère, alors même que l’école n’a plus de modèle lisible de projet éducatif. À partir d’une enquête qualitative réalisée auprès d’une centaine d’adolescents français, on dégagera différentes épreuves qui forment les adolescents (...) dans cette sphère, avant de montrer comment elles mettent en question les finalités, mais aussi les modalités de fonctionnement de l’école. Abstract: This article examines how school is questioned today by youth culture and its activities, especially outside school where teenagers are notably autonomous. These chosen activities (digital music, sports, dance, musical activities ...) could be described as an informal education which tends to build the identity and the self of the youth. From a qualitative survey conducted among a hundred French teenagers, we will first identify different trials teenagers face in this specific sphere, before showing how they are questioning the goals, but also the operating procedures of school. Certains écrivains contemporains choisissent de raconter leur vie ou un épisode de leur vie de manière originale, délaissant ainsi l’autobiographie traditionnelle ainsi que la plus récente autofiction. Cette originalité peut résider dans des choix énonciatifs déconcertants (Annie Ernaux, Les Années, Michel Rostain, Le Fils, Régis Jauffret, Lacrimosa, Daniel Pennac, Le Journal d’un corps) et/ou dans l’inscription même de la démarche d’écriture dans le texte : l’auteur raconte sa vie passée et l’écriture du récit que nous lisons (Les Années, Delphine de (...) Vigan, Rien ne s’oppose à la nuit). Cette narration inattendue a pour conséquence de refléter un lien particulier entre le passé raconté et le présent de l’écriture, car là où l’autobiographie traditionnelle permettait un clair départ entre le temps raconté et le temps de la narration, on mesure ici combien le passé s’écrit à l’aune du présent de l’écriture. Cette mise en abîme de l’écriture montre comment elle participe de la quête identitaire qui est au cœur de tout récit autobiographique. Les écrivains sont à la recherche d’une forme qui leur permettra d’approcher au plus près leur vérité ou au moins de tenter de l’approcher, et ils rendent compte de cette recherche. (shrink)

This article comprises a lyrical exposition of the ‘in-betweenness’ that underlies pedagogical relations and musical practice. The latter comprises making, performing, teaching, and indeed listening to music, phenomena encapsulated in the term ‘musicking’, first coined by the musicologist Christopher Small in 1999. Improvisatory practice is inscribed into the very process of writing as a means of mainstreaming the power of connection; and troubling the notion of seeking as the hallmark of ‘poor pedagogy’ (Masschelein, Citation2010) or weak education (Ingold, Citation2018). Drawing (...) inter alia on a scintillating improvisational performance by the musician Bobby McFerrin, we argue that pedagogical relations, construed as the art of encounter, are about finding rather than seeking. Finding implies experiencing a sense of connection, ‘that feeling of landing in the present tense’, of being immersed in whatever occupies you, paying close attention to the details of experience’ (Tempest, Citation2020, p. 5). This is a far cry from education in a major key, the secure territory of understanding that is premised on codified knowledge that pre-empts attention or marginalises it altogether. We draw on examples from music education to explore the deleterious effects of power dynamics. We end with a rallying cry for education as a common chorus. (shrink)

In this text the author takes up Luce Irigaray’s call upon women to image their ‘God’: a quality or attribute that makes them divine women when they realize it in their lives. She presents ‘human dignity’ as such a ‘divine’ quality and as a value that is understood by many to be the ultimate of our human being. Inspired by Ina Praetorius’ expression that seven billion dignitaries walk the earth, the author connects the different aspects of the concept of human (...) dignity with the image of the dignitary, arguing that this image evokes the idea that the subject is free to move and is meant to flourish according to their capabilities. The author also discusses the difficulties of the idea of human flourishing as an ideal for humanity, especially in the context of disability and illness. She offers the notion of ‘becoming, infinitely’ as an alternative for flourishing. (shrink)
