Qualitative validation consists in showing that a model is able to mimic available observed data. In population level biological models, the available data frequently represent a group status, such as pool testing, rather than the individual statuses. They are aggregated. Our objective was to explore an approach for qualitative validation of a model with aggregated data and to apply it to validate a stochastic model simulating the bovine viral-diarrhoea virus (BVDV) spread within a dairy cattle herd. Repeated measures of the (...) level of BVDV-specific antibodies in the bulk-tank milk (total milk production of a herd) were used to summarise the BVDV herd status. First, a domain of validation was defined to ensure a comparison restricted to dynamics of pathogen spread well identified among observed aggregated data (new herd infection with a wide BVDV spread). For simulations, scenarios were defined and simulation outputs at the individual animal level were aggregated at the herd level using an aggregation function. Comparison was done only for observed data and simulated aggregated outputs that were in the domain of validation. The validity of our BVDV model was not rejected. Drawbacks and ways of improvement of the approach are discussed. (shrink)

This research questions the prevention and health promotion practices developed for young people, in France, and re-examines the nature of competences enlisted in the classical model of empowerment in health education. Moving away from the epidemiological postulate of evidence based medicine (EBM), it explores another approach of these educational practices. The research uses the perspective of narrative mediation; it tries also to identify issues about a better knowledge of oneself, making more comprehensive the issues of young students’ health.

Texting while walking is a dangerous behavior that can lead to injury and even death. While several studies have examined the relationship between smartphone use and stress, to our knowledge no studies have yet investigated the relationship between stress and TWW. The objective of the present study was to investigate this relationship by examining the effects of stress on TWW, the effects of TWW on subsequent stress, and the effect of stress on multitasking performance. A total of 80 participants completed (...) two sequential tasks in a laboratory while they walked on a treadmill and responded to a biological motion stimulus imitating the movement of another pedestrian. In the unrestricted task, participants were given the choice to use their personal phones. In the controlled task, they carried a text conversation with a research assistant while they walked and responded to the stimulus. Stress was measured via questionnaire and saliva collection for measure of cortisol before and after each task. Results show that greater psychological stress and cortisol variations were associated with a greater number of phone uses during the unrestricted task. Greater phone use during the unrestricted task was associated with lower subsequent psychological stress in women and total time of phone use was correlated with subsequent cortisol levels. Stress measured before the controlled task had no effect on multitasking performance, but participants with moderate performance were those with the highest cortisol levels. Our results suggest that stress could be a precursor to TWW and that it could affect a pedestrian’s ability to stay safe when using their smartphone. (shrink)

Digital communication between a patient and their clinician offers the potential for improved patient care, particularly for young people with long term conditions who are at risk of service disengagement. However, its use raises a number of ethical questions which have not been explored in empirical studies. The objective of this study was to examine, from the patient and clinician perspective, the ethical implications of the use of digital clinical communication in the context of young people living with long-term conditions. (...) A total of 129 semi-structured interviews, 59 with young people and 70 with healthcare professionals, from 20 United Kingdom -based specialist clinics were conducted as part of the LYNC study. Transcripts from five sites were read by a core team to identify explicit and implicit ethical issues and develop descriptive ethical codes. Our subsequent thematic analysis was developed iteratively with reference to professional and ethical norms. Clinician participants saw digital clinical communication as potentially increasing patient empowerment and autonomy; improving trust between patient and healthcare professional; and reducing harm because of rapid access to clinical advice. However, they also described ethical challenges, including: difficulty with defining and maintaining boundaries of confidentiality; uncertainty regarding the level of consent required; and blurring of the limits of a clinician’s duty of care when unlimited access is possible. Paradoxically, the use of digital clinical communication can create dependence rather than promote autonomy in some patients. Patient participants varied in their understanding of, and concern about, confidentiality in the context of digital communication. An overarching theme emerging from the data was a shifting of the boundaries of the patient-clinician relationship and the professional duty of care in the context of use of clinical digital communication. The ethical implications of clinical digital communication are complex and go beyond concerns about confidentiality and consent. Any development of this form of communication should consider its impact on the patient-clinician-relationship, and include appropriate safeguards to ensure that professional ethical obligations are adhered to. It provides a new approach to understanding how eighteenth-century aesthetic theories are indebted to seventeenth-century religious, philosophical, and literary ideas.

La modernité, dès ses débuts, attribue un rôle-clef aux passions : qu’elles soient hostiles à la Raison ou au contraire ses alliées, dangereuses ou fascinantes, elles marquent le rôle du corps, du désir, du langage et de l’imagination dans la nature de l’homme. La même époque voit se développer différentes variantes du matérialisme. Presque toutes réévaluent ce que la raison classique avait tendance à réprimer ou à considérer comme révélateur de la faiblesse humaine : le corps et tout ce qui, (...) dans l’âme ou dans la société, porte les traces de l’activité et de la positivité du corps. On peut donc s’attendre à ce que les matérialistes fassent un sort particulier aux passions, à ce qu’ils y reconnaissent des lois et non pas seulement des manques ou des vices, à ce qu’ils essaient d’en repérer l’efficace dans l’ensemble des activités humaines. Encore faut-il se demander comment chaque matérialisme procède, par quelle configuration propre il rend compte de ces phénomènes ou comment il dévie les discours de la théorie classique pour les forcer à passer par les objets qui sont les siens. Plutôt que de supposer l’existence d’une théorie matérialiste unique, une enquête s’impose donc qui prenne en vue la diversité de ces auteurs, les situe dans leur contexte et repère les points d’inflexion que rencontre chez chacun d’entre eux cette problématique générale des passions qui semble avoir gouverné plusieurs siècles. Since the 1980s, there has been an increasing acknowledgement of the importance of recognising the ethical dimension of clinical decision-making. Medical professional regulatory authorities in some countries now include ethical knowledge and practice in their required competencies for undergraduate and post graduate medical training. Educational interventions and clinical ethics support services have been developed to support and improve ethical decision making in clinical practice, but research evaluating the effectiveness of these interventions has been limited. We undertook a systematic review of (...) the published literature on measures or models of evaluation used to assess the impact of interventions to improve ethical decision making in clinical care. We identified a range of measures to evaluate educational interventions, and one tool used to evaluate a clinical ethics support intervention. Most measures did not evaluate the key impact of interest, that is the quality of ethical decision making in real-world clinical practice. We describe the results of our review and reflect on the challenges of assessing ethical decision making in clinical practice that face both developers of educational and support interventions and the regulatory organisations that set and assess competency standards. The American term Bioethics has been adopted over the last ten years and the development of Bioethics committees on the American model testifies this influence, even before the official appointment of a National Committee in 1983. This phenomenon acknowledged as the emergence of French bioethics is in fact the final outcome of a long-lasting crisis in the medical profession, in quest for a new style of ethics, breaking with the traditional professional ethics (French Déontologie, through the Ordre des Médecins). Among (...) other factors of conceptual and institutional change, the increase of biomedical research comes first: a major consequence is the sharing of moral responsibilities in decision-making with outsider scientists and finally the involvement of the whole population as potential moral subjects.The designation of these events as the emergence of French bioethics is hardly appropriate for an account of this dramatic shift in ethical norms and roles in medicine. Background: Family members are often required to act as substitute decision-makers when health care or research participation decisions must be made for an incapacitated relative. Yet most families are unable to accurately predict older adult preferences regarding future health care and willingness to engage in research studies. Discussion and documentation of preferences could improve proxies' abilities to decide for their loved ones. This trial assesses the efficacy of an advance planning intervention in improving the accuracy of substitute decision-making and increasing (...) the frequency of documented preferences for health care and research. It also investigates the financial impact on the healthcare system of improving substitute decision-making. Methods/DesignDyads (n = 240) comprising an older adult and his/her self-selected proxy are randomly allocated to the experimental or control group, after stratification for type of designated proxy and self-report of prior documentation of healthcare preferences. At baseline, clinical and research vignettes are used to elicit older adult preferences and assess the ability of their proxy to predict those preferences. Responses are elicited under four health states, ranging from the subject's current health state to severe dementia. For each state, we estimated the public costs of the healthcare services that would typically be provided to a patient under these scenarios. Experimental dyads are visited at home, twice, by a specially trained facilitator who communicates the dyad-specific results of the concordance assessment, helps older adults convey their wishes to their proxies, and offers assistance in completing a guide entitled My Preferences that we designed specifically for that purpose. In between these meetings, experimental dyads attend a group information session about My Preferences. Control dyads attend three monthly workshops aimed at promoting healthy behaviors. Concordance assessments are repeated at the end of the intervention and 6 months later to assess improvement in predictive accuracy and cost savings, if any. Copies of completed guides are made at the time of these assessments. DiscussionThis study will determine whether the tested intervention guides proxies in making decisions that concur with those of older adults, motivates the latter to record their wishes in writing, and yields savings for the healthcare system. For nearly a century, the French have entertained an unshakable conviction that their ability to recognize themselves-to know and transmit the essence of Frenchness-depended on the teaching of the history of France. In effect, history was a discourse on France, and the teaching of history-"la pédagogie centrale du citoyen"-the means by which children were constituted as heirs and carriers of a common collective memory that made them not only citizens, but family. In this essay, I examine the rhetorical (...) and conceptual effects on history writing that emerge out of this preoccupation with the elaboration of a continuous, coherent national identity. Focusing on schoolbooks, I begin by looking at the dominant, nearly hegemonic model of French history created by Ernest Lavisse in the 1890s-a model informed by the dream of a unified, unitary French nation, embodied in and articulated through the history of France-and at the disruption of this paradigm in the aftermath of the Great War. I then consider a text written in the 1990s specifically to repudiate the kind of nationalist narratives that prevailed for most of this century-a new supranational history of Europe. I argue that, in their different experiments with fixing history, both Lavisse and the contemporary textbook authors did not so much repair a deficient history as produce a historical fixation, creating mythicized histories that are complete, closed, predictable, and at bottom ahistorical. Finally, I turn to a recent World War I novel, A Very Long Engagement by Sébastien Japrisot, in order to suggest ways in which the narrative strategies of a fiction writer may be useful to historians in thinking about a different kind of historical project. La métaphore du réseau s'est imposée durant les années quatre vingt dix à l'occasion de ce qu'il est convenu d'appeler le « topical turn », pour décrire les logiques de développement de la vie culturelle et leur analyse en termes spatiaux plutôt que temporels. Simultanément, il arrive que la constitution de réseaux soit bel et bien déterminée historiquement notamment de manière générationnelle ou sociopolitique. Les contributeurs de cet ouvrage s'inspirent de cette réflexion pour traiter d'un phénomène de réverbération dans l'histoire (...) culturelle, en particulier d'expression allemande, des années vingt aux années soixante du XXe siècle. Au plan métacritique, la réception répétée de la philosophie de Frédéric Nietzsche, mais aussi d'Ernst Cassirer notamment chez Michel Foucault et Gilles Deleuze apparaît comme un lien constitutif de ce déploiement métaphorique. D'autres modèles explicatifs sont présentés tel celui des paradigmes en science ou celui d'un équilibrage pulsionnel, de type psychanalytique. Des études de cas précis illustrent également cette hésitation entre le modèle métaphorique et le modèle historique, concernant les modes d'affirmation de certaines avants-gardes, l'étude de la reprise poétique chez Heissenbüttel ou Rühmkorf, filmique chez Godard, ou théâtrale chez les héritiers d'Artaud. Die Metapher des Netzwerkes hat sich im Laufe der neunziger Jahre anlässlich des sogenannten «topical turn» zur Beschreibung der logischen Mechanismen in der Entwicklung des kulturellen Lebens und deren Analyse in eher räumlichen als zeitlichen Begriffen durchgesetzt. Der Aufbau von Netzwerken mag aber auch historisch bedingt sein, zum Beispiel durch Generationszugehörigkeit und sozial-politische Umstsozial-politische Umstände. Die Beiträger dieses Bandes gehen von dieser Überlegung aus, um sich dem Phänomen der Spiegelung in der Kulturgeschichte - insbesondere in der deutschsprachigen - von den zwanziger bis sechziger Jahren des 20. Jahrhunderts auseinanderzusetzen. Auf metakritischer Ebene erscheint die wiederholte Rezeption der Philosophie Friedrich Nietzsches, aber auch Ernst Cassirers, insbesondere bei Michel Foucault und Gilles Deleuze als eine Verbindung, die wesentlich ist für die Entfaltung der räumlichen Metaphorik. Weitere Erklärungsmodelle werden vorgeschlagen, wie das paradigmatisch-wissenschaftliche oder das triebhaft-psychoanalytische. Fallstudien illustrieren zudem dieses Zögern zwischen metaphorischem und historischem Modell, was die Selbstbehauptung gewisser Avantgarden sowie die Untersuchung bestimmter Rezeptionsmomente betrifft, im poetischen Bereich bei Heissenbüttel oder Rühmkorf, im filmischen bei Godard, oder im dramatischen bei den Erben Artauds. This article analyzes the published testimonies of French shipwreck survivors to trace the emergence of a settler colonial ideal in nineteenth-century France. Emerging from the encounters of French survivors with the men of the Anglo-World, this ideal encouraged compassionate, paternalist authority as a solution to the ongoing conflict of paternal despotism and disorderly fraternal freedom in France. The community of sentiment imagined in shipwreck testimonies was gendered and racialized, cultivating white compassion across colonial empires. These transimperial affective ties (...) allowed the settler colonial ideal to persist in the early twentieth century, despite the abandonment of further projects for French settlement. (shrink)

Michel Foucault’s _History of Sexuality_ has been one of the most influential books of the last two decades. It has had an enormous impact on cultural studies and work across many disciplines on gender, sexuality, and the body. Bringing a new set of questions to this key work, Ann Laura Stoler examines volume one of _History of Sexuality_ in an unexplored light. She asks why there has been such a muted engagement with this work among students of colonialism for whom (...) issues of sexuality and power are so essential. Why is the colonial context absent from Foucault’s history of a European sexual discourse that for him defined the bourgeois self? In _Race and the Education of Desire, _Stoler challenges Foucault’s tunnel vision of the West and his marginalization of empire. She also argues that this first volume of _History of Sexuality_ contains a suggestive if not studied treatment of race. Drawing on Foucault’s little-known 1976 College de France lectures, Stoler addresses his treatment of the relationship between biopower, bourgeois sexuality, and what he identified as “racisms of the state.” In this critical and historically grounded analysis based on cultural theory and her own extensive research in Dutch and French colonial archives, Stoler suggests how Foucault’s insights have in the past constrained—and in the future may help shape—the ways we trace the genealogies of race. _Race and the Education of Desire_ will revise current notions of the connections between European and colonial historiography and between the European bourgeois order and the colonial treatment of sexuality. Arguing that a history of European nineteenth-century sexuality must also be a history of race, it will change the way we think about Foucault. (shrink)

Deux préjugés ont grevé la réception de l'œuvre d'Arendt, en particulier en France. L'un : que l'étude des " origines du totalitarisme " et de sa nature (comme modèle politique portant bien au-delà du prototype nazi) ne définirait pas seulement le point de départ de ses questions, mais en bornerait les enjeux. L'autre : qu'à travers les étapes et les détours d'une pensée de la politique et de l'histoire contemporaines, elle n'aurait tendu qu'à rejoindre la tradition philosophique (la méditation (...) sur " la vie de l'esprit "). Convoquant tour à tour l'analyse des révolutions, la critique du travail et du mouvement ouvrier, le questionnement sur la publicité, la société du spectacle et la capacité de juger, le présent ouvrage entend restituer ce qui fait l'originalité de la réflexion d'Arendt sur l'histoire et la politique. Il la replace dans la lignée de Machiavel, de Montesquieu et de Tocqueville, dans la confrontation déterminante avec Marx, dans l'opposition constante à la philosophie politique. Justifiant la posture non-philosophique qu'elle n'a cessé de revendiquer. Démontrant par là même quel bénéfice réel la philosophie peut tirer d'une provocation qui l'expose à la puissance sauvage de l'événement, et à l'étonnement qu'il doit susciter. (shrink)

In the UK, the Recommended Summary Plan for Emergency Care and Treatment (ReSPECT) is a widely used process, designed to facilitate shared decision-making between a clinician and a patient or, if the patient lacks capacity to participate in the conversation, a person close to the patient. A key outcome of the ReSPECT process is a set of recommendations, recorded on the patient-held ReSPECT form, that reflect the conversation. In an emergency, these recommendations are intended to inform clinical decision-making, and thereby (...) enable the attending clinician—usually a general practitioner (GP) or paramedic—to act in the patient’s best interests. This study is the first to explore the extent to which ReSPECT recommendations realise their goal of informing best interests decision-making in community contexts. Using a modified framework analysis approach, we triangulate interviews with patients and their relatives, GPs and nurses and care home staff. Our findings show that inconsistent practices around recording patient wishes, diverging interpretations of the meaning and authority of recommendations and different situational contexts may affect the interpretation and enactment of ReSPECT recommendations. Enacting ReSPECT recommendations in an emergency can be fraught with complexity, particularly when attending clinicians need to interpret recommendations that did not anticipate the current emergency. This may lead to decision-making that compromises the patient’s best interests. We suggest that recording patients’ values and preferences in greater detail on ReSPECT forms may help overcome this challenge, in providing attending clinicians with richer contextual information through which to interpret treatment recommendations. According to Diderot's article "Art" in the Encyclopédie, secrecy is frequently related to guilds and refers to monopolizing practices and "routine". It appears therefore as an obstacle to innovation. But may we consider secrecy exclusively in this negative light? Studies on mining and metallurgy provide us with a different perspective, for they show how the protection of secrets from the gaze of outsiders also allowed for technical exchanges between the craftsmen themselves and thus implied a certain measure of technical openness. (...) From this point of view, it seemed interesting to look also at the urban craftsmen's practices, the so-called "Metiers jurés." A micro-historical study of guilds in a small town of mid-eighteenth century France, presented in this essay, shows that things were indeed in a state of flux. On the one hand, the very ritualistic practice of producing a chef d'aeuvre, which included even the temporary confinement of the candidate, was still in existence. Secrecy still formed a part of the qualifying process of urban artisans. On the other hand, both the ritual and the chef d'aeuvre were undergoing change, the ritual developing more slowly than its content. In the process, secrecy tended to become pure dissimulation, allowing in fact the spread of a practice like subcontracting, which was officially prohibited. (shrink)

Newly discovered photographic collages by early-twentieth-century photographer Karl Blossfeldt. Karl Blossfeldt (1865-1932) achieved overnight fame in the late 1920s with the first publication of his photographs of plants. Those photographs, which revealed the inner structures of the organic forms, immediately made him a pioneer of New Objectivity—an innovative movement in art and photography of the 1920s and 1930s. Blossfeldt, however, was neither a trained photographer nor a botanist. He was a sculptor and art professor who did his photographic work to (...) generate teaching material for his students.The publication of this book is the result of an extraordinary event—the 1997 discovery in Blossfeldt's estate of sixty-one previously unknown collages, in virtually mint condition, of photographic contact prints arranged on large cardboard sheets. Blossfeldt apparently used these to study the relation and similarity of the photographs and to compare them graphically and aesthetically. On some, Blossfeldt had made marks or handwritten notations. Others show lines for cropping. The collages, published here for the first time, unveil a hidden treasure of modern photography and cast fresh light on the systematic approach Blossfeldt used in his photographic studies. All collages are reproduced in four colors. Introducing the book is an essay by Swiss art historian Ulrike Meyer-Stump, a Lecturer at the Zurich University of the Arts.
