Dans cet article, l’auteure interroge son contre-transfert au travers du récit de la thérapie d’une fratrie adulte qui lui a semblé ambivalente dans son rapport au cadre. Elle y voit la manifestation d’alliances inconscientes, en lien avec le négatif de la famille, notamment la fratrie paternelle, elle questionne son écoute et son contre-transfert, gênés par la particularité du dispositif de soin fraternel qu’elle a accepté à la demande des patients. Le deuil d’une sœur handicapée décédée dans l’enfance a gravement (...) porté atteinte au lien fraternel qu’ils souhaitent « réparer » en consultant ensemble, transformant le dispositif en aire de jeu transitionnelle. Dans l’après-coup de cette thérapie, des questions émergent sur ce qui n’a pas pu être pensé dans le néo-groupe et que les éprouvés corporels de la thérapeute lui ont signalé. (shrink)

La psychanalyse de famille et de couple prend en compte le corporel pulsionnel, les ressentis, le sensoriel, les comportements, dans le champ transféro-contretransférentiel et intertransférentiel. Dans la rencontre intersubjective, patients et thérapeutes sont dans la co-émotionalité. La médiation, fréquemment utilisée en séance familiale ou de couple, vise à mobiliser dans l’ici et maintenant des affects, des sensations, pour aller vers l’émotion partagée. Elle permet de passer de l’indicible, innommable, impensable, à la figuration, l’imaginaire, puis à l’ordre symbolique grâce au récit.

Aujourd’hui, le thérapeute psychanalytique de couple ( tpc ) ne se réfère que rarement dans sa pratique au sexuel et à la sexualité. L’auteur étudie cette évolution en proposant deux hypothèses. Les orientations théoriques les plus répandues parmi les tpc auraient-elles une influence sur la difficulté à intégrer le sexuel et les troubles sexuels du couple dans l’analyse des cas? La spécificité de ce cadre, un couple/un analyste, n’y joue-t-elle pas un certain rôle? Dans la clinique psychanalytique des « pathologies de contenant généalogique », le processus narratif en thérapie familiale psychanalytique permet l’émergence d’un néocontenant narratif. Dans cet article, l’auteur part d’une séquence de thérapie familiale psychanalytique pour présenter les notions de remaillage narratif des contenants familiaux effractés. Le néo-contenant narratif a une fonction thérapeutique d’étayage et de reprise du contenant généalogique démaillé, troué ou déchiré. C’est l’expression, au niveau topique du préconscient groupal, d’une corêverie groupale famille-thérapeutes. Cette approche témoigne d’une (...) créativité alternative par rapport à la clinique du refoulement et de l’interprétation. Au confluent de la narratologie et de la psychologie, le concept de narrativité est un outil thérapeutique primordial. Dans le cadre de thérapies post-traumatiques, le thérapeute est l’interlocuteur dont l’écoute soutient les fonctions de l’enveloppe narrative du patient : cette élaboration qui transforme les éléments bruts d’un réel effractant en éléments pensables puise dans la vie fantasmatique qui préexistait à l’événement traumatique. Ces « mythèmes » organisateurs sont venus nourrir la structuration de ces contes à usage privé qui se construisent (...) dans le cadre de la thérapie. Mais ce travail engage un jeu particulier où les identités du témoin, de la victime et du coupable sont floues. Wegen der engen Symbiose zwischen Mutter und Kind, die charakteristisch für die Schwangerschaft ist, können sich Interessenskonflikte zwischen den beiden Individuen entwickeln. Konflikte, bedingt durch die Bedrohung der mütterlichen Gesundheit einerseits und das Recht des Fetus auf Leben, stellen Ausnahmesituationen dar und können in der Regel ohne Opferung der Fetus gelöst werden. Bei unerwünschten Schwangerschaften stellt sich die Frage nach der Priorität von dem Recht der Mutter auf Autonomie und Wahrung ihrer psychosozialen Gesundheit und dem Recht auf Leben des Fetus. (...) Auch die frühzeitige Diagnose eines kranken Fetus mit Aussicht auf ein schwerbehindertes Kind kann – bedingt durch die psychische Belastung – als Bedrohung der mütterlichen Gesundheit aufgefaßt werden. Die gesetzlichen Vorgaben zur Lösung dieser Konflikte variieren zwischen verschiedenen Ländern und unterliegen ihrerseits Anpassungen und Veränderungen. In Anbetracht der verschiedenen kulturellen und religiösen Einflüsse moderner Gesellschaften kann es keinen übergreifenden Konsens zur Lösung dieser Konflikte geben, und die Entscheidung wird im Einzelfall in Abhängigkeit von den moralischen und religiösen Auffassungen der Betroffenen unter Beachtung der rechtlichen Rahmenbedingungen gefällt. In der Schweiz ist nach wie vor eines der ältesten und restriktivsten Gesetze über den Schwangerschaftsabbruch gültig, das eine Unterbrechung nur in Situationen ernsthafter Bedrohung des Lebens oder der Gesundheit der Mutter nach entsprechender Bestätigung durch einen medizinischen Fachexperten zuläßt. Die Anwendung bzw. die Auslegung des Gesetzes hat in den letzten 50 Jahre eine erhebliche Ausweitung erfahren und in der Mehrzahl der Kantone wird die psychosoziale Notlage im Zusammenhang mit einer unerwünschten Schwangerschaft als ernste Bedrohung der Gesundheit der Mutter akzeptiert, so daß der Schwangerschaftsabbrucht straffrei ist. Dem Parlament liegt eine neue Initiative zur Anpassung des Gesetzes an die herrschende Rechtspraxis vor, in dem der straffreie Schwangerschaftsabbruch bis zu 14 Wochen nach der letzten Periode auf Wunsch der Frau gefordert wird. Jenseits dieses Zeitpunkt wird dem Recht des Fetus auf Leben zunehmend Gewicht eingeräumt, so daß die Priorität des Rechtes der Mutter auf Autonomie und Selbstbestimmung nicht mehr gegeben ist. Der Schwangerschaftsabbruch beschränkt sich jenseits von 14 Wochen in der Regel auf Fälle mit schwerer nachgewiesener Pathologie des Fetus. Nach Erreichen der extrauterinen Lebensfähigkeit des Fetus (22–24 Wo-chen) wird der Abbruch auch bei nachgewiesener Pathologie des Fetus zunehmend restriktiv gehandhabt und beschränkt sich auf die Fälle mit schwerer Pathologie ohne Aussicht auf ein extrauterines Überleben oder ein Minimum an kognitiver Entwicklungsfähigkeit wie etwa bei Trisomie 13, 18, Anencephalus oder beidseitiger Nierenagenesie.Durch die Fortschritte der pränatalen Diagnostik ist eine präzise Abklärung fetaler Erkrankungen mit detaillierter Beurteilung der Prognose möglich. Die Methoden der Molekularbiologie ermöglichen den Nachweis oder Ausschluß von genetischen Erkrankungen wie auch von Virusbefall fetaler Zellen, so daß die Entscheidung für einen Schwangerschaftsabbruch in diesen Fällen nicht mehr auf Grund von Wahrscheinlichkeitsangaben, sondern abgestützt auf den definitiven Nachweis diskutiert werden kann.Die fetalen Erkrankungen, die einer intrauterinen Behandlung zugängig sind, bilden eine zunehmend länger werdende Liste, die mit der Transfusion von Blut bei Hämolyse wegen Rhesusinkompatibilität begonnen hat. Dazu kommt eine Reihe chirurgischer Eingriffe wie die Laserkoagulation kommunizierender Gefäße beim Zwillingstransfusionssyndrom oder die Ableitung von Flüssigkeit aus verschiedenen Körperhöhlen des Fetus in die Amnionhöhle. Diese Eingriffe werden unter Ultraschallsicht durchgeführt. Größere Eingriffe, die eine Eröffnung des Uterus erforderlich machen, beschränken sich auf Zwerchfellhernien, die Entfernung großer Teratome sowie auch die Behebung obstruktiver Uropathien. Die Erfolgsrate dieser invasiven Eingriffe, die nur in wenigen Zentren durchgeführt werden, bleibt begrenzt. Medizinische Behandlungen des Fetus können durch Verabreichung von Medikamenten an die Mutter mit transplazentarem Übertritt zu dem Fetus oder durch direkte Injektion in Nabelschnurgefäße erfolgen. Die häufigste Indikation stellen Tachyarrhythmien mit Herzinsuffizienz des Fetus dar. (shrink)

Feeding and Being Fed. Supply Cycles and Nutrition Regimes at the Royal Prussian Institute for Experimental Therapy, 1900 to 1910. The article explores the everyday life and especially the feeding practices in the laboratories of the Institute for Experimental Therapy and the Georg Speyer House in Frankfurt, Germany, in the decade after 1900. The text focuses on the experimental animals and their entangled relationship to humans in the two institutes where life‐scientists tested the therapeutic effect of chemicals and dyestuffs on (...) pathogens and tumors. For this purpose countless animal experiments were conducted in the fields of chemotherapy and cancer research. The article focuses on the importance of animal feeding and feeding practices in the experimental processes and serum testing procedures. The behaviour of the animals, their health status, their appetite and weight were constantly monitored and deemed fundamental to the outcome of the experiment. Animal feeding was therefore an important factor within the epistemological process of knowledge production. Animal feeding was both orientated towards the needs of the experiments and governed by economic needs. Within their economic (life) cycles, the ontological status of the animals changed: they became vectors of pathogens and information, economic resources, objects of exchange, living resources and life stock for ongoing experiments, patients, and sometimes even food for other animals. Overall, feeding experimental animals played an important role in the research process. (shrink)

Expressions belonging to the lexical fields of medicine and psychology recur repeatedly throughout Ludwig Wittgenstein’s writings since the 1930s. He uses therapeutic vocabulary mostly in the context of metaphilosophical reflections, i.e. reflections about the activity of philosophizing. But how are we to understand such expressions? Even though some interpreters admit their metaphorical nature, the methodological background of using figurative language has hitherto been neglected concerning this matter. Here, I argue that Wittgensteinian therapy is what G. Lakoff and M. Johnson have (...) termed a “structural metaphor”. Thus we do not only obtain a better understanding of Wittgenstein’s metaphilosophy, but also of the role metaphor use plays within his methodology. I shall try to show that he uses therapeutic metaphors as argumentative and epistemological devices and thereby investigates the philosophically problematic expression ‘philosophy’ grammatically. We would like to conduct a phenomenological analysis of illness beyond the characteristics which define it scientifically, in order to consider the suffering experience of the sick person which, in reality, constitute its authentic truth. Following Michel Henry’s philosophy which defines life as pathetic self-affection, we will understand that illness may and must first of all be considered as an intense form of this unsurmountable link to one’s self which concerns the very essence of everyone of us as a living (...) ipseity. Then, if “One does not feel”, if suffering is always a founding individual experience, the principle that medicine should be attentive to the subjective experience of patients is consequently established. Then we can also begin to understand the enigma of a patient’s attachment to his illness, to the point of refusing to be cured, as an inappropriate existential expression of an ontological fear of losing oneself. L’analyse des relations d’argent dans les cures psychanalytiques en couple reste souvent trop superficielle. La double fonction à la fois réelle et symbolique de l’argent suscite de faciles résistances coalisées entre les deux partenaires avec parfois une complicité du thérapeute. L’usage nécessaire de l’argent dans la réalité sert à masquer son rôle prépondérant dans les jeux de relation et notamment de pouvoir. Dépasser ces résistances implique d’analyser l’argent dans sa fonction de signifiant fondamental du lien de couple et en particulier (...) le croisement inconscient des représentations que s’en font les deux partenaires, quitte à exploiter des dispositifs adaptés. Zusammenfassung. Im Wandel vom paternalistischen zum partnerschaftlichen Arzt-Patient-Verhältnis werden auch Konfliktsituationen am Ende des Lebens aus einem anderen Blickwinkel gesehen. Der Fortschritt der Intensivmedizin und die zunehmende Zahl chronisch Kranker schaffen hier neue ethische Probleme. Am Fall eines Patienten, der an einer Amyotrophen Lateralsklerose litt, wird der Autonomiekonflikt zwischen Selbst- und Fremdbestimmung verdeutlicht. Juristische, medizinische und ethische Argumente werden fallbezogen dargestellt und der Stellenwert von Patientenverfügungen und das derzeitige deutsche Betreuungsrecht diskutiert.

The present paper intends to study Avicenna’s and Nietzsche’s approaches to the relationship between philosophy and medicine by examining their understanding of the multiple analogous meanings of “sickness” and “health” with regard to human existence. Avicenna considers philosophy itself as a form of “medicine” insofar as it can cure the soul from the sceptical doubt concerning the possibility of knowing the truth and thus create a perfect harmony between body, soul, and spirit. But in a second step, he claims that (...) earthly life itself is an “illness” that keeps us from the perfect contemplation of God as supreme intelligible object and, therefore, has to be “cured” by death. Nietzsche shares with Avicenna not only the particular interest in dietetics but also the conviction that philosophy has a deeply therapeutic dimension. However, he replaces the vertical movement of metaphysical ascension towards absolute, divine truth with the horizontal journey of philosophical transformation that allows each human being to find their own inalienable concept of “sickness” and “health” and in doing so, become their own healer. ‘Dancing mania’ has often been understood as an expression of purportedly ‘typical medieval’ mass hysteria. Yet evidence suggests that a better interpretation would be to see it as a disease, the idea of which was shaped by patterns tracing back to antique cosmology. During the later Middle Ages, this concept became reality as a form of suffering primarily determined by spiritual forces which typically struck only individuals or small groups in narrowly defined regions. This article closely examines a key shift (...) in the semiotic setting of how this disease was interpreted: During the 15th and early 16th centuries, it became medicalised and desacralized. Evidence of this development can be found in isolated instances of ‘dancing mania’ in towns of the Rhine and Moselle area which at first glance would appear to be of little significance. As a medical concept, ‘dancing mania’ would survive the Reformation, and as a concept of primarily medical understanding it would later be re-integrated into the renewed Catholic culture of the late 16th and 17th centuries. _Background:_ Art therapy (AT) is frequently offered to children and adolescents with psychosocial problems. AT is an experiential form of treatment in which the use of art materials, the process of creation in the presence and guidance of an art therapist, and the resulting artwork are assumed to contribute to the reduction of psychosocial problems. Although previous research reports positive effects, there is a lack of knowledge on which (combination of) art therapeutic components contribute to the reduction of psychosocial problems (...) in children and adolescents. _Method:_ A systematic narrative review was conducted to give an overview of AT interventions for children and adolescents with psychosocial problems. Fourteen databases and four electronic journals up to January 2020 were systematically searched. The applied means and forms of expression, therapist behavior, supposed mechanisms of change, and effects were extracted and coded. _Results:_ Thirty-seven studies out of 1,299 studies met the inclusion criteria. This concerned 16 randomized controlled trials, eight controlled trials, and 13 single-group pre–post design studies. AT interventions for children and adolescents are characterized by a variety of materials/techniques, forms of structure such as giving topics or assignments, and the use of language. Three forms of therapist behavior were seen: non-directive, directive, and eclectic. All three forms of therapist behavior, in combination with a variety of means and forms of expression, showed significant effects on psychosocial problems. _Conclusions:_ The results showed that the use of means and forms of expression and therapist behavior is applied flexibly. This suggests the responsiveness of AT, in which means and forms of expression and therapist behavior are applied to respond to the client's needs and circumstances, thereby giving positive results for psychosocial outcomes. For future studies, presenting detailed information on the potential beneficial effects of used therapeutic perspectives, means, art techniques, and therapist behavior is recommended to get a better insight into (un)successful art therapeutic elements. In the modern day, it is understood that the role of the teacher comprises aspects of therapy directed towards the child. But to what extent should this relationship be developed, and what are its concomitant responsibilities? This book offers a challenging philosophical approach to the inherent problems and tensions involved with these issues.

Gene therapy and gene editing are revolutionising the treatment of genetic diseases, most notably haematological disorders. This paper evaluates the use of both techniques in hereditary blood disorders. Many studies have been conducted in this field, especially with gene therapy, with very promising results in diseases such as haemophilia, certain haemoglobinopathies and Fanconi anaemia. The application of these techniques in clinical practice and the foreseeable development of these approaches in the coming years suggest that it might be useful to evaluate (...) the results achieved thus far. It is also essential to reflect on the possible bioethical concerns raised by the use of both techniques, especially in terms of safety issues for patients, potential side effects, treatment duration, accessibility and cost of treatment, and the heritability of genetic changes produced if germline cells are used.
