Despite their crucial role in the translation of pre-clinical research into new clinical applications, phase 1 trials involving patients continue to prompt ethical debate. At the heart of the controversy is the question of whether risks of administering experimental drugs are therapeutically justified. We suggest that prior attempts to address this question have been muddled, in part because it cannot be answered adequately without first attending to the way labor is divided in managing risk in clinical trials. In what (...) follows, we approach the question of therapeutic justification for phase 1 trials from the viewpoint of five different stakeholders: the drug regulatory authority, the IRB, the clinical investigator, the referring physician, and the patient. Our analysis shows that the question of therapeutic justification actually raises multiple questions corresponding to the roles and responsibilities of the different stakeholders involved. By attending to these contextual differences, we provide more coherent guidance for the ethical negotiation of risk in phase 1 trials involving patients. We close by discussing the implications of our argument for various perennial controversies in phase 1 trial practice. (shrink)

Most models of response time (RT) in elementary cognitive tasks implicitly assume that the speed-accuracy trade-off is continuous: When payoffs or instructions gradually increase the level of speed stress, people are assumed to gradually sacrifice response accuracy in exchange for gradual increases in response speed. This trade-off presumably operates over the entire range from accurate but slow responding to fast but chance-level responding (i.e., guessing). In this article, we challenge the assumption of continuity and propose a phase transition model (...) for RTs and accuracy. Analogous to the fast guess model (Ollman, 1966), our model postulates two modes of processing: a guess mode and a stimulus-controlled mode. From catastrophe theory, we derive two important predictions that allow us to test our model against the fast guess model and against the popular class of sequential sampling models. The first prediction—hysteresis in the transitions between guessing and stimulus-controlled behavior—was confirmed in an experiment that gradually changed the reward for speed versus accuracy. The second prediction—bimodal RT distributions—was confirmed in an experiment that required participants to respond in a way that is intermediate between guessing and accurate responding. (shrink)

I analyze the extent to which classical phase transitions, both first order and continuous, pose a challenge for intertheoretic reduction. My contention is that phase transitions are compatible with a notion of reduction that combines Nagelian reduction and what Thomas Nickles called Reduction2. I also argue that, even if the same approach to reduction applies to both types of phase transitions, there is a crucial difference in their physical treatment: in addition to the thermodynamic limit, in continuous (...) phase transitions there is a second infinite limit involved, which marks an important difference in the reduction of first-order and continuous phase transitions. In preparation for the development of a rapid tissue donation programme, we surveyed healthcare providers in our institution about knowledge and attitudes related to RTD with lung cancer patients. A 31-item web based survey was developed collecting data on demographics, knowledge and attitudes about RTD. The survey contained three items measuring participants’ knowledge about RTD, five items assessing attitudes towards RTD recruitment and six items assessing HCPs’ level of agreement with factors influencing decisions to discuss RTD. Response options were presented (...) on a 5-point Likert scale. Ninety-one HCPs participated in the study. 66% indicated they had never heard of RTD prior to the survey, 78% rated knowledge of RTD as none or limited and 95.6% reported not having ethical or religious concerns about discussing RTD with patients. The majority were either not comfortable or not sure if they felt comfortable discussing RTD with cancer patients . 56.1% indicated their knowledge of RTD would play an integral role in their decision to discuss RTD with patients. 71.4% reported concerns with RTD discussion and the emotional state of the patient. Physicians and nurses play an important role in initiating conversations about recruitment and donation to research that can ultimately influence uptake. Increasing HCP knowledge about RTD is a necessary step towards building an RTD programme. Our study provides important information about characteristics associated with low levels of knowledge and practice related to RTD where additional education and training may be warranted. (shrink)

This paper examines whether social workers and other direct service practitioners can find utility in ethnomethodology despite or even because of the policy of “indifference”. Garfinkel, the father of ethnomethodology, sets out “ethnomethodological indifference” to insist that EM studies do not supplement, formulate remedies, develop humanistic arguments, or encourage discussions of theory. While at first blush such limits on EM might appear to be a barrier for most social workers this paper argues against first impressions. It is argued that EM (...) provides an important redirection for social work practice and research. Additionally, it is proposed that by approaching EM through Dorothy Smith’s Institutional Ethnography social workers can bridge Garfinkel’s quest for haeccities with extended social relations and actual courses of actions to find congruence between EM and accomplished professional practice. This paper looks at an emerging major economic trend which appears to be, in part, a consequence of neoliberal globalization. This development is the rise of a huge segment of the world’s population, in both developed and developing countries, comprising a redundant or unneeded group of workers, both rural and urban. These make up “the precarious classes.” The paper initially presents background ideas to set the stage for discussing these findings. It looks at data summarizing the consequences of globalization to (...) date in the U.S. and in the rest of the world. The rise of the “working poor” in the U.S. is first documented and then we summarize Samir Amin’s work on what he calls the emergence of “precarious classes” around the globe. Finally, we tie this apparent trend to related global problems and look at what is needed to further research this potentially ominous development. (shrink)

We classify the phase transition thresholds from provability to unprovability for certain Friedman-style miniaturizations of Kruskal's Theorem and Higman's Lemma. In addition we prove a new and unexpected phase transition result for ε0. Motivated by renormalization and universality issues from statistical physics we finally state a universality hypothesis.

Biomedical research involving human participants is highly regulated and subject to stringent ethical requirements. Clinical research ethics, regulation and policy have tended to focus almost exclusively on the protection of participants' interests against harms that might result from taking part in research. Less consideration, however, has been given to the interests that patients may themselves have in research participation, even in trials that may be beyond the bounds of current clinical research practice. In this paper, we consider the case of (...) a suggested extension to clinical trial protocols to explore the ethics of participation in ‘risky’ research. We argue that patients may have a strong interest in taking part in research, and that even when greater-than-usual risks may be present, such research can be both ethically and scientifically justified. Finally, we suggest that there might be scope in some cases to assert a right to participate in research, and that the possibility of such a right merits further consideration. (shrink)

This paper advances an assessment of Kant’s Critique of Pure Reason made from a bird’s eye view. Seen from this perspective, the task of Kant’s work was to ground the spontaneity of human reason, preserving at the same time the strict methods of science and mathematics. Kant accomplished this objective by reviving an old philosophical discipline: the peirastic dialectic of Plato and Aristotle. What is more, he managed to combine it with logic. From this blend, Kant’s transcendental idealism appeared as (...) a new logic that paralleled Aristotle’s syllogistic logic. The first result of this move was that philosophy became a formal study that treats even such subjects as ethics with rigour. Another outcome was that it established philosophy as a professional – school – discipline. In the twentieth century academy, this development was echoed by the emergence of analytic philosophy, in which Kant’s new logic evolved into a philosophical logic. (shrink)

This paper examines the literary representation of the beginnings of the Northern Irish Troubles with regard to a gender variable, in the selected poems by Heaney, Durcan, Boland, Meehan and Morrissey. The reading of Heaney’s “Punishment” will attempt to focus not solely on the poem’s repeatedly criticized misogyny but on analyzing it in a broader, historical context of the North’s conflict. In Durcan’s case, his prominent nationalist descent or his declared contempt for any form of paramilitary terrorism do not seem (...) to prevent him entirely from immortalizing female victims of the Troubles. Boland’s attitude seems the most unequivocal: the clear aversion to the language of death and rendering Irish women’s experiences in this discourse. The article concludes with analysis of Meehan’s “Southern” guilt for the situation of Catholics in the North with the simultaneous critique of perpetrated violence and Morrissey’s complicated standpoint: atheist/neutral/Protestant/communist and her striving for the impossible impartiality in a war-ridden and politically divided country. Trying to avoid systemic victimization of Irish women, the paper intends to analyze the historical and political circumstances which made them more susceptible to various forms of attacks at the beginnings of the Troubles, as reflected in the titular labels.
