In 2009, health authorities from Taiwan formally attended the 62nd World Health Assembly of the World Health Organization as observers, marking the country’s participation for the first time since 1972. The long process of negotiating this breakthrough has been cited as an example of successful global health diplomacy. This paper analyses this negotiation process, drawing on government documents, formal representations from both sides of the Taiwan Strait, and key informant interviews. The actors and their motivations, (...) along with the forums, practices and outcomes of the negotiation process, are detailed. While it is argued that non-traditional diplomatic action was important in establishing the case for Taiwan’s inclusion at the WHA, traditional concerns regarding Taiwanese sovereignty and diplomatic representation ultimately played a decisive role. The persistent influence of these traditional diplomatic questions illustrates the limits of global health diplomacy. (shrink)

As modern foreign policy and international relations encompass more and more scientific issues, we are moving towards a new type of diplomacy, known as "Science Diplomacy." Will this new diplomacy of the 21st century prove to be more effective than past diplomacy for the big issues facing the world, such as climate change, food and water insecurity, diminishing biodiversity, pandemic disease, public health, genomics or environmental collapse, mineral exploitation, health and international scientific endeavours such (...) as those in the space and the Antarctic? Providing a new area of academic focus that has only gathered momentum in the last few years, this book considers these questions by bringing together a distinguished team of international specialists to look at various facets of how diplomacy and science are influenced by each other. The book not only dissects the ways that politics, science and diplomacy have become intertwined, but also highlights how the world's seemingly most intractable problems can be tackled with international collaboration and diplomacy that is rooted in science, and driven by technology. This article explores medical diplomacy as a means of navigating distinct but related nation-building and internationalist projects during the Cold War. It examines how medical professionals from the Democratic Republic of Vietnam (DRV) utilized their expertise to bolster foreign relations and assert national independence. This article focuses on how three tuberculosis (TB) specialists – Đặng Đức Trạch, Phạm Ngọc Thạch and Phạm Khắc Quảng – adopted, adapted and circulated techniques of TB control, including a modified version of bacillus Calmette–Guérin (...) (BCG) vaccine. Amidst these endeavours, these medical-doctors-cum-diplomats navigated various forms of internationalism while soliciting medical assistance. Their roles within the DRV's state apparatus were prominently showcased from the 1950s to the 1970s, particularly at international gatherings such as the conferences of Ministers of Health of the Socialist Countries (MOHOSC). Because of the political complexities inherent in socialist internationalism, these conferences provided a crucial platform for dialogue among socialist nations when other avenues were limited. Consequently, the DRV's medical experts cultivated goodwill and garnered political support, despite encountering mixed results in their TB control initiatives. (shrink)

Selection pressure from health risk is hypothesized to have shaped adaptations motivating individuals to attempt to become valued by other individuals by generously and recurrently providing beneficial goods and/or services to them because this strategy encouraged beneficiaries to provide costly health care to their benefactors when the latter were sick or injured. Additionally, adaptations are hypothesized to have co-evolved that motivate individuals to attend to and value those who recurrently provide them with important benefits so they are willing (...) in turn to provide costly care when a valued person is disabled or in dire need. Individuals in egalitarian foraging bands can provide a number of valuable benefits, such as defense, diplomacy, food, healing, information, technical skill, or trading savvy. We therefore expect that humans have evolved psychological mechanisms motivating the pursuit and cultivation of a difficult-to-replace social role based on the provisioning of a benefit that confers a fitness advantage on its recipients. We call this phenomenon social niche specialization. One such niche that has been well-documented is meat-sharing. Here we present cross-cultural evidence that individuals cultivate two other niches, information and tool production, that serve (among other things) to buffer health risk. (shrink)

The United Nations (UN) General Assembly High-Level Meeting (HLM) on pandemic prevention, preparedness and response (PPPR) was a missed opportunity to bring high-level commitment and momentum to the global governance of health emergencies. Intended to bring much-needed attention to a policy issue that is rapidly slipping down the international agenda, the fraught diplomacy among member states, lack of consensus on key issues, and weak UN Political Declaration in New York foreshadow a difficult road ahead for upcoming negotiations under (...) the World Health Organization (WHO) in Geneva. This article traces the historical co-evolution of microbiology, bacteriology, and virology, framed within industrial and agricultural contexts, as well as their role in colonial and national history between the end of the 19th century and the first decades of the 20th century. The epistemology of germ theory, coupled with the economic interests of European colonies, has shaped the understanding of human-microbial relationships in a reductionist way. We explore a brief history of the medical and biological sciences, focusing on microbes and (...) the difficulty of implementing germ theory outside of biology laboratories. Furthermore, we highlight the work of Lynn Margulis, who conceptualized microbes within their ecological contexts. Such research shows the active role microbes play in handling life-sustaining biological and biochemical processes. We outline how the industrial and technological advancements of the last two centuries not only impacted almost all human societies, but also changed the world on microbial, biological, and geological levels. The narration of these histories is a complex task, and depends on how national, international, and intergovernmental institutions (such as the World Health Organization) conceive of the selective environmental pressures exerted by industry and biotechnological companies. This book is the product of a conference held by the Albert Schweitzer Institute at Quinnipiac University in 2005. The conference re-examined the life and work of Albert Schweitzer, particularly his idea of "Reverence for Life," and assessed the relevance of his ideas for the twenty-first century. The essays in this book represent various perspectives on Schweitzer's life and works, including: reminiscences from individuals who worked with or were directly influenced by Schweitzer's life, including Jane Goodall (who was the keynote (...) speaker at the conference); philosophical examinations of Schweitzer's ideas in light of present concerns; and practical applications of Schweitzer's ideas to current problems in global issues including arms control, medical ethics, education, and state building. The Democratic Republic of the Congo is facing two crises: a potentially explosive Ebola epidemic and a major insurgency. But they are not wholly distinct from each other: the first is intertwined with the second, and public mistrust and political violence add a dangerous dimension to the Ebola epidemic. The World Health Organization and other health emergency responders will increasingly find themselves fighting outbreaks in insecure, misgoverned or ungoverned zones, possibly experiencing active conflict. Yet the WHO has neither (...) the mission nor the capabilities to navigate these security threats. We cannot expect that the usual public health strategy will succeed when health workers’ lives are directly imperiled and community resistance runs deep. Tackling health emergencies amidst complex humanitarian crises requires fresh thinking. Here, we offer a blueprint for fighting diseases in complex humanitarian emergencies. The building blocks of security and trust include high‐level political support, street‐level diplomacy, community engagement, enhanced funding, and protection of health professionals working in conflict or disaster zones. Responding to a major pandemic and planning for allocation of scarce resources under crisis standards of care requires coordination and cooperation across federal, state and local governments in tandem with the larger societal infrastructure. Maryland remains one of the few states with no state-endorsed ASR plan, despite having a plan published in 2017 that was informed by public forums across the state. In this article, we review strengths and weaknesses of Maryland’s response to COVID-19 and the role of the Maryland (...) Healthcare Ethics Committee Network in bridging gaps in the state’s response to prepare health care facilities for potential implementation of ASR plans. Identified “lessons learned” include: Deliberative Democracy Provided a Strong Foundation for Maryland’s ASR Framework; Community Consensus is Informative, Not Normative; Hearing Community Voices Has Inherent Value; Lack of Transparency & Political Leadership Gaps Generate a Fragmented Response; Pandemic Politics Requires Diplomacy & Persistence; Strong Leadership is Needed to Avoid Implementing ASR … And to Plan for ASR; An Effective Pandemic Response Requires Coordination and Information-Sharing Beyond the Acute Care Hospital; and The Ability to Correct Course is Crucial: Reconsidering No-visitor Policies. Vaccines and vaccination are richly explored areas of study within the history of science and medicine, connecting related fields of the history of science and technology, and spanning across subfields such as biomedical sciences, animal studies, colonial and postcolonial history, and the history of global health. Vaccination is a thoroughly political act that is at once an intimate and local issue and a transnational one, with its particular set of politics connecting stakes for the individual and the community. Vaccination (...) also maps on narratives and temporal frameworks of disease with an ultimate goal of ending epidemics. Therefore, the essay takes these three analytical entry points to discuss the historiography of vaccination: the geographical, the political, and the temporal. We argue that through these lenses we can gain a more nuanced understanding of historical narratives we privilege, and in return, this understanding can enable us to explore past and current questions of health inequalities, validation practices, power relations and resistance and vaccine diplomacy. During asymmetric war, state armies must care for their local allies, detainees and the civilian population in two contexts: acute care for those wounded during military operations and medical care for the general population as required by the Geneva Conventions. Constrained by scarce resources, state armies face a number of moral dilemmas that affect care on the ground.Triage. As they deploy, state armies allocate in-theater medical resources to care for their soldiers. In-theater care does not provide for long-term treatment. Its (...) purpose is to return warfighters to active duty or to quickly evacuate them to the US or Europe for comprehensive medical care. Limited beds and personnel make it impossible to provide the local population with the level of care that Western soldiers receive. As a result, the US and other armies develop complex and often arbitrary rules to treat civilians. The inability to provide high-level care to all, creates tensions with local civilians and host country allies.Ensuring local medical care. The Geneva Conventions require occupying armies to “ensure the medical needs of the civilian population.” Specifying these needs is a medical and moral challenge. What level satisfies this requirement? How much aid should the occupying power provide? How best to distribute existing resources? How should state armies protect NGOs that provide significant medical care and often find themselves under attack by insurgents?Medical Diplomacy. While the purpose of medical care is to treat the sick and injured, medical care also serves security needs. Medical care is a long-established tool to win the hearts and minds of the local population. Since the Vietnam War, however, critics have charged that medical diplomacy subverts the purpose of medicine, places medical personnel in the service of war and provides poor medical care. (shrink)

Thomas Frank has challenged this view, arguing that countercultural rebellion now provides the primary source of consumerism in our society, and that ‘cool’ has become its central ideological expression. Epicurus had a distinctive position on pleasure: the greatest possible pleasure consists in the absence of pain. The pain in question may be physical or psychological. Not to be hungry, cold, or otherwise distressed is the greatest pleasure that the body can know; to be free of fear, particularly the kind of vague, undirected anxiety that Lucretius called cura, is the most pleasant state that the mind can achieve. As Lucretius exclaims, "Do you not see that our nature cries out (...) for nothing other than that pain be absent from the body and that it may enjoy in the mind pleasant sensations, far from anxiety and fear?". This study explored the challenges of informed consent and understanding of the research process among Black and Latino men under community supervision. Between February and October 2012, we conducted cognitive face-to-face interviews using open-ended questions on the significant areas of research participation among 259 men aged 35 to 67 under community supervision in Bronx, New York. Content analysis of the open-ended questions revealed limited knowledge concerning the understanding of research participation. The study participants appeared to generally understand concepts such as (...) compensation after research participation and confidentiality. Participants demonstrated a lack of understanding of certain aspects of the research process—informed consent, human subject, Institutional Review Board, and clinical trials. These findings are informative to researchers conducting studies with criminal justice populations and Institutional Review Boards reviewing research studies. (shrink)
