Objective: The objectives of this study are to understand the current functions, structure and operation of hospital ethics committees (HECs) in Shanghai and to facilitate their improvement. Methods: (1) A questionnaire survey, (2) interviews with secretaries and (3) on-site document reviews of HECs in Shanghai were used in the study, which surveyed 33 hospitals. Results: In Shanghai, 57.56% of the surveyed hospitals established HECs from 1998 to 2005. Most HECs used bioethical review of research involving human subjects as well as (...) bioethical review or consultation regarding medical care services and administrative decision- making. Of the surveyed HECs, 14.3% did not provide any formal bioethical training to the HECs’ members and many HECs had no standard operating procedures. Some HECs had no clear definition of what was “conflict of interest” that should be considered by the HECs, while 44.4% of the HECs did not perform continuing review. Discussion: After the issues of related national regulations, more and more hospitals established HECs in Shanghai, but the functions of HECs need to be further developed and formal training on bioethics should be provided to HEC members. To assure the independence and good performance of HECs, the conflict of interest procedure, the standard operating procedures and bioethical review should be improved. Conclusion: HECs in Shanghai had developed in the preceding 10 years and they played great roles in protecting the rights and welfare of human subjects and patients; some areas need improvement. Objectives The objectives of the study are to understand the current practice of informed consent in medical research in public hospitals in Shanghai, and to share our views with other countries, especially developing countries. Methods In the study, 145 consent forms (CFs) of the selected research projects in eight public hospitals with ethics committees in Shanghai were audited, and the principle investigators (PIs) of these research projects and 40 student subjects who had participated in clinical drug tests were surveyed by (...) questionnaires. Results The CFs of medical researches in public hospitals with ethics committees in Shanghai were generally acceptable. However, there were some defects in the CFs. Although most of the surveyed PIs had correct recognition of informed consent, some processes of informed consent were not in accordance with generally accepted requirements. A large number of the PIs considered the greatest difficulty with informal consent was lack of correct recognition of subjects or legally authorised representatives on medical research. Discussion Informed consent in medical research should consider the research ethics, the background of potential subjects, the local resources and culture of medical research. In addition, special protection is needed for student subjects in informed consent as well as efforts for building and restoring the public's trust in biomedical research. Conclusion The informed consent in Shanghai's public hospitals with ethics committees was generally acceptable and the achievement of adequate informed consent is influenced by many factors. William Gerber’s study of American liberalism is a valuable compendium of the varied, changing, and often conflicting uses of that "slippery" word, liberalism, in the United States, past and present, and in antecedent Western political thought. But Gerber identifies himself as having "set his sights on trying to build an adequate definition of liberalism". The problem is introduced by chapter 1, which asks if liberalism is dying or already dead, and by chapter 2, which asks why liberalism has not brought (...) about the "millennium" it promised. Chapter 3 calls attention to the "Difficulty of Isolating the Essence of the Liberal Viewpoint" from many competing claims, and stipulates three criteria. 1) The definition should encompass only those features of liberalism with which all experts are in agreement. This requirement leads Gerber to extensive and sometimes strained marshalling of quotations showing the frequency of disagreement among liberals with regard to those beliefs commonly associated with liberalism: freedom, majority rule, improvement of society, preference for change, perfectibility, progress, etc. 2) The definition should be exclusive to liberalism and encompass no tenets shared by other than liberals. 3) The definition should be value-neutral. Understandably, these criteria result in a definition of liberalism at a high level of abstraction: "Liberalism is the belief that individuals and institutions, including governments, should so act—or refrain from acting—as to liberate as many individuals as possible from as many shackles as possible without overturning basic social machinery". In chapters 4-8 the "pedigree" of American liberalism is traced from Hammurabi, Moses, Buddha, Laotse, and Jesus to the ancient Greeks and Romans, the Enlightenment and the Glorious Revolution, to 19th century British liberalism. Chapters 6-8 examine American liberalism before 1900, from 1900 to 1945, and since World War II. Chapters 9-11 discuss the stand of liberals on political, economic, and social issues, and chapter 12 looks to the future of liberalism. (shrink)

In many research studies, tumor biopsies are an unavoidable requirement for achieving key scientific aims. Yet some commentators view mandatory research biopsies as coercive and suggest they should be optional, or at least optional until further data are obtained regarding their scientific usefulness. Further complicating the ethical picture is the fact that some research biopsies offer a potential for clinical benefit to trial participants. We interviewed and surveyed a convenience sample of participants in phase I clinical trials at a single (...) institution. Our primary aim was to describe phase I participants’ understanding of whether a research biopsy offered them the prospect of medical benefit. We also endeavored to describe participants’ views about biopsies—specifically, the benefits of biopsies, if any, and whether biopsies were acceptable, risky, or discouraged trial participation. Finally, we collected data on demographics and attitudes to see if any strong correlations with misunderstanding, acceptability, or riskiness existed. Overall, the respondents tended to view research biopsies as acceptable, though they did not succeed in identifying the lack of benefit of a research biopsy. These findings call for renewed efforts in consent conversations and documents to carefully describe the benefits, or lack thereof, of research biopsies. (shrink)

Pre-natal exposure to acute maternal trauma or chronic maternal distress can confer increased risk for psychiatric disorders in later life. Acute maternal trauma is the result of unforeseen environmental or personal catastrophes, while chronic maternal distress is associated with anxiety or depression. Animal studies investigating the effects of pre-natal stress have largely used brief stress exposures during pregnancy to identify critical periods of fetal vulnerability, a paradigm which holds face validity to acute maternal trauma in humans. While understanding these effects (...) is undoubtably important, the literature suggests maternal stress in humans is typically chronic and persistent from pre-conception through gestation. In this review, we provide evidence to this effect and suggest a realignment of current animal models to recapitulate this chronicity. We also consider candidate mediators, moderators and mechanisms of maternal distress, and suggest a wider breadth of research is needed, along with the incorporation of advanced -omics technologies, in order to understand the neurodevelopmental etiology of psychiatric risk. Conceptions of omissions standardly come in two flavours: omissions are construed either as mere absences of actions or are closely related to paradigmatic ‘positive’ actions. This paper shows how the semantics of the verb ‘to omit’ constitutes strong evidence against the view of omissions as involving actions. Specifically, by drawing from an influential fourfold typology of verbal predicates popularised by Zeno Vendler, I argue that declarative statements involving reference to omissions are semantically stative, which is a finding that makes serious (...) trouble for the conception of omissions as being closely related to paradigmatic actions. But references to omissions, in certain linguistic contexts, undergo a shift of meaning to describe processes or activities engaged in by the agent. Still, despite the semantic flexibility of the verb ‘to omit’, its processive reading does not straightforwardly support the second conception of omissions. A subsidiary aim of this paper is to offer a sketch of the metaphysics of processive action in order to show what those who claim that omissions are closely related to actions might be committed to. Does being green facilitate product innovation? This study examines whether green management in firms operating in China fosters radical product innovation to a greater extent than it does incremental product innovation and investigates the underlying institutional mechanisms involved in the relationship between green management and product innovation. The findings show that green management is more likely to lead to radical product innovation than to incremental product innovation. Moreover, government support as a formal institutional benefit more strongly mediates the effect of (...) green management on radical product innovation than its effect on incremental product innovation; whereas social legitimacy as an informal institutional benefit more strongly mediates the effect of green management on incremental product innovation than its effect on radical product innovation. Language is central to the interactional nature of the social life within which it is situated. To react or respond in a particular situation, we must be able to recognize the social situation. Growing evidence has demonstrated the negative impact of perceived loneliness on late-life executive functions. Yet little is known about how social factors impact language processing for older people. The current study aims to fill this gap, first by assessing age-related changes in lexical processing during Chinese word reading, (...) second, by examining whether older adults’ individual differences, such as processing speed and verbal abilities, modulate meaning retrieval and, third, by investigating whether perceived loneliness can hinder word reading. The use of compound words in Chinese enables significant sublexical ambiguity, requiring varying executive load during word recognition: when a word’s constituent characters carry multiple meanings, readers must consider the meaning contributions of both constituent characters and use top-down word information to determine the most accurate meaning of the ambiguous character, a process termed “sublexical ambiguity resolution.” In this study, adults read real Chinese words and pseudowords, and they were asked to make lexical decisions. Older adults exhibited greater lexicality effects and similar sublexical ambiguity effects compared with young adults. Among older participants, processing speed could account for their ability to differentiate between words and pseudowords. In contrast, the level of perceived loneliness modulated the efficacy of sublexical ambiguity resolution: the participants with higher perceived loneliness displayed a greater sublexical ambiguity disadvantage effect. These results indicate that perceived loneliness may affect the use of contextual information in meaning retrieval during reading. Uncertainties about how to achieve sustainable and reliable biobased value chains can be remedied by inclusion of local biomass producers. Such inclusion implies that the knowledge, values, interests and skills of these local producers are integrated into the set-up, design, development and associated distribution of risk and benefits of the specific value chain. To make sure that this inclusion is both fair and effective, capabilities of relevant actors need to be taken into account, i.e. the capabilities of biomass producers and (...) of companies and local governments. Building on these capabilities can lead to context sensitive design of biorefineries that allow for true inclusion of local stakeholders. In contrast to mainstream theories focusing on adolescent developmental deficits, the positive youth development (PYD) approach highlights adolescent developmental plasticity and potentials. There are rich empirical research and review studies showing that PYD attributes promote adolescent well-being. However, the existing literature shows several limitations. First, while there are many Western studies, Chinese studies are sparse, particularly studies in mainland China. Second, most PYD studies are cross-sectional studies with data collected at one single time point. Third, researchers in different Chinese contexts (...) seldom employed validated Chinese scales assessing different domains of PYD attributes. Fourth, few studies have examined the relationships between PYD attributes (measures of eudaimonic well-being) and life satisfaction (measure of hedonic well-being) and hopelessness (measure of negative well-being) in a single study. Finally, the role of life satisfaction or hopelessness as a mediator of the relationship between PYD attributes and psychological well-being is unclear. To explore the importance of PYD attributes to Chinese adolescent psychological well-being, we conducted a longitudinal study with two waves of data collected from junior high school students in mainland China ( N = 2,648). At both times, validated measures of PYD attributes, life satisfaction, and hopelessness were employed. Results showed that PYD attributes concurrently and longitudinally predicted life satisfaction and hopelessness with small effect sizes. Cross-lagged panel analyses showed that while Wave 2 life satisfaction did not serve as a mediator of the relationship between Wave 1 PYD attributes and Wave 2 hopelessness, Wave 2 hopelessness served as a mediator of the link from Wave 1 PYD attributes to Wave 2 life satisfaction. In view of the paucity of research findings in this area, the present findings clarify the association between PYD attributes and hedonic well-being (indexed by life satisfaction) and negative psychological well-being (indexed by hopelessness) in Chinese adolescents.
