Evidence suggests that one reason doctors provide certain interventions in assisted reproductive technologies (ART) is because of patient demand. This is particularly the case when it comes to unproven interventions such as ‘add‐ons’ to in vitro fertilisation (IVF) cycles, or providing IVF cycles that are highly unlikely to succeed. Doctors tend to accede to demands for such interventions because patients are willing to do and pay ‘whatever it takes’ to have a baby. However, there is uncertainty as to what moral (...) weight should be placed on patient‐led demands in ART, including whether it is acceptable for such demands to be invoked as a justification for intervention. We address this issue in this paper. We start by elucidating what we mean by ‘patient‐led demand’ and synthesise some of the evidence for this phenomenon. We then argue that a doctor's professional role morality (PRM) yields special responsibilities, particularly in commercialised healthcare settings such as ART, because of the nature of professions as social institutions that are distinct from markets. We argue on this basis that, while there may be reasons (consistent with PRM) for doctors to accede to patient demand, this is not always the case. There is often a gap in justification between acceding to patient‐led demands and providing contested interventions, particularly in commercial settings. As a result, acceding to demand in such settings needs a strong justification to be consistent with PRM. (shrink)

BackgroundSchool violence and bullying are prevalent problems that affect health in general, especially through the development of emotional and behavioral problems, and can result in the deterioration of the academic performance of the student victim. The objective of this study was to determine the prevalence rates of aggressive behaviors according to types of school violence and bullying, sociodemographic characteristics, and variation by department, region, and time in the period between 2014 and 2018 in Peru.MethodsThe design was observational and cross-sectional based (...) on data from the Specialized System for Reporting Cases of School Violence (Sistema Especializado en Reporte de Casos sobre Violencia Escolar—SíseVe) in Peru, which covers a population of 23,641 students at the initial, primary, and secondary levels of Basic Regular Education [Educación Básica Regular (EBR)], for the 2014–2018 period. The prevalence rates of the different types of school violence and bullying, the sociodemographic characteristics, and the variation by department, region, and time in the period between 2014 and 2018 were estimated.ResultsPsychological violence/bullying occurred at higher prevalence rates (185.8 and 62.6 per 100,000 residents). Women from public institutions reported greater sexual violence, mostly by teachers (67.8%) than by other students (32.2%). The Selva region had the highest prevalence rate of sexual violence (10.1 per 100,000 residents). The departments of Tacna and Piura had the highest and lowest rates of psychological/verbal violence and bullying in 2018 (95.79 and 25.31 per 100,000 residents).ConclusionPsychological/verbal violence and bullying is highly prevalent among students; women report being victims of sexual violence by administrative personnel of public institutions. The Selva region had the highest rate of sexual violence, and Piura and Tacna had the highest and lowest rates of violence and psychological/verbal bullying. Based on these results, it is suggested to conduct evidence-based prevention programs in Peruvian schools to reduce these social problems. (shrink)

Background: Limited knowledge of the experiences of conflicts of conscience found in nursing literature. Objectives: To explore the individual experiences of a conflict of conscience for neonatal nurses in Alberta. Research design: Interpretive description was selected to help situate the findings in a meaningful clinical context. Participants and research context: Five interviews with neonatal nurses working in Neonatal Intensive Care Units throughout Alberta. Ethical consideration: Ethics approval from the Health Research Ethics Board at the University of Alberta. Findings: Three common (...) themes emerged from the interviews: the unforgettable conflict with pain and suffering, finding the nurse’s voice, and the unique proximity of nurses. Discussion and conclusion: The nurses described a conflict of conscience when the neonate in their care experienced undermanaged pain and unnecessary suffering. During these experiences, they felt guilty, sad, hopeless, and powerless when they were unable to follow their conscience. Informal ways to follow their conscience were employed before declaration of conscientious objection was considered. This study highlights the vital importance of respecting a conflict of conscience to maintain the moral integrity of neonatal nurses and exposes the complexities of conscientious objection. (shrink)

Purpose-driven businesses have a stated objective to contribute to the welfare of society and the planet alongside generating shareholder value. As interest in purpose-driven businesses grows, an emerging “purpose ecosystem” of advisers, investors, and enablers offers different types of support for businesses wanting to transition to sustainability. This paper examines how the transition towards purpose-driven business in Australia and the United Kingdom requires addressing challenges facing this support ecosystem at three levels. First, at the individual level where support providers need (...) to build the capabilities of managers who are experiencing tensions around integrating societal and environmental purpose while facing pressure for maximizing shareholder value. Second, the support providers working within the purpose ecosystem offering professional advice and finance face their own tensions between environmental or social objectives and commercial pressures. Third, there are challenges facing actors in the ecosystems aiming to change the wider policy and institutional environment but facing lobbying from those wanting to keep “business as usual.” We identify practical implications for those parts of the purpose-driven business ecosystem providing support. This includes building capabilities to combine social, environmental, and commercial purpose; coordination among support providers; and creating an institutional environment to avoid “purpose wash.”. Drawing on insights from conversation analysis, discursive psychology and social psychology, this article describes some interactional features of two celebrity TV confessionals and the resources used by the TV interviewers and celebrity guests to attribute, accept or deny responsibility for their transgressions. The analytic interest lies in how confessions are locally and interactionally managed, that is, how ‘doing confessing’ is achieved in the television interview context. We show how the host’s opening turn constrains the celebrity guest’s contribution and secures overt (...) admission of guilt, while simultaneously inviting the celebrity guest to tell their side of the story. We also show how celebrity guests produce descriptions which minimize the extent and severity of their transgressions, reduce agency and transform the character of their transgression. In doing so, we argue that celebrity interviewees can convey mitigations and extenuations which diminish the extent of their responsibility – calling into question the very nature of their confession. We propose that our findings demonstrate the hybrid nature of interviewing in the celebrity TV confessional and contribute to our understanding of how ‘doing confessing’ in the public eye is discursively and interactionally negotiated. Lloyd (2009) contends that climate models are confirmed by various instances of fit between their output and observational data. The present paper argues that what these instances of fit might confirm are not climate models themselves, but rather hypotheses about the adequacy of climate models for particular purposes. This required shift in thinking—from confirming climate models to confirming their adequacy-for-purpose—may sound trivial, but it is shown to complicate the evaluation of climate models considerably, both in principle and in practice.

In Against Democracy, Jason Brennan argues that public ignorance undermines the legitimacy of democracy because, to the extent that ignorant voters make bad policy choices, they harm their own and one another’s interests. The solution, he thinks, is epistocracy, which would leave policy decisions largely in the hands of social-scientific experts or voters who pass tests of political knowledge. However, Brennan fails to explain why we should think that these putative experts are sufficiently knowledgeable to avoid making errors as damaging (...) as those made by voters. Given the strong link between political knowledge and ideological dogmatism, as well as the tendency of social scientists to disagree with one another, the case for epistocracy is deeply implausible, at best. Moreover, given that there are important non-instrumental justifications of democracy—justifications of which Brennan appears to be radically ignorant—the epistocratic alternative would be unnecessary even if it were viable. Unable to answer the call of our patients: mental health nurses’ experience of moral distress When health practitioners’ moral choices and actions are thwarted by constraints, they may respond with feelings of moral distress. In a Canadian hermeneutic phenomenological study, physicians, nurses, psychologists and non‐professional aides were asked to identify care situations that they found morally distressing, and to elaborate on how moral concerns regarding the care of patients were raised and resolved. In this paper, we describe the experience of (...) moral distress related by nurses working in mental healthcare settings who believed that lack of resources (such as time and staff) leads to dispiritedness, lack of respect, and absence of recognition (for both patients and staff) which severely diminished their ability to provide quality care. The metaphors of flashlight and hammer are used to elaborate nurses’ possible responses to intolerable situations. (shrink)

The stir caused in the civilised world by the writings of Bergson, particularly during the past decade, is evidenced by the volume of the stream of exposition and comment which has flowed and is still flowing. If the French were to be tempted to set up, after the German manner, a Bergson-Archiv they would be in no embarrassment for material, as the Appendix to this book - limited though it wisely is - will show. Mr. Gunn, undaunted by all (...) this, makes a further, useful contribution in his unassuming but workmanlike and well-documented account of the ideas of the distinguished French thinker. It is designed to serve as an introduction to Bergson's philosophy for those who are making their first approach to it, and as such it can be commended. (shrink)

The themes explored include political liberty, "legal tyranny," defences of influence in government, recognition of the Opposition, and the development of organic categories of political analysis - the latter in a chapter that explodes the association often presumed between organicism and conservative modes of thought. A chapter on the "Fourth Estate" examines the gradual process of legitimation of "interests," culminating in the influence of the press. Central to the account of new political forces and their recognition is the idea of (...) public opinion, which evolved during this period from the notion of public spirit. Chapters on the classical legacy of the century and on the High-Tories examine two backward-looking aspects of the political cultrure. Tracing the persistent influence of High-Toryism, Gunn questions the conventional wisdom about eighteenth-century ideological consensus in general and Whig solidarity in particular. He demonstrates that theories of government from the seventeenth century survived to a degree not previously admitted by modern scholarship. (shrink)

We all ‘know’ that public opinion came to prominence in the political vocabulary of the late eighteenth century. It may be that this dates its rise a bit late, but it is not relevant to argue the matter here. My concern is rather that we be equally aware of the purposes for which people made use of the concept. Here I wish to consider various possible contexts for speaking or writing of public opinion, or ‘opinion’, as it was usually called (...) prior to the mid-eighteenth century. It may be possible to define, more fully than heretofore, the work that the expression did in eighteenth-century thought. As contemporary students of public opinion have been learning, an answer to this question may not even be wholly irrelevant to the task of specifying the nature of public opinion in our own time. Most accounts of intellectual humility build from work in virtue epistemology, the study of knowledge as the state that results when agents are epistemically virtuous (or, perhaps, the view that the proper object of study for epistemology is the intellectually virtuous agent). After briefly reviewing some of the empirical and philosophical literature suggesting that there may be an adaptive role for delusion formation, we discuss the results of a recent study consisting of in-depth interviews with people experiencing delusions. We analyse three such cases in terms of the circumstances preceding the development of the delusion; the effects of the development of the delusion on the person’s situation; and the potential protective nature of the delusional belief as seen from the first-person perspective. We (...) argue that the development of the delusional belief can play a short-term protective function and we reflect on the implications that this might have for our understanding of psychotic symptoms, for the stigma associated with mental health issues, and for treatment options. Conducting research on adolescents raises a number of ethical issues not often confronted in research on younger children. In part, these differences are due to the fact that although assent is usually not an issue, given cognitive and social competencies, the life situations and behavior of youth make it more difficult to balance rights and privacy of the adolescents. In this article, the three ethical principles of beneficence, justice, and respect for persons are discussed in terms of their application to (...) the study of adolescents. Then, seven vignettes are presented to illustrate how these principles apply to real-life situations. How to balance the rights of adolescents and their parents is discussed, using adolescent girls and their parents for illustrative purposes. (shrink)

_Beyond Solidarity_ is an impassioned argument for a sharable morality in a world increasingly fractured along lines of difference. Giles Gunn asks how human solidarity can be reconceived when its expressions have become increasingly exceptionalist and outmoded, and when the pressures of globalization divide as much as they unify. He finds the terms for answering these questions in a more inclusive, cosmopolitan pragmatism—one willing to explore fundamental values without recourse to absolutist arguments. Drawing on the work of William and (...) Henry James, John Dewey, Primo Levi, Richard Rorty, and many others, as well as postcolonial writing, Jewish literature of the Holocaust, and the cultural and religious experience of African Americans in slavery, Gunn points pragmatism in a transnational direction and shows how it can better account for the consequences of diversity. _Beyond Solidarity_, then, is a study of the difference that difference makes in a globalized world. If we’re aiming to have well-founded beliefs, then we generally think it’s a good idea to listen to a wide range of arguments. Listening to dissenting views, in particular, is important for avoiding epistemic dispositions to dogmatism and closed-mindedness. We might say we have an epistemic responsibility to listen to others in order to ensure we do our due diligence when forming beliefs. But do we also have to listen to the arguments of people with whom we disagree online? Received (...) wisdom suggests no, and that not listening online is something of a life raft for epistemic self-preservation. Search engines like Google help to facilitate this self-preservation by filtering the testimony we will come across. There are legitimate concerns about the way that this sort of online personalization undermines our ability to access a sufficiently wide range of information. I explore here a related worry: how might our reliance on Google filtering be affecting our ability to listen responsibly and meet our duties to be good listeners? I propose that our received wisdom is misguided, and in fact search engines like Google have raised the stakes for what counts as good listening. (shrink)

Finding ways to foster intellectual humility (IH)—the willingness to own one’s limitations—is an important goal for facilitating effective learning. We report the results of a longitudinal, quasi-experimental study, conducted across six undergraduate, culturally diverse (58% racial/ethnic minority) introductory philosophy courses, that evaluates how social perceptions and cross-cultural reasoning change following a course on epistemology and social ethics. Critically, we manipulated whether each class received a standardized lesson in IH at the start of the course or not. Participants provided self-ratings of (...) IH, gave round-robin judgments of their classmates, and completed a wise-reasoning writing exercise focusing on a cultural conflict at both the beginning and end of the summer course session. Results revealed no change in self-reported IH, but an increase in perceived peer IH and conscientiousness within classes where IH was taught. Moreover, an analysis of the participants’ writing suggested greater levels of compromise-seeking when dealing with a cultural conflict among the students taught about IH, relative to those in the control classes. This study provides initial insights regarding the relationship between IH and cultural humility, as well as the efficacy of facilitating the development of IH among undergraduate students. (shrink)

Wendy Brown's work commands widespread attention and respect, and there has been considerable interest as to how it would develop after "States of Injury." This book will not disappoint.

ABSTRACTIn Democratic Reason, Hélène Landemore argues that deliberation and the aggregation of citizens' dispersed knowledge should tend to produce better consequences than rule by the one or the few. However, she pays insufficient attention to the epistemic processes necessary to realize these democratic goods. In particular, she fails to consider the question of where citizens' beliefs and ideas come from, with the result that the democratic decision mechanisms she focuses on are insufficiently powerful to justify her consequentialist defense of mass (...) decision making. If “the few” are technocratic experts, Landemore supplies little reason to resist their rule on epistemic grounds, for she does not secure a knowledge base for the citizens that might compete with the knowledge that is often attributed to such “experts.” Aggregating and deliberating about poor information is no substitute for good information. Her book can therefore be seen as a call for a new phase of epistemic political theory that compares the real-world knowledgeability of ordinary citizens and putative experts, but it does not convincingly deliver on its goal to demonstrate the epistemic superiority of the former. The problem of Time is one of the most fascinating and yet most difficult of those questions to which the human mind applies itself in philosophical thought. Dean Inge, in his Philosophy of Plotinus , has referred to this problem as ‘the hardest in metaphysics,’ and we know that “from the time of Parmenides and Zeno to that of Mr. Bradley and M. Bergson, there has been no other problem that has seemed so baffling as that of Time.”.

ABSTRACT One of the promises of the internet was its power to unite individual knowers with one another, democratizing knowledge and spurring our collective efforts toward truth. In what sense is our current epistemic life a collective effort? This article examines the idea of the epistemic community. I contrast epistemic community with a collection of individual epistemic agents aiming for truth. I propose that this latter conception of epistemic life permits neglecting our epistemic and moral duties. I argue that healthy (...) epistemic community requires joint commitments to truth and epistemic trust, and where epistemic trust requires conditions of mutual epistemic respect. (shrink)
