BackgroundAs a discipline, neuroethics addresses a range of questions and issues generated by basic neuroscientific research, and its use and meanings in the clinical and social spheres. Here, we present Part 4 of a four-part bibliography of the neuroethics literature focusing on clinical and social applications of neuroscience, to include: the treatment-enhancement discourse; issues arising in neurology, psychiatry, and pain care; neuroethics education and training; neuroethics and the law; neuroethics and policy and political issues; international neuroethics; and (...) discourses addressing "trans-" and "post-" humanity.MethodsTo complete a systematic survey of the literature, 19 databases and 4 individual open-access journals were employed. Searches were conducted using the indexing language of the U.S. National Library of Medicine. A Python code was used to eliminate duplications in the final bibliography.ResultsWhen taken with Parts 1-3, this bibliography aims to provide a listing of international peerreviewed papers, books, and book chapters published from 2002 through 2016. While seeking to be as comprehensive as possible, it may be that some works were inadvertently and unintentionally not included. We therefore invite commentary from the field to afford completeness and contribute to this bibliography as a participatory work-in-progress. (shrink)

Los estudios de ciencia, tecnología y sociedad revelan las interrelaciones entre la ciencia y la tecnología como procesos sociales. Este artículo persigue como objetivo: valorar la significación social de un entorno virtual en la enseñanza aprendizaje de la Estadística descriptiva en la carrera de Medicina. El diagnóstico preliminar mediante de la observación, la encuesta y el análisis documental, mostró que existen insuficiencias en el uso de las tecnologías de la información y las comunicaciones en el proceso de enseñanza aprendizaje (...) de la Estadística. Se diseñó un entorno virtual constituido por cinco módulos que incluyen contenidos de Estadística descriptiva, ejercicios, bibliografía y ayuda. Con la validación a partir del criterio de especialistas y la sistematización con la introducción en la práctica, se evaluó el mismo como muy adecuado, fue considerado útil, necesario y aplicable al contexto objeto de estudio, lo que contribuye a la formación científica de los estudiantes desde una posición materialista. Science, technology and society studies reveal the interrelations between science and technology as social processes. This article pursues the following objectives: to assess the social significance of a virtual environment in the teaching and learning of descriptive Statistics in Medicine degree course. The preliminary diagnosis through the observation, opinion poll and documentary analysis, showed that there are insufficiencies in the use of Information and Communication technologies in teaching and learning Statistics. It was designed a virtual environment with five modules that include contents of descriptive Statistics, exercises, bibliography and help. With the validation of specialists and the systematization after putting it in practice, the virtual environment was assessed as adequate, useful, necessary and applicable to the context being studied, which contributes to the scientific training of students from a materialistic position. This paper describes the present resource allocation problems in the Danish tax-based public health care system and presents an analysis of the two policy options put forward as a solution to these problems: (1) explicit rationing of services, and (2) the introduction of two-tiered health care. It is argued that a two-tiered system with a private second tier is unlikely to be acceptable and viable in Denmark, whereas an introduction of a second tier within the public system may be more (...) acceptable. It is further argued that present implicit rationing methods may be unethical and ought to be replaced by explicit rationing. Renamed “Patient Rights and Organization Ethics,” these standards are based on the realization that a hospital’s obligation to its (...) patients is derived from two relationships: the patient-provider relationship governed by “clinical ethics,” and the customer-supplier relationship governed by “business ethics.” “[This] boundary between ‘clinical’ ethics and ‘business’ ethics is not clear and in many cases is nonexistent. While marketing and admission practices are seen as issues related to ‘business’ ethics, they can lead to unneeded admissions or demand for unneeded services, both of which can unnecessarily expose the patient to the risk of side effects or complications. Issues of race have traditionally been addressed in medical school curricula in a didactic manner. However, medical school curricula often lack adequate opportunity for the application of learning material relating to race and culture. When confronted with acts of racism in clinical settings, students are left unprepared to respond appropriately and effectively. Forum Theatre offers a dynamic platform by which participants are empowered to actively engage with and become part of the performance. When used in an educational context, Forum Theatre (...) can be a powerful tool for students to interact with a wide variety of social issues. This paper describes the process by which one medical school designed a workshop in the Forum Theatre style to equip students to respond to racism observed in clinical settings. Based on real student experiences, the Responding to Racism in the Clinical Setting workshop was designed to give students an opportunity to combine cultural humility, communication theory and conflict resolution skills in order to prepare for interactions in clinical stages of medical education. As a result of workshop evaluations, surveys, and written reflections, the authors propose that Forum Theatre is a novel teaching modality for incorporating issues of race and culture into medical curriculum. In lieu of an abstract, here is a brief excerpt of the content:Histories of Science in Early Modern Europe:IntroductionRobert GouldingIn 1713, Pierre Rémond de Montmort wrote to the mathematician Nicolas Bernoulli:It would be desirable if someone wanted to take the trouble to instruct how and in what order the discoveries in mathematics have come about.... The histories of painting, of music, of medicine have been written. A good history of mathematics, especially of geometry, would be a much more interesting (...) and useful work.... Such a work, if done well, could be regarded to some extent as a history of the human mind, since it is in this science, more than in anything else, that man makes known that gift of intelligence that God has given him to rise above all other creatures.1Such a history of mathematics was attempted by Jean-Etienne Montucla in his Histoire des mathématiques (first printed in 1758, and reissued in a [End Page 33] greatly expanded form in 1799).2 Montucla's great work is generally acknowledged as the first genuine history of mathematics. According to some modern historians, previous attempts at such a history had amounted to little more than collections of anecdotes, biographies or exhaustive bibliographies: "jumbles of names, dates and titles," as one writer in the Dictionary of Scientific Biography characterized them.3 Montucla, on the other hand, was thoroughly animated by the Enlightenment project expressed in Rémond's letter, and his Histoire was a philosophical history of the "development of the human mind," as he himself described it.4 It was precisely Montucla's vision of what mathematics meant and his conviction that mathematics itself must change in order to reflect the historical elevation of the human intellect, that allowed him to transform the scattered dates and anecdotes of his predecessors into a genuine history. All subsequent histories of mathematics—until the most recent social histories5 —have been in a sense "footnotes to Montucla."The papers here argue that there were indeed histories of mathematics before Montucla which are worthy of scholarly attention, and that the dismissal of Renaissance histories of science as nothing more than a "cloud of fine adjectives and metaphors" is unfair (although it contains a germ of truth). Anthony Grafton has argued in an article on the historical writings of Cardano, Rheticus, and Kepler, that the purpose of such texts was not so much to trace what actually happened, as to justify the study of a subject often derided—by both humanist and scholastic writers—as obscure, useless and undignified.6 The following four papers go further in arguing that writings in this genre (or, at least, some writings in this genre) were not limited to this characteristically humanist objective. Authors did not just use their histories to persuade others. They themselves relied on historical narratives in order to think about their discipline, define its parts, distinguish among its acceptable and unacceptable forms and prescribe its content [End Page 34] and method of teaching. By placing their discipline into a historical context shared by other, more mainstream humanistic arts, moreover, they could draw upon the large, narrative structures which Renaissance humanists had adapted to understand human intellectual and cultural development, origins, progress and decline.James Byrne's article highlights a disparity of vision between those humanists who treated the origin of the arts as just another subject for humanistic display; and those (like Regiomontanus) who were invested in the arts and wrote their histories from an insider's perspective. Alongside Regiomontanus, one could include several other authors treated in this collection, such as Thomas Vaughan, Peter Ramus, and Henry Savile. In each case, history provided a means for working through problems of the legitimacy and nature of their disciplines. While their writings are not "histories of the human mind," their interests went beyond mere rehearsal of anecdotes. Ramus's histories, for instance, written 200 years before Montucla's Histoire, had the kind of unifying vision which de Montmort demanded, and shared Montucla's concern to find a meaning in the development of mathematics. The social ethics of medicine is the study and ethical analysis of social structures which impact on the provision of health care by physicians. There are many such social structures. Not all these structures are responsive to the influence of physicians as health professionals. But some social structures which impact on health care are prompted by or supported by important preconceptions of medical practice. In this article, three such elements of the philosophy of medicine (...) are examined in terms of the negative impact on health care of the social structures to which they contribute. The shortage of organs (...) for transplantation has been an ongoing problem for more than 20 years. In June1981, Representative Philip Crane (R-IL) introduced legislation in the U.S. House of Representatives proposing a $25,000 income tax deduction and a $25,000 estate tax exclusion for donating an organ (Tax Incentives 1981). Still the National Organ Transplant Act enacted in October 1984 prohibited the offering of any "valuable consideration" in return for organs (United States 1984).The developments of a legal market for tissues in the United States and a black market for organs around the world have raised the question of whether donation without compensation is the only ethically viable option. Some critics of the current system claim reliance on donation actually promotes a black market in organs (Kaserman and Barnett 2002, p. 38; Palmer 1999, p. 36). In addition, a new set of compensation issues has arisen as living kidney donor transplantations begin to rival those performed with cadaveric organs (United States 2001).As a result, current suggestions for incentives go far beyond the traditional death benefit payment to include such ideas as medals of honor for donors, reimbursement for funeral expenses, exchange programs for organ donation, and special medical insurance for living organ donors. In October 2002, the U.S. Department of Health and Human Services awarded $5.2 million in grants to study both clinical and behavioral interventions for increasing organ donations that do not involve incentives. The inspiration for some of the funded proposals comes from the successes of Spain's social services and the United Kingdom's clinical administration of organ procurement programs (Robeznieks 2002). This essay explores various ways in which bibliographies have exhibited “sociality.” Bibliographies are both products of the social contexts that have created them and engines of social interaction in scholarly communities. By tracing the history of the Isis Bibliography, the longest-running and most comprehensive bibliography in its field, this essay explains how different Isis classification systems have been tied to major twentieth-century cataloging efforts. By looking at classification, the essay also attends to the ways in which (...) aspects of the Isis Bibliography in different decades have reflected social mores of their period. Finally, it demonstrates how critical the Isis Bibliography was in the formation of the discipline of history of science and goes on to discuss how that disciplinary connection is evolving in the twenty-first century. By thinking of the bibliography as a network of scholars, not just scholarly works, the essay asks us to reflect on the nature and purpose of bibliography in the digital age. (shrink)

Extensive scientific, philosophical and artistic activities were carried out in the Islamic World’s various science and civilization centers during the early Middle Ages. In these centers, noteworthy works of mathematics, astronomy, geography, medicine, pharmacology, optics, botany, chemistry and other fields of science, which would later determine improvement paths for these fields, were created. Abu al-Izz Ismail ibn al-Razzaz al-Jazari (12th-13th centuries), was a magnificent Muslim scientist known for his work named The Book of Knowledge of Ingenious Mechanical Devices (Kitab (...) fi ma‘rifati’l-hiyal al-handasiyya) and he was born in geography where many scientists were trained. He contributed to the development of science and civilization, the joint legacy of humanity and demonstrated his true capability and success in the field of ilm al-hiyal (the scientific field which addresses the information and practices regarding mechanical technology and engineering in Islamic science history). From this perspective, it is evident that al-Jazari had a wide field of interest, made his mark in the age he lived in with his brave and innovative ideas, and was a wise engineer. al-Jazari’s detailed descriptions and explanations of water clocks, oil lamp clocks and water pumping machines, as well as their construction, were astounding. In addition, al-Jazari’s studies on metal casting techniques, cone valves, wheel balance and the real-world use of aspiration tubes are important in contemporary engineering studies. When all these are considered, it becomes clear that his designation as the “Leonardo da Vinci of the Islamic World” is not a coincidence. Many studies on al-Jazari’s scientific legacy have been conducted in various parts of the world. There are various manuscripts of his aforementioned book. Based on these manuscripts, the work has been -fully or partially- translated into many languages, including Turkish. The German and English translations served as a basis for studies on al-Jazari in other languages. In recent years, studies conducted on al-Jazari in Türkiye (books, articles, domestic and international symposiums, congresses etc.) have demonstrated his brilliance and competence as a scientist. Furthermore, studies on al-Jazari are still ongoing in Türkiye. As for Russia, the scientific legacy of the scientists raised in the Islamic World such as Avicenna, al-Farabi, Abu Bakr al-Razi, Abu Rayhan al-Biruni, Abu al-Qasim al-Zahrawi, Muhammad ibn Musa al-Khwarizmi, al-Battani, al-Kashi, Abd al-Rahman al-Khazini, Nasir al-Din al-Tusi was researched within the scope of a special plan and program. Depending on the relevant studies, al-Jazari and his work drew attention as well. A small portion of the aforementioned Russian studies is known in Türkiye. It is safe to assume that al-Jazari studies in Russia are virtually unknown in Türkiye. The present study primarily aims to contribute to the literature on the scientific legacy of al-Jazari, a Middle Age Islamic scientist best known for his famous work in the mechanical field. More specifically, the present study aims to: 1) to identify the studies on al-Jazari in Russia, and in the former USSR countries, where academic studies are still in Russian after the Soviet Union period, to create a chronological biography and to set forth their compilation; in other words, to create a bibliography which includes Russian studies on al-Jazari; 2) to provide information on the structure and content of the studies in question, to detect the methodological issues encountered in these works, and make solution recommendations to solve these issues; 3) to contribute to continuing and future studies in Türkiye on al-Jazari in terms of source and material index. In conclusion, it should be stated that comparative reading was conducted centered on The Book of Knowledge of Ingenious Mechanical Devices (Kitab fi ma‘rifati’l-hiyal al-handasiyya). It should also be noted that Russian popular internet publications (which are not based on academic research and are created/reproduced for commercial and personal purposes) were excluded from the assessment. The success of precision medicine depends on obtaining large amounts of information about at-risk populations. However, getting consent is often difficult. Why? In this commentary I point to the differentials in social status involved. These differentials are inevitable once personal information is surrendered, but are particularly intense when the studied populations are socioeconomically or socioculturally disadvantaged and/or ethnically stigmatized groups. I suggest how the deep distrust of the latter groups can be partially justified as a lack of confidence (...) that their core values or interests will sufficiently be taken into account. Hence, the ethical challenge here lies not in avoiding status differentials, but in dealing with them appropriately. Scientists should not assume trust from others but adopt a norm of “demonstrating trustworthiness”. (shrink)

A project accomplished largely by Roman members of Catholic Action. It is a compendium of maxims and discussion in moral theology, arranged alphabetically by subject. The book shows a sustained effort to incorporate evidence from, and problems posed by, contemporary jurisprudence, medicine and psychiatry, and political and social theory; but the moral authority founded upon revelation, Catholic tradition, and the pronouncements of Pius XII is never qualified. An introduction spells out in detail the purpose and standpoint of the (...) work. A forty-page bibliography is organized under special and general topics. The many articles upon matters sexual and familial make for interesting browsing; the general topic heading of the longest section is "marriage." Comments upon other religions, and non-Catholic philosophical and political views, are brief and harsh. For purposes of reference to a definitive Catholic point of view upon questions of practical morality, the work is of interest.--E. W. (shrink)

Calling on the methodology of psychology, the authors explore the way illness alters the self-image of the sick person, and the way the experience changes the person who is ill. The reader is taken through the psychological impacts of the first clinical moment when the patient realizes he or she is in the altered state of illness, as well as the subsequent effects of pain, hospitalization, being bed-ridden, fatigued or disabled. The central thesis is that an integral picture of (...) class='Hi'>medicine must include social-cultural aspects, the institutionalization of health care, and an attempt to provide patient-centered care. The approach taken is the empirically based psychosocial reflection of the disturbances to a person's self-image caused by various kinds of illness. Focusing on the world of the patient, the authors relate to problems concerning self-image, pain, exhaustion, isolation, handicaps, aging, dying and death. Important achievements were obtained in the fields of mathematics, medicine, chemistry, astronomy, physics, optics, mechanics, zoology, botanic, mineralogy, geography and etc. in the Turkish-Islamic world between the ninth and thirteenth centuries. According to the leading historians of science, scholars living in different parts of the TurkishIslamic world not only surpassed their Greek and Byzantine predecessors. They also paved the way for development of these fields of science for following centuries. It is possible to mention many physicians that left non-erasable (...) marks in the medicine history among these distinguished scholars. One of these names is an Andalusian surgeon-physician called Abul-Qāsim alZahrāwī. He is known for his solid and unique contributions to the surgical world and regarded as the founder of surgery in the TurkishIslamic world. al-Zahrāwī wrote many books. One of these books, called al-Taṣrīf (The Method of Medicine) regarded as a medical encyclopedia has reached until today. The book attracted more attention in the European countries than in the Turkish-Islamic world and had an impact on Europe. Different chapters from the book were translated into languages such as Hebrew, French, English and especially Latin. al-Zahrāwī’s scientific heritage has also been discussed with all its aspects in Russia where scientists in the Turkish-Islamic world have especially been studied. Even though it was translated at a later period than Avicenna’s al-Qānūn fī al-Ṭibb (The Canon of Medicine), al-Taṣrīf was translated into Russian. The aim of this study is to identify Russian studies on Zahrawi and his main work, al-Taṣrīf -within the framework of available opportunitiesand to present prominent ones among these. In conclusion, this article also aims to contribute to the Turkish literature on al-Zahrāwī and al-Taṣrīf. Therefore, the author wishes to assist the relevant ongoing studies and the ones to be conducted in the future in terms of bibliography and material index. (shrink)

Purpose: This review identifies the prominent topics in the literature pertaining to the ethical, legal, and social issues raised by research investigating personalized genomic medicine. Methods: The abstracts of 953 articles extracted from scholarly databases and published during a 5-year period were reviewed. A total of 299 articles met our research criteria and were organized thematically to assess the representation of ELSI issues for stakeholders, health specialties, journals, and empirical studies. Results: ELSI analyses were published in both scientific (...) and ethics journals. Investigational research comprised 45% of the literature reviewed and the remaining 55% comprised normative analyses. Traditional ELSI concerns dominated the discourse including discussions about disclosure of research results. In fact, there was a dramatic increase in the number of articles focused on the disclosure of research results and incidental findings to research participants. Few papers focused on particular disorders, the use of racial categories in research, international communities, or special populations. Conclusion: Considering that strategies in personalized medicine increasingly target individuals’ unique health conditions, environments, and ancestries, further analysis is needed on how ELSI scholarship can better serve the increasingly global, interdisciplinary, and diverse PGM research community. Samuel Thomson , a New Hampshire farmer, devised a form of medical treatment that became popular in the United States for about three decades to the middle of the nineteenth century. Thomson relied on steaming and botanical substances—mainly cayenne pepper and lobelia—to increase the body's temperature and restore health. He practiced on others, acquired a patent for his medicine, sold a “right” to others wishing to practice his methods, and formed the “Friendly Botanic Society.” In 1822 the first of (...) many editions of his New Guide to Health; or, Botanic Family Physician appeared, along with A Narrative of the Life and Medical Discoveries of Samuel Thomson. His practices were lucrative: even to buy the New Guide to Health at $2, a follower must already have purchased a right for $20. Going well beyond self‐help medicine, his publications also demanded reform in education, politics, religion, and attitudes to class and gender, among other spheres. Thomson's followers then produced their own books and journals, spreading Thomsonianism among reform‐minded Americans from North to South and into the Midwest.The People's Doctors, by John Haller, assumes that readers know this story and its context. The book opens not with Thomsonianism but with phrenology. A quotation from Daniel Drake, a prominent physician, seems then to herald a sophisticated study of this sect as an antiestablishment group of reformers. As Haller points out, such a study would be a prolegomenon to the later history explored in his three other books on American botanic medical practices. Instead, The People's Doctors primarily documents the development of Thomson's sect in three parts: “The Man and His Medicines,” “The Middle Years,” and “The Diaspora.” The central chronology is framed by two four‐page sections that introduce and explain Thomson's role in the history of American medicine. Ten appendixes present organizational records of Thomsonianism .Steeped in Thomsonian journals, this study demonstrates their richness for historical investigation. Thomson appears here in a gentler historical light than that of untutored lay healer; indeed, adopting modern terms, Haller portrays Samuel Thomson as a pioneer in the concept of a national prepaid health‐care plan . These journals also reveal the extent to which Thomsonianism was not monolithic, despite Samuel Thomson's bids to control both his medicines and his followers: by the late 1830s, “new light Thomsonians” emerged to pursue improvements on his original botanic medicines.Reliance on Thomsonian journals and Thomson's autobiography simultaneously leads to the adoption of the views of those studied. From the outset, for instance, the study refers to “Dr.” Thomson; it is some time before the uninformed reader learns that Thomson was not a doctor in the usual sense. The eventual split among followers is attributed in part to Thomson's having become “an angry old man” , drawn “deeper and deeper into petty quarrels,” who “seemed too opinionated” —despite evidence that, from an early age, Thomson had an unbridled urge for control exercised through lawsuits, defamation of character, and ostracism. More important, this reliance raises questions about context. The sect's publishing, organizing, and educational activities are not connected to the activities of mainstream medicine in the same period. Haller situates the sect historically only in his last section, “Reassessment,” but without citations; in fact, other studies on Thomsonianism are relegated mainly to the acknowledgments, with many excluded from the bibliography.A rich and potentially rewarding subject for social historians, Thomsonianism reflected intricately woven assumptions about reform issues in a turbulent period in American history. Haller has contributed an internal, organizational perspective on the sect that may aid future researchers. Yet while his observation that the “history of Thomsonism is illustrative of the divided soul of American sectarian medicine” may be true, we need a broad‐based, balanced history to be able to evaluate it. Few diseases are extensively diffused as influenza, but though flu pandemics occur with regularity throughout history the bibliography is dominated by the 1918-1919 “Spanish influenza” pandemic. This review argues that this preoccupation is largely a product of historical epidemiology and retrospective statistical analysis which has made the Spanish flu the reference point against which other modern respiratory pandemics, including COVID-19, are measured—hence the Spanish flu’s importance for the 21st century pandemic imaginary. The review identifies six distinct thematic areas within (...) the historiography of H1N1 Spanish influenza. These include medical writings which attempt to read the history of the Spanish flu backwards to “learn” public health “lessons” for the mitigation of future pandemics, and ecological writings in which influenza is seen as the paradigm of an emerging infectious disease and a model for the genesis of epidemics and pandemics from zoonotic reservoirs. Scholarship since 1997 also reflects a growing interdisciplinarity, one in which bioarchaeology and molecular dating techniques have furnished new insights into the history of influenza and the Spanish flu’s evolutionary origins, bringing the life sciences into closer dialogue with the medical and environmental humanities. These scientific insights have spurred both academic and popular writings on the Spanish flu, rendering its characterization as the “forgotten pandemic” something of an oxymoron. Indeed, if anything, the centenary of the Spanish flu in 2018 and the 2019-2023 COVID pandemic have provoked renewed interest in several of the themes identified in this review: including, most particularly, the writings of social historians of medicine, cultural historians, and disease demographers. The branch of clinical medicine most likely to qualify as a social science is family medicine. Whether family medicine is a social science is addressed in four steps. First, the nature of family medicine is outlined. Second, the extent to which social science knowledge is used in family practice is discussed. Third, the extent to which family medicine can qualify as a social science is considered with respect to an orthodox model (...) of the social sciences, that is, one that emphasizes affinities between the natural and social sciences. Finally, the same question is addressed with respect to an unorthodox model of the social sciences, that is, one that stresses the evaluative nature of the social sciences. Se realizó una revisión bibliográfica de materiales disponibles en revistas electrónicas de la base SciELO con el objetivo de fundamentar la contribución de la Medicina Tradicional y Natural a la Salud Pública cubana y las interrelaciones ciencia-tecnología-sociedad. La perspectiva Ciencia, Tecnología y Sociedad contribuye a construir una cultura científica para que la población en general pueda llegar a sentirla como propia, lo cual requiere priorizar la aplicación de la Medicina Tradicional y Natural socialmente útil y culturalmente relevante con el compromiso (...) social basado en la erradicación de la pobreza, la armonía con la naturaleza y el desarrollo sustentable. Se enfatizó en la importancia de la medicina preventiva, además de poner en evidencia la necesidad de reconceptualizar la investigación en las ciencias médicas sobre la base de una visión interdisciplinaria que permita revelar fenómenos no explicados hasta el momento, aspecto que necesita del enfoque desde la perspectiva de la ciencia, tecnología y sociedad como complemento esencial. We conducted a bibliographic review of materials available in electronic journals in SciELO base aiming to substantiate the contribution of Natural and traditional medicine to the Cuban public health and the interrelationships of science-technology - society. Science, technology and society perspective contributes to build a scientific culture so the population can in general get to feel it as their own, which requires to prioritize the application of traditional medicine and Natural, socially useful and culturally relevant social commitment based on the eradication of poverty, the harmony with nature and sustainable development. Emphasized the importance of preventive medicine, as well as highlight the need to reconceptualize the research in the medical sciences on the basis of an interdisciplinary view that reveal phenomena not explained so far, issue that needs the approach from the perspective of science, technology and society as essential complement. Bengt Brülde in his article ``The Goals of Medicine. Towards a Unified Theory'' has proposed a normative theory of the goals of medicine within which the concept of quality of life plays a crucial role. In Brülde's analysis, however, the very concept of medicine is deliberately left quite vague and it is therefore difficult to see how the goals of medicine are related to the goals of closely allied enterprises such as health promotion and social (...) welfare. In this reply I therefore propose an analysis of these related conceptual areas. I do this mainly in two respects. Following the nomenclature in a previously published article I propose a systematic conceptual framework for all varieties of health enhancement and distinguish different notions of medicine within this framework. A consequence of this analysis is, for instance, that the means and also the immediate goals of medicine in its broadest sense are more diversified than the means and immediate goals of medicine in its narrowest sense. From this position I expand the topic further by comparing medicine and health enhancement with social welfare and try to trace the basic features between – as well as the common properties of – these different enterprises. The author proceeds from empirical basis to show how far the two Rawls's principles of justice can be implemented in Africa. Positions not only of Rawls but also of other philosophers are presented, reconstructed and commented. They are also opposed with critiques and other theories, so that the appropriate position for Africa can be explained. The author comes to the conclusion that the fundamental liberties are still in the making in Africa. A long colonial past and Aparthied have deprived Africa (...) from its blossoming. However, a groping democracy of full hope is pointing ahead. So that freedom and democracy gain more room the author suggests the establishment of federalism based on ethnic groups. To speak of democracy and freedom without a substantial economic support seems illusive. In order to improve the lot of the least advantaged the author proposes betterment of education and health. Technologies for enhancement of the human body historically have taken the form of an apparatus: a technological device inserted in, or appended to, the human body. The margins of these devices were clearly discernible and materially circumscribed, allowing the distinction between the corporeality of the human body and the “machine” to remain both ontologically and materially secure. This dualism has performed some important work for human rights theorists, regulators, and policy makers, enabling each to imagine they can establish where the (...) human ends and the other begins. New regenerative products such as Infuse™ and Amplify™ subsist, as animal-derived scaffolds seeded with growth hormone implanted within a prosthetic device. They are much more materially complex, and their identities thus remain open to contestation. Following Lochlann Jain’s 2006 work, I thus attend closely to their social lives, particularly the stories that are told about them and how these are employed to construct understandings of what kind of a phenomenon they are: systemic drug, biologic, or combinatorial medical device. The significance of this classificatory project is revealed in the final section of this paper, which explores how these stories shape understandings of “product failure,” liability, and causation when such products overflow their material and ontological categorization and their recipients become disturbingly “more than human.”. SummaryThis article analyses the viewpoints of doctors, patients and the state of the phenomenon of unconventional methods of treatment/alternative medicine in Slovenia. The doctors’ viewpoints are taken from the official documents of the Medical Chamber of Slovenia and the Slovene Medical Association. The patients’ viewpoints are established on the basis of public opinion and epidemiological research, carried out in 1994 and 1996 on two representative samples of the Slovene population from the Celje area. The estimation of the attitude of (...) the state is based on legislation and other regulations connected with alternative medicine. The findings of our analysis show a strongly negative attitude of doctors to any form of alternative medicine. In contrast, as many as half the patients express a favourable opinion on alternative methods of treatment, and more than a third of them actually make use of them. From existing laws it is evident that the state in general does not deal with alternative medicine, except for possibilities in such cases when a doctor or some other healthcare professional uses alternative methods of treatment. Alternative medicine is, from the point of view of social medicine, a phenomenon that should be followed, analysed and controlled. Dangerous as well as protective factors which accompany the exceution of alternative methods should be recognised so as to be able to inform the public of the results in a timely and accurate manner. Because of the need for the legal and healthcare security of Slovene citizens, the state should regulate existing unconventional treatments. (shrink)

Resumo Em 1747, John Wesley, spiritus rector do movimento metodista, publicou a primeira edição do seu guia medicinal Primitive Physic[k] . Qual era o seu propósito num mundo onde a academia real, herbalistas, curandeiros/as, exorcistas e charlatães competiam pela atenção da população? O artigo apresenta os diferentes grupos que atuaram, ou pretendiam atuar, em prol da saúde na Inglaterra do século 18, e compara o conteúdo do guia Primitive Physic[k] com suas propostas e estratégias terapêuticas. Conclua-se que uma parte significativa (...) do guia é composta por orientações da academia real de medicina, mas que sempre se favorecem remédios caseiros, com ingredientes acessíveis para as classes mais humildes. Quanto à chamada Spiritual Physick , menciona-se a oração como medida complementar, mas ignora-se plenamente a prática do exorcismo. What was its purpose in a world where the Royal Academy, herbalists, healers, exorcists and quacks competed for the attention of the population? The article introduces the different groups who promoted or pretended to promote health in 18th century England and compares the contents of the guide Primitive Physic[k] with their proposals and therapeutic strategies. The conclusion is that a significant portion of the guide consists of guidelines of the Royal Academy of Medicine, but that it always favors homemade remedies with ingredients available to humbler classes. In relation to the so called Spiritual Physick, prayer is mentioned as a complementary measure, but the practice of exorcism is totally ignored.
