The purpose of this article is to describe the development of a model of moral distress in military nursing. The model evolved through an analysis of the moral distress and military nursing literature, and the analysis of interview data obtained from US Army Nurse Corps officers (n = 13). Stories of moral distress (n = 10) given by the interview participants identified the process of the moral distress experience among military nurses and the dimensions of the military nursing moral distress (...) phenomenon. Models of both the process of military nursing moral distress and the phenomenon itself are proposed. Recommendations are made for the use of the military nursing moral distress models in future research studies and in interventions to ameliorate the experience of moral distress in crisis military deployments. The development of nursing ethics as a field of inquiry has largely relied on theories of medical ethics that use autonomy, beneficence, and/or justice as foundational ethical principles. Such theories espouse a masculine approach to moral decision-making and ethical analysis. This paper challenges the presumption of medical ethics and its associated system of moral justification as an appropriate model for nursing ethics. It argues that the value foundations of nursing ethics are located within the existential phenomenon of human caring within (...) the nurse/patient relationship instead of in models of patient good or rights-based notions of autonomy as articulated in prominent theories of medical ethics. Models of caring are analyzed and a moral-point-of-view (MPV) theory with caring as a fundamental value is proposed for the development of a theory of nursing ethics. This type of theory is supportive to feminist medical ethics because it focuses on the subscription to, and not merely the acceptance of, a particular view of morality. (shrink)

The article proposes Logic of Secrets in Collaboration Networks, a formal logical system for reasoning about a set of secrets established over a fixed configuration of communication channels. The system’s key feature, a multi-channel relation called independence, is a generalization of a two-channel relation known in the literature as nondeducibility. The main result is the completeness of the proposed system with respect to a semantics of secrets.

Traditional flat-structured bigram and trigram models of phonotactics are useful because they capture a large number of facts about phonological processes. Additionally, these models predict that local interactions should be easier to learn than long-distance ones because long-distance dependencies are difficult to capture with these models. Long-distance phonotactic patterns have been observed by linguists in many languages, who have proposed different kinds of models, including feature-based bigram and trigram models, as well as precedence models. Contrary to flat-structured bigram and trigram (...) models, these alternatives capture unbounded dependencies because at an abstract level of representation, the relevant elements are locally dependent, even if they are not adjacent at the observable level. Using an artificial grammar learning paradigm, we provide additional support for these alternative models of phonotactics. Participants in two experiments were exposed to a long-distance consonant-harmony pattern in which the first consonant of a five-syllable word was [s] or [∫] (“sh”) and triggered a suffix that was either [-su] or [-∫u] depending on the sibilant quality of this first consonant. Participants learned this pattern, despite the large distance between the trigger and the target, suggesting that when participants learn long-distance phonological patterns, that pattern is learned without specific reference to distance. In Democracy without Shortcuts, Cristina Lafont identifies proceduralist or ‘deep pluralist’ conceptions of democracy alongside epistemic and lottocratic approaches as shortcuts that avoid the more challenging but, in her view, preferable path of engaging with and attempting to sway competing views, values and beliefs of fellow citizens. I argue that with the wholesale dismissal of proceduralist accounts of democracy Lafont herself takes two shortcuts: The first concerns the characterization of deep pluralism as unable to explain substantive disagreement after a decision (...) is settled, and the second undervalues proceduralism’s ability to evaluate and criticize the substance of the political decision-making process. While her critique is fitting for minimalist conceptions of proceduralism, a theory of normative proceduralism shares many objectives with Lafont’s vision of a participatory deliberative democracy. Integrating those approaches instead of dismissing proceduralism outright would render her project appealing to theorists who would not otherwise consider themselves deliberative democrats. (shrink)

Employers and governments are interested in the use of serological testing to allow people to return to work before there is a vaccine for SARS-CoV-2. We articulate the preconditions needed for the implementation of antibody testing, including the role of the U.S. Food & Drug Administration.

Advances in molecular technologies have the potential to help remedy health inequities through earlier detection and prevention; if, however, their delivery and uptake are not more carefully considered, there is a very real risk that existing inequities in access and use will be further exacerbated. We argue this risk relates to the way that information and knowledge about the technology is both acquired and shared, or not, between health practitioners and their patients.A healthcare system can be viewed as a complex (...) social network comprising individuals with different worldviews, hierarchies, professional cultures and subcultures and personal beliefs, both for those giving and receiving care. When healthcare practitioners are not perceived as knowledge equals, they would experience informational prejudices, and the result is that knowledge dissemination across and between them would be impeded. The uptake and delivery of a new technology may be inequitable as a result. Patients would also experience informational prejudice when they are viewed as not being able to understand the information that is presented to them, and information may be withheld.Informational prejudices driven by social relations and structures have thus far been underexplored in considering equitable implementation and uptake of new molecular technologies. Every healthcare interaction represents an opportunity for experiencing informational prejudice, and with it the risk of being inappropriately informed for undertaking such screening or testing. Making knowledge acquisition and information dissemination, and experiences of informational prejudice, explicit through sociologically framed investigations would extend our understandings of equity, and offer ways to affect network relationships and structures that support equity in delivery and uptake. Current facts about soil erosion, groundwater “mining,” and impact of toxic substances suggest a resource crisis in our farming system. Yet traditional checks on the exploitation of farmland, capsulized in the “stewardship ethic,” proceed from too limited a viewpoint to adequately address the root of the exploitation and proffer an alternative. After briefly examining the stewardship ethic, I consider the developmentof a “partnership ethic” to guide the use of land for farming which builds its essential elements out of the reflections (...) of feminist thinkers on the relationship between humankind and nature. Instead of using “rights” language to express the ethic, I develop a theory of appropriate use analogous to the appropriate use of another person’s capabilities-i.e., that such moral use should respect and not destroy the other and that it should return something of value to the other in exchange for the use. Finally, those principles are examined for their practical implications for farmland use and national farm policy. (shrink)

A fundamental criterion considered essential to deem the procedure of vital organ procurement for transplantation ethical is that the donor must be dead, as per the Dead Donor Rule (DDR). In the case of Donation after Circulatory Death (DCD), is the donor genuinely dead? The main aim of this article is to clarify this uncertainty, which primarily arises from the fact that in DCD, death is determined based on cardiac criteria (Circulatory Death, CD), rather than neurological criteria (Brain Death, BD), (...) and that to allow the procurement procedure, physicians reperfuse the organs in an assisted manner. To ensure that the cessation of circulation leads to the irreversible loss of brain functions, DCD regulations require that physicians wait a certain period after CD before commencing vital organ procurement. However, during this “no-touch period,” the organs are at risk of damage, potentially rendering them unsuitable for transplantation. When DCD is performed on patients whose CD follows a Withdrawal of Life-Sustaining Treatment (WLST) (DCD Maastricht III category), how long should the no-touch period last? Does its existence really make sense? Does beginning the procedure of vital organ procurement immediately after WLST constitute a violation of the DDR that can be ethically justified? The discussion aims to provide arguments in support of the non-absoluteness of the DDR. (shrink)

This article has the double goal of reflecting on the concept of narrative identity in Paul Ricoeur’s Thinking and of evaluating its contribution to the resolution of the general problem of personal identity. Accordingly, this article will develop the following thesis: 1) narrative identity results from a permanent dialectic between character and selfhood, that is, between subject’s power to relate continuously to himself during all his life through narrative mediation and subject’s psychological and physical traits; 2) personal identity is the (...) continuous ethical and aesthetical recreation and narrative identity brings perfectly together these two domains. In this paper I will compare Hertz and Wittgenstein in order to bring forth a new interpretation of the Tractatus. I shall look at Wittgenstein’s ideas about simple objects and compare them with Hertz’s material particles. I shall then claim that if one understands Hertz’s material particles as logical entities that are more co-ordinate like than physical, one can reach an interpretation of the Tractatus that is deliberately silent about the nature of reality, therefore escaping both objectivist and subjectivist interpretations (...) of the book. Whenever the legitimacy of a new or ethically contentious medical intervention is considered, a range of influences will determine whether the treatment becomes accepted as lawful medical treatment. The development and introduction of abortion, organ donation, gender reassignment, and non-therapeutic cosmetic surgery have, for example, all raised ethical, legal, and clinical issues. This book examines the various factors that legitimatise a medical procedure. Bringing together a range of internationally and nationally recognised academics from law, philosophy, medicine, health, economics, and sociology, (...) the book explores the notion of a treatment, practice, or procedure being proper medical treatment, and considers the range of diverse factors which might influence the acceptance of a particular procedure as appropriate in the medical context. Contributors address such issues as clinical judgement and professional autonomy, the role of public interest, and the influence of resource allocation in decision-making. In doing so, the book explores how the law, the medical profession, and the public interact in determining whether a new or ethically contentious procedure should be regarded as legitimate. In this introduction to part 2 of the Common Knowledge symposium “Peace by Other Means,” the journal's editor reflects on the difference between the contributions to parts 1 and 2. Whereas the first installment concentrated on ethnography, the second focuses on the peacemaking repertoire of the Greco-Latin tradition, whose basis is psychological. That tradition is characterized by its refusal of wishful thinking about human nature and, in particular, by its doubt about claims that human drives other than thumos — the (...) rage for self-aggrandizement — motivate the initiators of wars. Given this assumption about motive, the Greco-Latin tradition tends also to regard negotiations based on the rational discussion of material interests as unlikely to succeed. Success requires symbolic and ritual gestures — acts of self-humiliation on the part of those apparently with the greatest power — by which thumos is propitiated and pacified. Most of the introduction considers cases of such settlements, including two contemporary efforts at ritual peacemaking, a successful one by Queen Elizabeth II in Ireland and an unsuccessful one by Pope Francis in the Middle East. While the activity of imagination is present in much writing about environmental ethics, little direct attention has been given to clarifying its role. Both its significant presence and provocative theoretical work showing the central role of imagination in ethics suggest a need for discussion of its contributions. Environmental ethicists especially should attend to imagination because of the pervasive influence of metaphorical constructs of nature and because imaginative work is required to even partially envision the perspective of a nonhuman being. Without (...) clear awareness of the limits of contemporary Western metaphoric constructs of nature, environmental ethicists may overlook or even contribute to the cultural extinction of ideas of nature present in the imaginative visions of indigenous cultures. In this article, I briefly review the reasons why the dominant Western philosophical tradition ranks imagination below the power of abstract reasoning, survey contemporary ideas about the role of imagination in ethics, and consider the implications of these ideas for environmental ethics. The work of imaginative empathy in constructing what might be the experience of nonhuman beings, the role of diverse metaphors and symbols in understanding nature, and the process of envisioning the possible future are developed as three central contributions of imagination to environmental ethics. Imaginative work is not peripheral, butcomplementary to the work of reason in shaping an environmental ethic. (shrink)

Sara Nadal-Melsió The reception of Georg Lukács's theorizing of realist narratives has been complicated and controversial, often relying on an artificial division of Lukács's oeuvre. On the one hand, the widely admired History and Class Consciousness stands as the mouthpiece for anything philosophical or political in Lukács. On the other, one finds a generalized dismissal of (...) Lukács's "reactionary" defenses of realism. But the role of realism in Lukács's oeuvre cannot be trivialized or rejected as "doctrinaire" or "outmoded"—two of the most common charges against Lukácsean literary criticism—without damaging an understanding of his political and philosophical contributions to Western Marxism.I thus propose a revisiting of Lukács's writings on literature during the 1930s. I consider them to be a development of the philosophical insights of History and Class Consciousness and as a corrective to the aesthetic theory of Theory of the Novel in the form of an axiological understanding of literary change and preservation. One cannot accept the outcome of Lukács's philosophical investigations and dismiss his often authoritarian radical advocacy of realism—for they are mutually inclusive. Lukács's project takes shape through his use of the aesthetic to salvage ideological and epistemological issues raised by his critical adoption of Marxism. What I propose is similar to what has become a trademark of Gramscian studies.1 In the same way that one does not begin an analysis of Gramsci's philosophy without addressing his molding and obsessive study of the Italian language, his twisting and redefinition of words ("hegemony," "intellectual," and so on), one should not, indeed cannot, embark on the exegesis of Lukács's philosophical writing without addressing his use of literary criticism and of nineteenth-century realism to rethink philosophical Marxism and to rename some of his terminology; "naturalism" and "realism" became, from the 1930s onward, codewords for a reworking of his earlier "reification" and "totality." After History and Class Consciousness, the superstructure, culture, and its changing values, not only became the condition of possibility for a Lukácsean/Marxist philosophy but, in a move against determinism, the only, quasi-utopian transformative force able to explain Marxism to itself. After Lukács, it comes as no surprise that Marx's last, never completed, project was a critical study of Balzac's La comédie humaine.I will focus on two moments of Lukács's "turn to literature": his famously controversial intervention in the Expressionism Debate and his less prominent entrance into the realm of "literary criticism," the essays "Narrate or Describe" and "The Intellectual Physiognomy in Characterization. " I will also read Lukács's failure to theorize contemporary realism in terms of his own insights regarding the historicity of subjective standpoints. Paradoxically, this failure functions as an example and proof of the Lukácsean narrative of literary change.While the first critical work to acknowledge the philosophical implications of Lukács's defense of realism, Fredric Jameson's 1971 Marxism and Form, remains [End Page 62] widely read, it did not bring about a reconsideration of Lukács's literary criticism. It is Jameson's contention that Lukács's entire oeuvre can be accounted for through an epistemology of narrative. Thus, Lukács's numerous writings emerge as part of a coherent and single-minded project. According to Jameson, "Lukács's work may be seen as a continuous and lifelong meditation on narrative, on its basic structures, its relationship to the reality it expresses, and its epistemological value when compared with other, more abstract and philosophical, modes of understanding" [163].2 If one agrees with Jameson's statement, one is also forced to acknowledge the sustained coherence of the Lukácsean project together with its enforced complexity. The chronology of such a lifelong project (1885–1971) began with the ambiguities of "fin-de-siècle" aestheticism and ended shortly after May '68. In this paper we argue that while individual private interests such as autonomy and the need for a medical procedure or treatment are important in the provision and delivery of health care and the utilization of biotechnologies, these concepts need to be balanced with other interests such that in certain situations they do not take priority. We use as an example a particular developing biotechnology, xenotransplantation, to suggest that interest in the health of the public is such that this biotechnology (...) should not be permitted to move to the clinical trial stage because of the particular risk of harm it poses to the potential xeno-recipient, their close contacts and the wider population. This is despite the interest of those in need of a transplant in allowing such clinical trials to proceed. We derive support for our position from John Stuart Mill's harm principle. (shrink)
