The purpose of this article is to describe the development of a model of moral distress in military nursing. The model evolved through an analysis of the moral distress and military nursing literature, and the analysis of interview data obtained from US Army Nurse Corps officers (n = 13). Stories of moral distress (n = 10) given by the interview participants identified the process of the moral distress experience among military nurses and the dimensions of the military nursing moral distress (...) phenomenon. Models of both the process of military nursing moral distress and the phenomenon itself are proposed. Recommendations are made for the use of the military nursing moral distress models in future research studies and in interventions to ameliorate the experience of moral distress in crisis military deployments. Mind blanking is a mental state in which attention does not bring any perceptual input into conscious awareness. As this state is still largely unexplored, we suggest that a comprehensive understanding of mind blanking can be achieved through a multifaceted approach combining self-assessment methods, neuroimaging, and neuromodulation. In this article, we explain how EEG and TMS could be combined to help determine whether mind blanking is associated with a lack of mental content or a lack of linguistically or conceptually determinable (...) mental content. The development of nursing ethics as a field of inquiry has largely relied on theories of medical ethics that use autonomy, beneficence, and/or justice as foundational ethical principles. Such theories espouse a masculine approach to moral decision-making and ethical analysis. This paper challenges the presumption of medical ethics and its associated system of moral justification as an appropriate model for nursing ethics. It argues that the value foundations of nursing ethics are located within the existential phenomenon of human caring within (...) the nurse/patient relationship instead of in models of patient good or rights-based notions of autonomy as articulated in prominent theories of medical ethics. Models of caring are analyzed and a moral-point-of-view (MPV) theory with caring as a fundamental value is proposed for the development of a theory of nursing ethics. This type of theory is supportive to feminist medical ethics because it focuses on the subscription to, and not merely the acceptance of, a particular view of morality. (shrink)

This paper problematizes the analogy that Hubert Dreyfus has presented between phenomenology and cognitive science. It argues that Dreyfus presents Merleau-Ponty''s modification of Husserl''s phenomenology in a misleading way. He ignores the idea of philosophy as a radical interrogation and self-responsibility that stems from Husserl''s work and recurs in Merleau-Ponty''s Phenomenology of Perception. The paper focuses on Merleau-Ponty''s understanding of the phenomenological reduction. It shows that his critical idea was not to restrict the scope of Husserl''s reductions but to study (...) the conditions of possibility for the thetic acts. Merleau-Ponty argued, following Husserl''s texts, that the thetic acts rest on the basis of primordial pre-thetic experience. This layer of experience cannot, by its nature, be explicated or clarified, but it can be questioned and unveiled. Traditional flat-structured bigram and trigram models of phonotactics are useful because they capture a large number of facts about phonological processes. Additionally, these models predict that local interactions should be easier to learn than long-distance ones because long-distance dependencies are difficult to capture with these models. Long-distance phonotactic patterns have been observed by linguists in many languages, who have proposed different kinds of models, including feature-based bigram and trigram models, as well as precedence models. Contrary to flat-structured bigram and trigram (...) models, these alternatives capture unbounded dependencies because at an abstract level of representation, the relevant elements are locally dependent, even if they are not adjacent at the observable level. Using an artificial grammar learning paradigm, we provide additional support for these alternative models of phonotactics. Participants in two experiments were exposed to a long-distance consonant-harmony pattern in which the first consonant of a five-syllable word was [s] or [∫] (“sh”) and triggered a suffix that was either [-su] or [-∫u] depending on the sibilant quality of this first consonant. Participants learned this pattern, despite the large distance between the trigger and the target, suggesting that when participants learn long-distance phonological patterns, that pattern is learned without specific reference to distance. This paper addresses the question of the role of ostracism in democratic Athens. I argue that the frequent expulsion of aristocrats by rival aristocrats in the predemocratic polis is the key to understanding the function of ostracism in the democratic polis. I show that aristocratic "politics of exile" was a fundamental political problem in the archaic polis and that democratic political power, symbolized by the institution of ostracism, was the polis' solution to the problem. In the archaic polis, the expulsion (...) of aristocrats often led to an endless cycle of exiles and returns and consequent political instability. In 508/7, the Athenian demos usurped the power over decisions of exile by banishing Isagoras and recalling Cleisthenes from exile. By taking control of decisions of exile, the Athenian demos took over political power insofar as power over decisions of exile had become synonymous with political power. The creation of the institution of ostracism, whereby the people decided collectively whether to banish a single citizen for ten years, provided not only a mechanism for the symbolic expression of democratic power, but also a means for the practical and ideological distinction between oligarchic and democratic rule. While aristocrats had banished their opponents without limit in numbers and duration of exile, the Athenian democracy decided each year whether to banish a single individual for a limited time period. In most years, the Athenian demos chose not to exercise the power of exile. Nevertheless, the annual question in the assembly served to remind aristocrats that the demos held the fundamental political power, and hence deterred aristocrats from attempting to overthrow the democracy. The limited nature of ostracism further deterred exiled aristocrats from attempting to return by force. Finally, the infrequent and moderate use of exile as a means of resolving political conflict helps to explain the extraordinary stability of the Athenian democracy. (shrink)

The article proposes Logic of Secrets in Collaboration Networks, a formal logical system for reasoning about a set of secrets established over a fixed configuration of communication channels. The system’s key feature, a multi-channel relation called independence, is a generalization of a two-channel relation known in the literature as nondeducibility. The main result is the completeness of the proposed system with respect to a semantics of secrets.

Advances in molecular technologies have the potential to help remedy health inequities through earlier detection and prevention; if, however, their delivery and uptake are not more carefully considered, there is a very real risk that existing inequities in access and use will be further exacerbated. We argue this risk relates to the way that information and knowledge about the technology is both acquired and shared, or not, between health practitioners and their patients.A healthcare system can be viewed as a complex (...) social network comprising individuals with different worldviews, hierarchies, professional cultures and subcultures and personal beliefs, both for those giving and receiving care. When healthcare practitioners are not perceived as knowledge equals, they would experience informational prejudices, and the result is that knowledge dissemination across and between them would be impeded. The uptake and delivery of a new technology may be inequitable as a result. Patients would also experience informational prejudice when they are viewed as not being able to understand the information that is presented to them, and information may be withheld.Informational prejudices driven by social relations and structures have thus far been underexplored in considering equitable implementation and uptake of new molecular technologies. Every healthcare interaction represents an opportunity for experiencing informational prejudice, and with it the risk of being inappropriately informed for undertaking such screening or testing. Making knowledge acquisition and information dissemination, and experiences of informational prejudice, explicit through sociologically framed investigations would extend our understandings of equity, and offer ways to affect network relationships and structures that support equity in delivery and uptake. This research investigated whether perceived situational appropriateness (defined as the degree of fit between product and intended usage situations) is predictive of consumer choices for foods and beverages, on the theoretical premise that intended usage situation acts as a frame of reference in orienting choices. Extant research on the topic, though suggestive of a link, is very limited in scope and almost completely lacking with regards to choice behaviour (as opposed to other aspects such as food acceptability or intake). To (...) address the hypotheses, data collected in a series of 15 experiments ($N= 2731$ consumers in total) -- covering a wide range of product categories and usage situations -- are presented. In all studies, participants evaluated a set of stimuli varying with respect to perceived appropriateness (low to high), and evaluated each stimulus either monadically using a choice likelihood scale or by performing a discrete choice task. Regression analyses from all studies consistently indicated that perceived appropriateness significantly predicted choice response. The results were robust with respect to variation in product category and experimental protocol and, overall, strongly support the notion that appropriateness can provide a simple yet powerful (in some case accounting for over 50\% of variance) predictor of consumer choice. Effect sizes varied substantially: perceived appropriateness explained from a minimum of 3\% to over 65\% of variance in consumer choice, and this variation was linearly related to the degree of product heterogeneity in the product sets. This research also investigated possible moderators of the link between appropriateness and choice, by relating the results to consumers’ product familiarity and involvement. While both traits significantly (and positively) affected choice, they did not interact with appropriateness. Possible explanations for these results, as well as other possible candidate moderators to explore in future research, are highlighted. (shrink)

Background: Direct–to–consumer genetic testing (DTCGT) offers risk estimates for a variety of complex diseases and conditions, yet little is known about its impact on actual users, including their decisions about sharing the information gleaned from testing. Ethical considerations include the impact of unsolicited genetic information with variable validity and clinical utility on relatives, and the possible burden to the health care system if revealed to physicians. Aims: The qualitative study explored primary care patients’ views, attitudes, and decision making considerations regarding (...) DTCGT. This article focuses on the disclosure decisions participants made regarding participation, testing, and results of DTCGT, a topic which arose as a secondary aim of the study. Methods: Through four longitudinal interviews (pre–test, results, 3 and 12 months post–test) we examined twenty primary care patients’ decisions, expressed intentions, and actions regarding disclosure to immediate and extended family, friends and coworkers, and physicians about participation in and results of DTCGT. Individual interviews were analyzed using qualitative content analysis and a summative approach to describe the global themes. Results: Most participants disclosed to some immediate family; less than half disclosed to extended family; approximately half talked to friends. Most participants stated they would or might disclose to physicians about DTCGT and a few did. Conceptual themes that emerged from the data analysis include ambivalence about disclosure, consistency between intention and actual disclosure behavior and decisions, and conditional information sharing. Conclusions: Participants’ intentional and actual disclosure patterns offer insight into how they view DTCGT, weigh results, and the potential impact of DTCGT. (shrink)

In this paper I will compare Hertz and Wittgenstein in order to bring forth a new interpretation of the Tractatus. I shall look at Wittgenstein’s ideas about simple objects and compare them with Hertz’s material particles. I shall then claim that if one understands Hertz’s material particles as logical entities that are more co-ordinate like than physical, one can reach an interpretation of the Tractatus that is deliberately silent about the nature of reality, therefore escaping both objectivist and subjectivist interpretations (...) of the book. This, I think, is just one of the great results of re-thinking Wittgenstein’s ideas through Hertz’s. (shrink)

Whenever the legitimacy of a new or ethically contentious medical intervention is considered, a range of influences will determine whether the treatment becomes accepted as lawful medical treatment. The development and introduction of abortion, organ donation, gender reassignment, and non-therapeutic cosmetic surgery have, for example, all raised ethical, legal, and clinical issues. This book examines the various factors that legitimatise a medical procedure. While the activity of imagination is present in much writing about environmental ethics, little direct attention has been given to clarifying its role. Both its significant presence and provocative theoretical work showing the central role of imagination in ethics suggest a need for discussion of its contributions. Environmental ethicists especially should attend to imagination because of the pervasive influence of metaphorical constructs of nature and because imaginative work is required to even partially envision the perspective of a nonhuman being. Without (...) clear awareness of the limits of contemporary Western metaphoric constructs of nature, environmental ethicists may overlook or even contribute to the cultural extinction of ideas of nature present in the imaginative visions of indigenous cultures. In this article, I briefly review the reasons why the dominant Western philosophical tradition ranks imagination below the power of abstract reasoning, survey contemporary ideas about the role of imagination in ethics, and consider the implications of these ideas for environmental ethics. The work of imaginative empathy in constructing what might be the experience of nonhuman beings, the role of diverse metaphors and symbols in understanding nature, and the process of envisioning the possible future are developed as three central contributions of imagination to environmental ethics. Imaginative work is not peripheral, butcomplementary to the work of reason in shaping an environmental ethic. (shrink)

Current facts about soil erosion, groundwater “mining,” and impact of toxic substances suggest a resource crisis in our farming system. Yet traditional checks on the exploitation of farmland, capsulized in the “stewardship ethic,” proceed from too limited a viewpoint to adequately address the root of the exploitation and proffer an alternative. After briefly examining the stewardship ethic, I consider the developmentof a “partnership ethic” to guide the use of land for farming which builds its essential elements out of the reflections (...) of feminist thinkers on the relationship between humankind and nature. Instead of using “rights” language to express the ethic, I develop a theory of appropriate use analogous to the appropriate use of another person’s capabilities-i.e., that such moral use should respect and not destroy the other and that it should return something of value to the other in exchange for the use. Finally, those principles are examined for their practical implications for farmland use and national farm policy. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Athenian Democratic Ideology and Herodotus' HistoriesSara ForsdykeIt is an oft-repeated refrain in studies of Athenian politics that we have no ancient treatises that systematically describe Athenian beliefs about the value of democracy.1 Although we can reconstruct some democratic values from the institutions and practices of the Athenian state, scholars have had to rely on elite writers such as Thucydides, Plato, and Aristotle, or on the more allusive references in (...) Athenian drama in order to get at the positive tenets of democratic political theory. Recently, oratory has been shown to be a valuable source for democratic political values. Great progress has been made in elucidating Athenian political thought and ideology from all these sources.2In this article, I wish to point to a rather neglected source for Athenian democratic ideology, namely Herodotus' Histories. While parts of the Histories (especially the constitutional debate) have been studied for Greek political thought, less has been made of the Histories as a source for Athenian democratic political beliefs.3 I argue that democratic [End Page 329] political values form the background against which several of Herodotus' narratives resonate and that thus these narratives may provide fruitful ground from which to extract some central themes, symbols, and ideas of Athenian democratic ideology.Herodotus' narratives dealing with Athens are the obvious place to look for signs of Athenian political values, since they ultimately derive from Athenian sources. Yet full utilization of these narratives as a source for democratic beliefs has been hampered by the view, argued by Jacoby and accepted by many current historians, that Herodotus relied primarily on the aristocratic family traditions of the Alcmeonidae for his Athenian narratives and hence that his narratives do not reflect the beliefs of the Athenians as a whole but only those of a particular aristocratic family.4 [End Page 330]This view has been challenged, most recently by Rosalind Thomas in her pathbreaking study of the nature of oral traditions in archaic and classical Greece. Thomas argues that Herodotus' Histories, and in particular his account of the liberation of Athens from the Peisistratid tyranny, reflects a wide variety of traditions, including popular or polis traditions as well as family ones. Certain elements of Herodotus' account, she argues, are decidedly unflattering to the family of the Alcmeonidae and can only be explained by Herodotus' utilization of non Alcmeonid and, in particular, wider polis traditions.5Thomas's revisions of the dominant view of Herodotus' versions of Athenian history open the way for a new evaluation of Herodotus' narratives as a source for polis traditions, or widespread Athenian beliefs about their history and the origins and nature of their political system.6 Furthermore, since these polis traditions were orally transmitted, and oral traditions are shaped by the values of the group that transmits them, they should reveal the values of the fifth-century Athenians from whom Herodotus heard them.7 Herodotus' narratives about early Athenian history, therefore, can be used as a source for widespread, fifth-century Athenian political beliefs and values, as well as partisan family traditions.8In the first part of this article, I argue that comparison of Herodotus' use of the verb in his Athenian narratives with the occurrence of this word in Athenian dramatic and historical texts reveals that this was a key term in the Athenians' understanding of the value of democracy. Specifically, I argue that the word was used by the Athenians to describe the forceful subjection of a people by a tyrant and was associated with Athenian ideas about the weakness of societies ruled by [End Page 331] a tyrant in contrast to the strength of free societies with a democratic political system. In the second half of this article, I argue that the theme of tyranny/civic weakness versus democracy/civic strength underlies Herodotus' presentation of some non-Athenian narratives. This article examines a dramatic story about a slave rebellion on the island of Chios and argues that such stories were the medium through which masters and slaves negotiated compromise between the slaves' desire to be free and the masters' desire to control their slaves. This interpretation arises from a recognition of the complexity of a story that on the one hand celebrates the triumphs of a heroic slave over his masters, but on the other hand suggests that such resistance (...) can be contained if masters treat their slaves humanely. Numerous parallels are drawn with contemporaneous and later historical societies involving compromises between subordinate groups and those who would control them. Finally, the article demonstrates that religious cults could be a medium for negotiating a middle ground, and that the story of the slave hero on Chios illustrates this function by making the slave the object of cultic worship by both masters and slaves. In this paper we argue that while individual private interests such as autonomy and the need for a medical procedure or treatment are important in the provision and delivery of health care and the utilization of biotechnologies, these concepts need to be balanced with other interests such that in certain situations they do not take priority. We use as an example a particular developing biotechnology, xenotransplantation, to suggest that interest in the health of the public is such that this biotechnology (...) should not be permitted to move to the clinical trial stage because of the particular risk of harm it poses to the potential xeno-recipient, their close contacts and the wider population. This is despite the interest of those in need of a transplant in allowing such clinical trials to proceed. We derive support for our position from John Stuart Mill's harm principle. (shrink)

Background: Caring for a partner with primary malignant brain tumour can be a dramatic life-changing event. Primary malignant brain tumour is known to give poor life expectancy and severe neurological and cognitive symptoms, such as changed behaviour and personality, which demand greater caring responsibilities from spouses. Aim: The aim of the study is to explore ethical dilemmas spouses experience in the everyday care of a partner in treatment for primary malignant brain tumour. Research design, participants and research context: A phenomenological (...) and hermeneutic qualitative descriptive design was adopted as a method for collecting and analysing data. Ten spouses were interviewed twice using an in-depth, semi-structured interview guide. The interviews took place at the spouses’ homes or at the hospital. Ethical consideration: Ethical matters were considered throughout the research process. Permission from The National Committee on Health Research Ethics and the Danish Data Protection Agency was obtained. Findings: The analysis showed that the spouses perceived daily ethical dilemmas in caring for a partner with primary malignant brain tumour. Their life as well as their partner’s life had changed considerably. The main theme that emerged therefore was ‘oscillating in a changing relationship’. This theme was further elaborated in three subthemes that in more detail demonstrated the dilemmas: ‘doing the right thing in unpredictable daily situations’; ‘torn between patience and guilt’; and ‘living in a time of uncertainty, hope and despair’. Conclusion: Caring for a partner with changed behaviour and personality due to primary malignant brain tumour may involve exhausting ethical caring dilemmas. Spouses’ married life may change to a semi-professional asymmetrical relationship, which is challenged by the oscillation between acting responsibly for their partners’ well-being and caring dilemmas with no answer for what the right thing to do is. Mixed feelings of right and wrong, patience and guilt, hope and despair seem to be spousal companions through their partners’ progressing illness. (shrink)
