The writers respond to the commentary “Physician Burnout Calls for Legal Intervention,” by Sharona Hoffman, in the November‐December 2019 issue of the Hastings Center Report.

No writer or public intellectual of our era has been as sensitive to the role of faith in the lives of ordinary Americans as Robert Coles. Though not religious in the conventional sense, Coles is unparalleled in his astute understanding and respect for the relationship between secular life and sacredness, which cuts across his large body of work. After 40 years of attributing high rates of physician career dissatisfaction, attrition, alcoholism, divorce and suicide to ‘burnout’, there is growing recognition that these outcomes may instead be caused by moral injury. This has led to a debate about the relative diagnostic merits of these two terms, a recognition that interventions designed to treat burnout may be ineffective, and much perplexity about how—if at all—this changes anything. The current research seeks to develop the construct of moral injury outside military contexts, (...) generate more robust validity tests and more fully describe and measure the experiences of persons exposed to moral harms. Absent from the literature is a mechanism through which to move from the collective moral injury experience of physicians to a systematic change in the structure of medical practice. To address this, after providing a brief history, definitions and contrasts between burnout, moral distress and moral injury, we review the interplay of moral and ethical codes in the context of moral injury. We conclude by suggesting that professional associations can potentially prevent moral injury by providing protections for physicians within their codes of ethics. There are no data in this work. I propose a modest Bayesian reductionism as an alternative to Quine's moderate confirmational holism. Employing only first-order predicate logic with identity and elementary probability theory, I present two models of confirmation for individual hypotheses within blocks of theoretical sentences. Testing these models, I consider: (1) the old evidence problem; (2) the raven paradox; (3) a version of the the thesis of underdetermination which says that the evidence never provides adequate epistemic grounds for deciding between rival theories; and (4) the confirmation (...) of modal conditionals. Introduction: There is significant overlap in the type of structural language impairments exhibited by children with autism spectrum disorder and children with attention deficit hyperactivity disorder. This similarity suggests that the cognitive impairment contributing to the structural language deficits in ASD and ADHD may be shared. Previous studies have speculated that procedural memory deficits may be the shared cognitive impairment. The procedural deficit hypothesis argues that language deficits can be explained by differences in the neural structures underlying the procedural memory (...) network. This hypothesis is based on the premise that the neural structures comprising the procedural network support language learning. In this study, we aimed to test the PDH in children with ASD, ADHD, and typical development.Methods: One hundred and sixty-three participants : 91 with ASD, 26 with ADHD, and 46 with TD, completed standardized measures of cognitive and language ability as well as structural magnetic resonance imaging. We compared the structural language abilities, the neural structures underlying the procedural memory network, and the relationship between structural language and neural structure across diagnostic groups.Results: Our analyses revealed that while the structural language abilities differed across ASD, ADHD, and TD groups, the thickness, area, and volume of the structures supporting the procedural memory network were not significantly different between diagnostic groups. Also, several neural structures were associated with structural language abilities across diagnostic groups. Only two of these structures, the inferior frontal gyrus, and the left superior parietal gyrus, are known to be linked to the procedural memory network.Conclusions: The inferior frontal gyrus and the left superior parietal gyrus, have well-established roles in language learning independent of their role as part of the procedural memory system. Other structures such as the caudate and cerebellum, with critical roles in the procedural memory network, were not associated with structural language abilities across diagnostic groups. It is unclear whether the procedural memory network plays a fundamental role in language learning in ASD, ADHD, and TD. (shrink)

In order to solve problems, humans are able to synthesize apparently unrelated concepts, take advantage of serendipitous opportunities, hypothesize, invent, and engage in other similarly abstract and creative activities, primarily through the use of their visual systems. In _Scenario Visualization_, Robert Arp offers an evolutionary account of the unique human ability to solve nonroutine vision-related problems. He argues that by the close of the Pleistocene epoch, humans evolved a conscious creative problem-solving capacity, which he terms scenario visualization, that enabled (...) them to outlive other hominid species and populate the planet. Arp shows that the evidence for scenario visualization -- by which images are selected, integrated, and then transformed and projected into visual scenarios -- can be found in the kinds of complex tools our hominid ancestors invented in order to survive in the ever-changing environments of the Pleistocene world. Arp also argues that this conscious capacity shares an analogous affinity with neurobiological processes of selectivity and integration in the visual system, and that similar processes can be found in the activities of organisms in general. The evolution of these processes, he writes, helps account for the modern-day conscious ability of humans to use visual information to solve nonroutine problems creatively in their environments. Arp's account of scenario visualization and its emergence in evolutionary history suggests an answer to two basic questions asked by philosophers and biologists concerning human nature: why we are unique; and how we got that way. Robert Arp is Postdoctoral Research Fellow at the National Center for Biomedical Ontology. His areas of specialization include philosophy of biology and philosophy of mind. To mark the book’s fiftieth anniversary, Jonathan Day revisits this pivotal work and contributes a thoughtful and revealing critical commentary.

Michel Foucault’s “Standing Vigil for the Day to Come” was a review of Roger Laporte’s novel, La Veille, published by Gallimard earlier that year. Although Laporte’s work never received the wide readership it deserved, Foucault held it in high esteem, praising it in his assessment as one of the “most original” and “most difficult” of his time and, subsequently, urging Derrida to read it. This article is most appropriately situated in the series of literary reviews Foucault composed between 1961 and (...) 1966, in which his marked attempts to understand the relationship between language and thought drew him to the works of Roussel, Klossowski, Hölderlin, Mallarmé, and, of course, Laporte. Foucault finds Laporte’s treatment of the subject-matter particularly satisfying because it provides a non-reductive account of thought and its relationship to language; thought is neither identical with nor distinct from language. Foucault sees Laporte as relying on an important Nietzschean insight that thought is both too funda- mental and too archaic to be reduced to philosophy or to require a Cartesian ego. In this way La Veille is naturally of interest to Foucault because it deals with the relationship of a writer to an anonymous other; it is this other — not the writer — that makes writing possible. With the role of the subject de-emphasized, Foucault finds in Laporte a starting point for talking about language in contemporary literature and thought in post- Cartesian philosophy. (shrink)

The concept of brain death was recently described as being “at once well settled and persistently unresolved.” Every day, in the United States and around the world, physicians diagnose patients as brain dead, and then proceed to transplant organs from these patients into others in need. Yet as well settled as this practice has become, brain death continues to be the focus of controversy, with two journals in bioethics dedicating major sections to the topic within the last two years.By way (...) of background, the Uniform Determination of Death Act states that “[a]n individual who has sustained either: irreversible cessation of circulatory and respiratory functions, or irreversible cessation of all functions of the entire brain, including the brain stem, is dead. A determination of death must be made in accordance with accepted medical standards.” In other words, death can be defined by either cardiorespiratory or neurological criteria, with “brain death” representing the loss of all brain function. This standard, or closely related variants, has become the accepted approach throughout the United States and in many parts of the world. (shrink)

A major philosophical mind in his day, William Paley wrote in a lucid style that made complex ideas more accessible to a wide readership. This work, first published in 1785, was based on the lectures he gave on moral philosophy at Christ's College, Cambridge. Cited in parliamentary debates and remaining on the syllabus at Cambridge into the twentieth century, it stands as one of the most influential texts to emerge from the Enlightenment period in Britain. 1. We are unknown to ourselves, we knowing ones, we to our own selves, and for a good reason. We have never sought ourselves – so how could it happen, that one day we would find ourselves? Someone once correctly said: “where your treasure is, there your heart will be also”;1 our treasure is where the beehives of our knowledge are. We are always on the way to finding it; as winged creatures and honey-gatherers of the spirit, we truly care (...) for only one thing from the heart: to “bring something home.” Whatever else concerns life, the so-called “experiences” – who among us has enough seriousness for them? Or enough time? In these matters we have been, I fear, somewhat distracted: our hearts have not quite been in it, and not even our ears! More often, like some divine scatter-brain absorbed-in-himself into whose ear the bell has tolled the twelve strikes of noon at full power, who then suddenly wakes up and asks himself, “What was really just struck?”, just like that we rub our ears afterwards and ask, completely amazed, completely embarrassed, “what did we really experience?” and further, “who are we really?” and afterwards, as said, count the twelve trembling bell-strikes of our experience, our life, our being – ugh! and miscount them . . . We necessarily remain strangers to ourselves, we do not understand ourselves, we must get ourselves wrong, for us the saying “Each is furthest from himself” holds for all eternity – we are not “knowing ones” with respect to ourselves. Understanding cooperation and punishment in small-scale societies is crucial for explaining the origins of human cooperation. We studied warfare among the Turkana, a politically uncentralized, egalitarian, nomadic pastoral society in East Africa. Based on a representative sample of 88 recent raids, we show that the Turkana sustain costly cooperation in combat at a remarkably large scale, at least in part, through punishment of free-riders. Raiding parties comprised several hundred warriors and participants are not kin or day-to-day interactants. Warriors incur substantial (...) risk of death and produce collective benefits. “If good design tells the truth,” writes Robert Grudin in this path-breaking book on esthetics and authority, “poor design tells a lie, a lie usually related... to the getting or abusing of power.” From the ornate cathedrals of Renaissance Europe to the much-maligned Ford Edsel of the late 1950s, all products of human design communicate much more than their mere intended functions. In the article, I demonstrate how realistic philosophy of religion can be employed in order to obtain a preliminary verification of the truthfulness of the doctrine proclaimed by a particular religious community. The first element of a religious doctrine that qualifies for philosophical evaluation is its non-contradictory character. For this reason I endeavour to reconstruct one such doctrine and show how it is possible to demonstrate, through philosophical analyses, that such a doctrine does not meet the aforementioned criterion. For the (...) object of my studies I chose the Church of Jesus Christ of Latter-day Saints (called the Mormon Church for short), as I find it particularly interesting among the religious movements alternative to Christianity. Among new religious movements currently in existence the Mormon Church stands out in several respects. First, this religious community utilises a variety of sources of revelation. Furthermore, although their sacred story, that spanning a dozen or so centuries, begins in Israel it mostly takes place on American soil. Finally, the Mormon Church sets forth a vision of a future ‘holy land,’ and the concept of salvation that is essentially different from those of other Bible-based religious movements. The Mormon concept of salvation, referred to as the plan of eternal progress shows both unique goal and unique means to achieve it. People are to become gods and populate multiple worlds. According to Mormons there will be different levels of salvation associated with what life a person lived on earth. Reconstruction and philosophical analysis of the main elements of the Mormon doctrine shows its weak points, inconsistencies, and oversimplifications. From this perspective, the Mormon ‘sacred story’ hardly could be regarded as something more than a colourful and interesting myth. But the question of reasons why Mormons still put their faith in such an inconsistent doctrine lies outside the scope of these considerations. (shrink)
