Background: Treating medication-refractory freezing of gait in Parkinson’s disease remains challenging despite several trials reporting improvements in motor symptoms using subthalamic nucleus or globus pallidus internus deep brain stimulation. Pedunculopontine nucleus region DBS has been used for medication-refractory FoG, with mixed findings. FoG, as a paroxysmal phenomenon, provides an ideal framework for the possibility of closed-loop DBS.Methods: In this clinical trial, five subjects with medication-refractory FoG underwent bilateral GPi DBS implantation to address levodopa-responsive PD symptoms with open-loop stimulation. Additionally, PPN (...) DBS leads were implanted for CL-DBS to treat FoG. The primary outcome of the study was a 40% improvement in medication-refractory FoG in 60% of subjects at 6 months when “on” PPN CL-DBS. Secondary outcomes included device feasibility to gauge the recruitment potential of this four-lead DBS approach for a potentially larger clinical trial. Safety was judged based on adverse events and explantation rate.Findings: The feasibility of this approach was demonstrated as we recruited five subjects with both “on” and “off” medication freezing. The safety for this population of patients receiving four DBS leads was suboptimal and associated with a high explantation rate of 40%. The primary clinical outcome in three of the five subjects was achieved at 6 months. However, the group analysis of the primary clinical outcome did not reveal any benefit.Interpretation: This study of a human PPN CL-DBS trial in medication-refractory FoG showed feasibility in recruitment, suboptimal safety, and a heterogeneous clinical effect in FoG outcomes. Principles of Philosophy. Author’s Letter to the Translator (Preface).

The blind spot of philosophy: to live a life Age-old philosophical texts, from Plato to Montaigne or from Lao-tse to Rousseau, are still being read by many. What makes these texts so attractive and ‘contemporary’ today, other than scientific texts from long ago? Most of the time these texts do not address empirical or conceptual questions, but address the problem to understand ourselves as creatures that try to live a life. Academic philosophy has left this question more and more aside, (...) in an ongoing process of ‘scientification’, and concentrated more on conceptual and epistemological questions or on the reconstruction of the philosophical tradition. Yet, in the margins of academic philosophy, in neighboring fields of research and in the public sphere, we can discern examples of a philosophical practice that rephrases the classical philosophical question in a new manner. In this essay an attempt is made to draw the contours of this new practice of philosophy and the writings it produces. Starting with (among others) Nietzsche and Benjamin, and inspired by recent lectures by Norman Lear, a portrait is created of an author who operates somewhere on the borders between science and literature, as a placeholder – or more precise: as an ethnographer who, more than the average scholar, is closely involved with the object of his research, and who, more than the average novelist, addresses ‘the weird problem of having a life’ in a direct way, starting with the problematic and problematizing experiences of others. This philosophical attitude comes close to what Michel Foucault called ‘an ontology of the present’ that writes about the question of who or what we are here and now from a ironical involvement with everyday problematizations of our cultural, political or personal identity. The essay ends with some concrete examples of this philosophical practice and with some suggestions for a philosophical institute in which such a practice could be a central concern. (shrink)

These two 1985 volumes provide a translation of the philosophical works of Descartes, based on the best available Latin and French texts. They are intended to replace the only reasonably comprehensive selection of his works in English, by Haldane and Ross, first published in 1911. All the works included in that edition are translated here, together with a number of additional texts crucial for an understanding of Cartesian philosophy, including important material from Descartes' scientific writings. The result should meet the (...) widespread demand for an accurate and authoritative edition of Descartes' philosophical writings in clear and readable modern English. (shrink)

Since the UN Guiding Principles on Business and Human Rights were adopted by the UN Human Rights Council in 2011, they have diffused into policy frameworks, laws, and regulations across the globe. This special issue seeks to advance the interdisciplinary field of human rights research by examining key elements of the emerging transnational regime for the regulation of business and human rights. In seven original contributions, scholars from political science, law, accounting, and philosophy critically reflect on the theoretical foundations of (...) the UNGPs, they analyze the effectiveness of implementation mechanisms and current regulatory practice, and they advance proposals for the future development of the business and human rights regime. In this introduction, we prepare the ground for these analyses, proceeding in three steps. Firstly, we argue that the adoption of the UNGPs has triggered a norm cascade which requires a distinctive, empirically oriented research agenda focusing on the scope, governance, and effectiveness of corporate human rights accountability norms and instruments. Secondly, we explain how the articles in this special issue contribute to that research agenda by addressing these themes. A controversy among ancient geographers about the localisation of the Argonauts’ quest for the golden fleece has deeper methodological implications because it reflects divergent views on how to fill gaps in the account that serves as one’s source. Reconstructing the controversy step by step, the article aims to bring to light these methodological implications. In so doing, it also demonstrates that the extant editions unjustifiably curtail the text of the relevant fragment from Demetrius of Scepsis. Current perspectives on attractiveness-related prosocial biases emphasize the contribution of evolutionarily shaped mating drives. Here, we extend these concepts by highlighting the pivotal role of the hypothalamic peptide oxytocin in augmenting the salience and rewarding value of social stimuli, including the partner's face, thereby fostering social bonding in general and the stability of monogamous pair bonds and offspring care in particular.

Frequently, rationally less justifiable arguments and often so called implicit assumptions (which are not explicitly clarified) regarding our conceptions of reality are perhaps an underestimated but crucial factor causing controversial discussions between the representatives of equal and different disciplines. The essay conceptualizes the separation of academic disciplines from a new theoretical viewpoint by looking at the fundamental structures of reality on the level of the roots, not the fruits. Basic assumptions and deeply rooted convictions and conceptions of reality, theories and (...) models in quantum physics, consciousness studies and art may thereby be regarded as a cause for communication problems. Can a wider scope of how to define the foundation of science support the concept of a necessary initiation of a movement towards a more expansive and deliberate synchronization of the separate disciplines into new synthetic combinations of knowledge? The essay speculatively concludes with some central points of how new modes of knowledge production may be envisioned. It includes a perspective on the origin of knowledge with a wider scope that goes beyond the conventional frameworks of scientific and artistic exploration. (shrink)

The theologian Hugo Rahner argued that the homo ludens is a man of ‘Ernstheiterkeit’, a person who can smile under tears but also recognizes the gravity in all earthly cheerfulness. The primary aim of this study was to test the validity of this notion: Do homines ludentes exist? Two hundred sixty-three adult subjects were measured for seriousness and cheerfulness and playfulness. Results provided unequivocal support for Rahner's thesis. Numerous subjects scored high in both seriousness and cheerfulness thus confirming the existence (...) of homines ludentes. It was further found that these subjects were among those scoring highest in playfulness. Subjects scoring high in cheerfulness but low in seriousness were, however, even more closely associated with playfulness. The scores for homines ludentes did not differ from the rest of the subjects in the creative and dynamic facets of playfulness. The importance of developing empirical research to investigate hypotheses derived from non-scientific concepts is discussed. (shrink)

In one of his admirable letters to Princess Elizabeth, Descartes asserts that the best way to overcome the annoyances of life is to “divert one's imagination and one's senses away from them as much as possible and to make use merely of one's understanding in dealing with them.” This advice is not easy to follow, and one of the devil's principal tricks is to identify himself so profoundly with our intimate concerns that, in disowning him, we come to believe that (...) we disown ourselves. It sometimes seems that man cares more for his misery than for his pleasure.But whether easy or not to follow, the advice is good. To convert the evil which oppresses you, insofar as possible, into a knot to untie, a safety lock to take apart, a problem to resolve, is to detach it from yourself; it is no longer me, it is mine. I can censure it and perhaps even pin it down. When one can state the evil, then what remains is only half-evil. (shrink)

The (...) Discourse on The Method is best known as the source of the famous quotation "Je pense, donc je suis" ("I think, therefore I am"), which occurs in Part IV of the work. Good sense is, of all things among men, the most equally distributed; for every one thinks himself so abundantly provided with it, that those even who are the most difficult to satisfy in everything else, do not usually desire a larger measure of this quality than they already possess. And in this it is not likely that all are mistaken the conviction is rather to be held as testifying that the power of judging aright and of distinguishing truth from error, which is properly what is called good sense or reason, is by nature equal in all men; and that the diversity of our opinions, consequently, does not arise from some being endowed with a larger share of reason than others, but solely from this, that we conduct our thoughts along different ways, and do not fix our attention on the same objects. For to be possessed of a vigorous mind is not enough; the prime requisite is rightly to apply it. The greatest minds, as they are capable of the highest excellences, are open likewise to the greatest aberrations; and those who travel very slowly may yet make far greater progress, provided they keep always to the straight road, than those who, while they run, forsake it. The anonymous author of the Syriac Liber Graduum never mentions his theological opponents. The article analyses a few examples taken from his biblical exegesis and from his most typical theological concepts and shows that these peculiar features are better explained as a hidden polemic against Marcionism, thus casting new light on the nature of the Liber Graduum and providing new data for the study of Syriac Marcionism.

Hydrogen is troublesome in any periodic table classification. This being so it may as well be placed in a position that confers desirable attributes to the arrangement of the elements, while notionally recognising its lineage to the group 1 alkali metals and the group 17 halogens. Since the noble gases bridge the halogens and the alkali metals, and hydrogen encompasses the transition from the alkali metals to the halogens, there is more to the idea of hydrogen over helium.
