Indexicality, the ability of language to evoke the context in which it usually occurs, is a concept commonly drawn upon in sociolinguistics and linguistic anthropology. This article applies the framework of orders of indexicality to political discourse about a controversial topic in Pittsburgh, United States, the construction of bike lanes. A concordance analysis of the term bike lanes in news media, blogs and online news comments helps to explain the variation in the indexical meanings of bike lanes between those who (...) oppose and those who support bike lanes. We argue that the orders of indexicality approach helps explain how groups with different interests can reinforce or challenge the possible discursive associations of a loaded political term. Finally, it examines the aims of wildlife conservation with a view to whether they are compatible with de-extinction, or not. -/- CHAPTER 2 examines three prime candidates for de-extinction—namely, the aurochs, the (...) woolly mammoth, and the passenger pigeon. It is about what these animals were like, why people want to resurrect them, and the methods by which their resurrections could be accomplished. -/- CHAPTER 3 is about the authenticity of de-extinct animals. Critics of de-extinction have offered many reasons for thinking that the products of de-extinction will be inauthentic. The bulk of the chapter is taken up with surveying their arguments. We attempt to show that none are convincing, and end the chapter by offering and defending two arguments in favour of the view that authentic de-extinctions are possible. -/- CHAPTER 4 surveys and critically evaluates all the main arguments both for and against de-extinction. BackgroundCommunity engagement within biomedical research is broadly defined as a collaborative relationship between a research team and a group of individuals targeted for research. A Community Advisory Board is one mechanism of engaging the community. Within genomics research CABs may be particularly relevant due to the potential implications of research findings drawn from individual participants on the larger communities they represent. Within such research, CABs seek to meet instrumental goals such as protecting research participants and their community from research-related risks, (...) as well as intrinsic goals such as promoting the respect of participants and their community. However, successful community engagement depends on the degree to which CABs legitimately represent and engage with communities targeted for research. Currently, there is little literature describing the use of CABs in genomics research taking place in developing countries, and even less in the field of genomics research relating to mental illness. The aim of this article is to describe and consider the contributions made by a researcher-driven, population-specific CAB in a genomics of schizophrenia research project taking place in South Africa, from the perspective of the research team.DiscussionFour broad discussion topics emerged during the CAB meetings namely: 1) informed consent procedures, 2) recruitment strategies, 3) patient illness beliefs and stigma experiences, and 4) specific ethical concerns relating to the project. The authors consider these discussions in terms of their contributions to instrumental and intrinsic goals of community engagement.SummaryThe CAB gave valuable input on the consent processes and materials, recruitment strategies and suggested ways of minimizing the potential for stigma and discrimination. All of these contributions were of an instrumental nature, and helped improve the way in which the research took place. In addition, and perhaps more importantly, the CAB made a unique and important contribution relating to intrinsic functions such as promoting the respect and dignity of research participants and their community. This was particularly evident in ensuring sensitivity and respect of the community’s traditional beliefs about schizophrenia and its treatment, and in this way promoting a respectful relationship between the research team and the participants. This volume examines the nature and significance of transformative experiences as they occur across a variety of contexts in human life. By treating these events as social as well as individual phenomena, the essays bring to light the various ways in which cultural and institutional forces influence narratives of personal change. The ease with which we identify transformative experiences shows their importance for our sense of the potentialities inherent in human life, even while their disruptive character threatens confidence in our (...) capability to make rational decisions concerning our future well-being. Yet, narratives of transformation are not just individual artefacts, but are also given support and structure through social forces including shared languages, practices, and institutions. What are the cultural and institutional contexts which enable this form of self-conceptualisation, and what happens when social changes undermine the cogency of these narratives? The chapters in this volume investigate these issues through a blend of philosophical theory and applied cases, working across the disciplinary boundaries of philosophy and social anthropology. Contributors investigate topics including recovery from trauma; the role of narratives in gender transition; climate activism; the ethical ramifications of war; the role of media in framing narratives of ethical change; and the university as a site of transformative experience. Cell line immortalisation is a growing component of African genomics research and biobanking. However, little is known about the factors influencing consent to cell line creation and immortalisation in African research settings. We contribute to addressing this gap by exploring three questions in a sample of Xhosa participants recruited for a South African psychiatric genomics study: First, what proportion of participants consented to cell line storage? Second, what were predictors of this consent? Third, what questions were raised by participants during (...) this consent process? 760 Xhose people with schizophrenia and 760 controls were matched to sex, age, level of education and recruitment region. We used descriptive statistics to determine the proportion of participants who consented to cell line creation and immortalization. Logistic regression methods were used to examine the predictors of consent. Reflections from study recruiters were elicited and discussed to identify key questions raised by participants about consent. Approximately 40% of participants consented to cell line storage. The recruiter who sought consent was a strong predictor of participant’s consent. Participants recruited from the South African Eastern Cape, and older participants, were more likely to consent; both these groups were more likely to hold traditional Xhosa values. Neither illness nor education were significant predictors of consent. Key questions raised by participants included two broad themes: clarification of what cell immortalisation means, and issues around individual and community benefit. These findings provide guidance on the proportion of participants likely to consent to cell line immortalisation in genomics research in Africa, and reinforce the important and influential role that study recruiters play during seeking of this consent. Our results reinforce the cultural and contextual factors underpinning consent choices, particularly around sharing and reciprocity. Finally, these results provide support for the growing literature challenging the stigmatizing perception that people with severe mental illness are overly vulnerable as a target group for heath research and specifically genomics studies. Ludwig Wittgenstein’s relation to Gestalt Psychology is typically understood through the lens of his engagement with Wolfgang Köhler’s work. Though valuable, this framing may obscure certain broader points of similarity between Wittgenstein’s methodological approach and the Gestalt school considered as a whole. In what follows I pursue this line of thought by comparing Wittgenstein’s discussion of rule-following in the Philosophical Investigations with Max Wertheimer’s concept of productive thinking. I argue that Wertheimer’s emphasis on the shared and public aspects of Gestalt (...) phenomena counteracts Köhler’s tendency to characterise Gestalts as essentially private entities. I suggest that in this respect at least, Wertheimer’s approach is closer than Köhler’s to Wittgenstein’s later philosophical orientation. (shrink)

Since 1980, the number of twin births in the United States has increased 76 percent, and the number of triplets or higher‐order multiples has increased over 400 percent. These increases are due in part to increased maternal age, which is associated with spontaneous twinning. But the primary reason for these increases is that more and more people are undergoing fertility treatment. Despite an emerging (but not absolute) consensus in the medical literature that multiples, including twins, should be a far less (...) frequent outcome of fertility treatment, American clinicians currently practice fertility medicine in ways likely to result in twins and, occasionally, triplets—often, they tell us, at the request of their patients. At first blush, one might conclude that these practice patterns show that patients and their doctors have weighed the risks associated with twins against the benefits of swiftly completing their families and decided that the risks are worth taking. That is, their requests for twins must represent their free and informed choices. But there are reasons to be skeptical of this interpretation, including that the preferences of many patients seem to be very strongly shaped, if not sometimes completely constrained, by concerns about costs—costs that mainly result from the routine exclusion of in vitro fertilization from health insurance coverage. It is quite possible, we believe, that these cost concerns actually undermine the autonomy of fertility patients, pushing many of them to take risks with their health and the health of their hoped‐for future children. Descartes’s Meditations on First Philosophy, regarded by many as his masterpiece, has been the subject of significant philosophical debate since its publication in 1641. Yet the Meditations is remarkable not only for its philosophical ideas but also for the style in which it was written. Two of the most notable stylistic elements of the Meditations are the use of temporal markers—a significant departure from analogous philosophical treatises of the same period—and the fact that the text is written in such a (...) way as to invite readers to subsume themselves into the role of the narrator, so as to experience its arguments for themselves. Many commentators have hinted at the importance of the narrator. But there has been little attempt at a sustained engagement. The function of the text as a series of days of meditation has also been insufficiently explored. In order to further investigate the roles of time and narrative within the Meditations, this thesis uses various reading methods provided by narrative theory, with particular focus on Monika Fludernik’s experiential model of narrative. Fludernik’s model allows for a clearer articulation of the role readers play in enacting meaning, and the way in which readers will in a sense “author” a text on their own terms. Reading the Meditations as an experiential narrative also illuminates significant issues to do with Descartes’s distinction between geometric and discursive argument, his conception of time, the specific expression of the Cogito in the Meditations, and the “reorientation” at the heart of the text—which I am calling a sort of conversion. The conversion at the heart of the Meditations will be explored in parallel with Saint Augustine’s Confessions and Saint Ignatius of Loyola’s Spiritual Exercises, two texts which—like the Meditations—can be thought of as experiential narratives designed to bring about some kind of conversion. I argue that such an experiential reading, drawing on the roles of time and narrative in the text, offers to enrich our understanding of the Meditations. (shrink)

This article explores Descartes's philosophy of time as articulated in the 'non-endurance doctrine' in the Third Meditation. I argue that what has been overlooked in prior readings of this doctrine is acknowledgement that it is articulated by a fictional character within a fictional span of time. The text, therefore, expresses a temporal reality, as well as articulating a temporal theory. I explore the temporality of the text, and the fictionality of the narrator, through the lens of Monika Fludernik's theory of (...) experiential narrative. (shrink)

Drug Safety Communications are used by the Food and Drug Administration to inform health care providers, patients, caregivers, and the general public about safety issues related to FDA-approved drugs. To assess patient knowledge of the messaging contained in DSCs related to the sleep aids zolpidem and eszopiclone, we conducted a large, cross-sectional patient survey of 1,982 commercially insured patients selected by stratified random sampling from the Optum Research Database who had filled at least two prescriptions for either zolpidem or eszopiclone (...) between July 1, 2012 and June 30, 2013. Among the 594 respondents, two-thirds reported hearing generally about drug safety information prior to starting a new drug, with the remaining one-third “rarely” or “never” hearing such information. Providers and pharmacists were primary sources of drug safety information. Two-thirds of zolpidem users and half of eszopiclone users reported having heard about the related DSC messages, ability to accurately identify the major factual messages was limited. Respondents reacted to new drug safety information about their sleep aids by reporting that they would want to learn about alternative ways to help them sleep and seek out more information about the safety of their specific sleeping pill. Opportunities may exist for the FDA to work with providers and pharmacies to help ensure the DSC information is more widely received and is more fully understood by those taking the affected medications. In this chapter, I consider the prospects for deploying the concept of public reason in settling practical bioethical questions, focusing in particular on entitlements to healthcare. I begin by tracing the origins of the concept of public reason to the aspirations of the liberal political theorist to find a justification for the authority of government, which reconciles a basic belief in the autonomy of the individual with the legitimacy of the coercive institutions that create and govern the public sphere. I (...) then consider how the concept of public reason may be used in order to justify a universal entitlement to healthcare, paying particular attention to the work of Norman Daniels. Finally, I briefly consider the objection that an entitlement to healthcare grounded on such terms is insufficiently robust, because it downplays the values that healthcare encodes. I conclude by reflecting on whether the concept of public reason can be stretched to include a more robust conception of the value of health. (shrink)

This book explores and elaborates three theories of public reason, drawn from Rawlsian political liberalism, natural law theory, and Confucianism. In a recent article, Matej Cíbik claims that some tattoos are immoral because they are imprudent. (Cíbik 2020) In response, I argue that a tattoo can only be immoral if it causes harm to a third party, so that no tattoo is immoral simply because it is reckless. Conflating prudential and moral requirements in the way that Cíbik does would strike at the heart of liberalism, and has deeply counter-intuitive consequences, as we can see when we consider Cíbik's own discussions (...) of suicidal individuals and smokers. After discussing the role of the self/other distinction in liberal moral philosophy, I affirm both the moral neutrality of tattoos and the importance of adopting non-judgemental attitudes towards the choices which a person makes concerning her own body. The concepts of time and identity seem at once unproblematic and frustratingly difficult. Time is an intricate part of our experience -- it would seem that the passage of time is a prerequisite for having any experience at all -- and yet recalcitrant questions about time remain. Is time real? Does time flow? Do past and future moments exist? Philosophers face similarly stubborn questions about identity, particularly about the persistence of identical entities through change. Indeed, questions about the metaphysics of (...) persistence take on many of the complexities inherent in philosophical considerations of time.
