Situations in which patients lack medical decision-making (MDM) capacity raise ethical challenges, especially when the patients decline care that their surrogate decision makers and/or clinicians agree is indicated. These patients are a vulnerable population and should receive treatment that is the standard of care, in line with their the values of their authentic self, just as any other patient would. But forcing treatment on patients who refuse it, even though they lack capacity, carries medical and psychological risks to the patients (...) and the hospital staff. It is also often impractical to force some treatments, especially in the long term. For example, independent of the ethical “should” question, how would one force hemodialysis for the rest of a patient’s life, or force a surgery that requires weeks of post-operative physical therapy? In this article we present a novel algorithm that can help clinicians with ethical and practical decision making, with the goals of achieving the best outcomes for patients and reducing moral distress for their caretakers and clinicians. Regis College Toronto, Ontario HETHER ONE IS rethinking the content of a course and the place of one's study in the context of a broader curriculum, or whether there is a moment of fundamental questioning which grows out of rereading a classic text or engaging in contemporary debates, such moments of questioning are essential. This (...) essay proposes one of those simple and general questions: in what does the work of the philosophy of religion consist? My consideration of the question, as will become evident, is an exercise in prolegomena, and after this brief introduction there will be two parts to my essay in which the context for a full answer to the question will be explored, before a summary conc1usion will propose an agenda for ful1ther work. Though concept clarification is essential to philosophical reflection, attention to historical development is of equal importance. The two inner parts of this essay will give attention to both, first by discussing a specific texrt and,author, and then by sorting out some fundamental matters of definition. My choice of a classic text and context is David Hume's Dialogues Concerning Natural Religion.1 Such a choice is not to imply that the philosophy of religion begins there or that Hume's notion of either philosophy or religion is: somehow normative. However, I wish to propose a reading of Hume's text which can be very helpful not only to understand what he is about but also to encourage a rethinking of what the philosophy of religion should be about. Simply as a text, Hume's Dialogues is intriguing. He withheld its publication on the advice of seveml of his friends and continued to revise it until his death, altering nearly every page styJistica1ly and substantively changing all hut four sections. :Before he died, he ensured that his nephew David would have the work published and was busy with some of the most lengthy additions to the text in the last months before his death.2 The work and its contents dearly remained a matter of importance and preoccupation for Hurne, and both the contents and form of the text present essentials of Hume's mature thought. The apparent meandering of the text, as a dialogue among the characters Cleanthes, Philo, and Demea, belies its careful construction. Despite Cleanthes' accusation that Philo proposes his notions in a " rambling way," the logic of the text is a masterpiece of exposition, movement, internal reference, and cumulative rhetorical effect. David Livingston has recently discussed the dialect~cal and narrative character of Hume's writing considered in general as a philosophy of "common life." 3 Hume is seen as both the critic of false philosophy and the artificer of the true by means of the application of his principles of the association of ideas, of nrutural beliefs, of sympathy. Forty-five children between the ages of nine and twelve years, who were forced to flee their native Sudan and seek refuge in Egypt, were interviewed about their everyday life in Cairo. Phenomenological analyses of the transcripts revealed the physical, social and technological dimensions to their encounter with a new cultural world. The interviews also revealed the extent to which the children had to face racism, discrimination and social exclusion. Specific analyses of children’s difficulties in learning a new form of Arabic (...) and their involvement in play and games indicated that a refugee child develops his or her self-identity as a stranger by reflecting on particular confrontations with the new environment. Finally, comparative analyses across age groups led to the construction of a phenomenological-developmental model of the child refugee. Both the model and the findings are discussed in the context of Alfred Schutz’s (1964a) essay The Stranger, George Herbert Mead’s (1967) communicative model of the self and Binnie Kristal-Andersson’s (2000) psychological framework for understanding migration. Best known for his influential History of Greece, the historian and politician George Grote wrote this account of Plato's dialogues as a philosophical supplement to the History. First published in 1865, Grote's account of Plato's works includes substantial footnotes and marginalia. With three volumes each running to over six hundred pages, Grote's scholarship is formidably comprehensive. Volume 1, which includes a general preface and extensive introductory material, focuses on Plato's early and transitional dialogues, all of which feature Sokrates. Volume (...) 2 covers the transitional and middle dialogues including Gorgias and Symposium. Volume 3 contains extensive treatments of the Republic and the Laws as well as later dialogues and apocryphal works. It also contains the index to all three volumes. The publication of Plato and the Other Companions of Sokrates confirmed Grote as one of the greatest authorities on Plato in the nineteenth century. (shrink)

The focus on the institutional implications of democratization in postcolonial plural societies invites the following conclusion. While procedural democracy, the general rules for aggregating preferences, is easily defeated, an alternative formulation, proportional representation or consociational democracy is defended. Consociational democracy has explicable reparable flaws and can be brought into coherence. The tools for repair require the embodiment of deliberative principles in the organs of consociational democracy. I argue in my conclusion that this theoretical argument can be utilized to explicate the (...) ethnic conflict in Burundi. (shrink)

The long-running debates between between conservatives and libertarians are vigorous and highly charged, dealing with ideas about the very nature of liberty and morality. Like no other single work, _Freedom and Virtue_ explores what unites and divides the adherents of these two important American traditions—shedding much light on our current political landscape.

As part of an attempt to give a “libertarian” account of some aspects of human agency, the author articulates and defends a modified interpretation of “internalism” which makes coherent the notion of a genuinely, self-determined choice amongst fundamental conceptions of practical reason. That such choices are “nomologically irreducible” is evidenced by the fact that although (contextually) unavoidable, they are nonetheless under-determined with respect to any combination of the agent’s (specific) desires and circumstances. Alternatively, to the extent that orthodox “externalism” subordinates (...) reason to the field of externally determined “passions,” it is rejected, in conclusion, as yielding a naive and excessively reductive analysis of human agency.
