There is a broad consensus in nutritional-microbiota research that high-fat (HF) diets are harmful to human health, at least in part through their modulation of the gut microbiota. However, various studies also support the inherent flexibility of the human gut and our microbiota’s ability to adapt to a variety of food sources, suggesting a more nuanced picture. In this article, we first discuss some problems facing basic translational research and provide a different framework for thinking about diet and gut health (...) in terms of metabolic flexibility. We then offer evidence that well-formulated HF diets, such as ketogenic diets, may provide healthful alternative fuel sources for the human gut. We place this in the context of cancer research, where this concern over HF diets is also expressed, and consider various potential objections concerning the effects of lipopolysaccharides, trimethylamine-N-oxide, and secondary bile acids on human gut health. We end by providing some general suggestions for how to improve research and clinical practice with respect to the gut microbiota when considering the framework of metabolic flexibility. (shrink)

Summary Background Self-binding directives instruct clinicians to overrule treatment refusal during future severe episodes of illness. These directives are promoted as having potential to increase autonomy for individuals with severe episodic mental illness. Although lived experience is central to their creation, service users’ views on self-binding directives have not been investigated substantially. This study aimed to explore whether reasons for endorsement, ambivalence, or rejection given by service users with bipolar disorder can address concerns regarding self-binding directives, decision-making capacity, and human (...) rights. Methods This study used qualitative data from an internet-based survey distributed to the mailing list of the UK charity Bipolar UK, which contained multiple closed and open questions on advance decision-making in bipolar disorder. Quantitative analysis of a closed question about self-binding directives had already demonstrated endorsement amongst a very high proportion of participants with bipolar disorder who completed the survey. We conducted thematic analysis of responses from those participants who answered a subsequent open question about reasons for their view. Research was co-produced within a multi-disciplinary team, with clinical, legal, and ethical expertise, and lived experience of bipolar disorder. Ideas and methodologies associated with all these areas of expertise were used in the analysis of these reasons and to gain insight into the thoughts of individuals with bipolar disorder about self-binding directives and associated issues. Findings Between Oct 23 and Dec 5, 2017, 932 individuals with a self-reported clinical diagnosis of bipolar disorder completed the internet survey, with 565 (154 men; 400 women; 11 transgender or other) providing free text answers to the open question. A large majority of respondents endorsed self-binding directives, nearly all describing a determinate shift to types of distorted thinking and decision-making when unwell as their key justification. Responses indicating ambivalence were dominated by logistical concerns about the drafting and implementation of self-binding directives, while those who rejected self-binding directives also cited logistical concerns, validity of their thinking when unwell, and potential contravention of human rights. Interpretation This study is, to our knowledge, the first large study of reasons why mental health service users might endorse or reject the use of self-binding directives. The findings provide empirical support for introducing self-binding directives into mental health advance decision-making practice and policy and may help to address enduring ethical concerns surrounding possible implementation of the directive while a person retains decision-making capacity. The opinions expressed here in responses given by multiple service users with bipolar disorder challenge a prominent view within international disability rights debates that involuntary treatment and recognition of impaired mental capacity constitute inherent human rights violations. How is it that we think and refer in the first-person way? For most philosophers in the analytic tradition, the problem is essentially this: how two apparently conflicting kinds of properties can be reconciled and united as properties of the same entity. What is special about the first person has to be reconciled with what is ordinary about it. The range of responses reduces to four basic options. The orthodox view is optimistic: there really is a way of reconciling these (...) apparently contradictory properties as contained within the same thing. Lucy Allais presents an original interpretation of Kant's transcendental idealism. She argues that his distinction between things in themselves and things as they appear to us has both epistemological and metaphysical components. Kant is committed to a genuine idealism about things as they appear to us, but this is not a phenomenalist idealism. He is committed to the claim that there is an aspect of reality that grounds mind-dependent spatio-temporal objects, and which we cannot cognize, but he does not (...) assert the existence of distinct non-spatio-temporal objects. On Allais's account, we cannot understand Kant's idealism without a clear account of his notion of intuition and of the role of intuition in cognition: she understands Kantian intuitions as representations that give us acquaintance with the objects of thought. This enables us to make sense of Kant's central argument for his idealism in the Transcendental Aesthetic, and to see why he takes the complete idealist position to be established there. (shrink)

In this paper, we introduce the Japanese philosopher Tetsurō Watsuji’s phenomenology of aidagara (“betweenness”) and use his analysis in the contemporary context of online space. We argue that Watsuji develops a prescient analysis anticipating modern technologically-mediated forms of expression and engagement. More precisely, we show that instead of adopting a traditional phenomenological focus on face-to-face interaction, Watsuji argues that communication technologies — which now include Internet-enabled technologies and spaces — are expressive vehicles enabling new forms of emotional expression, shared experiences, (...) and modes of betweenness that would be otherwise inaccessible. Using Watsuji’s phenomenological analysis, we argue that the Internet is not simply a sophisticated form of communication technology that expresses our subjective spatiality (although it is), but that it actually gives rise to new forms of subjective spatiality itself. We conclude with an exploration of how certain aspects of our online interconnections are hidden from lay users in ways that have significant political and ethical implications. (shrink)

In this paper James’s pluralism is examined in light of his critiques of ‘intellectualism’ and monistic idealism in order to elucidate his relationship to Hegel. Contrary to the strong anti-Hegelianism found throughout the writings of James, Hegel’s dialectic and speculative logic are able to give a rational account of the continuity of objects and relations within experience that James struggled to articulate in A Pluralistic Universe. Neither James nor Hegel is an absolute pluralist or monist due to the interdependence of (...) the concepts of unity and plurality, aptly described by Hegel in his Logic, and alluded to by James in various places throughout his work. Thus, the ambiguity of the nature of James’s pluralism previously noted by scholars is explained, and the relevance of Hegel and dialectic for pragmatist theory is elucidated. (shrink)

Despite its long history of investigating sociality, phenomenology has, to date, said little about online sociality. The phenomenological tradition typically claims that empathy is the fundamental way in which we experience others and their experiences. While empathy is discussed almost exclusively in the context of face-to-face interaction, I claim that we can empathetically perceive others and their experiences in certain online situations. Drawing upon the phenomenological distinction between the physical, objective body and the expressive, lived body, I: (i) highlight that (...) empathy involves perceiving the other’s expressive, lived body, (ii) show that the lived body is not tied to the physical body and that empathy can take place outside of face-to-face interactions, and (iii) argue that the lived body can enter online space and is empathetically available to others there. I explore two ways in which the other’s lived body enters online space and can be empathetically perceived: first, in cases where our face-to-face encounter is technologically-mediated over video link and, second, by showing how the other’s texts, as speech, can form part of the other’s lived body. Investigating empathy online not only furthers our understanding of online encounters but also leads to a refined conception of empathy more generally. (shrink)

ABSTRACT Research groups around the world are developing non‐invasive methods of prenatal genetic diagnosis, in which foetal cells are obtained by maternal blood test. Meanwhile, an increasing number of genetic tests are sold directly to the public. I extrapolate from these developments to consider a scenario in which PNGD self‐testing kits are sold directly to the public. Given the opposition to over‐the‐counter genetic tests and the continuing controversy surrounding PNGD, it is reasonable to expect objections to PNGD self‐testing kits. I (...) focus on one potential objection, that PNGD self‐testing kits would undermine the autonomy of potential test subjects. More specifically, that ‘direct to the public’ PNGD would fail to ensure that consumers exercise autonomy in the following PNGD‐related choices: • Should I use PNGD? • Based on the results of the PNGD test, should I continue or terminate my pregnancy? Under the current system, PNGD is provided by health care practitioners, who are required to counsel women both before and after the test. In contrast, ‘direct to the public’ PNGD would allow women to make their PNGD‐related decisions outside the context of the health care system. I compare these two decision‐making contexts, arguing that the health care system is not unequivocally better at promoting the autonomy of potential test subjects. Therefore the promotion of autonomy does not constitute a strong argument against such test kits. Other objections may be more persuasive, so I do not offer an overall assessment of the acceptability of ‘direct to the public’ PNGD. (shrink)

Like any discipline, bioethics is a developing field of academic inquiry; and recent trends in scholarship have been towards more engagement with empirical research. This ‘empirical turn’ has provoked extensive debate over how such ‘descriptive’ research carried out in the social sciences contributes to the distinctively normative aspect of bioethics. This paper will address this issue by developing a practical research methodology for the inclusion of data from social science studies into ethical deliberation. This methodology will be based on a (...) naturalistic conception of ethical theory that sees practice as informing theory just as theory informs practice – the two are symbiotically related. From this engagement with practice, the ways that such theories need to be extended and developed can be determined. This is a practical methodology for integrating theory and practice that can be used in empirical studies, one that uses ethical theory both to explore the data and to draw normative conclusions. The internet provides us with a multitude of ways of interacting with one another. In discussions about how technological innovations impact and shape our interpersonal interactions, there is a tendency to assume that encountering people online is essentially different to encountering people offline. Yet, individuals report feeling a sense of togetherness with one another online that echoes offline descriptions. I consider how we can understand people’s experiences of being together with others online, at least in certain instances, as arising out (...) of their feeling together as a we. Using Walther’s phenomenological framework of communality, I explore whether the following might take place online: habitual communal experiences and actual we-experiences. While neither of these sketches amount to a full account of how we find ourselves with others online, I suggest that they reveal how insights from the phenomenology of sociality can be used to deepen our understanding of online communality. What is more, I suggest that the strength of this approach is that in some cases it allows us to circumvent tricky questions about embodiment online and, in others, prompts us to ask to what extent a fully-embodied interaction is really required for we-experiences. Anorexia Nervosa (AN) is a complex disorder characterised by self-starvation, an act of self-destruction. It is often described as a disorder marked by paradoxes and, despite extensive research attention, is still not well understood. Much AN research focuses upon the distorted body image that individuals with AN supposedly experience. However, based upon reports from individuals describing their own experience of AN, I argue that their bodily experience is much more complex than this focus might lead us to believe. Such research (...) often presents an overly cognitive understanding of bodily experience in AN, underplaying the affective, felt experience of individuals with AN, as well as descriptions of empowerment, strength and control reported in the early stages of AN. This paper seeks to enrich our understanding of bodily experience in AN as it progresses throughout the various stages of the disorder. I show how the classical phenomenological distinction between the body-as-subject and the body-as-object, as well as Leder’s conception of the visceral body, can inform our understanding of bodily experience in AN. I suggest that the project of self-starvation is an attempt to overcome the noisy demands of the visceral body, which are experienced as threatening the body-as-subject, through a process of objectifying the body-as-object. By cashing out AN as a project of radical bodily control that, tragically, comes to control the individual, we can capture important aspects of the bodily experience of AN and the temporal progression of the disorder. -/- . This paper gives an interpretation of Kant's argument for transcendental idealism in the Transcendental Aesthetic. I argue against a common way of reading this argument, which sees Kant as arguing that substantive a priori claims about mind-independent reality would be unintelligible because we cannot explain the source of their justification. I argue that Kant's concern with how synthetic a priori propositions are possible is not a concern with the source of their justification, but with how they can have objects. I (...) argue that Kant's notion of intuition needs to be understood as a kind of representation which involves the presence to consciousness of the object it represents, and that this means that a priori intuition cannot present us with a mind-independent feature of reality. Unethical pro-organizational behavior (UPB) is unethical behavior driven by an intention to assist an organization. This study is one of the first attempts to examine the consequences of UPB. We argue that such types of behaviors can induce failure in self-regulation and thereby give rise to counterproductive work behavior (CWB). Based on self-regulation theory, we theorize that the breakdown in three fundamental mechanisms (i.e., moral standards, monitoring, and discipline) explains the link between UPB and CWB. Moreover, moral identity internalization can (...) temper these breakdown processes such that employees with higher levels of moral identity internalization are less likely to experience moral disengagement, workplace entitlement, or self-control depleting after enacting UPB, and therefore, will engage in less CWB. We conducted a 7-day experience sampling study of 95 financial service employees in a Chinese bank to test our theoretical model. The results indicated that transgressions committed with the intention of helping the organization would ultimately harm the organization, especially for those with lower moral identity internalization. Individuals suffering from depression consistently report experiencing a lack of connectedness with others. David Karp (2017, 73), in his memoir and study of depression, has gone so far to describe depression as “an illness of isolation, a disease of disconnectedness”. It has become common, in phenomenological circles, to attribute this social impairment to the depressed individual experiencing their body as corporealized, acting as a barrier between them and the world around them (Fuchs 2005, 2016). In this paper, I offer an (...) alternative view of the experience of social disconnectedness in depression, suggesting that rather than necessarily experiencing their body as object-like, the depressed individual’s bodily is saturated with experiences of lethargy, tiredness, heaviness, sadness, hopelessness and so on, to the exclusion of being able to bodily connect to others. I suggest that depression does not involve a complete social impairment but a specific impairment of affective forms of interpersonal experience. (shrink)

NOTE FROM THE AUTHORS The context of our co-authored contribution to the ‘Aesthetic Relations’ conference-publication is a performance devised by Lucie in 2020 to which she invited Kiraṇ as a collaborator. Due to international travel restrictions however, our physical co-pres-ence in a studio and on stage remained suspended throughout that year. Our exchanges nevertheless continued in adaptive turns both before and after that performance. It is this condition of, at once, compromised yet consistent relation with each other that we refer (...) to as ‘suspension’. For us this suspension was at first admittedly a source of disruption, even anxiety of interrupting our artistic flow. Yet there came a point during the year when this swelling tension ruptured, and a strange ease set in. Nothing had changed on the surface (travel restrictions were only extended), yet something had shifted under our skins. While still physically across continents, our online presentation at the conference was a joint essay at articulating, both textually and performatively, the unassuming complexity of this thing that is suspension. By the time of this publication in 2022, we have somewhat broken this suspension by physically meeting in Switzerland in August 2021. As such we have decided not to edit the text that was performed at the conference back in January 2021, and only append it with this contextual note. Here we must add that the text in one of the sections to follow (which begins with “Please turn your head to your left..”) is intended as a score for action. Originally spoken in our voices, we now ask you to invite another person to read this section aloud to you at an unhurried pace, while you respond to the text’s suggestions. As for us, we continue our collabora-tive artistic research of working in and with suspension as artistic strategy through our ongoing exchanges and upcoming projects. (shrink)

: Interpretations of Kant's transcendental idealism have been dominated by two extreme views: phenomenalist and merely epistemic readings. There are serious objections to both of these extremes, and the aim of this paper is to develop a middle ground between the two. In the Prolegomena, Kant suggests that his idealism about appearances can be understood in terms of an analogy with secondary qualities like color. Commentators have rejected this option because they have assumed that the analogy should be read in (...) terms of either a Lockean or a Berkelean account of qualities such as color, and have argued, rightly, that neither can provide the basis for a coherent interpretation of Kant's position. I argue that the account of color that the analogy requires is one within the context of a direct theory of perception, as opposed to Locke's representative account. Using this account of color, the secondary quality analogy enables us to explain how appearances can be mind-dependent without existing in the mind. (shrink)

The presbytery pavement of Novara Cathedral in Piedmont, which dates from the early twelfth century, includes the symbols of the four Evangelists on either side of where the altar once stood. The fourteenth-century ordinal for the cathedral indicates that these symbols were used as markers for participants in the prebaptismal ceremony known as the aurium apertio, in particular during the reading of the initial passages of the Gospels . This rare mention of the liturgical use of images in a figurative (...) floor mosaic challenges our preconceptions of what it might have meant to stand on an image and poses the question whether the pavement could have been designed with such a use in mind. However, several factors complicate the matter. Most important is the substantial gap in time between the laying of the floor and the writing of the manuscript, which means that the liturgical custom might equally have responded to the design of the floor. Secondly, there is the position of Novara as a suffragan diocese within the archdiocese of Milan, when the traditio evangeliorum was not part of the Milanese rite. Thirdly, there is the location of the symbols on the presbytery pavement, where they may not have been visible to the laity, although a central aspect of the rite is the explanation of the Evangelist symbols to the baptismal candidates. Taking these points into consideration, this article attempts to elucidate the relationship between pavement and ceremony. It traces the place of the traditio evangeliorum within the history of prebaptismal liturgy and considers the significance of the rite for those who took part in it, before examining the audience and function of both the readings of the Gospel initia and the mosaic symbols themselves. Although this editorial will be published in the February 2025 issue of the Journal of Medical Ethics, I am writing it in the run up to the Christmas and the winter holidays, after a busy term, and this is, for some of us, a time to reflect on the past year (although as an academic the year is only half-way through!). It is also the journal’s 50th anniversary in 2025. This prompts further reflection, thinking about how and why the journal (...) started, where its focus has been and what the future holds for both the journal and the field of medical ethics more broadly. Healthcare, those who work in it and the systems in which it is delivered, are facing pressures and challenges of huge global proportions, global conflict, the climate crisis and, in England, ongoing austerity all exacerbate existing health inequities. The stark disparities in healthcare access between the global south and north, and within countries such as the UK, raise profound questions of social justice. The JME has addressed many of these in articles and editorials over the years. As well as these current concerns, critical issues at the beginning and end of life are still as pertinent today as they were when the journal was first published in 1975. Résumé Selon la perspective de l’œuvre ouverte développée par Eco, « un texte est un tissu d’espaces blancs, d’interstices à remplir … un mécanisme paresseux qui vit sur la plus-value de sens qui y est introduite par le destinataire » (Eco 1985, Lector in fabula ou la coopération interprétative dans les textes narratifs, Myriem Bouzaher (trad.). Paris: Grasset, 66–67). C’est à cette plus-value de sens, concomitante à l’activité interprétative, que la présente étude s’attardera, et ce, afin d’identifier d’abord des éléments (...) de l’album d’enfance nécessitant l’interprétation d’un lecteur modèle et d’en convoquer par la suite les significations qui peuvent être appliquées au texte. Plus précisément, il s’agira de décrire et d’analyser les mécanismes intertextuels qui enrichissent l’organisation syntaxique et sémantique de quatre albums d’enfance parus au Québec entre 1992 et 2002 et qui nécessitent l’intervention d’un lecteur modèle pour identifier les systèmes de signification et les savoirs qui y sont rattachés. Les albums soumis à l’investigation reprennent le schéma narratif de contes de fée connus et celui d’une nouvelle contemporaine. L’hypothèse est la suivante: si les composantes génériques de l’album dévoilent à un jeune lecteur des univers de références connus et familiers, le processus de l’intertextualité sollicite des contenus qui, soumis à l’interprétation d’un lecteur modèle, font parfois surgir des structures de signification inattendues.
