PurposeThis study aims to describe the adverse events following immunization of SARS-CoV-2 vaccination in cancer patients/survivors associated with their psychological distress.MethodsA cross-sectional study was conducted to assess AEFIs after the receipt of SARS-CoV-2 vaccines in cancer patients/survivors attending a university hospital in Malaysia. Psychological distress was measured using the Hospital Anxiety and Depression Scale before and after the first and second doses of COVID-19 vaccine.ResultsA total of 217 complete responses were received. Compared with before vaccination, both HADS Anxiety and HADS (...) Depression scores were significantly reduced after the first and second dose of the SARS-CoV-2 vaccine. Most of the participants had mild-or-moderate systemic and local AEFIs, with the most common being pain at the injection site, tiredness, and headache for both the first and second doses of the vaccine. Positive correlations between the total AEFI score and HADS-A and HADS-D scores were observed after the first dose of the SARS-CoV-2 vaccine. Similarly, positive associations were observed between the total AEFI score and HADS-A and HADS-D scores after the second dose of the SARS-CoV-2 vaccine.ConclusionMild-to-moderate AEFIs found in this study help address vaccine hesitancy in cancer patients/survivors. Receiving the SARS-CoV-2 vaccine had a positive effect on decreasing psychological distress in cancer patients/survivors. High severity of an AEFI was associated with higher anxiety and depressive symptoms. (shrink)

ObjectivesWorking-age cancer patients face barriers to resuming work after treatment completion. Those resuming work contend with reduced productivity arising from persisting residual symptoms. Existing studies of return to work after cancer diagnosis were done predominantly in Western countries. Given that employment and RTW in cancer survivors likely vary regionally due to healthcare provision and social security differences, we documented rates and correlates of RTW, work productivity, and activity impairment among Chinese cancer survivors in Hong Kong at one-year post-treatment.MethodsOf 1,106 cancer (...) patients assessed at six-months post-cancer treatment, 593 previously worked; detailed work status, psychological distress, physical symptom distress, supportive care needs, health-related quality of life, and illness perception were assessed. Six months later, work productivity and activity impairment were assessed. Descriptive analyses examined RTW rate. Fully adjusted regressions determined RTW, work productivity, and activity impairment predictors.ResultsAt baseline, 39% were working, 26% on sick leave, and 35% were unemployed. Compared to patients returning to work, unemployed participants were older, likely manual/service-oriented workers, and had lower family income, chemotherapy, fewer unmet health system and information needs, poorer physical functioning, and negative illness perceptions. Sick leave participants were likely service-oriented workers, who had head and neck cancer, chemotherapy, and poor physical functioning. At FU, baseline depressive symptoms, physical symptom distress, and negative illness perceptions predicted presenteeism and work productivity loss; gynecological cancer, fewer unmet health system and information needs, and greater unmet sexuality needs predicted absenteeism; physical symptom distress, negative illness perception, and poor physical functioning predicted activity impairment.ConclusionCancer survivors who had more physically demanding jobs and poorer physical functioning delayed RTW. Unmanaged physical symptom and psychological distress hindered work productivity. (shrink)

For much of the twentieth century, Confucianism was condemned by Westerners and East Asians alike as antithetical to modernity. Internationally renowned philosophers, historians, and social scientists argue otherwise in Confucian Political Ethics. They show how classical Confucian theory--with its emphasis on family ties, self-improvement, education, and the social good--is highly relevant to the most pressing dilemmas confronting us today. The United Kingdom case R v Lee EWCA Crim 1404 resulted in a pharmacist being convicted for an inadvertent dispensing error and paved way for the decriminalisation of such errors by way of a due diligence defence enacted in 2018. In relation to Hong Kong, what is its legal position for dispensing errors, and can it follow the decriminalising steps of UK? The primary objective of this paper is to explore whether and how HK can reach the normative position for (...) a dispensing error legal regime: I posit that the normative position for healthcare professional liability for dispensing errors should prioritise the public interest of minimisation of future dispensing errors over the retribution of past wrongs; I illustrate HK’s current position for the liabilities of HCPs on dispensing errors, focusing analysis on the relatively controversial aspects of HK’s criminal liability, referencing the landmark cases Hin Lin Yee v HKSAR 13 HKCFAR 142 and Kulemesin v HKSAR 16 HKCFAR 195 to assist my analysis of the requisite mental element for relevant statutory offences; through comparison with UK’s development post-R v Lee and application of Rule of Law principles, HK’s current position is critiqued, coming to the conclusion that while there are compelling reasons for the decriminalisation of dispensing errors in HK, the prerequisite for this to happen is an overhaul of regulatory frameworks by significantly increasing levels of accountability. Complex tensions define us, and that is why rational evaluative analysis and the deliberate application of principles to cases can, at best, claim to account for only a limited register in the full compass of ethical voice. Close analysis of brief texts from the "Mencius" and Dante's "Inferno" discloses in both an approach to ethical reflection that aims to expand the capacity for virtue, the ethical skillfulness exercised in response and evaluation, through affective engagement of the reader. This approach, a (...) kind of "re- flexive tropology," may involve us in a Han-tan walk. Nevertheless, if virtue and models of the self are interconnected, then right self-understanding is where the foundations of virtue are laid. Because the religious orientation calls into question the false fixities that inform ordinary activities and ordi- nary models of the self, it is crucial to the work of distinguishing among true and false forms of virtue. Modern warfare cannot be conducted without civilians being killed. In order to reconcile this fact with the principle of discrimination in just war theory, the principle is applied through the doctrine of double effect. But this doctrine is morally inadequate because it is too permissive regarding the risk to civilians. For this reason, Michael Walzer has suggested that the doctrine be supplemented with what he calls the idea of double intention: combatants are not only to refrain from intending to harm (...) civilians; they are also to take precautions to reduce risk to civilians, even at the expense of increasing risk to themselves. The article develops the idea of double intention by addressing two questions: What does it mean to intend to reduce civilian risk, and how much should civilian risk be reduced? The results of this discussion are then used to consider a moral issue that arises in technologically asymmetric warfare, namely, the extent to which the use of precision-guided munitions, which allow more accurate targeting, can by itself bear the moral burden imposed by the principle of discrimination. This study gathered preliminary baseline data on the moral development of journalists using the Defining Issues Test, an instrument based on Kohlberg's 6 stages. Results show that a sample of journalists scored 4th highest among professionals tested using the DIT. The journalists ranked behind seminarians/philosophers, medical students, and physicians but above dental students, nurses, graduate students, undergraduate college students, veterinary students, and adults in general. No significant differences were found between various groups of journalists, including men and women, and broadcast (...) and print journalists. The journalists in the study scored significantly higher on the 3 journalism-specific dilemmas than on 3 nonjournalism dilemmas. (shrink)

Although disoriented young children reorient themselves in relation to the shape of the surrounding surface layout, cognitive accounts of this ability vary. The present paper tests three theories of reorientation: a snapshot theory based on visual image-matching computations, an adaptive combination theory proposing that diverse environmental cues to orientation are weighted according to their experienced reliability, and a modular theory centering on encapsulated computations of the shape of the extended surface layout. Seven experiments test these theories by manipulating four properties (...) of objects placed within a cylindrical space: their size, motion, dimensionality, and distance from the space’s borders. Their findings support the modular theory and suggest that disoriented search behavior centers on two processes: a reorientation process based on the geometry of the 3D surface layout, and a beacon-guidance process based on the local features of objects and surface markings. Some of these will be observations; some will be practical inferences. One of them, though, involves the crossing of an expectation, or maybe a fear, I had brought with me to Minneapolis. Since this has to do with the whole tone of the conference, we might as well (...) begin with it.It, of course, was the question of the polemical nature of this gathering. Would this conference be diagnostic and denunciatory in nature, or would it seek to start something truly new? Certainly we all feel the need of something new—not new, either, in the sense that movements or ideological positions claim to be new, but in the sense of a fresh gathering of common attention in the place where literature is, the place where each of us who loves literature already goes alone. This would be a possibility for something original rather than just another novelty—which makes the question of polemics all the more urgent. An association that founded itself in complaint would be only one more identity-group making a virtue out of its grudge. And the grudge, in that case, would have been a sad reflex of feeling superseded and powerless.But where have the polemics against current orthodoxies been? I am delighted not to have heard them in any strenuous capacity. Even their relative absence hasn’t commanded any extended attention, and that may be an even more telling point. I take this to be partly the result of foresight and good sense. But the clean air here has been more than a reward for our mutual constraint. Instead, it seems evidence of the fact [End Page 193] that, when literature occupies our attention, there is no need for self-defense. Justice, or justification, happens by itself when we “do our own work,” as Socrates recommended.What is our own work, then? This association has made a point, it seems, of declining to prescribe what it should be. The only thing we are absolutely sure of is that its subject is literature, and that its standards and rewards should be of a literary nature. From there the field opens up—and this conference has succeeded in keeping it open in some very concrete ways.Let’s think briefly about the two panels we attended yesterday. The morning panel on “Intellectual Craftsmanship” seemed unified in its concern with the disciplined ways by which we commit ourselves to literary experience. Literature itself, as well as what we do with it, teaches us this discipline, which is “aesthetic” in every sense of that word. Roger Shattuck set the tone when he described the discipline of reading as an issue of preparation and assimilation on the one hand, and as a kind of complete perceptual openness on the other. Such openness, in our normal experience, allows us to be surprised, to see actual things, perhaps, instead of their institutional categories. But literature also requires that we craft our receptivity. The surprises we prepare ourselves to undergo are achieved breakthroughs, which makes them no less unpredictable but all the more deeply affecting.Criticism cultivates such surprising experience. (I think experience is the crucial word, by the way, since everyone on that first panel seemed to be devoted to a kind of free empiricism.) The spectacular moments of visionary power that Robert Alter isolated in Dickens are places that startle and disturb us—and we track our own surprise, and intensify it, when we look closely at how those dark places of power emerge from Dickens’s refiguring of a world where darkness otherwise tends toward an entirely unredeemable opacity. We have to read “synesthetically,” as Shattuck says, “with both eyes open,” to let ourselves be for a moment blinded by such literary places.Once again, the discipline of reading—devotedly and resistantly—frees us for the achieved experience. Slowing down, in Sven Birkerts’s sense of “retardation and deceleration,” is precisely a matter of stepping into a field of experience shaped (or, in an auditory sense, fostered and conserved) by the special demands of literature. Disoriented animals from ants to humans reorient in accord with the shape of the surrounding surface layout: a behavioral pattern long taken as evidence for sensitivity to layout geometry. Recent computational models suggest, however, that the reorientation process may not depend on geometrical analyses but instead on the matching of brightness contours in 2D images of the environment. Here we test this suggestion by investigating young children's reorientation in enclosed environments. Children reoriented by extremely subtle geometric properties of the 3D (...) layout: bumps and ridges that protruded only slightly off the floor, producing edges with low contrast. Moreover, children failed to reorient by prominent brightness contours in continuous layouts with no distinctive 3D structure. The findings provide evidence that geometric layout representations support children's reorientation. (shrink)

BackgroundAssisted dying has wide support among the general population but there is evidence that those providing care for the dying may be less supportive. Senior doctors would be involved in implementing the proposed change in the law. We aimed to measure support for legalising physician assisted dying in a representative sample of senior doctors in England and Wales, and to assess any association between doctors' characteristics and level of support for a change in the law.MethodsWe conducted a postal survey of (...) 1000 consultants and general practitioners randomly selected from a commercially available database. The main outcome of interest was level of agreement with any change in the law to allow physician assisted suicide.ResultsThe corrected participation rate was 50%. We analysed 372 questionnaires. Respondents' views were divided: 39% were in favour of a change to the law to allow assisted suicide, 49% opposed a change and 12% neither agreed nor disagreed. Doctors who reported caring for the dying were less likely to support a change in the law. Religious belief was also associated with opposition. Gender, specialty and years in post had no significant effect.ConclusionMore senior doctors in England and Wales oppose any step towards the legalisation of assisted dying than support this. Doctors who care for the dying were more opposed. This has implications for the ease of implementation of recently proposed legislation. (shrink)

Experimental work has revealed causal links between physical cleansing and various psychological variables. Empirically, how robust are they? Theoretically, how do they operate? Major prevailing accounts focus on morality or disgust, capturing a subset of cleansing effects, but cannot easily handle cleansing effects in non-moral, non-disgusting contexts. Building on grounded views on cognitive processes and known properties of mental procedures, we proposegrounded proceduresof separation as a proximate mechanism underlying cleansing effects. This account differs from prevailing accounts in terms of explanatory (...) kind, interpretive parsimony, and predictive scope. Its unique and falsifiable predictions have received empirical support: Cleansing attenuates or eliminates otherwise observed influences of prior events (1) across domains and (2) across valences. (3) Cleansing manipulations produce stronger effects the more strongly they engage sensorimotor capacities. (4) Reversing the causal arrow, motivation for cleansing is triggered more readily by negative than positive entities. (5) Conceptually similar effects extend to other physical actions of separation. On the flipside, grounded procedures ofconnectionare also observed. Together, separation and connection organize prior findings relevant to multiple perspectives (e.g., conceptual metaphor, sympathetic magic) and open up new questions. Their predictions are more generalizable than the specific mappings in conceptual metaphors, but more fine-grained than the broad assumptions of grounded cognition. This intermediate level of analysis sheds light on the interplay between mental and physical processes. (shrink)

According to Lee, a hierarchy of biological and individual needs provides an objective basis for ethics. The anthropocentric hierarchy of needs provides a model for examining the needs of the environment as well. A sound world order must be based on an ethical theory that integrates the needs of humans and the environment of which they are a part. Hospitalized patients lacking the mental capacity to consent to treatment may demand to be discharged from the hospital against medical advice. Forced custody of these patients, including the use of restraints, may be required if the plan is to proceed with treatment. This raises ethical concerns with regard to depriving people of their liberty. The determination of the wishes and values of the patient and her best interests may sometimes vary, depending on the assessor or the clinical team entrusted to (...) perform the evaluation. We therefore propose the following triad for clinicians when determining the best interests for this group of patients to ensure ethical and legal soundness as well as in providing consistency in approach. Firstly, the treatment should serve as a rescue response. Secondly, any restraint deployed must be proportionate with a foreseeable end. Her liberty and autonomy interests should be enhanced by the treatment. Thirdly, the patient’s family must be supportive. This paper analyzes the use of the triad approach on two clinical cases, both assessed as lacking in mental capacity to decide on treatment and insisting to be discharged. Using this approach, we could justify how the contrasting outcomes, one in which the voluntary discharge was acquiesced versus the other, which was not acquiesced, were reached. We also examine the compatibility of the triad approach with the obligations under the United Nations Convention on the Rights of Persons with Disabilities (UNCRPD). (shrink)

It is generally acknowledged by proponents of ‘new mechanism’ that mechanistic explanation involves adopting a perspective, but there is less agreement on how we should understand this perspective-taking or what its implications are for practising science. This paper examines the perspectival nature of mechanistic explanation through the lens of the ‘mechanistic stance’, which falls somewhere between Dennett’s more familiar physical and design stance. We argue this approach implies three distinct and significant ways in which mechanistic explanation can be interpreted as (...) perspectival: ‘phenomenon perspectivism’, ‘pattern perspectivism’ and ‘hierarchy perspectivism’. We evaluate the strength of the perspective-dependency implied by each of these, and along the way, discuss their significance for wider debates within the new mechanism literature, such as the nature of function attribution and an ontic vs epistemic understanding of explanation. This paper explores how the meaning of evidentiality is temporally constrained, by investigating the meaning of Korean evidential sentences with –te. Unlike evidential sentences in languages that have previously been formally analyzed , e.g. Cuzco Quechua and Cheyenne, Korean evidential sentences with –te are compatible with both direct and indirect evidence types. In this paper, I analyze –te as an evidential that lexically encodes the meaning of a ‘sensory observation’. I account for the availability of both direct and indirect evidential (...) readings in terms of the variable temporal relation between relevant eventualities. I show that this temporal relation is compositionally determined by the interaction between –te and tense, and that it in turn constrains possible (direct vs. indirect) evidence types. I also provide empirical evidence for the modal meaning contributions of –te sentences, and develop a formal analysis in terms of Kratzer’s modal theory. The paper concludes by discussing the empirical and theoretical improvements of the proposed analysis over earlier analyses by Chung, and the implications for crosslinguistic studies of evidentials. This paper critically appraises the arguments that have been offered for what can be called ‘the expressive function of punishment’. According to this view, what distinguishes punishment from other kinds of non-punitive hard treatment is that punishment conveys a censorial/reprobative message about what the punished has done, and that this expressive function should therefore be accepted as part of the nature and definition of punishment. Against this view, this papers argues that the standard account of punishment, according to which punishment (...) is a kind of hard treatment that is imposed on an alleged offender in response to her alleged wrongdoing, can already properly account for punishment and distinguish it from other kinds of hard treatment when it is properly clarified and understood. Thus there is no need to accept the expressive function of punishment in addition to the standard account when it comes to the nature and definition of punishment. (shrink)

In the second Critique, Kant claims that the moral law is given as a fact of reason. In this paper, contra the standard view, I argue that there is a non-dogmatic way of defending this claim. And Kant’s principle of morality is widely taken to be a formal principle. How then can such a formal principle be reconciled with our substantial moral end? In this paper, I also argue that Kant’s principle of morality can be construed as a formal principle (...) in the sense that it provides correctness conditions for moral laws, rather than providing specific moral laws.Dans sa deuxième Critique, Kant affirme que la loi morale se donne comme un fait de raison. Dans cet article, contrairement à la position standard, nous prétendons qu’il existe une autre façon — non dogmatique — de défendre cette affirmation. Par ailleurs, le principe de moralité de Kant est largement considéré comme étant formel. Comment un tel principe formel peut-il être réconcilié avec notre fin morale substantielle? The view that human beings begin to exist at fertilization (namely conceptionism) faces a serious challenge from the twinning argument, that identical twins coming from the same zygote must be numerically distinct from the zygote and so did not exist at fertilization. Recently, some philosophers have claimed that the twinning argument rests on a particular metaphysical theory of persistence, namely endurantism, on which a human being, for example, is wholly present at every moment of her existence. And we can easily (...) refute the argument, they claim, by employing perdurantism or exdurantism, according to which a human being is a temporally extended entity with temporal parts or a momentarily existing stage who has other momentarily existing stages as counterparts. I argue that such claims are mistaken. The twinning argument does not rest on endurantism and can be formulated in terms of perdurantism to provide a good reason for perdurantists to reject conceptionism. And exdurantism does not have any advantage in defending conceptionism either, for it already concedes more than what the twinning argument aims to show. (shrink)

Are the circumstances in which moral testimony serves as evidence that our judgement-forming processes are unreliable the same circumstances in which mundane testimony serves as evidence that our mundane judgement-forming processes are unreliable? In answering this question, we distinguish two possible roles for testimony: (i) providing a legitimate basis for a judgement, (ii) providing (‘higher-order’) evidence that a judgement-forming process is unreliable. We explore the possibilities for a view according to which moral testimony does not, in contrast to mundane testimony (...) play role (i), but can play role (ii). We argue that standard motivations for rejecting this hybrid position are unpersuasive but suggest that a more compelling reason might be found in considering the social nature of morality. (shrink)

Research on delinquency reduction often highlights the importance of identifying and sanctioning antisocial and illegal activities so as to reduce the likelihood of future offending. The rise of digital technology complicates the process of detecting cybercrimes and technology enabled offenses, as individuals can use devices from anywhere to engage in various harmful activities that may appear benign to an observer. Despite the growth of cybercrime research, limited studies have examined the extent to which technology enabled offenses are detected, or the (...) behavioral and attitudinal factors associated with being unobserved or caught for one’s actions. The current study addresses this gap in the literature by estimating a multinomial regression model for self-reported computer hacking behavior and the likelihood of those actions being detected in a large international sample of juveniles (N = 51,059). The findings demonstrate significant differences between youth who hack without detection compared to those who are caught. The implications of this analysis for our understanding of cybercrime and its relationship to traditional delinquency are explored in depth. (shrink)
