This research investigates gender differences in employer-sponsored medical insurance coverage among full-time male and female workers in the United States and assesses the relevance of human capital theory and its compensating differentials corollary in predicting coverage. Data are analyzed from a subsample of the Quality of Employment Survey, a national probability sample of workers in the United States. Results indicate that men were more likely to have medical insurance coverage from their employers than were women; however, gender differences were minimized (...) in a multivariate context when human capital and structural variables were controlled. Furthermore, human capital variables are, overall, weak predictors of whether both women and men received coverage. More generally, the data suggest that structural conditions of the occupation, rather than individual human capital, may be most relevant for future research. (shrink)

Background and AimsEnd-of-life discussions can be difficult for seriously ill adolescents and young adults. Researchers aimed to determine whether completing Voicing My CHOiCES —a research-informed advance care planning guide—increased communication with family, friends, or health care providers, and to evaluate the experience of those with whom VMC was shared.MethodsFamily, friends, or HCPs who the AYAs had shared their completed VMC with were administered structured interviews to assess their perception of the ACP discussion, changes in their relationship, conversation quality, and whether (...) the discussion prompted changes in care. Open-ended responses underwent thematic analysis.ResultsOne-month post-completion, 65.1% of AYA had shared VMC completion with a family member, 22.6% with a friend, and 8.9% with an HCP. Among a sample of respondents, family and friends reported a positive change in their relationship with the AYA. Participant descriptions of the experience fell into five themes: positive experience, difficult experience, appreciated a guide to facilitate discussion, provided relief, and created worry/anxiety. Only 1 HCP noted a treatment change. Family, friends, and HCP did not think the AYA would have discussed EoL preferences without completing VMC.ConclusionsVMC has potential to enhance communication about ACP between AYA and their family and friends, though less frequently with HCPs. Participants reported a positive change in their relationship with the AYA after discussing VMC, and described experiencing the conversation as favorable, even when also emotionally difficult. This paper compares the views of Hegel and Schelling regarding the problem of individuation, i.e. the question of what makes an individual (a) numerically distinct from others and (b) the very individual it is. My focus is on how Hegel approaches this problem in his metaphysics and how that relates to Schelling’s views as articulated in his ‘negative philosophy’. While Hegelians like Robert Stern and Karen Ng are optimistic that Hegel can solve the problem of individuation, I argue that (...) Recent defenses of shaming as an effective tool for identifying bad practice and provoking social change appear compatible with feminism. I complicate this picture by examining two instances of online feminist shaming that resulted in shame backlashes. Shaming requires the assertion of social and epistemic authority on behalf of a larger community, and is dependent upon an audience that will be receptive to the shaming testimony. The concept of phronesis is venerable and is experiencing a resurgence in contemporary discourses on professional life. Aristotle’s notion of phronesis involves reasoning and action based on ethical ideals oriented towards the human good. For Aristotle, humans possess the desire to do what is best for human flourishing, and to do so according to the application of virtues. Within health care, the pervasiveness of economic agendas, technological approaches and managerialism create conditions in which human relationships and moral reasoning are becoming (...) increasingly de‐valued. This creates a tension for nurses, and nursing leaders, as the desire to do what is morally right is often in conflict with contextual demands. In this paper, Aristotle’s writing on phronesis is examined with a focus on his classic conceptions of eudaimonia, the virtues, deliberation, judgement, and praxis. Building on Aristotle’s work, a number of contemporary views are explored with a focus on what various conceptualizations offer for the discipline of nursing. These expanded conceptions of phronesis include attention to: embodiment in practice; open‐mindedness including the capacity to stay curious and open to recognizing what we do not know; perceptiveness as a disposition towards insight and aesthetic understanding; and reflexivity as an ongoing process of interrogation and inquiry into ourselves and our actions. Drawing on these concepts, we discuss the affordances of phronesis as a morally informed guiding force to attend to modern‐day challenges in nursing practice and nursing leadership. (shrink)

I agree with Jecker et al that “the headline-grabbing nature of existential risk (X-risk) diverts attention away from immediate artificial intelligence (AI) threats…”1 Focusing on very long-term speculative risks associated with AI is both ethically distracting and ethically dangerous, especially in a healthcare context. More specifically, AI in healthcare is generating healthcare justice challenges that are real, imminent and pervasive. These are challenges generated by AI that deserve immediate ethical attention, more than any X-risk issues in the distant future. We are ‘doing better’ because numerous advances in medical technologies are saving more lives and improving the quality of our lives. But we are ‘feeling worse’ because the additional costs are unsustainable, are threatening funding for other social goods and are increasing injustices regarding the allocation of those resources. This article is based on a distinctive study that seeks to analyse the nascent role of teenagers’ activism and protests for climate change action. With the increasing realisation of children's rights to participation, the past few years have marked the rise of the new dispensation of climate activism and protests in which teenagers have occupied the centre stage. We pay specific reference to Greta Thunberg, a Swedish child climate activist, in as much as she can set a framework upon which (...) Africa can draw parallels, lessons and insights for climate activism and protests. Even though the context may be different, the paper attempts to inform vibrant climate action through activism by children and ultimately climate policies, laws and management for environmental sustainability in Africa. We engaged document review and thematic approaches, and it emerged that children climate activism and protests in Africa are not as vibrant as they should be given the prevalence of climate inaction across the continent. Therefore, there is much to learn from Thunberg by pushing national governments and regional organisations to increase the decision-making space of children in the fight against climate change. The development of Web 2.0 has opened new opportunities for teaching and learning - an entire ecology of online learning. This ecology has not previously been systematically investigated. This book offers the first comprehensive conceptual map for this territory.

Using qualitative data based on interviews with 22 married infertile couples living in western New York State, we describe the ways in which husbands and wives interact in the process of constructing their infertility. The wives experienced infertility as a cataclysmic role failure. Husbands tended to see infertility as a disconcerting event but not as a tragedy. Couples tended to see infertility as a problem for wives. Frustration and lack of communication were typical consequences of the confrontation of husbands' and (...) wives' perspectives on infertility. Interactions with medical professionals tended to reinforce these consequences. These interactions between wives, husbands, and medical professionals may lead to taking wrong directions in treatment and to ignoring treatment options. Some of the problems we describe could be lessened by adopting a model of couple-centered treatment in a setting that incorporated routine counseling. (shrink)

Sunstein's characterization of moral blunders jointly indicts an intuitive process and the structure of heuristics. But intuitions need not lead to error, and the problems with moral heuristics apply also to moral principles. Accordingly, moral development may well involve more, rather than less, intuitive responsiveness. This suggests a novel trajectory for future research into the development of appropriate moral judgments.

In ethical theory, different concepts of autonomy can be distinguished. In this article we explore how these concepts of autonomy are combined in theory in the citizenship paradigm, and how this turns out in the practice of care for people with acquired brain injury. The stories of a professional caregiver and a client with acquired brain injury show that the combination of various concepts of autonomy in practice leads to tensions between caregivers and clients. This article compares the ways in which Saba Mahmood’s The Politics of Piety and Cressida Heyes’ Self-Transformations: Foucault, Ethics, and Normalization, unlike current governmentality studies, employ the later Foucault’s ethical theory. By explaining the theoretical framework of the ‘middle’ Foucault and the ‘later’ Foucault and then comparing Mahmood and Heyes’ use of Foucault’s work, it is argued that Mahmood and Heyes’ analyses, though thought-provoking and incisive, overlook aspects of Foucault’s later work, ultimately preventing them from offering productive ‘feminist strategies’. The (...) author seeks to link this discussion to contemporary debates and analyses of agency, freedom and Muslim women in the media.
