A cross-sectional study to ascertain what the Singapore population would regard as material risk in the anaesthesia consent-taking process and identify demographic factors that predict patient preferences in medical decision-making to tailor a more patient-centered informed consent. A survey was performed involving patients 21 years old and above who attended the pre-operative evaluation clinic over a 1-month period in Singapore General Hospital. Questionnaires were administered to assess patients’ perception of material risks, by trained interviewers. Patients’ demographics were obtained. Mann–Whitney U (...) test and Kruskal-Wallis one-way analysis of variance was used. Statistical significance was taken at p < 0.05. Four hundred fourteen patients were eligible of which 26 refused to participate and 24 were excluded due to language barrier. 364 patients were recruited. A higher level of education, being employed and younger age group are factors identified in patients who wanted greater participation in medical decisions. Gender, marital status, type of surgery, and previous surgical history did not affect their level of participation. The complications most patients knew about were Nausea, Drowsiness and Surgical Wound Pain. Patients ranked Heart Attack, Death and Stroke as the most significant risks that they wanted to be informed about in greater detail. Most patients wanted to make a joint decision with the anaesthetist, instead of letting the doctor decide or deciding for themselves. Discussion with the anaesthetist is the preferred medium of communication compared to reading a pamphlet or watching a video. Age and educational level can influence medical decision-making. Despite the digital age, most patients still prefer a clinic consult instead of audio-visual multimedia for pre-operative anaesthetic counselling. The local population appears to place greater importance on rare but serious complications compared to common complications. This illustrates the need to contextualize information provided during informed consent to strengthen the doctor-patient relationship. (shrink)

How much novocaine a patient receives affects how much pain they will feel during dental surgery. -/- The metaphysics of causation asks questions about what it takes for claims like these to be true—what kind of relation the claims are about, and in virtue of what these relations obtain.

It is often suggested that we are equipped with a set of cognitive tools that help us to filter out unreliable testimony. But are these tools effective? I answer this question in two steps. Firstly, I argue that they are not real-time effective. The process of filtering, which takes place simultaneously with or right after language comprehension, does not prevent a particular hearer on a particular occasion from forming beliefs based on false testimony. Secondly, I argue that they are long-term (...) effective. Some hearers sometimes detect false testimony, which increases speakers’ incentives for honesty and stabilizes the practice of human communication in which deception is risky and costly. In short, filtering prevents us from forming a large number of beliefs based on false testimony, not by turning each of us into a high-functioning polygraph but by turning the social environment of human communication into one in which such polygraphs are not required. Finally, I argue that these considerations support strong anti-reductionism about testimonial entitlement. (shrink)

At some distance from a high-rate source in an elastic half-space, the dominant wave motion at the free surface is a Rayleigh surface wave. The calculation of surface waves generated by a concentrated force in a half-space is a basic problem in elastodynamics. By straightforward manipulations, the result can be used to obtain surface waves for other kinds of wave-generating body-force arrangements. For example, appropriate combinations of double-forces (or dipoles) can be used to represent the surface loading due to laser (...) irradiation, or due to acoustic emission from the opening of a sub-surface crack or from sliding over a fault surface. The surface wave motion is usually obtained by the application of integral transform techniques and the subsequent extraction of the surface waves as the contributions from poles in the integral for the inverse transform. In this paper, we use a much simpler approach based on the elastodynamic reciprocity theorem. We consider a transversely isotropic solid whose axis of symmetry is normal to the surface of the half-space. Surface wave pulses generated by a single force, by irradiation from a laser source, and by opening of a crack, have been determined. (shrink)

This paper will develop the concept of semiogenesis – a process of novel sign generation – and how instances of this process, such as agency, relate to their built environment and beyond. Section two will build on Hoffmeyer’s discussion of swarms, specifically the idea of overlapping swarms and its manifestation in the creation of termite mounds, in order to introduce three types of structure. Building upon this real-world example explored in section two, the third section will present a heuristic for (...) semiogenesis: a neuromorphic dynamic system model grounded in information and thermodynamic theory. Through the lens of this heuristic model, the penultimate section will more rigorously characterize the similarities and differences between termite and human agency in their respective built milieus. The language of phenomenology includes terms such as intentionality, phenom- enon, insight, analysis, sense, not to mention the key term of Edmund Husserl’s manifesto, “the things themselves” to return to . But what does the “things them- selves” properly mean? How come the term is replaced by the “findings” over time? And what are the findings for? The investigation begins by looking at the tricky legacy of the modern turn, trying to clarify ties to past masters, including Francis- co Suárez (...) and Augustine of Hippo . The former, because his influence goes beyond René Descartes reaching undoubtedly Franz Brentano and his students, as well as Martin Heidegger . The latter, because Augustine gives a personal component to the Greek inheritance, marked by the “inward turn .” However, it would not be possible to review the history of thought without the help offered by Jan Patočka's analyses . Patočka discloses the “care” of the Greek philosophers, Plato and Dem- ocritus among others, “for the soul”, we would say with Patočka for “being,” whose sense “does not leave us indifferent” as the leitmotiv of Ancient Philosophy . Nev- ertheless, in his lectures on Plato and Europe, Patočka points out that you must be careful not to confuse the phenomena of things, of existens, with the phenomena of being . Finally, Patočka’s legacy is found in the efforts to reconcile the life-feeling with the modern construction of reality, which means “a radical reconstruction of the naive and natural world of common sense .” In some ways, intentionality is to be revised . J. Arthur Baird is the author of several important books in New Testament studies, his best known perhaps being his Audience Criticism and the Historical Jesus. At his untimely death, he left a nearly complete manuscript, now published here. In this timely and relevant manuscript, Baird offers first a critical introduction to the historical paradigm, pointing out its limitations in terms of tracing the paradigm of New Testament formation. He then traces this development himself, beginning with the starting point of (...) the Holy Word and noting its development through the context, theological interpretation, and application, to its manifestation in the apostles, schools and fathers. He concludes with comments on its current implications. This book represents the last and lasting final thoughts of a scholar who was very much devoted to the importance of the Holy Word. (shrink)

Research questions in mainstream epistemology often take for granted a cognitive internalist picture of the mind. Perhaps this is unsurprising given the seemingly safe presumptions that knowledge entails belief and that the kind of belief that knowledge entails supervenes exclusively on brainbound cognition. It will be argued here that the most plausible version of the entailment thesis holds just that knowledge entails dispositional belief. However, regardless of whether occurrent belief supervenes only as the cognitive internalist permits, we should reject the (...) idea that dispositional belief supervenes only in cognitive internalist-friendly ways. These observations, taken together, reveal two things: first, that a cognitive internalist picture of the mind is much more dispensable in epistemology than has been assumed and, second, that pursuing questions in extended epistemology need not involve any radical departure from the commitments of more traditional epistemological projects. Austria, by the end of the nineteenth century, clearly lagged behind its more developed Western neighbours in matters of intellect and science. The Empire had witnessed a relatively late process of urbanization, bringing also a late development of those liberal habits and values which would seem to be a presupposition of the modern, scientific attitude. It therefore lacked institutions of scientific research of the sort that had been founded in Germany since the time of von Humboldt. On the other hand, (...) as more liberal ways began to be established in Austria - effectively in the second half of the nineteenth century - the desire to enjoy the trappings of a modern enlightened culture made itself felt. The Austrians were not of course in a position to summon forth the means to create reputable institutions and traditions of science in the narrow sense, and this created a vacuum which the theory of a practice so attractively pursued elsewhere could then fill. This essay aims to continue to develop the thesis that the welter of political-economic, social, technological, and subjective transformations that characterized the final decades of the 20th century and the first decades of the 21st necessitate a re-thinking of the relationship between social criticism and the critique of political economy. Herein the focus is directed towards the critique of reification and industrial rationalization as developed in the works of Georg Lukács and Cornelius Castoriadis. Drawing on recent phenomenological and psychological analyses (...) of work within contemporary capitalism, I argue for both the historical obsolescence of the critique of reification as it has been traditionally understood and, consequently, the need for novel conceptual responses to the pressing needs of our time. Whereas previous research has focused on the link between workload and task performance, less is known about the intervening mechanisms influencing this relationship. In the present study, we test the moderating roles of daily recovery and total sleep time in the relationship between work pressure and daily task performance. Using performance and recovery theories, we hypothesized that work pressure relates positively to daily task performance, and that both daily recovery in the form of psychological detachment and relaxation, and total sleep (...) time independently enhance this relationship. Our hypotheses were tested in a 30-day diary study with 110 officer cadets on a cross-Atlantic voyage on a Naval sail ship. The results of multilevel modeling lend support to all three hypotheses. Taken together, our findings suggest that recovery and sleep duration between shifts play a key role in the relationship between daily work pressure and task performance. We discuss the implications of these findings for the stressor-detachment model. (shrink)

This is an attempt to clarify the nature of extreme, or complete, “wokeness” in its modern sense. The central thesis is that it is an inverted form of fascism, and thereby even worse than some of its critics assume. In fact, it is far worse than ordinary fascism whether or not it is correct to see it as an inverted form. As this is a thesis, it is not a definition. I adopt embedded argumentative reasoning processes as satisfying criteria to analyze the expert function in a new socio technical environment that has changed deeply the mechanism and tools to access and to aggregate information. The open access to information and institutional arrangements addressed towards team knowledge could offer other kind of tools to affront the conflict, even its possible benefits. -/- The “crowd expertise” is emerging as an actual possibility and it must be incorporated to affront with conflicts. The very possibility of obtaining knowledge generated by “many minds”, collective wisdoms, brings up a real challenge to the conservative or elitist conception of the masses, because masses now emerge as a smart collective user, with new mechanisms to select and produce quality knowledge. These new collective actions differ deeply from the traditional modes of social organization. A new mass society is emerging now as a hybrid one that breaks some conceptual traditional models, such as Ortega y Gasset’s ones, and induces a structured way of flourishing both new practices and new knowledge with transforming capabilities. He also remarks that prior to 1920 suicide was relatively rare among Hungarian intellectuals, and as a possible explanation he refers to their more intensive political activity. The present paper investigates relations between a society's intellectual life and its general suicidal tendencies. In so doing it takes up a central theme of T. G. O objetivo deste ensaio é examinar a proposta de Sandy Goldberg, segundo a qual há divisão de trabalho epistêmico em certos processos de aquisição de conhecimento – ao menos em se tratando de conhecimento testemunhal. Goldberg propõe mostrar a veracidade desta alegação salientando a nossa dependência epistêmica em relação a outros indivíduos, ou mesmo comunidades inteiras. Nós, então, vamos propor o tratamento de um famoso caso tipo-Gettier que, segundo Gilbert Harman, revelaria algumas dimensões sociais do conhecimento. Por fim, nós esperamos (...) tirar algumas lições da explicação oferecida por Goldberg, dentre as quais a mais importante a ser tomada é que, havendo tal divisão de trabalho epistêmico em processos de aquisição de conhecimento, muitas vezes, não são apenas as propriedades epistêmicas do sujeito que determinam a sua posse de conhecimento ou ignorância, de modo que o conhecimento é mais social do que individual em se tratando de uma ampla gama de casos. Displaying his subtlety of thought and expression as do few other documents, this book of his lectures discusses the moral and cultural implications of developments in modern physics.

From the Hippocratic Oath on, deontological codes and other professional self-regulation mechanisms have been used to legitimize and identify professional groups. New technological challenges and, above all, changes in the socioeconomic environment require adaptable codes which can respond to new demands.We assume that ethical codes for professionals should not simply focus on regulative functions, but must also consider ideological and educative functions. Any adaptations should take into account both contents (values, norms and recommendations) and the drafting process itself.In this article (...) we propose a process for developing a professional ethical code for an official professional association (Colegio Oficial de Ingenieros Industriales de Valencia (COIIV) starting from the philosophical assumptions of discursive ethics but adapting them to critical hermeneutics.Our proposal is based on the Integrity Approach rather than the Compliance Approach. A process aiming to achieve an effective ethical document that fulfils regulative and ideological functions requires a participative, dialogical and reflexive methodology. This process must respond to moral exigencies and demands for efficiency and professional effectiveness.In addition to the methodological proposal we present our experience of producing an ethical code for the industrial engineers’ association in Valencia (Spain) where this methodology was applied, and we evaluate the detected problems and future potential.
