The medical concept of prognosis is analysed into its basic constituents: patient data, medical intervention, outcome, utilities and probabilities; and sources of utility and probability values are discussed. Prognosis cannot be divorced from contemplated medical action, nor from action to be taken by the patient in response to prognostication. Regrettably, the usual decision-theoretic approach ignores this latter aspect. Elicitation of utilities, decision contemplation and prognostic counselling interweave, diagnostics playing a subsidiary role in decision-oriented clinical practice. At times the doctor has (...) grounds for withholding information. As this is known to the patient, prognostic counselling becomes a conflict-prone and rationality-thwarting activity. The meaning of standard phrases such as prognosis of a disease, the prognosis of this patient, the prognosis is unknown, is examined. (shrink)

DNA gyrase, an enzyme unique to prokaryotes, has been implicated in almost all processes that involve DNA. Although efficient inhibitors of this protein have been known for more than 20 years, none of them have enjoyed prolonged pharmaceutical success. It is only recently that the mechanisms of inhibition for some of these classes of drugs have been established unequivocally by X‐ray crystallography. It is hoped that this detailed structural information will assist the design of novel, effective inhibitors of DNA gyrase.

Consistent with the planning–control model, recent fMRI data reveal that the inferior parietal lobe, the frontal lobes, and the basal ganglia are involved in motor planning. Inconsistent with the planning–control model, however, recent behavioral data reveal a spatial repulsion effect, indicating that the visual context surrounding the target can sometimes influence the on-line control of goal-directed action.

Alonso-Meijide and Fiestras-Janeiro (2002, Annals of Operations Research 109, 213–227) proposed a modification of the Banzhaf value for games where a coalition structure is given. In this paper we present a method to compute this value by means of the multilinear extension of the game. A real-world numerical example illustrates the application procedure.

Background: The American Medical Association, the British Medical Association and the Canadian Medical Association have guidelines that specifically discourage physicians from self-prescribing or prescribing to family members, but only the BMA addresses informal prescription requests between colleagues. Objective: To examine the practices of paediatric providers regarding self-prescribing, curbsiding colleagues, and prescribing and refusing to prescribe to friends and family. Methods: 1086 paediatricians listed from the American Academy of Paediatrics 2007 web-based directory were surveyed. Results: 44% of eligible survey respondents returned (...) usable surveys. Almost half of respondents had prescribed for themselves. An equal number had informally requested a prescription from a colleague. Three-quarters stated they had been asked to prescribe a prescription drug for a first-degree or second-degree relative, and 51% had been asked by their spouse. Eighty-six per cent stated that they had refused to write a prescription on at least one occasion for a friend or family member. The following reasons “strongly influenced” their decision to refuse a prescription request: outside of provider’s expertise ; patient’s need for his or her own physician ; not medically indicated ; need for a physical examination. Conclusion: These data confirm that most physicians have engaged in self-prescribing or curbside requests for prescriptions. It can be argued that curbsiding is more morally problematic than self-prescribing because it implicates a third party, and should be discouraged regardless of whether the requester is a colleague, family member or friend. Our age is trying in many ways to deal with the problem of the survival of ethnic minorities. Being confronted with a centralized political power which destroys cultural differences, many of these minorities have become conscious of their originality (which is often hidden away in folklore) and are in search of a new balance with the dominant culture. They try to achieve this new balance by bringing their basic characteristics up to date or even deliberately entering a phase of counter (...) acculturation and adopting an attitude of political liberation much like that of the formerly colonized peoples. The European Clinical Trials Directive (EU 2001; 2001/20/EC) was introduced to improve the efficiency of commercial and academic clinical trials. Concerns have been raised by interested organizations and institutions regarding the potential for negative impact of the Directive on non-commercial European clinical research. Interested researchers within the European Group for Blood and Marrow Transplantation (EBMT) were surveyed to determine whether researcher experiences confirmed this view. Following a pilot study, an internet-based questionnaire was distributed to individuals in key research positions in (...) the European haemopoietic SCT community. Seventy-one usable questionnaires were returned from participants in different EU member states. The results indicate that the perceived impact of the European Clinical Trials Directive has been negative, at least in the research areas of interest to the EBMT. (shrink)

Synchronization is a special case of self-organization in which one can observe close mimicry in behavior of the system components. Synchrony in body movements, autonomic arousal, and EEG activity among human individuals has attracted considerable attention for their possible roles in social interaction. This article is specifically concerned with autonomic synchrony and finding the best model for the dyadic relationships, with regard to both theoretical and empirical accuracy, that could be extrapolated to synchrony levels for groups and teams of three (...) or more people. The four models that are compared in this study have different theoretical origins: the two-variable linear regression function, a three-parameter nonlinear regression function, the logistic map function stated in polynomial form, and the logistic map function stated as an exponential regression structure. The data for this study were electrodermal responses collected from a team of four people engaged in an emergency response simulation that produced 12 dyadic time series. Results shows strong levels of fit between the data and all four models, although there were significant differences among them. Further research directions point toward finding conditions that favor one model over another and exploring other possible nonlinear structures. (shrink)

There is an increasing interest in publications about the sources of meaning in life; books about the art of living are immensely popular. This article discusses whether one of the ancient predecessors of current 'art of living' theories, the Stoa and more particularly Seneca, can be of interest to educators today. Seneca's explicit writings on education are relatively few, but in his letters to his friend Lucilius we find several ideas as to how educators can assist students to become wise (...) and virtuous adults. The main characteristic of the virtuous sage is his ability to maintain tranquillity of mind. While we disagree with the radicalism of Seneca's view on the extirpation of emotions, we have discovered insights that we believe can be a valuable source for educators and students in their reflections on the meaning of education for the business of life. Response to the Commentary on ‘The View from Within’ The numerous commentators to this Special Issue have greatly enhanced its focus and usefulness. We thank them all very sincerely for their efforts. Within the restricted space of this rejoinder we cannot respond in detail to all the issues raised. Instead, we shall concentrate first on some fundamental criticisms.The remaining additions and complementary ideas will only be touched on briefly, merely to see them in perspective. We shall start with our two (...) main critics of the global enterprise, Baars and Nixon. They both offer arguments that our enterprise is misguided, but their arguments fall on two opposite extremes: empirical and hermeneutical. (shrink)

The article recently published in this journal by Dr B A Rix, a member of the Danish Council of Ethics (DCE), was heavily criticised by Dr David Lamb and Mr Christopher Pallis in subsequent commentaries. The editorial column by Professor Raanan Gillon also criticised the position put forward by Rix. In this article I contend that the definition of death put forward by Pallis and Lamb suffers certain philosophical shortcomings, that the position put forward by Rix deserves fuller consideration, and (...) that Rix is not to be dismissed easily.
