As part of a multifactorial approach to address weak incentives for innovative antimicrobial drug development, market entry rewards are an emerging solution. Recently, the Duke-Margolis Center for Health Policy released the Priority Antimicrobial Value and Entry Award proposal, which combines a MER with payment reforms, transitioning from volume-based to “value-based” payments for antimicrobials. Here, the PAVE Award and similar MERs are reviewed, focusing on further refinement and avenues for implementation.

Starting in June 2012, a series of articles in the journal Nature and in the online journals of the Public Library of Science made public the first results of a massive, international collaborative scientific endeavor known as the “Human Microbiome Project” . This project, which is attempting to categorize the vast number of microbiological species and organisms that live in and on the “healthy” human body, raises important questions about what it means to be a whole individual organism, especially if (...) that individual involves a community of some hundreds of trillions of others. Within medical education, there is an increasing need to provide ethics education for learners embarking on service learning. The concept of hospitality, as illustrated in Homer’s classic epic the Odyssey, has the potential to underpin such ethics education. This paper describes the implementation and evaluation of an ethics education session for medical students, built on the concept of hospitality, before they embark on a three-year longitudinal service-learning home visit program. In preparation for their household visits, second-year students at a medical (...) school in Miami, Florida, participate in weekly didactic sessions. One of these classes consisted of a two-hour ethics session, involving a brief introductory large-group lecture, followed by self-administered small-group discussions of a series of case scenarios. An innovative web-based platform called Rise allowed the students to run their own small group activities. To assess the impact on the students’ ability to identify, and on their confidence in handling, ethical dilemmas, we administered pre- and post-session surveys. Students indicated statistically significant improvement in median responses to all questions, for two consecutive years. Students also listed three adjectives, both before and after the session, that they associated with good and bad hosts and guests. The consistency of the terms listed suggests that hospitality is a robust concept for the learners. In our experience, the concept of hospitality provides a robust framework for the students, and the mixed large-group and self-facilitated small-group format offers a valuable tool for ethics education related to service learning in medicine. Our study supports the utility of the concept as a teaching tool, and the format facilitates a session using minimal faculty. The notion of hospitality seems particularly fruitful for various ethical considerations in medicine, even beyond those that involve home visits. (shrink)

The background to this event, and the controversy which followed it, are described in detail in this article, which draws on many hitherto unpublished materials. Searching for God draws upon the traditional categories of systematic theology as it guides readers through the Catholic theological thought process involved in the search for God. This book approaches ethical and legal issues in medicine from the patient's viewpoint and argues that many patients do not want the full burden of decision making that contemporary bioethics has thrust upon them.

Drawing on the empirical premise that attention makes objects look more intense, Ned Block has argued for mental paint, a phenomenal residue that cannot be reduced to what is perceived or represented. If sound, Block's argument would undermine direct realism and representationism, two widely held views about the nature of conscious perception. We argue that Block's argument fails because the empirical premise it is based upon is false. Attending to an object alters its salience, but not its perceived intensity. We (...) also argue that salience should be equated with mental primer, a close cousin of mental paint that reintroduces difficulties for direct realism and representationism. The upshot is that direct realism and representationism are still in trouble, but not for the reason that Block thinks. (shrink)

Many philosophers think that part of what makes an event lucky concerns how probable that event is. In this paper, I argue that an epistemic probability account of luck successfully resists recent arguments that all theories of luck, including probability theories, are subject to counterexample (Hales 2016). I argue that an event is lucky if and only if it is significant and sufficiently improbable. An event is significant when, given some reflection, the subject would regard the event as significant, and (...) the event’s epistemic probability, determined by the subject’s evidence, is the only kind of probability that directly bears on whether or not the event is lucky. I conclude with some lessons that are applicable to probability theorists of luck generally, including those defending non-epistemic probability theories. (shrink)

From 1953 to 1959 Gödel worked on a response to Carnap’s philosophy of mathematics. The drafts display Gödel’s familiarity with Carnap’s position from The Logical Syntax of Language, but they received a dismissive reaction on their eventual, posthumous, publication. Gödel’s two principal points, however, will here be defended. Gödel, though, had wished simply to append a few paragraphs to show that the same arguments apply to Carnap’s later views. Carnap’s position, however, had changed significantly in the intervening years, and to (...) a significant extent, this mature position realizes much of what Gödel himself wanted out of a philosophy of mathematics. Social media is changing social accountability practices. The release of the Panama Papers on April 3, 2016 by the International Consortium of Investigative Journalists (ICIJ) unleashed a tsunami of over 5 million tweets decrying corrupt politicians and tax-avoiding business elites, calling for policy change from governments, and demanding accountability from corporate and private tax avoiders. The current study uses 297,000+ original English-language geo-codable tweets with the hashtags #PanamaGate, #PanamaPapers, or #PanamaLeaks to examine the trajectory of Twitter-based social accountability conversations and (...) the potential for the emergence of a longer-term social accountability user network. We propose that it is the combination of financial inscriptions and evaluative ethical utterances that incite and sustain social accountability conversations and social accountability networks. Financial inscriptions simultaneously remind audiences of both the information event that fomented the initial public reaction and the monetary magnitude of the event. Value-based ethical messaging, in turn, enunciates an ethical stance that simultaneously evaluates existing practices and emphasizes the need for accountability. It is the combining of these two types of messaging that helps to construct and sustain a normative narrative about social accountability. The results illustrate how the repetition and re-working of these two forms of messaging facilitated the construction of a normative narrative that coalesced into a social accountability network which persisted beyond the initial Panama Paper information event and which was re-activated in 2017 when the ICIJ published the Paradise Papers. This paper will deal with three questions regarding Carnap's transition from the position he held at the time of writing Syntax to the doctrines he held during his semantic phase: (1) What was Carnap's attitude towards truth at the time of writing Syntax? (2) What was Carnap's position regarding questions of reference and ontology at the time of writing Syntax? (3) Was Carnap's acceptance of Tarski's analysis of truth and reference detrimental to his philosophical project? Section 1 of this paper (...) will deal with the first of these questions. Special attention will be paid to identifying what it was that prevented Carnap from defining a truth predicate for descriptive languages in Syntax. Section 2 of this paper will deal with the question of Carnap's attitude towards reference and ontology in Syntax. It will be shown that the attempt in Syntax to address ontological questions is seriously defective. Section 3 of this paper addresses the last of the questions posed above. It is argued that in the light of what is established in Sections 1 and 2, Carnap could not have retained the position of Syntax with respect to truth and reference. (shrink)

Quine’s ‘Two Dogmas of Empiricism’ is generally seen as overturning Carnap’s epistemological picture of mathematics and the sciences. However, I wish to stress how this paper grew out of arguments not having anything to do with large-scale epistemological concerns, but ones originally presented against quantified modal logic. Quine thought he could demonstrate the impossibility of adding anything like ordinary quantification to modal logic, but Barcan Marcus did exactly this. In fact, as I will argue, ‘Two Dogmas ...’ can be seen (...) as growing out of a 1947 paper of Quine’s where Barcan Marcus is the explicit target. Later, in certain exchanges with Quine, Barcan Marcus puts forward and defends a consistent and interesting position on matters of ontology and the philosophy of language. This chapter will examine Barcan Marcus’s role in both the debate over analyticity and the debate on (meta-)ontology. (shrink)

Thomas Piketty’s Capital in the Twenty-First Century makes hardly any reference to the ethics of inequality. Surprisingly, this is an omission shared by most of his critics. In this paper I investigate the literature on which he and his reviewers might have drawn and speculate on the reasons why they did not. I outline the four ‘views of society’ and the related issues in moral philosophy that were presented by Michael Schneider in his book on the distribution of wealth. (...) I then summarise the criticisms of Piketty made by those few reviewers who did show some interest in ethical questions and examine the slightly earlier and quite different case against reducing inequality made by one of these critics, N. Gregory Mankiw. I consider the economic, political and social costs of inequality identified in a book-length study of Piketty’s work by Steven Pressman, and conclude by reflecting on the reasons for the widespread neglect of moral philosophy by mainstream economists. There is a curious letter of 24 April 1928, reproduced in Russell’s Autobiography. It is from Russell to Max Newman. It is my thesis that there is a crucial “not” missing from the text and interpretations of the letter. This small point, if it is correct, has a very large impact for clarifying how Russell saw Newman’s challenge to his structural realism according to which all of our empirical knowledge in physics concerns structure alone.

Abstract At the time of The Logical Syntax of Language (Syntax), Quine was, in his own words, a disciple of Carnap’s who read this work page by page as it issued from Ina Carnap’s typewriter. The present paper will show that there were serious problems with how Syntax dealt with ontological claims. These problems were especially pronounced when Carnap attempted to deal with higher order quantification. Carnap, at the time, viewed all talk of reference as being part of the misleading (...) material mode of speech, and as such dismissed, rather than addressed, ontological problems. Central to the analysis in the present paper is the concept of an explication, which was seen by both Carnap and Quine as being of great philosophical importance. It will be shown that the concept of explication played a significant role in how each formulated their mature position on ontology. Both these final positions on ontology can also be seen as a evolving in reaction to Carnap’s flawed handling of ontological matters at the time of Syntax. Carnap, influenced by Tarski’s work on semantics, comes to believe that the concept of reference can be given an acceptable explication, and that by doing so we can see reference to abstract objects as unobjectionable. As a result, Carnap develops a position very different from the one presented in Syntax. Quine strongly rejected the instrumentalism of Syntax, and sought to give an explication of ontological questions that was language independent. In this book Gregory Browne rejects the views of David Hume and the Logical Positivists, and argues that there are necessary factual truths, which include a wide range of truths from many fields of knowledge. Browne argues for the necessity of Newton's Laws and truths about natural kinds, and for the factuality of definitional truths and truths of logic and mathematics. Browne synthesizes the work of Kripke, Putnam, Quine and others, but goes beyond the usual discussions of the meanings (...) and definitions of terms to discuss the references of various kinds of terms, and specifically to develop a theory of kinds, distinguishing 'Deep Kinds' and 'Shallow Kinds' . His theory of Deep Kinds does not accept all of the assumptions commonly associated wtih a theory of natural kinds. (shrink)

There are many wonderful puzzles concerning Principia Mathematica, but none are more striking than those arising from the crisis that befell Whitehead in November of 1910. Volume 1 appeared in December of 1910. Volume 2 on cardinal numbers and Russell's relation arithmetic might have appeared in 1911 but for Whitehead's having halted the printing. He discovered that inferences involving the typically ambiguous notation ‘Nc‘α’ for the cardinal number of α might generate fallacies. When the volume appeared in 1912, it was (...) extensively emended by Whitehead and accompanied by a Prefatory Statement of Symbolic Conventions. This paper endeavors to recover from Whitehead's bad emendations—including his bewildering thesis that since ‘‘α’ is ‘true whenever significant,’ ‘α is to be accepted. It is supposedly a fallacy to apply Modus Ponens and infer Nc‘α from ‘α and‘‘α. (shrink)

For many theologians and philosophers, scientism is among the greatest of intellectual sins. In its most commonly cited form, scientism consists in claiming that science is the only source of real knowledge and, therefore, that what science does not discover does not exist. Because the charge of scientism is frequently levied, it is important to be clear about what exactly is being claimed in its name. I argue that scientism can best be understood as a fallacy, specifically as a kind (...) of category mistake. Being clear about this requires an examination of the relationship of scientism to the question of demarcation between science and nonscience, a question that has potential implications for theology. First, there is considerable reason to be pessimistic about instantaneous destructive uploading's ability to preserve identity or to produce continuations of the original person. Second, there is also good reason to be pessimistic about gradual destructive uploading's ability to preserve identity or produce continuations, since exactly the same issues arise in the context of gradual uploading. Erving Goffman's distinctive contribution to an understanding of others was grounded in his information control and ritual models of the interaction process. This contribution centered on the forms of the interaction order rather than self-other relations as traditionally conceived in phenomenology. Goffman came to phenomenology as a sympathetic but critical outsider who sought resources for the sociological mining of the interaction order. His engagement with phenomenological thinkers (principally Gustav Ichheiser, Jean-Paul Sartre and Alfred Schutz) has to be understood in these (...) terms. The article traces basic differences in analytical focus through a range of phenomenological critiques of Goffman and a comparison of salient aspects of Schutz's and Goffman's writings. While the contrasts have perhaps been overplayed, I conclude that Goffman's thinking about others probably owed more to his pragmatist roots than to his later encounters with phenomenology. (shrink)
