BackgroundEverybody has the right to decide whether to receive specific medical treatment or not and to provide their free, prior and informed consent to do so. As dementia progresses, people with Alzheimer’s dementia (PwAD) can lose their capacity to provide informed consent to complex medical treatment. When the capacity to consent is lost, the autonomy of the affected person can only be guaranteed when an interpretable and valid advance directive exists. Advance directives are not yet common in Germany, and their (...) validity is often questionable. Once the dementia diagnosis has been made, it is assumed to be too late to write an advance directive. One approach used to support the completion of advance directives is ‘Respecting Choices’®—an internationally recognised, evidence-based model of Advance Care Planning (ACP), which, until now, has not been evaluated for the target group of PwAD. This study’s aims include (a) to investigate the proportion of valid advance directives in a memory clinic population of persons with suspected AD, (b) to determine the predictors of valid advance directives, and (c) to examine whether the offer of ACP can increase the proportion of valid advance directives in PwAD.MethodWe intend to recruit at leastN = 250 participants from two memory clinics in 50 consecutive weeks. Of these, the first 25 weeks constitute the baseline phase (no offer of ACP), the following 25 weeks constitute the intervention phase (offer of ACP). The existence and validity of an advance directive will be assessed twice (before and after the memory clinic appointment). Moreover, potential predictors of valid advance directives are assessed.DiscussionThe results of this study will enhance the development of consent procedures for advance directives of PwAD based on the ACP/Respecting Choices (R) approach. BackgroundThe UN Convention on the Rights of Persons with Disabilities, and the reformed guardianship law in Germany, require that persons with a disability, including people with dementia in Alzheimer’s disease (PwAD), are supported in making self-determined decisions. This support is achieved through communication. While content-related communication is a deficit of PwAD, relational aspects of communication are a resource. Research in supported decision-making (SDM) has investigated the effectiveness of different content-related support strategies for PwAD but has only succeeded in improving understanding, (...) which, although one criterion of capacity to consent, is not sufficient to ensure overall capacity to consent. The aim of the ‘spatial intervention study’ of the DECIDE project is to examine an innovative resource-oriented SDM approach that focuses on relational aspects. We hypothesise that talking to PwAD in their familiar home setting (as opposed to a clinical setting) will reduce the complexity of the decision-making process and enhance overall capacity to consent. MethodsPeople with a suspected or confirmed diagnosis of dementia in Alzheimer’s disease will be recruited from two memory clinics (N = 80). We will use a randomised crossover design to investigate the intervention effect of the decision-making place on capacity to consent. Besides reasoning capacity, which is part of overall capacity to consent and will be the primary outcome, various secondary outcomes (e.g., other aspects of capacity to consent, subjective task complexity, decisional conflict) and suspected moderating or mediating variables (e.g., meaning of home, demographic characteristics) will be assessed.DiscussionThe results of the study will be used to develop a new SDM strategy that is based on relational resources for PwAD. If a change in location achieves the anticipated improvement in capacity to consent, future research should focus on implementing this SDM strategy in a cost-effective manner in clinical practice.Trial registration: DRKS00030799. Prompted by recent recognitions of the omnipresence of horizontal gene transfer among microbial species and the associated emphasis on exchange, rather than isolation, as the driving force of evolution, this essay will reflect on hybridization as one of the central concerns of nineteenth-century biology. I will argue that an emphasis on horizontal exchange was already endorsed by ‘biology’ when it came into being around 1800 and was brought to full fruition with the emergence of genetics in 1900. The true revolution (...) in nineteenth-century life sciences, I maintain, consisted in a fundamental shift in ontology, which eroded the boundaries between individual and species, and allowed biologists to move up and down the scale of organic complexity. Life became a property extending both ‘downwards’, to the parts that organisms were composed of, as well as ‘upwards’, to the collective entities constituted by the relations of exchange and interaction that organisms engage in to reproduce. This mode of thinking was crystallized by Gregor Mendel and consolidated in the late nineteenth-century conjunction of biochemistry, microbiology and breeding in agro-industrial settings. This conjunction and its implications are especially exemplified by Wilhelm Johannsen’s and Martinus Beijerinck’s work on pure lines and cultures. An understanding of the subsequent constraints imposed by the evolutionary synthesis of the twentieth century on models of genetic systems may require us to rethink the history of biology and displace Darwin’s theory of natural selection from that history’s centre. The cell is not only the structural, physiological, and developmental unit of life, but also the reproductive one. So far, however, this aspect of the cell has received little attention from historians and philosophers of biology. I will argue that cell theory had far-reaching consequences for how biologists conceptualized the reproductive relationships between germs and adult organisms. Cell theory, as formulated by Theodor Schwann in 1839, implied that this relationship was a specific and lawful one, that is, that germs of (...) a certain kind, all else being equal, would produce adult organisms of the same kind, and vice versa. Questions of preformation and epigenesis took on a new meaning under this presupposition. The question then became one of whether cells could be considered as autonomous agents producing adult organisms of a given species, or whether they were the product of external, organizing forces and thus only a stage in the development of the whole organism. This question became an important issue for nineteenth-century biology. As I will demonstrate, it was the view of cells as autonomous agents which helped both Charles Darwin and Gregor Mendel to think of inheritance as a lawful process. (shrink)

Summary In 1979, Robert C. Olby published an article titled ?Mendel no Mendelian??, in which he questioned commonly held views that Gregor Mendel (1822?1884) laid the foundations for modern genetics. According to Olby, and other historians of science who have since followed him, Mendel worked within the tradition of so-called hybridists, who were interested in the evolutionary role of hybrids rather than in laws of inheritance. We propose instead to view the hybridist tradition as an experimental programme characterized by (...) a dynamic development that inadvertently led to a focus on the inheritance of individual traits. Through a careful analysis of publications on hybridization by Carl Linnaeus (1707?1778), Joseph Gottlieb Koelreuter (1733?1806), Carl Friedrich Gärtner (1772?1850), and finally Mendel himself, we will show that this development consisted in repeated reclassifications of hybrids to accommodate anomalies, which in the end allowed Mendel to draw analogies between whole organisms, individual traits, and ?elements? contained in reproductive cells. Mendel's achievement was a product of normal science, and yet a revolutionary step forward. This also explains why, in 1900, when the report he gave on his experiments was ?rediscovered?, Mendel could be read as a ?Mendelian? (shrink)

When Yogācāra specialists take on the task of trying to introduce the tradition to newcomers and nonspecialists, whether it be in a book-length project, or an article in a reference work, they inevitably choose different points of departure, depending on their particular approach to understanding Yogācāra, and Buddhism in general. Some will start with the explanation of the eight consciousnesses; some will start with the four parts of cognition; some will start with the three natures; others will start with the (...) doctrine of no-self, and so on. There is no special need to try to assess whether one of these approaches is better than the other, for indeed, in the vast and complex system that is known as Yogācāra, all of these different approaches and categories are ultimately tied into each other, and thus, starting with any one of them, one can eventually enter into all of the rest. Another approach, partially utilized in a recent introductory Yogācāra book by the Japanese Yogācāra specialist Yokoyama Kōitsu — Yasashii yuishiki ( "Easy Consciousness-Only" ), would be to take the two hindrances as a point of departure for an introduction to the Yogācāra soteriological system. This is also a viable approach, since there is nothing within the Yogācāra system that cannot be tied into or developed from the two basic categories of problems that Buddhist practitioners must work their way through: (1) afflictive/emotive disorders and (2) distorted apprehensions of reality. It is a commonplace that emotions are characteristically passive. As we ordinarily think of them, emotions are ways in which we are acted upon, that is, moved or affected by aspects of our environment. Moreover, we have no voluntary control over whether we feel them. In this paper, I call attention to a much-neglected respect in which emotions are active, which is no less central to our pretheoretical concept of them. That is, in having emotions, we are engaged with the (...) world insofar as we respond to aspects of our environment. In this context, to say that an emotion is a response to x is tantamount to saying that x is a reason for which we have it. Elaborating this claim in light of a historically prominent conception of the active/passive distinction, I will argue that emotions are a form of spontaneity in virtue of their responsive character and contrast in this respect with perceptions, which are fundamentally receptive. While this proposal is prima facie opposed to the ordinary image of emotions as passive, I will show that it actually allows us to make proper sense of it. (shrink)

Der Verzicht auf absolut gültige Erkenntnis, heute in den Naturwissenschaften beinahe schon selbstverständlich, ist erst jüngeren Datums. Noch im vergangenen Jahrhundert zweifelte die experimentelle Forschung kaum an der vollkommenen Begreifbarkeit der Welt. Diesen Wandel zu erkunden und aufzuzeigen ist Thema der vorliegenden Studie. Der erste Teil präsentiert verschiedene Typen neuzeitlicher und moderner Wissenschaftsauffassungen von Galilei über Newton bis hin zu Kant. Im zweiten Teil werden Entwicklung und Wandel der Wissenschafts- und Naturauffassung bei Helmholtz (1821-1895) erstmals mittels detaillierter Textanalysen einer umfassenden (...) Rekonstruktion unterzogen. Die Relativierung des Wahrheitsanspruchs erlaubt es Helmholtz, seine Naturauffassung trotz der antimechanistischen Kritik innerhalb der Physik, die im letzten Viertel des vergangenen Jahrhunderts laut wurde, als Hypothese aufrechtzuerhalten. Auch gewährt die Studie eine neue Sichtweise des Verhältnisses zwischen Helmholtz und Kant, das in der Vergangenheit kontroverse Beurteilungen erfuhr. (shrink)

Through two exchanges in this journal, a type of relativism has been advanced by a group of authors from Loughborough University with a view to demolishing what they see as ‘bottom line’ arguments for critical realism in the social sciences. Jauntily dismissing realism, they also soberly disown the supposed ‘extreme’ consequences that some realists insist follow naturally from relativist conceptions of social inquiry. In this article, I contest the Loughborough team’s arguments. Their presentation of relativism itself can be reconstructed in (...) at least three different ways, none of which suffices to undermine a generally realist account of explanatory endeavour. The most productive note in the critics’ conceptual register highlights the utility of rhetorical analysis; yet this can proceed effectively only if it is neutral with respect to different epistemic and ontological stances. The most productive thematic possibility raised in their articles, and shared with several other recent discussions, signals that realism and relativism may not be anything like fixed and opposed stances; yet the polemical thrust of their anti-realism compromises this insight. John Ellis McTaggart defended an idealistic view of time in the tradition of Hegel and Bradley. His famous paper makes two independent claims (McTaggart1908): First, time is a complex conception with two different logical roots. Second, time is unreal. To reject the second claim seems to commit to the first one, i.e., to a pluralistic account of time. We compare McTaggarts views to the most important concepts of time investigated in physics, neurobiology, and philosophical phenomenology. They indicate that a unique, (...) reductionist account of time is far from being plausible, even though too many conceptions of time may seem unsatisfactory from an ontological point of view. Nach einer kurzen Erinnerung an einige von Keplers Hauptwerken, in denen traditionelle und moderne Elemente eingehen (Abschnitt 1), wird zwei Beispielen die Differenz zwischen diesen beiden Elementen näher untersucht. Das erste Beispiel, Keplers Naturbegriff, dient zur Diskussion der Kritik qualitativer Unterscheidungen. Hierbei stehen Keplers Verhältnis zur aristotelischen Naturauffassung und die Relevanz dieser Relation für die moderne Wissenschaftsauffassung im Mittelpunkt (Abschnitt 2). Das andere Beispiel befasst sich mit dem absoluten Wahrheitsanspruch von Keplers Wissenschaft und rückt damit exemplarisch eine Differenz zur modernen (...) Wissenschaftsauffassung in den Vordergrund (Abschnitt 3). Anschließend werden umfassender traditionelle Elemente der frühneuzeitlichen Wissenschaft, wie sie Kepler vertrat, dem modernen Wissenschaftsverständnis gegenübergestellt. Nachdem damit die Entfernung Keplers zur Gegenwart gleichsam maximiert ist, wende ich mich den Wissenschaftsauffassungen von Wolfgang Pauli und Werner Heisenberg zu, die in bemerkenswerter Nähe zu Keplers vormodernen Ansichten stehen und doch ganz im Kontext der Moderne entwickelt wurden (Abschnitt 4). Obwohl also in jüngster Zeit ganz differente Einstellungen zu Keplers Verhältnis zur modernen Wissenschaft vertreten wurden, lässt sich doch eine Tendenz zur Abstandsvergrößerung in dieser Relation ausmachen (Abschnitt 5). Persistent disagreement is perhaps one of the defining features of modern liberal democracy. Disagreement, though, is not merely a philosophical nuisance, but a source of friction and conflict. The goal of this work is to inquire, whether we can conceive of a political order that might be judged superior to our current modus vivendi arrangements for settling conflict from a diverse set of normative standards or perspectives. In this essay we develop the concept of Polycentric Democracy.
