Background: Newer “closed-loop” neurostimulation devices in development could, in theory, induce changes to patients’ personalities and self-perceptions. Empirically, however, only limited data of patient and family experiences exist. Responsive neurostimulation (RNS) as a treatment for refractory epilepsy is the first approved and commercially available closed-loop brain stimulation system in clinical practice, presenting an opportunity to observe how conceptual neuroethical concerns manifest in clinical treatment. Methods: We conducted ethnographic research at a single academic medical center with an active RNS treatment program (...) and collected data via direct observation of clinic visits and in-depth interviews with 12 patients and their caregivers. We used deductive and inductive analyses to identify the relationship between these devices and patient changes in personality and self-perception. Results: Participants generally did not attribute changes in patients’ personalities or self-perception to implantation of or stimulation using RNS. They did report that RNS affected patients’ experiences and conceptions of illness. In particular, the capacity to store and display electrophysiological data produced a common frame of reference and a shared vocabulary among patients and clinicians .Discussion: Empirical experiences of a clinical population being treated with closed-loop neuromodulation do not corroborate theoretical concerns about RNS devices described by neuroethicists and technology developers. However, closed-loop devices demonstrated an ability to change illness experiences. Even without altering identify and self-perception, they provided new cultural tools and metaphors for conceiving of epilepsy as an illness and of the process of diagnosis and treatment. These findings call attention to the need to situate neuroethical concerns in the broader contexts of patients’ illness experiences and social circumstances. (shrink)

Drawing on previous models of anxiety, intermediate memory, the positive symptoms of schizophrenia, and goal-directed behaviour, a neuropsychological hypothesis is proposed for the generation of the contents of consciousness. It is suggested that these correspond to the outputs of a comparator that, on a moment-by-moment basis, compares the current state of the organism's perceptual world with a predicted state. An outline is given of the information-processing functions of the comparator system and of the neural systems which mediate them. The hypothesis (...) appears to be able to account for a number of key features of the contents of consciousness. However, it is argued that neitherthis nor any existing comparable hypothesis is yet able to explain why the brain should generate conscious experience of any kind at all. (shrink)

BackgroundBurnout is being experienced by medical students, residents, and practicing physicians at significant rates. Higher levels of Hardiness and Emotional Intelligence may protect individuals against burnout symptoms. Previous studies have shown both Hardiness and Emotional IntelIigence protect against detrimental effects of stress and can be adapted through training; however, there is limited research on how training programs affect both simultaneously. Therefore, the objective of this study was to define the association of Hardiness and Emotional Intelligence and their potential improvement through (...) hyper realistic immersion simulation training in military medical students.MethodsParticipants in this study consisted of 68 second year medical students representing five medical schools who were concurrently enrolled in the United States military scholarship program. During a six day hyper-realistic surgical simulation training course, students rotated through different roles of a medical team and responded to several mass-casualty scenarios. Hardiness and Emotional Intelligence were assessed using the Hardiness Resilience Gauge (HRG) and the Emotional Quotient Inventory (EQ-I 2.0) respectively, at two time points: on arrival (pre-event) and after completion of the course (post-event).ResultsHardiness and Emotional Intelligence scores and sub scores consistently improved from pre-event to post-event assessments. No difference in training benefit was observed between genders but differences were observed by age where age was more often associated with Emotional Intelligence. In addition, factor analysis indicated that the HRG and EQ-I 2.0 assessment tools measured predominately different traits although they share some commonalities in some components.ConclusionThis study indicates that Hardiness and Emotional Intelligence scores can be improved through immersion training in military medical students. Results from this study support the use of training course interventions and prompt the need for long term evaluation of improvement strategies on mitigating burnout symptoms. (shrink)

The theme of this year’s Spindel Conference was Social Ontologies of Race. This editorial introduction serves as both a general introduction to the topic of racial ontology and an introduction to this volume’s contributions. I will first explain some central ideas for discussions of ontology in general. I will then make some basic taxonomic distinctions common to discussions of racial ontology and suggest some clarifications. I will then go on to discuss the five contributions to this volume.

This paper argues that, at least in the context of employment, we should reconsider the applicability of the dominant framework in the contemporary literature on exploitation, which views exploitation as a micro-level moral wrong. I present a novel argument showing that these micro-level theories share commitments inconsistent with taking exploitation seriously as a moral wrong. Given the difficulties these theories face, I argue that we should pursue a structural theory of exploitation, and I give a brief sketch of what such (...) a theory might look like. (shrink)

Understanding the nature of errors in a simple, rule‐based task—programming a VCR—required analyzing the interactions among human cognition, the artifact, and the task. This analysis was guided by least‐effort principles and yielded a control structure that combined a rule hierarchy task‐to‐device with display‐based difference‐reduction. A model based on this analysis was used to trace action protocols collected from participants as they programmed a simulated VCR. Trials that ended without success (the show was not correctly programmed) were interrogated to yield insights (...) regarding problems in acquiring the control structure. For successful trials (the show was correctly programmed), steps that the model would make were categorized as matches to the model; steps that the model would not make were violations of the model. The model was able to trace the vast majority of correct keystrokes and yielded a business‐as‐usual account of the detection and correction of errors. Violations of the model fell into one of two fundamental categories. The model provided insights into certain subcategories of errors; whereas, regularities within other subcategories of error suggested limitations to the model. Although errors were rare when compared to the total number of correct actions, they were important. Errors were made on 4% of the keypresses that, if not detected, would have prevented two‐thirds of the shows from being successfully recorded. A misprogrammed show is a minor annoyance to the user. However, devices with the approximate complexity of a VCR are ubiquitous and have found their way into emergency rooms, airplane cockpits, power plants, and so on. Errors of ignorance may be reduced by training; however, errors in the routine performance of skilled users can only be reduced by design. (shrink)

What does unethical behavior look like in everyday professional practice, and how might it become the accepted norm? Examinations of unethical behavior often focus on failures of individual morality or on psychological blind spots, yet unethical behaviors are generated and performed through social interactions across professional practices rather than by individual actors alone. This shifts the focus of behavioral ethics research beyond the laboratory exploring motivation and cognition and into the organizations and professions where unethical behavior is motivated, justified, enabled (...) and supported in specific social contexts. For instance, when pharmaceutical firms fund academic research, how do the funding arrangements intersect with individual behaviors that are typical in academic research? In turn, how do academic-industry interactions affect scientific norms, particularly those involving compliance with rules, regulations, and ethical codes of conduct? A social organization approach to ethics allows us not only to examine ethics in practice, but also to tease apart unethical behaviors that might operate on an unreflective level and become accepted as “just the way it is.”. Prompted by a 2019 essay by Jonathan Moreno in the Hastings Center Report, the Center's board of directors undertook a careful examination of the name of its preeiminent award, the Henry Knowles Beecher Award, which has been given to twenty‐nine individuals who have made lifetime contributions to bioethics. citing new research that revealed that Beecher's earlier experimentation on drugs had involved nonconsenting adults, Moreno urged the Center to reevaluate honoring Beecher through this award. After reviewing the relevant published evidence and (...) holding extensive discussions, the board decided to rename the award. We also took the opportunity to reframe the award to emphasize quality of scholarship and impact on thinking, practice, and policy rather than lifetime achievement. The new name—the Founders’ Award of The Hastings Center—reflects our desire to honor not only the Center's cofounders, Daniel Callahan and Willard Gaylin, but also others who helped found the field of bioethics. With great pride, the Center has announced that Patricia King of the Georgetown University Law Center is the 2021 recipient of the Founders’ Award. A distinguished civil rights lawyer, King was instrumental in ensuring justice as one of the three principles of The Belmont Report. (shrink)

In Alan Bennett’s play The History Boys, Irwin, a sixth-form history tutor destined for a media career (based, it is rumored, on that specialist in historical controversy Niall Ferguson) sets out his views on how a difficult change in the law that will affect individual rights should be dealt with. The tactic Irwin advocates is for the Government to insist that the Bill, rather than reducing the liberty of the subject “amplifies it.” The use of paradox, notes Irwin, “works well (...) . . . the loss of liberty is the price we pay for freedom type thing” (Bennett 2004, 3). Recent work in science and technological studies has provided a clearer understanding of the way in which science functions in society and the interconnectedness among different strands of science, policy, economy and environment. It is well acknowledged that a different way of thinking is required in order to address problems facing the global community, particularly in relation to issues of risk and uncertainty, which affect humanity as a whole. However, approaches to education in science tend to perpetuate an outmoded way (...) of thinking that is incommensurable with preparing individuals for participation and decision-making in an uncertain, complex world. Drawing on experiences of interdisciplinary dialogue and practice in a higher education context, this book illustrates how reformulating the agenda in science and technology can have a revolutionary impact on learning and teaching in the classroom at all levels. Taking Habermas’s Die nachholende Revolution as a foil, I contend that in his discussions of 1989, Habermas has misunderstood the nature of the anti-Communist revolutions. Comparing them to his writings on the public sphere and the student protest movements in Germany, I argue that the revolutions do not represent the triumph of capitalism anymore than they represent the triumph of Western democracy. Calling the events catch-up revolutions is to frame the events as the expansion of modernity and nothing more. Rather, (...) the revolutions show that the revolutionaries in Eastern Europe were grappling with the same problem, namely the control of technical subsystems, that the students and revolutionaries of 1968 were. Viewed in the light of Habermas’s writings from the 1960s, we end up better understanding the twin extremes of capitalism and bureaucratic-totalitarianism. (shrink)

The advent and widespread adoption of modern technology has impacted our society in a significant and ubiquitous manner. I argue that our dependence on modern technology, specifically, has prompted a loss of human autonomy that corresponds directly to its advancement. I argue that this anti-reciprocal phenomenon is self-instituted. In this sense, autonomy is not lost like ones wallet or car keys, but rather handed over to modern technology in exchange for the streamlined processes and conveniences it promises. -/- I arrive (...) at this conclusion through exploring the research and findings of other thinkers situated in the field of philosophy of technology and the analyses made by other contemporary authors. I also provide my own analysis incorporating relevant phenomena that serve to validate and solidify my arguments. -/- For as much talk of technology that comes part and parcel with a work of this nature, I suggest something quite divergent from its leading critics. It is true that modern technology impacts nearly every aspect of our lives today, and in many ways, its influence defines the very way we have evolved as a society. In its use, there can be seen a wide spectrum of advocacy from philia to phobia and everything between. Yet, I recognize that the one common factor in the invention, adoption, advancement, and widespread utilization of modern technology is a human inventor, adopter, advancer, and end user. It is my contention that the blame can rest not in a thing for the phenomenon of human dependence and loss of autonomy. To truly understand the way modern technology affects us, we must look upon ourselves. (shrink)

In this paper I investigate some characteristics of anomalous or deviant belief systems, such belief systems which include the extraordinary diversity of paranormal claims. It is frequently suggested that the appropriate attitude to adopt to such belief systems is one of scepticism—and that is a conclusion which I will not dispute. However I also suggest that deviant belief systems have characteristics which reveal much of interest about human psychology and the way that we try to make sense of the world.

This article tackles caste-based poverty by a focus on the position of Dalit women in India. Of 200 million Dalits, nearly 50% are women, often referred to a ‘thrice Dalit’, as they suffer from the triple oppressions of poverty, being female and being female Dalits. They are frequently let down by both the Dalit movement itself as well as the women’s movement in India that focuses more on social problems like dowry deaths—more relevant for caste women and not those outside (...) the caste system. Many Dalit women are denied access to education, to meaningful employment, health provision and are the first to suffer the negative effects of globalization. Access to upper caste wells is forbidden. Worst of all, Dalit women are exposed to many forms of violence and are frequently raped as a way to humiliate Dalit men. The degrading work of ‘scavenging’—removing human excrement-falls mostly on Dalit women, since men are more likely to be ‘upwardly-mobile’. Despite all of this, a new strength now emerges in challenging caste boundaries, contributing to self esteem and a stronger sense of identity. The strong spirituality of Dalit women has sustained strength through songs and stories, and in some cases by subverting patriarchy through ironically re-shaping traditional myths. The article ends by suggesting forms of action from Church, society and feminist theology to show solidarity and effect social change for Dalit women. (shrink)

This article combines my journey in Feminist Theology from Liberation to Reconciliation, with a deep appreciation of the late Catherine Halkes, a great influence in my life and a European foremother for Feminist Theology. The emergence of Feminist Theology globally was based on the contextual struggle for justice in society and religion. This evoked new awareness, academic disciplines, culture and spirituality, and in an eccesial dimension inspired the Woman Church movement. Evaluating progress and acknowledging tensions, it is time to take (...) new directions – including visions of liberation that aim for peace with justice, flourishing and ecological wellbeing – contained appropriately by the concept ‘praxis of reconciliation’.
