Der finanzielle Druck auf die Krankenhäuser in Deutschland führt zu einer Ökonomisierung medizinischer Entscheidungen, die die Qualität der Patientenversorgung beeinträchtigt und das Gesundheitspersonal erheblich belastet. Der vorliegende Beitrag untersucht, welche Möglichkeiten ein organisationsethischer Ansatz bietet, den Herausforderungen durch die Ökonomisierung zu begegnen. Ausgewählte empirische Befunde sollen zunächst verdeutlichen, welche Auswirkungen die Ökonomisierung auf die Patientenversorgung und das Personal in den Krankenhäusern hat. Zudem liefern sie erste Hinweise auf mögliche Handlungsspielräume für die Krankenhäuser. Dabei wird deutlich, dass die Ökonomisierung einen organisationsethischen (...) Ansatz erfordert, der die Handlungsspielräume auf der Mesoebene, d. h. auf der Ebene der einzelnen Krankenhäuser nutzt. Der organisationsethische Ansatz wird in seinen Grundzügen vorgestellt und dann im Sinne eines Wertemanagements als Lösungsperspektive für die Herausforderungen der Ökonomisierung konkretisiert. Abschließend entwickelt der Beitrag Perspektiven, wie klinische Ethikkomitees mit organisationsethischer Arbeit den negativen Auswirkungen der Ökonomisierung auf die Patientenversorgung und das Personal im Krankenhaus entgegenwirken können. "The actuality of the revolution: this is the core fo Lenin's thought and his decisive link with Marx."This essay on Lenin, which appeared in 1924, was intended to head off the massive criticism leveled at Lukacs History and Class Consciousness by Communist Party leadership. It was a period in which Lukacs was decisively influenced by Lenin and by Rosa Luxemburg, and his intellectual development proceeded concretely toward a political interpretation of history and of literature.In a postscript Lukacs remains essentially unchanged (...) in his view of Lenin as a practitioner whose theoretical superiority lay in his ability to assess the sociohistorical uniqueness of any given situation that required action. Looking back, Lukacs regards the book as a document of the mid-twenties--of how a number of Marxists of the period saw Lenin's personality and mission and his place in world events. Ideas in the book were determined by the concepts of the period, its prejudices, illusions, and extravagances. Nevertheless, the book established certain spiritual verities in perceiving Lenin as the active-practical sage who had a skillfull tactical grasp of realpolitik which was neither empirical nor dogmatic but the culmination of a theoretical attitude. "His life was one of permanent action, of continuous struggle in a world in which he was profoundly convinced that there was no situation without a solution, for himself or his opponents. The leitmotiv of his life was accordingly: always be armed ready for action--for correct action." Lukacs further notes that an essential dimension of Lenin's activism was unceasingly self-education and constant openness to the lessons of experience.Lukacs also emphasizes a number of points in the book that remain methodologically valid, including criticisms of Lenin's behavior which were implicit and accurate critiques of Stalin's later development and the increasing bureaucratization and mechanization of the party. Welche Rolle spielen die Begriffe des Zwecks und der Funktion für die Biologie und die Bestimmung ihres Grundbegriffs, des Organismus?

In this paper, we develop an understanding of recognition in terms of individuals’ capacity for conflict. Our goal is to overcome various shortcomings that can be found in both the positive and negative conceptions of recognition. We start by analyzing paradigmatic instances of such conceptions—namely, those put forward by Axel Honneth and Judith Butler. We do so in order to show how both positions are inadequate in their elaborations of recognition in an analogous way: Both fail to make intelligible the (...) fundamental nexus between relations of recognition and individuals’ capacity for conflict. We then move on to reconsider aspects of Hegel's view of recognition—ones that, from our viewpoint, have been unjustly neglected in the debate about recognition: his focus on the constitution of relations of recognition in conflict and on the status of being an author of acts of recognition. On this basis, we then spell out in a more systematic way what we take to be a more convincing conception of recognition. This puts us in the position to gesture at some consequences of this conception in practical contexts, above all with regard to the justification, role and structure of political institutions. Trust constitutes a fundamental strategy to deal with risks and uncertainty in complex societies. In line with the vast literature stressing the importance of trust in doctor–patient relationships, trust is therefore regularly suggested as a way of dealing with the risks of medical artificial intelligence (AI). Yet, this approach has come under charge from different angles. At least two lines of thought can be distinguished: (1) that trusting AI is conceptually confused, that is, that we cannot trust AI; and (2) (...) that it is also dangerous, that is, that we should not trust AI—particularly if the stakes are as high as they routinely are in medicine. In this paper, we aim to defend a notion of trust in the context of medical AI against both charges. To do so, we highlight the technically mediated intentions manifest in AI systems, rendering trust a conceptually plausible stance for dealing with them. Based on literature from human–robot interactions, psychology and sociology, we then propose a novel model to analyse notions of trust, distinguishing between three aspects: reliability, competence, and intentions. We discuss each aspect and make suggestions regarding how medical AI may become worthy of our trust. (shrink)

In this paper, we approach the idea of group cognition from the perspective of the “extended mind” thesis, as a special case of the more general claim that systems larger than the individual human, but containing that human, are capable of cognition (Clark, 2008; Clark & Chalmers, 1998). Instead of deliberating about “the mark of the cognitive” (Adams & Aizawa, 2008), our discussion of group cognition is tied to particular cognitive capacities. We review recent studies of group problem-solving and group (...) memory which reveal that specific cognitive capacities that are commonly ascribed to individuals are also aptly ascribed at the level of groups. These case studies show how dense interactions among people within a group lead to both similarity-inducing and differentiating dynamics that affect the group's ability to solve problems. This supports our claim that groups have organization-dependent cognitive capacities that go beyond the simple aggregation of the cognitive capacities of individuals. Group cognition is thus an emergent phenomenon in the sense of Wimsatt (1986). We further argue that anybody who rejects our strategy for showing that cognitive properties can be instantiated at multiple levels in the organizational hierarchy on a priori grounds is a “demergentist,” and thus incurs the burden of proof for explaining why cognitive properties are “stuck” at a certain level of organizational structure. Finally, we show that our analysis of group cognition escapes the “coupling-constitution” charge that has been leveled against the extended mind thesis (Adams & Aizawa, 2008). (shrink)

In this paper we consider two major issues: conceptual–experimental approaches to the self, and the neuroanatomical substrate of the self. We distinguish content- and processed-based concepts of the self that entail different experimental strategies, and anatomically, we investigate the concept of midline structures in further detail and present a novel view on the anatomy of an integrated subcortical–cortical midline system. Presenting meta-analytic evidence, we show that the anterior paralimbic, e.g. midline, regions do indeed seem to be specific for self-specific stimuli. (...) We conclude that future investigation of the self need to develop novel concepts that are more empirically plausible than those currently in use. Different concepts of self will require novel experimental designs that include, for example, the brain’s resting state activity as an independent variable. Modifications of both conceptual and anatomical dimensions will allow an empirically more plausible account of the relationship between brain and self. (shrink)

Rudolf Carnap’s mature work on the logical reconstruction of scientific theories consists of two components. The first is the elimination of the theoretical vocabulary of a theory in terms of its Ramsification. The second is the reintroduction of the theoretical terms through explicit definitions in a language containing an epsilon operator. This paper investigates Carnap’s epsilon-reconstruction of theories in the context of pure mathematics. The main objective here is twofold: first, to specify the epsilon logic underlying his suggested definition of (...) theoretical terms and a suitable choice semantics for it. Second, to analyze whether Carnap’s approach is compatible with a structuralist conception of mathematics. (shrink)

The article presents the conceptual groundwork for an understanding of the essentially improvisational dimension of human rationality. It aims to clarify how we should think about important concepts pertinent to central aspects of human practices, namely, the concepts of improvisation, normativity, habit, and freedom. In order to understand the sense in which human practices are essentially improvisational, it is first necessary to criticize misconceptions about improvisation as lack of preparation and creatio ex nihilo. Second, it is necessary to solve the (...) theoretical problems that derive from misunderstandings concerning the notions of normativity, habit, and freedom – misunderstandings that revolve around the idea that rationality is a form that is developed out of itself and thus works in a way similar to algorithms. One can only make sense of normativity, habit, and freedom if one understands that they all involve conflictual relationships with the world and with others, which in turn enables one to adequately take into account their constitutive connection to improvisation, properly understood. In outlining these conceptual connections, we want to prepare the foundations for an explanation of rational practices as improvisational practices. The article concludes by stating that human rational life is improvisatory because the conditions of human practice arise out of practice itself. (shrink)

In moral epistemology, the method of reflective equilibrium is often characterized in terms of intuitions or understood as a method for justifying intuitions. An analysis of reflective equilibrium and current theories of moral intuitions reveals that this picture is problematic. Reflective equilibrium cannot be adequately characterized in terms of intuitions. Although the method presupposes that we have initially credible commitments, it does not presuppose that they are intuitions. Nonetheless, intuitions can enter the process of developing a reflective equilibrium and, if (...) the process is successful, be justified. Since the method of reflective equilibrium does not essentially involve intuitions, it does not constitute a form of intuitionism in any substantial sense. It may be classified as intuitionist only in the minimal sense of not reducing justification to a matter of inference relations alone. (shrink)

Georg Wilhelm Friedrich Hegel was not only a great philosopher but a great historian of philosophy. He invented the idea of the philosophical tradition as a discussion among philosophers extending over centuries centering on a few main philosophical problems. The conceptual scheme, widely accepted in histories of philosophy, emerged in Hegel's lectures at the same time as German idealism itself. This new abridgment of a well-known edition makes the main insights of Hegel's famous Lectures on the History of Philosophy (...) widely available in an inexpensive edition. In this student-oriented text, Professor Tom Rockmore selects the most significant material in a one-volume abridgment. A short introduction explains the purpose and principles of the selections and assesses the continued importance of the work. The exact relationship between qualia and the function of the brain remains elusive. The present approach focuses on the linkage between the neural mechanisms of the brain and the phenomenological and epistemological mechanisms of qualia. It is hypothesized that distinct characteristics of the ventral prefrontal cortical function may be crucial for the generation of these phenomenological and epistemological mechanisms this is reflected in the so-called 'neurophenomenological hypothesis'. The 'phenomenological—qualitative' character of qualia may be related with an early activation in the (...) ventromedial prefrontal cortex . The experience of 'presence' in qualia may be accounted for by a co-activation in both VMPFC and hippocampus and a concurrent deactivation in the ventrolateral prefrontal cortex and posterior cingulate. Past and future temporal dimensions may therefore be integrated within the experience of events, which in turn may account for the phenomenal characteristics of 'presence' and thus 'phenomenal time'. 'Non-structural homogeneity' in the experience of qualia may be accounted for by either 'supramodal character' or 'modality unspecifity' of >the VMPFC. The 'heterogenous' stimuli can therefore be included and integrated within one 'homogenous' event as experienced in qualia. Finally, 'transparency' in the experience of qualia may be related with the 'reciprocal suppression' between VMPFC and VLPFC. Simultaneous cognitive processing during the experience of qualia may be suppressed and may consequently account for their transparent character. Due to several methodological limitations, these 'neurophenomenological hypotheses' must be considered as preliminary. However, they may nevertheless serve as a starting point for the development of a more elaborate neuroscientific hypothesis of qualia in the future. (shrink)

There is a long-standing debate whether propositions, sentences, statements or utterances provide an answer to the question of what objects logical formulas stand for. Based on the traditional understanding of logic as a science of valid arguments, this question is firstly framed more exactly, making explicit that it calls not only for identifying some class of objects, but also for explaining their relationship to ordinary language utterances. It is then argued that there are strong arguments against the proposals commonly put (...) forward in the debate. The core of the problem is that an informative account of the objects formulas stand for presupposes a theory of formalization; that is, a theory that explains what formulas may adequately substitute for an inference in proofs of validity. Although such theories are still subject to research, some consequences can be drawn from an analysis of the reasons why the common accounts featuring sentences, propositions or utterances fail. Theories of formalization cannot refer to utterances qua expressions of propositions; instead they may refer to sentences and rely on additional information about linguistic structure and pragmatic context. The term ``neurophilosophy'' is often used either implicitly or explicitly for characterizing the investigation of philosophical theories in relation to neuroscientific hypotheses. The exact methodological principles and systematic rules for a linkage between philosophical theories and neuroscientific hypothesis, however, remain to be clarified. The present contribution focuses on these principles, as well as on the relation between ontology and epistemology and the characterization of hypothesis in neurophilosophy. Principles of transdisciplinary methodology include the `principle of asymmetry', the `principle of bi-directionality' and (...) the `principle of transdisciplinary circularity'. The `principle of asymmetry' points to an asymmetric relationship between logical and natural conditions. The `principle of bi-directionality' claims for the necessity of bi-directional linkage between natural and logical conditions. The `principle of transdisciplinary circularity' describes systematic rules for mutual comparison and cross-conditional exchange between philosophical theory and neuroscientific hypotheses. The relation between ontology and epistemology no longer is determined by ontological presuppositions i.e. ``ontological primacy''. Instead, there is correspondence between different `epistemological capacities' and different kinds of ontology which consecutively results in ``epistemic primacy'' and ``ontological pluralism''. The present contribution concludes by rejecting some so-called `standard-arguments' including the `argument of circularity', the `argument of categorical fallacy', the `argument of validity' and the `argument of necessity'. Background: Nursing home residents are a vulnerable population. Most of them suffer from multi-morbidity, while many have cognitive impairment or dementia and need care around the clock. Several ethical challenges in nursing homes have been described in the scientific literature. Most studies have used staff members as informants, some have focused on the relatives’ view, but substantial knowledge about the residents’ perspective is lacking. Objective: To study what nursing home residents and their relatives perceive as ethical challenges in Norwegian nursing (...) homes. Research design: A qualitative design with in-depth interviews with nursing home residents, and focus-group interviews with relatives of nursing home residents. The digitally recorded interviews were transcribed verbatim. Analysis was based on Interpretive Description. Participants and research context: A total of 25 nursing home residents from nine nursing homes in Norway, and 18 relatives of nursing home residents from three of these nursing homes. Ethical considerations: This study was reported to and approved by the Regional Ethics Committee in Oslo, Norway. Findings and discussion: The main ethical challenges in Norwegian nursing homes from the residents’ and relatives’ perspective were as follows: (a) acceptance and adaptation, (b) well-being and a good life, (c) autonomy and self-determination, and (d) lack of resources. The relationship with the staff was of outmost importance and was experienced as both rewarding and problematic. None of the residents in our study mentioned ethical challenges connected to end-of-life care. Conclusion: Residents and relatives experience ethical challenges in Norwegian nursing homes, mostly connected to “everyday ethical issues.”. (shrink)

The object lesson concerns the passage from the foundational aims for which various branches of modern logic were originally developed to the discovery of areas and problems for which logical methods are effective tools. The main point stressed here is that this passage did not consist of successive refinements, a gradual evolution by adaptation as it were, but required radical changes of direction, to be compared to evolution by migration. These conflicts are illustrated by reference to set theory, model theory, (...) recursion theory, and proof theory. At the end there is a brief autobiographical note, including the touchy point to what extent the original aims of logical foundations are adequate for the broad question of the heroic tradition in the philosophy of mathematics concerned with the nature of the latter or, in modern jargon, with the architecture of mathematics and our intuitive resonances to it. Activism undertaken by CEOs has been on the rise in recent years. Research on this practice has been primarily concerned with determining the conditions under which a CEO’s public statements on sociopolitical issues are beneficial or detrimental to her firm’s business performance. We complement this instrumental perspective on CEO activism with an ethical investigation of the implications of CEO activism for the democratic process. Drawing on political philosophy, we show that the answer to the question of whether CEO activism is (...) conducive to the democratic process depends on the view of democracy that is adopted. From the perspective of liberalism, the sole requirement that an instance of CEO activism must fulfill is that it is lawful, provided that the applicable law sufficiently protects people’s essential rights. However, from the viewpoint of republicanism, this is not a sufficient condition. Besides being law-abiding, CEOs should be “civic-minded” when intervening in public debates, i.e., concerned with the quality and fairness with which those debates are conducted. Based on the literature on republicanism, we suggest four possible criteria that civic-minded CEOs can apply to gauge the democratic conduciveness of a possible public intervention: added insight, timeliness, constructiveness, and transparency. Our article complements the predominantly instrumentally oriented literature on CEO activism and contributes, more broadly, to the literature that explores the normative dimensions of corporate political involvement, as well as to a growing strand of research that draws on philosophical theory to inform business leaders’ ethical decision-making.
