Objective To evaluate the modes of death and treatment offered in the last 24 h of life to patients dying in 10 Brazilian intensive care units (ICUs) over a period of 2 years. Design and setting Cross-sectional, multicentre, retrospective study based on medical chart review. The medical records of all patients that died in seven paediatric and three adult ICUs belonging to university and tertiary hospitals over a period of 2 years were included. Deaths in the first 24 h of (...) admission to the ICU and brain death were excluded. Intervention Two intensive care fellows of each ICU were trained in fulfilling a standard protocol (κ=0.9) to record demographic data and all medical management provided in the last 48 h of life. The Student t test, Mann–Whitney U test, χ2 test and RR were used for data comparison. Measurements and main results 1053 medical charts were included (59.4% adult patients). Life support limitation was more frequent in the adult group (86% vs 43.5%; p<0.001). A ‘do not resuscitate’ order was the most common life support limitation in both groups (75% and 66%), whereas withholding/withdrawing were more frequent in the paediatric group (33.9% vs 24.9%; p=0.02). The life support limitation was rarely reported in the medical chart in both groups (52.6% and 33.7%) with scarce family involvement in the decision making process (23.0% vs 8.7%; p<0.001). Conclusion Life support limitation decision making in Brazilian ICUs is predominantly centred on the medical perspective with scarce participation of the family, and consequently several non-coherent medical interventions are observed in patients with life support limitation. RESUMO O presente artigo pretende sistematizar os principais elementos do conceito de "cena", termo fundamental para a conexão entre teoria e método no pensamento do filósofo francês Jacques Rancière. O "método da igualdade" empregado por Rancière pretende, entre outras coisas, delimitar os diferentes momentos de conflito entre regimes de verdade e atos de tomada da palavra - independentemente de classificações feitas a priori como dados para se investigar um objeto de estudo - por meio do conceito de "literalidade", que demonstra (...) duas lógicas distintas de circulação das palavras: de um lado, uma lógica "policial"; de outro, uma lógica "política". Desta maneira, o conceito de "cena" é um elemento central em uma significativa mudança da relação teoria-metodologia-objeto defendida por boa parte do meio acadêmico, inclusive na área de história, nos dias atuais, uma vez que toma a própria noção de historicidade como descontínua e considera amplamente a racionalidade das palavras em vez da minuciosa articulação dos fatos. ABSTRACT This article intends to systematize the main elements of the concept of 'scene', a fundamental term for the connection between theory and method in the thought of the French philosopher Jacques Rancière. Rancière's 'method of equality' aims, among other things, to delineate the different moments of conflict between regimes of truth and acts of taking the word - regardless of classifications made a priori as data to investigate an object of study - through of the concept of 'literality', which demonstrates two distinct logics of word circulation: on the one hand, a 'police' logic; on the other, a 'political' logic. In this way, the concept of 'scene' is a central element in a significant change in the theory-methodology-object relation defended by a fair amount of the academic world, including in history field, nowadays, since it takes the very notion of historicity as discontinuous and broadly considers the rationality of words rather than the detailed articulation of facts. Finley’s life during and just after World War II, although neglected by biographers and others interested in his career, was a formative period for him. A successful activist for Soviet-solidarity organizations, he learned lessons in public outreach that would affect his teaching and later public activity, and absorbed experiences that found their way into his first well-known book, The World of Odysseus. During this period he changed his name from Finkelstein to Finley, a shift emblematic of his adjustment to a (...) postwar situation that would prove professionally difficult for him, yet intellectually fruitful. Inicia-se o estudo com uma breve exposição de alguns elementos da concepção de história no pensamento voltairiano e então se passa à consideração que o autor britânico faz deles.The present text aims to understand the reasons that took the Scottish philosopher and critic Hugh Blair to take Voltaire as a model to the modern historian. Context: The advent of a general artificial intelligence mechanism that learns like humans do would represent the realization of an old and major dream of science. It could be achieved by an artifact able to develop its own cognitive structures following constructivist principles. However, there is a large distance between the descriptions of the intelligence made by constructivist theories and the mechanisms that currently exist. Problem: The constructivist conception of intelligence is very powerful for explaining how cognitive development takes place. (...) However, until now, no computational model has successfully demonstrated the underlying mechanisms necessary to realize it. In other words, the artificial intelligence (AI) community has not been able to give rise to a system that convincingly implements the principles of intelligence as postulated by constructivism, and that is also capable of dealing with complex environments. Results: This paper presents the constructivist anticipatory learning mechanism (CALM), an agent learning mechanism based on the constructivist approach of AI. It is designed to deal dynamically and interactively with environments that are at the same time partially deterministic and partially observable. CALM can model the regularities experienced in the interaction with the environment, on the sensorimotor level as well, as by constructing abstract or high-level representational concepts. The created model provides the knowledge necessary to generate the agent behavior. The paper also presents the coupled agent environment system (CAES) meta-architecture, which defines a conception of an autonomous agent, situated in the environment, embodied and intrinsically motivated. Implications: The paper can be seen as a step towards a computational implementation of constructivist principles, on the one hand suggesting a further perspective of this refreshing movement on the AI field (which is still too steeped in a behaviorist influence and dominated by probabilistic models and narrow applied approaches), and on the other hand bringing some abstract descriptions of the cognitive process into a more concrete dimension, in the form of algorithms. Constructivist content: The connection of this paper with constructivism is the proposal of a computational and formally described mechanism that implements important aspects of the subjective process of knowledge construction based on key ideas proposed by constructivist theories.
