Patients with advanced dementia suffer from severe cognitive and functional impairment, including eating disorders. The focus of our research is on the issue of life-sustaining treatment, specifically on the social and ethical implications of tube feeding. The treatment decision, based on values of life and dignity, involves sustaining lives that many people consider not worth living. We explore the moral approach to caring for these patients and review the history of the debate on artificial nutrition and hydration showing the impact (...) of the varying perceptions of the value of these patients' lives on changing norms. We argue that in light of the value of solidarity, decisions about life-sustaining treatment for patients with advanced dementia should be made on a case by case basis, as with any other patient, in consideration of the medical implications of the intervention which might best serve the goals of care for the individual patient. Patients with advanced dementia are less likely than those with other terminal illnesses to receive palliative care. Due to the nature and course of dementia, there may be a failure to recognize the terminal stage of the disease. A possible and under-investigated explanation for this healthcare disparity is the healthcare practitioner who plays a primary role in end-of-life decision-making. Two potential areas that might impact provider decision-making are cognitive biases and moral considerations. In this analysis, we demonstrate how the cognitive (...) biases and moral considerations of practitioners related to clinical decision-making are inherent in clinical practice and may impact on providers’ accuracy related to diagnostic and treatment related decision-making associated with patients with advanced dementia. Anchoring, default, availability, representativeness and framing biases are cognitive biases based on the "Two System Model" that relate to decision-making in end-of-life care. In patients with advanced dementia, those biases may result in a tendency to adhere to traditional mandatory care, involving an aggressive approach to care, which values saving lives at all costs, without taking into account the possible suffering and long-term consequences. Aspects such as moral sensitivity and moral courage play an important role in ethical decision-making related to advanced dementia. Investigations of clinical decision-making that include the cognitive biases and ethical considerations of practitioners might advance the comprehensive understanding of the clinical decision-making process related to care of patients with advanced dementia and promote the quality of care given to this population. Are the circumstances in which moral testimony serves as evidence that our judgement-forming processes are unreliable the same circumstances in which mundane testimony serves as evidence that our mundane judgement-forming processes are unreliable? In answering this question, we distinguish two possible roles for testimony: (i) providing a legitimate basis for a judgement, (ii) providing (‘higher-order’) evidence that a judgement-forming process is unreliable. We explore the possibilities for a view according to which moral testimony does not, in contrast to mundane testimony (...) play role (i), but can play role (ii). We argue that standard motivations for rejecting this hybrid position are unpersuasive but suggest that a more compelling reason might be found in considering the social nature of morality. (shrink)

In classical Chinese philosophy, the best kind of life is a life lived in line with the Dao (the “Way”). A core feature of this kind of life is attaining the ideal of wu-wei. In early Daoist writings, wu-wei denotes an ideal way of acting. However, since wu-wei is normally translated as “no-action” these ancient texts give us a picture of the best kind of life that may appear paradoxical to many philosophers. In this paper, I suggest a way to (...) make sense of this classical ideal. I argue that by applying a Merleau-Pontyian framework of action we can arrive at a non-paradoxical reading of wu-wei. On this reading, wu-wei is essentially manifested in a specific way we are aware of what we are doing. (shrink)

The purpose of this study is to examine whether United States -style regulatory intervention to encourage whistle-blowing can be immediately effective if transplanted into another country with a distinctly different historical cultural background and institutional system. A total of 98 U.S. and 84 German accountants participated in a laboratory experiment relating to a case of financial statement fraud. The provision of anti-retaliation protection and monetary rewards for whistle-blowing were manipulated and participants were asked to assume the role of an internal (...) auditor. We hypothesize and find that the provision of anti-retaliation protection and monetary rewards encourage U.S. accountants to blow the whistle. In contrast, among German accountants, where their country features a historical fear and distrust of whistle-blowers, U.S.-style regulatory interventions are less effective. Together, our findings provide strong support for the theory of path-dependence, suggesting that whistle-blowing regulation should not be uniformly transplanted without due consideration of the unique history and culture of a country. This article shows how the legacy of Hans Christian Ørsted (1777-1851), and his discovery of electromagnetism in 1820, was reconfigured and used by the H.C. english abstracts Ørsted Committee from 1914 before and during the centenary of 1920. Combining Somsen’s (2008) concept of olympic internationalism with uses of the past, I suggest understanding the 1920-centenary as an instance of olympic commemoration [olympisk historiebrug]. I argue that central actors from the committee, e.g. Martin Knudsen (1871-1949) and Kirstine Meyer, née Bjerrum (1861-1941), (...) used Ørsted's nationality to substantiate Denmark’s cultural and scientific achievements. By comparison to pre-twentieth century biographies, I show how the committee re-described Ørsted, i.e. from a scientist [Naturforsker] to being a Danish scientist [dansk Naturforsker]. The committee was deeply concerned with proving the big contributions by the small nation of Denmark, and I show how this relates to cultural politics of the early twentieth century Denmark. The profession of medicine has developed codes of ethical conduct for thousands of years. From the Hippocratic Oath of ancient Greece onward to modern times, a universal and central element of such codes has expressed the imperative that a physician shall “Do no harm.”.

Researchers have recognized that interpersonal trust consists of different dimensions. These dimensions suggest that trust can be rational, cognitive, or affective. Affect, which includes moods and emotions, is likely to have a direct impact on the affective dimension. On the other hand, there are also studies showing that affect indirectly influence cognitive judgments. Nonetheless, in this chapter we argue that the impact of affect on judgment will not be the same on all individuals. In effect, the impact varies, depending on (...) the individual's attention to affect, motivation to use or guard against affect, or regulation of affect. All this may suggest that an individual's abilities or tendencies to manage affect will have implication on his or her trust relationships with others. (shrink)

Vann McGee claims that open-ended schemas are more innocuous than ordinary second-order quantification, particularly in terms of ontological commitment. We argue that this is not the case.

This paper attempts to delineate the interaction between trust, emotion, and ethical decision making. The authors first propose that trust can either incite an individual toward ethical decisions or drag him or her away from ethical decisions, depending on different situations. The authors then postulate that the feeling of guilt is central in understanding how trust affects the ethical decision making process. Several propositions based on these assumptions are introduced and implications for practice discussed.

This article investigates the impact of different emotions on trust decisions taking into account the experience of betrayal. Thus, an experiment was created that included one betrayal group and one control group. Participants in the betrayal group experienced more intense feelings governed by negative emotions than participants in the control group did. Moreover, participants in the betrayal group significantly lowered their trust of another stranger. On the other hand, we found some evidence that neuroticism exaggerated the relationship between experienced betrayal (...) and subsequent trust. (shrink)

By adopting social exchange theory and the affect-infusion-model, the hypothesis is made that emotional intelligence (EI) will have an impact on three perceptions of trustworthiness – ability, integrity and benevolence – at the beginning of a relationship. It was also hypothesized that additional information would gradually displace EI in forming the above perceptions. The results reveal that EI initially does not contribute to any of the perceptions of trustworthiness. As more information is revealed EI has an impact on the perception (...) of benevolence, but not on the perceptions of ability and integrity. This impact was observed to be negative when the nature of the information was negative. On the other hand, information alone was shown to have a significant impact on the perceptions of ability and integrity, but not on the perception of benevolence. Second-order axiomatizations of certain important mathematical theories—such as arithmetic and real analysis—can be shown to be categorical. Categoricity implies semantic completeness, and semantic completeness in turn implies determinacy of truth-value. Second-order axiomatizations are thus appealing to realists as they sometimes seem to offer support for the realist thesis that mathematical statements have determinate truth-values. The status of second-order logic is a controversial issue, however. Worries about ontological commitment have been influential in the debate. Recently, Vann McGee has argued that one (...) can get some of the technical advantages of second-order axiomatizations—categoricity, in particular—while walking free of worries about ontological commitment. In so arguing he appeals to the notion of an open-ended schema—a schema that holds no matter how the language of the relevant theory is extended. Contra McGee, we argue that second-order quantification and open-ended schemas are on a par when it comes to ontological commitment. (shrink)

Drug Safety Communications are used by the Food and Drug Administration to inform health care providers, patients, caregivers, and the general public about safety issues related to FDA-approved drugs. To assess patient knowledge of the messaging contained in DSCs related to the sleep aids zolpidem and eszopiclone, we conducted a large, cross-sectional patient survey of 1,982 commercially insured patients selected by stratified random sampling from the Optum Research Database who had filled at least two prescriptions for either zolpidem or eszopiclone (...) between July 1, 2012 and June 30, 2013. Among the 594 respondents, two-thirds reported hearing generally about drug safety information prior to starting a new drug, with the remaining one-third “rarely” or “never” hearing such information. Providers and pharmacists were primary sources of drug safety information. Two-thirds of zolpidem users and half of eszopiclone users reported having heard about the related DSC messages, ability to accurately identify the major factual messages was limited. Respondents reacted to new drug safety information about their sleep aids by reporting that they would want to learn about alternative ways to help them sleep and seek out more information about the safety of their specific sleeping pill. Opportunities may exist for the FDA to work with providers and pharmacies to help ensure the DSC information is more widely received and is more fully understood by those taking the affected medications. En lo que sigue queremos sintetizar una lectura de la obra de Herbert Marcuse que pone el énfasis en un cruce posible (...) entre su visión antropológica y su visión epistemológica desde una metodología que denominamos dialéctica. La confluencia propuesta supone considerar a la categoría racionalidad tecnológica como una categoría central de su obra que permite estructurar una lectura de conceptos que, aventuramos, siguen vigentes para pensar y re-pensar nuestra condición humana. Esta categoría reúne reflexiones filosóficas y políticas en el intento por desnudar la principal ideología que permite cohesionar a los individuos en los modelos sociales centrales del siglo XX. This text is a part of the M.S. Dissertation, "Scientific-technological rationality. Contributions to an epistemological reflection on Herbert Marcuse's work", under the supervision of PhD Delia Albarracín for the Master in Methodology for Scientific Investigation, Lanús University, directed by Esther Díaz, held on August 26th, 2011. What follows is a summary of our approach to Herbert Marcuse's work, which focuses on the crossing between the anthropological and the epistemological vision, from a methodological view that we call dialectics. The proposed convergence implies that the technological rationality category is central in his work and permits a certain reading of concepts that are still in force to think over our human condition. This category joins philosophical and political reflections in an attempt to unveil the main ideology that unites individuals in the XXth century's principal social models. (shrink)

This book challenges the conventional wisdom that improving democratic politics requires keeping emotion out of it. Marcus advances the provocative claim that the tradition in democratic theory of treating emotion and reason as hostile opposites is misguided and leads contemporary theorists to misdiagnose the current state of American democracy. Instead of viewing the presence of emotion in politics as a failure of rationality and therefore as a failure of citizenship, Marcus argues, democratic theorists need to understand that emotions (...) are in fact a prerequisite for the exercise of reason and thus essential for rational democratic deliberation and political judgment. Attempts to purge emotion from public life not only are destined to fail, but ultimately would rob democracies of a key source of revitalization and change. Drawing on recent research in neuroscience, Marcus shows how emotion functions generally and what role it plays in politics. In contrast to the traditional view of emotion as a form of agitation associated with belief, neuroscience reveals it to be generated by brain systems that operate largely outside of awareness. Two of these systems, "disposition" and "surveillance," are especially important in enabling emotions to produce habits, which often serve a positive function in democratic societies. But anxiety, also a preconscious emotion, is crucial to democratic politics as well because it can inhibit or disable habits and thus clear a space for the conscious use of reason and deliberation. If we acknowledge how emotion facilitates reason and is "cooperatively entangled" with it, Marcus concludes, then we should recognize sentimental citizens as the only citizens really capable of exercising political judgment and of putting their decisions into action. Dazu werden zunächst Grundzüge der Lebensphänomenologie von Michel Henry rekonstruiert, um anschließend Überlegungen im Versuch einer hermeneutisch-epistemologischen Anwendung das „Wie“ des Sakramentes als das In-Erscheinung-Treten des absoluten Lebens zu erschließen. Sakramente können damit als das In-Erscheinung-Tretens des Stiftungsereignisses des absoluten Lebens gedacht werden.

This fascinating study explores the difficulties faced by modern Westerners in their search for a meaningful life. It sheds light on this enduring cultural dilemma through a close reading of four popular film and television narratives: Pixar's animated feature film, Toy Story; Christopher Nolan's Batman Begins and The Dark Knight; the television romantic comedy, Sex and the City; and, finally, the mobster drama, The Sopranos. Neste artigo apresenta-se a proposta de compreender os textos restantes do livro perdido de Heráclito como arranjos de palavras a partir de padrões audiofocais. Assim, tais textos não apenas conteriam referências a sons como também explorariam criativamente grandezas psicoacústicas. Basilar para a argumentação aqui desenvolvida é a identificação de um movimento oscilatório presente tanto na distribuição dos vocábulos e frases quanto nas referências textuais. Tal movimento oscilatório aproxima a escrita de Heráclito de eventos físicos acústicos.
