Rationale, aims and objectives Given the increasing prevalence of cataract and demand for cataract extraction surgery, patients must often wait to undergo this procedure. We validated a previously developed priority scoring system in terms of clinical variables, pre-intervention health status, appropriateness of surgery and gain in visual acuity (VA) and health-related quality of life (HRQoL).Methods Explicit prioritization criteria for cataract extraction created by a variation of the Research and Development (RAND) and University of California Los Angeles appropriateness methodology were retrospectively (...) applied to a prospective cohort of 5257 patients on waiting lists to undergo cataract by phacoemulsification at 17 hospitals in Spain. Demographic data, clinical data and data related to surgical technique were collected by trained ophthalmologists. Patients were evaluated by their ophthalmologist before the intervention and 6 weeks afterward. They also completed, by mail, the Visual Function Index (VF-14) before the intervention and 3 months afterward.Results High-priority patients experienced greater improvement in VA and HRQoL than those classified as intermediate or low-priority (P < 0.0001), even after adjusting by VA and the VF-14 score at baseline. The time to intervention was the same for high-priority patients as it was for intermediate and low-priority patients.Conclusions The priority score we developed identified patients most likely to experience the greatest improvements from cataract extraction. Use of this tool could provide a fairer and more rational way to prioritize patients for cataract extraction. (shrink)

Empirical studies have revealed a disturbing prevalence of research misconduct in a wide variety of disciplines, although not, to date, in the areas of ethics and philosophy. This study aims to provide empirical evidence on perceptions of how serious a problem research misconduct is in these two disciplines in Spain, particularly regarding the effects that the model used to evaluate academics’ research performance may have on their ethical behaviour. The methodological triangulation applied in the study combines a questionnaire, a debate (...) at the annual meeting of scientific association, and in-depth interviews. Of the 541 questionnaires sent out, 201 responses were obtained, with a significant difference in the participation of researchers in philosophy and in ethics ; 26 researchers took part in the debate and 14 interviews were conducted. The questionnaire results reveal that 91.5% of the respondents considered research misconduct to be on the rise; 63.2% considered at least three of the fraudulent practices referred to in the study to be commonplace, and 84.1% identified two or more such practices. The researchers perceived a high prevalence of duplicate publication and self-plagiarism, use of personal influence and citation manipulation, in contrast to a low perceived incidence of data falsification or fabrication. The debate and the interviews corroborated these data. Researchers associated the spread of these misconducts with the research evaluation model applied in Spain. (shrink)

El diagnóstico y caracterización del estado actual de la formación socio-humanista de los estudiantes que ingresan a la Casa de Altos Estudios reveló las insuficiencias que existen, por lo cual el objetivo consistió en la elaboración de un sistema de talleres de apreciación literaria para la formación socio-humanista de los estudiantes de Medicina de primer año de la Universidad de Ciencias Médicas (...) de Camagüey que forma parte de un proyecto aprobado en el programa de ciencia, tecnología e innovación nacional número 2, Organización, eficiencia y calidad de los servicios de salud, con el código 152049. El mismo constituye un aporte al Trabajo Educativo y de Extensión Universitaria en la institución. Medical universities face the challenge of graduating a professional with a comprehensive cultural background. The diagnosis and characterization of the current situation in regards to the socio-humanistic education of the students entering Camaguey´s Medical University revealed several insufficiencies. Due to this, the objective of this research was to prepare a system of literary analysis workshops to enable the socio-humanistic education of first year Medical students at Camaguey´s Medical University. Qualitative description of movements can be very important for representation and reasoning about dynamic systems which are complex in structure or whenever numerical data are incomplete or inaccessible. For this reason, we present a hybrid approach based on the combination of qualitative reasoning, quantitative data and logical methods. In this article, we introduce a new propositional dynamic logic QM for representation and reasoning with relative movements of objects. In this way, we can infer additional information about movements by using axioms (...) and the logic apparatus. We present a sound and complete deduction system in dual tableaux style for the logic QM. The system can be used for verification of validity of formulas of the logic in question. (shrink)

Despite its successes, to date Artificial Intelligence (AI) is still characterized by a number of shortcomings with regards to different application domains and goals. These limitations are arguably both conceptual (e.g., related to the underlying theoretical models, such as symbolic vs.connectionist), and operational (e.g., related to robustness and ability to generalize). Biologically inspired AI, and more specifically brain-inspired AI, promises to provide further biological aspects beyond those that are already traditionally included in AI, making it possible to assess and possibly (...) overcome some of its present shortcomings. This article examines some conceptual, technical, and ethical issues raised by the development and use of brain-inspired AI. Against this background, the paper asks whether there is anything ethically unique about brain-inspired AI. The aim of the paper is to introduce a method that has a heuristic nature and that can be applied to identify and address the ethical issues arising from brain-inspired AI (and from AI more generally). The conclusion resulting from the application of this method is that, compared to traditional AI, brain-inspired AI raises new foundational ethical issues and some new practical ethical issues, and exacerbates some of the issues raised by traditional AI. Equational reasoning arises in many areas of mathematics and computer science. It is a cornerstone of algebraic reasoning and results essential in tasks of specification and verification in functional programming, where a program is mainly a set of equations. The usual manipulation of identities while conducting informal proofs obviates many intermediate steps that are neccesary while developing them using a formal system, such as the equationally complete Birkhoff calculus ${\mathcal{B}}$. This deductive system does not fit in the common manner of (...) doing mathematical proofs, and it is not compatible with the mechanisms of proof assistants. The aim of this work is to provide a deductive system ${\mathcal{B}}^{\textrm{GOAL}}$ for equality, equivalent to ${\mathcal{B}}$ but suitable for constructing equational proofs in a backward fashion. This feature makes it adequate for interactive proof-search in the approach of proof assistants. This will be achieved by turning ${\mathcal{B}}^{\textrm{GOAL}}$ into a transition system of formal tactics in the style of Edinburgh LCF, such transformation allows us to give a direct formal definition of backward proof in equational logic. L’évolution industrielle nous destine à vivre dans des environnements technologiques caractérisés par une dissémination informatique totale dans la multitude et d’objets connectés capables de traitement sémantique et d’interaction informationnelle avec les humains. Dans ce contexte d’Intelligence ambiante se déploient des dispositifs domotiques d’Interactions dites naturelles Homme / machine. Ainsi de Google Home présenté comme : « une enceinte à commande vocale qui fonctionne avec l'Assistant Google ». On pourra tout aussi bien discuter avec les agents de la concurrence : Amazon (...) Echo, Apple HomePod, etc. Ces dispositifs nous conduisent à interroger une étape contemporaine majeure de « la révolution technologique du langage » et de son automatisation, qui fait des machines de réels sujets parlants, inscrits dans un « couplage humano-machinique ». Dans ce contexte fortement marqué par les affordances, nous observons les modalités de cette interaction artéfactuelle et dite naturelle, caractérisée notamment par une simulation conversationnelle et une personnalisation des dispositifs technologiques. El presente artículo tiene como objetivo develar la implicación de los nexos entre la cultura y los procesos de comprensión y desarrollo psíquico para la concepción e instrumentación de una enseñanza desarrolladora atendiendo a que el desarrollo psíquico del sujeto es una condición esencial para lograr su papel activo y creador en el desarrollo social. Por ello su tránsito a niveles cualitativamente superiores, constituye una prioridad para los sistemas educativos. This article aims at revealing the implications of the links between culture and the processes of understanding and psychic development towards the conception and implementation of developmental teaching taking into account that an individual's psychic development is an essential condition to fulfil its active and creative role in social development. That is why its movement to qualitatively superior levels is a priority for educational systems. The historical-cultural theory gives special importance to the role of culture and its assimilation in the formation and development of the personality. Furthermore, disciplines such as semiotics recognize the role of texts and textual processes in the conservation, transmission and recreation of culture. Background: Electroconvulsive therapy is one of the most effective treatments for major depressive disorder. Recently, there has been increasing attention to evaluate the effect of ECT on resting-state functional magnetic resonance imaging. This study aims to compare rs-fMRI of depressive disorder patients with healthy participants, investigate whether pre-ECT dynamic functional network connectivity network estimated from patients rs-fMRI is associated with an eventual ECT outcome, and explore the effect of ECT on brain network states.Method: Resting-state functional magnetic resonance imaging data were (...) collected from 119 patients with depression or depressive disorder, and 61 healthy participants, with an age mean of 52.25 years old. The pre-ECT and post-ECT Hamilton Depression Rating Scale were 25.59 ± 6.14 and 11.48 ± 9.07, respectively. Twenty-four independent components from default mode and cognitive control network were extracted, using group-independent component analysis from pre-ECT and post-ECT rs-fMRI. Then, the sliding window approach was used to estimate the pre-and post-ECT dFNC of each subject. Next, k-means clustering was separately applied to pre-ECT dFNC and post-ECT dFNC to assess three distinct states from each participant. We calculated the amount of time each subject spends in each state, which is called “occupancy rate” or OCR. Next, we compared OCR values between HC and DEP participants. We also calculated the partial correlation between pre-ECT OCRs and HDRS change while controlling for age, gender, and site. Finally, we evaluated the effectiveness of ECT by comparing pre- and post-ECT OCR of DEP and HC participants.Results: The main findings include depressive disorder patients had significantly lower OCR values than the HC group in state 2, where connectivity between cognitive control network and default mode network was relatively higher than other states, Pre-ECT OCR of state, with more negative connectivity between CCN and DMN components, is linked with the HDRS changes. This means that those DEP patients who spent less time in this state showed more HDRS change, and The post-ECT OCR analysis suggested that ECT increased the amount of time DEP patients spent in state 2.Conclusion: Our finding suggests that dynamic functional network connectivity features, estimated from CCN and DMN, show promise as a predictive biomarker of the ECT outcome of DEP patients. Also, this study identifies a possible underlying mechanism associated with the ECT effect on DEP patients. (shrink)

This paper reflects on the notion and structural components of two concepts: the human condition and human nature. Likewise, it is proposed that both concepts should be included in the basic themes not only in the field of Educational Philosophy, but also in all disciplines where the human being occupies a privileged place. Human condition is understood as a state of subjection to everything with which human beings interact and represents the starting point to ascend to three natures that correspond (...) to human beings. Hegel emphasizes two natures of man. The first is characterized by having a biological structure governed by instincts. The second is culture and to ascend to it requires processes of intellection that can be objectified through consciousness. The third human nature proposed in this paper is rooted in Ernst Bloch’s principle of hope. It is materialized by human actions tending to the construction of the future from the discovery of what is not-yet-conscious that, when identified, is achieved through actions. (shrink)

In the editorial note, we shortly outline the reception of Kant’s philosophy in Finland and how Kant’s philosophy became an academic topic from the late 18th century onwards. We also provide some details about the translations of Kant’s works into Finnish. Finally, we introduce the contributions to the special section on Kantian studies in Finland.

_Background:_ Preterm infants are still at an increased risk for suboptimal neurodevelopmental outcomes when compared with term born infants. The development of a child born preterm can be jeopardized by suboptimal conditions during pregnancy, in addition to the suboptimal growth environment postnatally compared to the normal in utero environment. This review summarizes the literature on the role of chorioamnionitis, placental insufficiency, and maternal smoking on the developmental outcomes of preterm infants. _Methods:_ A systematic database search was performed to identify all (...) original articles published on or before September 12, 2018 that evaluated the impact of clinical or histological chorioamnionitis, abnormal prenatal fetal and placental blood flow, and prenatal smoking exposure on the neuropsychological and cognitive outcomes of preterm infants. We identified a total of 54 studies. Thirty five original articles evaluated the effects of clinical or histological chorioamnionitis; 15 studies evaluated the effects of abnormal blood flow patterns; and four studies evaluated the effects of maternal smoking during pregnancy. _Results:_ The studies on prenatal risk factors showed conflicting results about the impact on the neurodevelopment of preterm infants. The majority of the studies did not show that chorioamnionitis poses a direct risk to the development of preterm infants. The role of abnormal prenatal placental and fetal blood flow on the development of preterm infants remained inconclusive because the sample sizes were often small and methodological problems complicated the interpretation of the data. Maternal smoking during pregnancy was assessed only in one cohort which showed that maternal smoking is a risk for suboptimal cognitive and neuropsychological development in preterm infants. _Conclusions:_ This review summarizes the data on several prenatal risk factors which play a role in the developmental outcomes of preterm infants. To optimize the developmental outcomes, we need to first optimize the fetal wellbeing before birth. More research that extends from the fetal life to long-term developmental outcomes is needed. Stakeholder engagement and dialogue have a central role in defining the relations between organisations and their internal and external interlocutors. Drawing upon the analysis of dialogic motifs, power–conflict dynamics and sociopolitical perspectives, and based on a set of interviews with the stakeholders of a consumer-owned cooperative, the research explores the dialogic potential of stakeholder engagement. The analysis revealed a fragmented picture where the co-design and co-implementation aspects were mainly related to the non-business areas of cooperative life, while business logic dominated (...) the most central aspects. Stakeholder engagement was mainly related to consensus building, while dialogic engagement based on a pluralistic understanding was only partially considered and then neglected. The social capital in the local area, the growing size of the organisation and the related power structure embrace stakeholder engagement, influencing the orientation of the dialogic dynamic. The analysis indicates that a dialogic exchange is a relative concept which depends on the interests involved and the topics discussed. It also reveals that the key factors in the democratisation of stakeholder engagement are a mutual understanding and long-term opportunities. Common sociopolitical aspects are also important, but they do not necessarily guarantee the creation of dialogism paths. The research contributes to the critical dialogic literature in revealing whether and how stakeholder engagement has been implemented in a specific setting. It also shows the limitations of voluntarist stakeholder engagement initiatives. (shrink)

In the literature there are at least two models for probabilistic belief revision: Bayesian updating and imaging [Lewis, D. K. (1973), Counterfactuals, Blackwell, Oxford; Gärdenfors, P. (1988), Knowledge in flux: modeling the dynamics of epistemic states, MIT Press, Cambridge, MA]. In this paper we focus on imaging rules that can be described by the following procedure: (1) Identify every state with some real valued vector of characteristics, and accordingly identify every probabilistic belief with an expected vector of characteristics; (2) For (...) every initial belief and every piece of information, choose the revised belief which is compatible with this information and for which the expected vector of characteristics has minimal Euclidean distance to the expected vector of characteristics of the initial belief. This class of rules thus satisfies an intuitive notion of minimal belief revision. The main result in this paper is to provide an axiomatic characterization of this class of imaging rules. (shrink)

According to Article 8 of the Paris Agreement, parties to the treaty recognise and commit to address, avert and minimise losses and damages associated with adverse effects of climate change. For many societies, such as Small Island Developing States, loss and damage is a matter of survival. Global warming and sea-level-rise are threatening the territories and livelihoods of vulnerable low-lying island states and thereby undermining many of the fundamental human rights and freedoms of individuals living in these societies. This paper (...) looks at loss and damage through the concept of reparations in the context of the coloniality of climate. By studying the case of the Marshall Islands and its history with nuclear testing, the paper argues for a connection between irreversible loss and damage and the right to remedy and reparation by proposing that the nuclear legacy represents a grave violation of the human rights of the Marshallese and has led to a permanent loss of land and other severe impacts on health, culture and livelihoods, which now are exacerbated by climate change. Estudio del término sakīna en su evolución postcoránica en el que se muestra, en primer lugar, lo que la investigación ha dicho sobre él, para, a continuación, presentar, traducir y analizar cinco textos pertenecientes al ámbito islámico y al filosófico en donde aparece dicho término y que no habían sido tenidos en cuenta hasta ahora. Estos textos proceden de: Ibn Ḥabīb, al-Tawḥīdī, traducción árabe de los Versos áureos, y Comentarios de Jámblico y de Proclo a estos Versos áureos. La sakīna (...) coránica evoluciona en estos autores pasando a connotar una pacificación en los animales violentos en Ibn Habib y un estado cercano a la divinidad de caracteres próximos al de los sufíes en al-Tawhidi, mientras que en los Versos áureos se emplea sakīna para traducir daímōn, con sus efectos de eliminación de los males originados por la discordia innata a los hombres. En los Comentarios a los Versos áureos se dan explicaciones, de corte religioso y racionalista a la vez, de dicho término. (shrink)

Normality, for Husserl, is said in many ways. While the most detailed treatments of this technical Husserlian concept are usually found in discussions concerning the constitutive dimension of the lived body and intersubjectivity, little attention has been paid to the notion of normality understood as the tacit regularity of nature. Indeed, the normal can also be understood as the causal background which is presupposed, tentatively, in the anticipation of uniform processes of change, as well as in poieticinstrumental experiences, that is, (...) in experiences involving the production and use of objects. The subject that elaborates, manipulates or uses objects is installed in nature. Hence, in our productive-instrumental confrontation with objects in the world, we operate under the assumption that the potentially mutable causal scene surrounding our acting life will retain its style. This assumption of normality is the world, in one of the many ways in which Husserl characterizes this concept. The world qua background of normality becomes thematic, retrospectively, precisely when it ceases to cooperate with the agent’s initiatives. This, on my view, is what lies behind Husserl’s assertion that the world is a “super-object” or “higher-level theme”. (shrink)

Post‐translational modifications regulate each step of DNA replication to ensure the faithful transmission of genetic information. In this context, we recently showed that deubiquitination of SUMO2/3 and SUMOylated proteins by USP7 helps to create a SUMO‐rich and ubiquitin‐low environment around replisomes that is necessary to maintain the activity of replication forks and for new origin firing. We propose that a two‐flag system mediates the collective concentration of factors at sites of DNA replication, whereby SUMO and Ubiquitinated‐SUMO would constitute “stay” or (...) “go” signals respectively for replisome and accessory factors. We here discuss the findings that led to this model, which have implications for the potential use of USP7 inhibitors as anticancer agents. (shrink)
